This paper aims to bring reflections and notes for strengthening Brazilian structuring public policies, focusing on Popular Health Education in the Unified Health System (SUS) from the perspectives built in the Observatory of Popular Health Education and the Brazilian Reality. The Observatory is a valuable space for sharing health professionals' and popular educators' interpretations and experiences about local and Brazilian realities from the perspective of Popular Health Education. During its two years of activity, the Observatory has gathered summary interpretations of Popular Health Education for the crises that traverse the country's recent history in a dialogical and participatory way. In a panoramic view, the shared statements point to challenges for valuing the human approach to health promotion, including respecting local and community knowledge and social practices. Moreover, we underscore the importance of social participation in constructing participatory social processes in public health toward citizen autonomy and expanded democratic dynamics in the Brazilian State and its social equipment.

Acknowledging the contributions of the assessment area in supporting the performance of health policies, is to admit it in an ongoing and permanent way in the management context. This requires a set of procedures that go beyond monitoring and evaluation practices, known as performance management. The goal of this study was to analyze the logic of the Health Surveillance (HS) Evaluation Policy of Pernambuco, comparing it with the corresponding Canadian policy. For this purpose, a qualitative study of logical analysis of the program theory was carried out, using as a tool the design of the logical model of performance management and its respective matrix of analysis and judgment with the criteria to be evaluated. In HS, 9 key-informants were interviewed, and documents were analyzed; the Canadian model was analyzed based on a paper written by Lahey (2010). Both policies analyzed by this study are convergent and have the necessary elements for performance management. While the evaluation featured largely in the Canadian model, monitoring was the driving force behind the institutionalization of assessment practices in HS. Some lessons learned in the Canadian model can be recommended, such as the development of an assessment plan, based on the strategic and decision-making level of HS.

This paper aimed to analyze the contribution of Journal Ciência & Saúde Coletiva (C&SC) to the construction of the CH field, particularly in its relationships with the Health Policy, understood both as an academic discipline and as a scope of practice. We reviewed papers published between 1996 and 2019 in the C&SC. Titles and abstracts of the 397 documents on health policies were read to measure the magnitude and identify the main themes and theoretical-methodological approaches. Thirty-five documents were selected and read in full among the 142 revised ones to investigate the Journal's contribution to CH's construction. The analysis was based on Bourdieu's sociology. It revealed that C&SC was established as a space for the construction of CH in multiple dimensions, particularly concerning the reflexivity on the field. Specifically, concerning the Health Policy, despite the small percentage of documents on the issue (6.8%), it encompassed the different meanings of this subject for the field. The authors discuss the possible relationships between the characteristics identified and the historical process of incorporating this subject in the various areas of CH.

This paper aims to investigate public inter-municipal health consortium operation in Brazil. To this end, a qualitative documentary analysis was conducted on the content of the agreements between the consortia and the Federal Government, available at the Transparency Portal of the Federal Government, from 1996 to 2016. The results cover two categories: agreements concluded in Brazil and the content of the agreements signed by an inter-municipal public consortium (CIS). The agreements signed were concentrated mainly in the Southern and Southeastern regions and aimed to carry out regional actions, thus contributing to health regionalization. However, challenges related to the process of cooperation and coordination between the health management bodies persist, mainly related to the improvement of linkages between the consortium and the regional health coordination and in-depth social control of these organizations.

This paper analyzes recent policies in the field of Primary Health Care (PHC) and their possible implications for the care model in the Unified Health System (SUS). Initially, some of the concepts that influenced the models of care in the Brazilian public system are revived, and we argue that the Family Health Strategy (ESF) bases for reorienting care practices in primary care are consistent with the principles of the SUS. Below, we analyze the central elements of new federal policies for PHC. We show that changes in the PHC care model threaten the teams' multidisciplinarity, prioritize acute illness care, focus in individual care, weaken the community territorial approach and establish coverage by registration, which evidence redirection of the health policy, harming the principles of universality, integrality, and equity in the SUS.

This paper presents the issue of judicialization of the right to health in Brazil. Data from the National Council of Justice evidence a substantial increase in the number of lawsuits concerning the right to health. We emphasize that the national doctrine exhaustively discusses ways to make the authority more effective, but it does not, as a general rule, discuss the economic aspect of health judicialization. Using the concept of opportunity cost extracted from economics science, it is shown that the judge, by deferring the lawsuit formulated by the plaintiff, automatically forces the Executive Branch to reduce the scope of other policies to generate resources to meet the court order. In specific contexts, this setting ends up favoring individual rights at the expense of the collective rights of SUS users, in violation of the principle of isonomy and efficiency. Finally, the case of the judicialization promoted by the hemophiliac patients in the Federal District is shown as a way of evidencing, at the factual level, the consequences of judicialization in the SUS policies.

This article discusses the issue of social protection against the demands presented in the treatment of children and adolescents in Brazil. It aims to problematize the advances and limits of the Brazilian social protection system in face of the social demands arising from the treatment of children and adolescents that go beyond the specific limits of health care. It is a qualitative study that was structured based on the analysis of official documents, Laws, Decrees, Ordinances and Policies and the literature destined to children and adolescents in cancer treatment whose incapacitating consequences produce social demands that impact, above all, family life. The paper presents social policies as a fundamental resource for treatment, considering the demand for income and displacement, in order to enable an effective health care. However, the reflections indicate that there is now a retraction of social policies by the State that was expressed in the focus and transfer of responsibilities of the State to other sectors of civil society.

Objective: This paper examines two experiences of social networks developed as a space for strengthening public health policies in the Brazilian context. To describe and analyze some possibilities of using social networks as devices linked to public health policies based on the experience of two comparative cases currently underway in Brazil: The HumanizaSUS Network and the Community of Primary Care Practices., Methods: This is qualitative research with a comparative case study approach, with emphasis on the exploitation of data available in the public platform of both networks and publications around these two experiences., Results: Webometric data of the cases studied will be shown, pointing out aspects of differentiation and similarity between them from three axes of analysis: (1) theoretical-conceptual framework; (2) the design of the platform, its functionalities and its daily support process; (3) the singularities of the related policies. The discussion of these points indicates that social networks can function as devices for education, production of a collection of experiences, clinical collaboration and especially a collaborative creation of spaces for sharing experiences and collective reflection on the daily construction of public policy.

Governance is a concept with different hues in public policies. In this study, it is conceived as a policy network, with stakeholders who manage interests depending on available resources, mediated by rules and by their degree of influence. This paper analyzes the stakeholders, spaces of articulation, norms, and processes in health governance in two regions of the Legal Amazon and how managers shape their social network. This is a qualitative study with regional analysis plan, using as sources documents and interviews. An analysis matrix was formulated with adapted realms of conceptual models of governance and policy networks. Results point to differentiated policy networks. In the metropolitan region, stakeholders' diversity was more associated with state management and capital, given the articulation capacity and available resources. In the international border region, the policy network of municipal managers integrates stakeholders from diverse sectors in the face of regional specificity and articulation capacity. Regions, living territories, single out technical processes, leading stakeholders to seek partners and spaces beyond those established, to weave strategies closer to reality.

This article examines the strategy for construction of a System of Indicators for Monitoring the Public Policies and Health Situation of the Elderly (SISAP-Idoso). The System seeks to assess the production of health information with the political/institutional decision, serving as a management tool in the health of the elderly. Its focus is to meet the need of the Unified Health System municipal managers for information for planning and controlling health programs. The paper presents the methods used in the creation of the Conceptual Matrix for Health Dimensions and Conceptual Matrix for Monitoring and Evaluation of Policies. The first systematizes a panel of indicators to diagnose the health condition of the elderly, a procedure without which one cannot properly plan health actions. The second selects indicators to monitor implementation and evaluate the results of health policies and programs for the elderly. The System became an important tool to support the formulation and monitoring of health programs, particularly at the local level. Moreover, it is a powerful tool of social control and population empowerment. The full incorporation of the System depends on the consolidation of information and indicators as parts of the policy process.

Over recent decades, several Latin American health systems have undergone reforms. This paper analyzes health policies in Argentina, Brazil and Mexico from 1990 to 2014. It explores the reform strategies, explanatory factors and effects on the configuration of each health system. The analytical framework was based on the historical-comparative approach and considered the following aspects: political and economic context; health reform agendas, processes and strategies; changes in the health system configuration in terms of social stratification and de-commodification. The research methods involved literature review, document and data analysis and interviews. In the period, Argentina maintained an employment-based and fragmented healthcare system, expanded specific public programs and private health plans. Brazil created a public and universal health system, which coexists with a dynamic and growing private sector. Mexico maintained the employment-based health care and created a popular health insurance. Although the reform influences and strategies varied between the countries, social stratification and commodification persisted in the three health systems, under different arrangements.The transformation of these characteristics is essential to build universal health systems in Latin America.

This paper addresses the popular health movement in Nova Iguaçu-RJ in the 1970s and 1980s. Amidst political repression, residents organized themselves to find solutions to various problems, including health problems. Health demands are enhanced both by the dengue epidemic and linkages with the Brazilian health reform and a struggle for democracy. Using documental historical sources from newspapers, health dissemination papers and documents from the House of Oswaldo Cruz-Fiocruz collection, this paper concludes that the example of Nova Iguaçu reveals both the complexity of the process of political opening at the local level and efforts of conducting a health reform where it was needed the most. Resistance on the part of the medical corporation, private interests in health and the existence of a still incipient popular political organization and culture are some of the elements that account for the hardships in advancing the manifest wishes of policies. The specificities of local arenas also point to possible institutional arrangements, sometimes very peculiar and not reproducible in other settings.

The concept and approach of the Health Economic-Industrial Complex (HEIC) were present in the advancement of the Unified Health System (SUS) in the last decades, contributing to the incorporation of an agenda related to the national pattern of development. The paper reconstructs this approach to capture its dynamics and demarcate the vision developed in this paradigm. It reveals the thinking matrices of the field of political economy that provide the analytical substrate for its development, allowing us to confront the reductionist use of the concept. It highlights, therefore, the logical foundations that guided the public policies resulting from this approach, with emphasis on the systemic approach and the use of state purchasing power, through the Productive Development Partnerships (PDP), marking the effort to articulate the social and economic realms of development. At present, this analytical perspective is even more crucial, showing that well-being not only fits into GDP but can also be a lever for a pattern of development committed to the National Health Universal System (SUS), society and economic and technological sovereignty in health.

Evidence-informed policies can produce social and economic impacts and equity and health benefits. Interaction between researchers in politics depends on the interests of social stakeholders and favorable political environments. This paper seeks to understand the meanings and researchers' perspectives of interaction processes between scientists and decision-makers that would influence the research impact on the health policy. This is a qualitative content analysis study conducted in 2014 to identify the core meanings and relationships between research and politics. The paper builds on the RAPID program approach of the Overseas Development Institute. Fourteen researchers who conducted maternal and neonatal morbidity and mortality studies sponsored by the Health Ministry were interviewed. Researchers focused on the production of knowledge, strengthening of research capacities and dissemination of results. On some occasions, researchers also participated in the definition of clinical care policies and performance of health services. They pointed to barriers to interact and produce an impact on politics due to tensions in the political, economic and social context, as well as to institutional and organizational changes in the health sector and to the academic evaluation system.

This paper deals with the judicialization of the right to health, a phenomenon that provokes several studies in different perspectives. The Brazilian Supreme Court will judge two appeals (Recursos Extraordinários n. 566.471/RN e n. 657.718/MG) of great importance to guide the rest of the Judiciary branch, what reveals the actuality of the theme. In Brazil, the legal principles, especially because of the contribution offered by Robert Alexy's theory of principles, are being used by courts to decide concrete cases. In the case of right to health, this principle is invoked to provide medicaments or services not included in health's public policies. The balancing used, however, lacks of a formal structure offered by proportionality norm, a legal principle in David Duarte's vision, adopted in this study. This paper suggests how to apply the proportionality test to judge cases of right to health and integral health care, a material principle of the Brazilian Health System, with support in the Norman Daniels' concept of health. It identifies the kind of normative conflict and the correct form to structure the proportionality norm to examine positive duties arising from the right to health, in order to conclude when there will be a violation of this right. This paper highlights the important leadership role of the public health sector, working with other governmental sectors and nongovernmental entities, to advance environmental public health in Latin America and the Caribbean toward the achievement of 2030 Sustainable Development Goal 3: Health and Well-Being. The most pressing current and future environmental public health threats are discussed, followed by a brief review of major historical and current international and regional efforts to address these concerns. The paper concludes with a discussion of three major components of a regional environmental public health agenda that responsible parties can undertake to make significant progress toward ensuring the health and well-being of all people throughout Latin America and the Caribbean.

This paper aims to present an ex-post analysis of industrial and science, technology and innovation (sti) policies to the Health Industrial Complex, between 2003-2017 in Brazil, from the perspective of the public health value. We concluded that the intention to meet social health needs was present in the policies' design, however, the lack of objectivity and clarity in their definitions made it difficult to articulate the three fields of policies. Pharmaceutical services and the formulation of a medicines policy are SUS areas ensured by the organic health care law 8,080/90. Thus, after a widely participative process, involving stakeholders, the National Medicines Policy (NMP) was approved in 1998 by Ordinance 3,916.The NMP presents directives and priorities, aligned with organic health care law, which should guide the federal, states and municipals entities actions to achieve the policy goals. Considering almost 20 years of the NMP, this paper took stock discussed some of the directives in light of the SUS principles. It was not the objective to provide an exhaustive review of all the activities performed during this period. The authors tried to get close to those that have brought advances and dilemmas, with potential risk of regression. Efforts to implement an ambitious agenda applied to pharmaceutical services were identified. This agenda tried to deal with different challenges like the dynamics of the pharmaceutical market and the operation of pharmaceutical services to guarantee the supply of medicines aligned with principles and directives of SUS.

This paper examines how the Union of South American Nations (Unasur) concerned itself with ensuring democracy and participation in the construction of health policies. It aims to explain the meaning of health democracy, and its importance for making the right to health effective. The concern is that the decision on these politics of the bloc should be in harmony with the interests of their citizens, so as to identify ethical activity of the States in international relations. An analysis was made of the documents of constitution, and resolutions on health policy, of this bloc, and statements were found with objectives and principles about democracy and citizen participation; but no institutionalization of mechanisms that might make participative democracy in health operational was found.

Understanding that conditions of health are dependent on more general nationwide factors relating to the level of development means admitting that 'sectorization' of health service is something that tends to limit the formation of knowledge. It also points to the importance of investigating the relationship between health and development, including issues about the processes that feed the persistence of the polarization between modernization and exclusion which has been a history of Brazil's contemporary development. This paper, based on a review of the literature, aims to look more deeply at the structural conditioning factors of the relationship between health and development, and their reproduction in the formation of contemporary knowledge, based on a dialog with the ideas of Celso Furtado. It concludes that a 'virtuous' articulation between health and development calls for policies that are able to reconcile the antagonistic factors represented by the interests of capitalism, and the interests of social wellbeing, in a harmonious co-existence. In Furtado's conception, this would be possible, if the process of social homogenization is correlated with an efficient production system that has a degree of technological autonomy.

Health has become a priority issue on the agenda in relation to Brazilian international technical cooperation in the last decade. This applies to Mozambique which is a strategic partner of Brazil's when one takes into consideration the volume of projects that have been undertaken between the two countries and the available resources on offer. There has been a shift in the foreign policy paradigm which has resulted in a new Brazilian foreign policy posture that promotes bilateral agreements and international cooperation. On conducting a retrospective and current analysis it is clear that there are, however, major obstacles and challenges for bilateral agreements and technical international cooperation that require in-depth study and which need to be dealt with in the area of health. This paper seeks to identify and analyze these obstacles with a view to providing proposals for ways forward. It takes the approach of using the methodology of reviewing specialized journals on this topic as well as using qualitative research from the main actors and institutions involved in bilateral cooperation.

The limits for expenditure on personnel that were imposed by the Fiscal Responsibility Act (FRA) have been considered by local health managers as an obstacle to health sector policies. This paper analyzes the linear trend for the personnel expenses indicators and the correlation of this with the profile of spending on health care personnel in 5,356 Brazilian municipalities from 2004 to 2009. The study of the time series used data from the 'Finanças do Brasil' (Finbra) and data from the Information System on Public Health Budgets (SIOPS). There was a trend towards an increase of 1.3% in the annual average of total personnel expenditure in the municipalities, but the cost of health care staff did not follow that growth. There were no correlations between the indicators, and this result is contrary to the arguments given by the health managers. They attribute the problems with hiring workers and the expansion of health systems to the FRA. The availability of data from the Finbra and the Siops system is associated with a lack of knowledge on these issues. This makes it an opportune time for conducting new research.

This paper discusses part of the results obtained from a study carried out in two cities of the so-called ABCD Paulista Region in the period 2010-2012, in an attempt to spot the existence of non-state regulatory rationale towards access and consumption of health care services. The first stage includes interviews carried out with strategic stakeholders (managers and politicians) and key workers players. The second stage collected the stories of 18 very frequent users of health care services. This study revealed the leading role played by users to produce "health care maps", with emphasis on the frequent use of public and private resources in their itineraries, circumventing or merging with government regulation to obtain the care they need. The different formats of public-private mix transcend the still prevailing "official" concepts about the clear distinction between the two systems, which reveals the importance of this theme to public health management.

This article aims to analyze the extent to which Brazil's public sector has made progress in institutionalizing an integral and participative model for the healthcare of government workers, based on the principles of universality, integrality and workers' participation. Based on documents produced by the Rio de Janeiro Permanent Forum on Workers' Health, the paper analyses the process of implantation of the Integrated Public Workers' Healthcare Sub-System (SIASS) in the municipality of Rio de Janeiro over the period 2009-2016, based on the historical institutionalism. Although it was conceived as an integrated system referenced to the principles of workers' health, as from 2013 an inflexion was observed in this Subsystem, in the direction of a conservative model of occupational health, such as had been traditional and hegemonic in labor relations in Brazil. One factor that emerges is the loss of the universal character of the system due to the flexibility of employment relations in the public sector. There are various challenges: the need to expand the dialogue, and integration of the Policies on health, employment relations and management of the public administration, in such a way as to guarantee the principles of workers' health and the universality of the system, evolving from the concept of the 'public servant' to that of the 'public employee'.

This paper examines the role of the Technical Advisory Committee for antiretroviral therapy of the Brazilian AIDS program in mediating the decision-making process of including new antiretroviral (ARV) drugs in the Unified Health System services by the end of the 2000s. We conducted documental analysis and interviews with key informants from the governmental sphere and professionals. The work features the Technical Advisory Committee as an "expert community", defined as a network of individuals with expertise and competence in a particular sphere and whose knowledge is relevant in critical public policy decision areas. It also indicates that the decision-making process for inclusion of antiretroviral drugs in the Brazilian program was incremental, considering the expectations of the innovative leader companies of pharmaceutical market. The work describes thus the results of the interaction of government interests, pharmaceutical industry and experts in the implementation of a relevant international policy. It provides arguments and evidence for understanding the role of expert communities on a sectorial public policy so far analyzed predominantly from the perspective of social movements.

This paper aims to identify and analyze the political obstacles to the implementation of Organizational Contract of Public Action (COAP) based on the perceptions of municipal health secretaries of Bipartite Interagency Commissions (CIB). For this purpose, we interviewed 195 secretaries (92% of the total) from October 2015 to August 2016. Based on the approach of policy analysis, the main hurdles identified were, in short, a traditional obstacle (lack of resources), one that has been gaining strength in recent years (judicialization of politics) and another, perhaps unheard of: the party-political system and the State Executive Branch are the great absentees in the coalitions in support of SUS regionalization policies. We can conclude that such obstacles indicate an extremely negative setting for the implementation of the COAP and other SUS regionalization policies. Thus, it is incumbent upon those involved to reflect, negotiate and build consensus on improving the health of the population and overcome such obstacles if, of course, they embrace the authors' concept that regionalization is fundamental for the SUS.

This paper analyzes Brazilian health regions according to their service delivery capacity from the debate on the crisis of cooperative federalism in the SUS that resulted from decentralizing process established in the 1988 Constitution. Service delivery capacity tracer indicators were selected by regions and statistical analyses evidenced greater regional capacity in hospital care and large asymmetries with regard to the availability of physicians, high complexity equipment and private insurance coverage. In conclusion,we argue that further solutions are required to strengthen governmental capacity to reduce regional inequalities throughincreased central coordination.

Modern-day slave labor is one of the most pernicious and persistent social problems in Brazil. In the light of the need to implement a national occupational health policy, this paper discusses slave labor as a public health concern, highlighting possibilities for broadening strategies for vigilance and comprehensive care for this specific working population. Exploratory qualitative research was carried out based on the "social construction of reality" proposed by Lenoir, Berger and Luckmann. The investigation consisted of a theoretical review of modern-day slave labor on the national and international scene within the scope of the human, social and public health sciences and an analysis of social and political practices to tackle modern-day slave labor was conducted in the State of Rio de Janeiro. Semi-structured individual and group interviews with workers and representatives of social movements and public institutions were organized. The results reveal the theoretical and practical dimensions of slave labor and its relations with the health field and highlight the role and potential of public health in the enhancing of vigilance practices and health care of workers subjected to these chronic social conditions.

This paper describes the development process of the Pesquisa Nacional sobre Acesso, Utilização e Promoção do Uso Racional de Medicamentos (PNAUM - National Survey on Access, Use and Promotion of Rational Use of Medicines) based on an integrated approach to pharmaceutical services, science, technology and innovation. It starts by contextualizing health and development in Brazil and features elements of the National Policy for Science, Technology and Innovation in Health in Brazil and the National Policy for Pharmaceutical Services. On presenting pharmaceutical policy guidelines, it stresses the lack of nationwide data. This survey, commissioned by the Brazilian Ministry of Health, has two components: household survey and evaluation of pharmaceutical services in primary care. The findings point to perspectives that represent, besides the enhancement of public policy for pharmaceutical services and public health, results of government action aimed at developing the economic and industrial health care complex to improve the health conditions of the Brazilian population. RESUMO O artigo apresenta o processo de construção da Pesquisa Nacional sobre Acesso, Utilização e Promoção do Uso Racional de Medicamento a partir de uma concepção integradora da Assistência Farmacêutica, Ciência, Tecnologia e Inovação. Inicia-se contextualizando a saúde e o desenvolvimento no País e apresenta elementos da Política Nacional de Ciência Tecnologia e Inovação em Saúde no Brasil e da Política Nacional de Assistência Farmacêutica. Ao apresentar as diretrizes das Políticas Farmacêuticas, destaca-se a carência de dados de abrangência nacional. A presente pesquisa, encomendada pelo Ministério da Saúde, foi estruturada em dois componentes: inquérito domiciliar e avaliação dos serviços de assistência farmacêutica na atenção básica. Cardiovascular disease (CVD) is the leading cause of death in the Americas and raised blood pressure accounts for over 50% of CVD. In the Americas over a quarter of adult women and four in ten adult men have hypertension and the diagnosis, treatment and control are suboptimal. In 2021, the World Health Organization (WHO) released an updated guideline for the pharmacological treatment of hypertension in adults. This policy paper highlights the facilitating role of the WHO Global HEARTS initiative and the HEARTS in the Americas initiative to catalyze the implementation of this guideline, provides specific policy advice for implementation, and emphasizes that an overarching strategic approach for hypertension control is needed. The authors urge health advocates and policymakers to prioritize the prevention and control of hypertension to improve the health and wellbeing of their populations and to reduce CVD health disparities within and between populations of the Americas. This paper presents an analysis of the incorporation of the gender perspective in health care-related policies. Based on the recommendations of international organizations, the importance of the incorporation of the gender concept in the health field is analyzed, in order to design health policies that seeking to redress existing inequalities by virtue of sex/gender. This paper presents an analysis of the published guidelines in order to facilitate the incorporation of the gender perspective in health care systems. The article concludes with a proposal of gender-sensitive health indicators that can ensure the gender perspective in health care policies.
