2. Composite fat grafts for correcting facial dystrophy in a case of localized scleroderma

Localized scleroderma (linear morphea) is a rare disease that causes facial dystrophy and asymmetry in young women. We report the case of a 20-year-old female patient who was treated with novel composite fat grafts. After extracting lipoaspirate from deep subcutaneous fat, the microfat and the unfiltered nanofat were processed separately, and amalgamated, obtaining composite fat grafts. These were meticulously transplanted layer by layer to rebuild the subcutaneous tissue of the patient's face. After one year, a good volume of fat retention was observed with successful correction of asymmetry and restoration of facial balance.

Abstract Two siblings presented with clinical and biochemical features of rickets, initially suspected as hypophosphatemic rickets. There was no improvement initially, hence the siblings were reinvestigated and later diagnosed as having vitamin D-dependent rickets (VDDR) type 1 due to a rare mutation in the CYP27B1 gene encoding the 1α-hydroxylase enzyme. Both siblings improved with calcitriol supplementation. The initial presentation of VDDR is often confusing and algorithmic evaluation helps in diagnosis. We also present a brief review of the literature, including genetics.

Abstract Background and objectives: Focused Cardiac Ultrasound (FoCUS) has proven instrumental in guiding anesthesiologists’ clinical decision-making process. Training residents to perform and interpret FoCUS is both feasible and effective. However, the degree of knowledge retention after FoCUS training remains a subject of debate. We sought to provide a description of our 4-week FoCUS curriculum, and to assess the knowledge retention among anesthesia residents at 6 months after FoCUS rotation. Methods: A prospective analysis involving eleven senior anesthesia residents was carried out. At end of FoCUS Rotation (EOR) participants completed a questionnaire (evaluating the number of scans completed and residents’ self-rated knowledge and comfort level with FoCUS), and a multiple-choice FoCUS exam comprised of written- and video-based questions. Six months later, participants completed a follow-up questionnaire and a similar exam. Self-rated knowledge and exam scores were compared at EOR and after 6 months. Spearman correlations were conducted to test the relationship between number of scans completed and exam scores, perceived knowledge and exam scores, and number of scans and perceived knowledge. Results: Mean exam scores (out of 50) were 44.1 at EOR and 43 at the 6-month follow-up. Residents had significantly higher perceived knowledge (out of 10) at EOR (8.0) than at the 6-month follow-up (5.5), p = 0.003. At the EOR, all trainees felt comfortable using FoCUS, and at 6 months 10/11 still felt comfortable. All the trainees had used FoCUS in their clinical practice after EOR, and the most cited reason for not using FoCUS more frequently was the lack of perceived clinical need. A strong and statistically significant (rho = 0.804, p = 0.005) correlation between number of scans completed during the FoCUS rotation and 6-month follow-up perceived knowledge was observed. Conclusion: Four weeks of intensive FoCUS training results in adequate knowledge acquisition and 6-month knowledge retention.

6. Fortified and freeze-dried kiwi fruit (Actinidia deliciosa): quality and sensory assessment

Abstract Kiwi fruit (Actinidia deliciosa) (KF) is one of the best fruits available due to its large amount of nutrients. Despite its many health benefits, there are no previous reports on its preparation in other readily ingestible forms. The objective of the present study was to make a new food product from KF. The KF pulp was fortified and blended with several raw materials (such as rice flour and oat flour) using a stepwise short time addition and mixing methodology since this avoids unwanted biochemical and chemical reactions. The blended and reduced moisture KF paste was freeze-dried on a round silver coated steel plate (RSCSP), supplying the heat of sublimation using a newly designed cubic heater. The freeze-drying (FD) time was 4.5 h and the drying kinetics were studied using four established models. The effective moisture diffusivity (Deff) during FD (at 50 °C) was 1.532 x 10-6 m2/s and the activation energy (E) estimated for the FD was 28.35 kJ/mol. The freeze-dried sample was ground and placed under vacuum to reduce the weathering effects. The quality of the stored product was evaluated using the proximate analysis, physicochemical analysis and a sensory evaluation using a hedonic scale. The raw, fresh KF had a moisture content of 85.07% and the final freeze-dried product one of 3%. The carbohydrate, total sugar, protein, fat, total ash, crude fibre and vitamin C contents of the final product increased by 563%, 400%, 355%, 386%, 672%, 106%, and 117% respectively. Of the 66 panelists, the % consumer acceptances for the different attributes were: sweetness (68.18%), sourness (90.91%), saltiness (100%), bitterness (100%), flavour (95.45%), texture (77.27%) and overall acceptability (81.82%). Using conventional freeze-drying (CFD) for blended KF pulp without fortification, with the same RSCSP and the same cubic heater for sublimation, the drying time was found to be 7 h to reach the same final moisture content of 3%.

India is often interpreted as a country which is still influenced by the values of gerontocracy as well as joint family households. However modernizing trends are affecting the nooks and corners of every part of the society, and India is no left behind. The present paper seeks to explore the domain of physical and mental well being of the elderly population among the lower income households in the country. The findings of the paper show that in lower income households, nuclear families values are gradually penetrating, which is transforming the elderly- family interactions. It is seen that the elderly population which were previously acknowledged as the head of the household or one of the major decision maker are gradually sidelined or excluded from the household affairs. Awareness of being excluded is often a primary reason behind the degrading physical and mental well being of the elderly populace. They are often treated as an unwanted liability, which will be relieved only with their deaths. Case studies of 20 elderly respondents have been taken to explore the physical and mental health of the elderly populace among the lower income households.

It starts of the perspective that is necessary an alternative paradigm of development, clearly enounced, which reinforces the national identity in Latin America, India and the rest of the world in development. Such paradigm should favour the construction of mix and inclusives cultures able to create the sense of solidarity among great masses located in extensive areas with the objective to unblock, free and mobilize resistance against the predatory neoliberalism in the 21st century.

FUNDAMENTO: Recentes pesquisas tem se concentrado no uso de biomarcadores inflamatórios na previsão de risco cardiovascular. Entretanto, a informação é escassa em relação à associação entre esses marcadores inflamatórios com outros fatores de risco cardiovasculares em indianos asiáticos, particularmente em mulheres. OBJETIVO: Explorar a associação entre marcadores inflamatórios tais como proteína C-reativa de alta sensibilidade (PCR-as) e contagem de leucócitos (LEU) e fatores de risco cardiovascular tais como adiposidade geral e central, pressão arterial, variáveis lipídicas e lipoproteicas e glicemia de jejum. MÉTODOS: Conduzimos uma análise transversal de 100 mulheres com idade entre 35-80 anos. As participantes foram selecionadas através da metodologia de amostragem por cluster, de 12 distritos urbanos selecionadas ao acaso na Corporação Municipal de Kolkata, Índia. RESULTADOS: A PCR-as apresentou uma associação significante com o índice de massa corporal (IMC) (p < 0,001) e circunferência da cintura (CC) (p = 0,002). Associações significantes inversas foram observadas entre a lipoproteína de alta densidade colesterol (HDL-c) e ambos marcadores inflamatórios PCR-as (p = 0,031) e LEU (p = 0,014). A apo-lipoproteína A1 (Apo A1) também estava negativamente associada com a PCR-as. A contagem de leucócitos apresentou uma correlação significante com a glicemia de jejum e a razão colesterol total (CT) /HDL-C. Usando regressão logística ajustada para idade, IMC (odds ratio/OR, 1,186; intervalo de confiança/IC, 1,046-1,345; p=0,008) e LEU (OR, 1,045; IC, 1,005-1,087; p=0,027) foram as covariantes significantemente associadas com a PCR-as. CONCLUSÃO: No presente estudo, os fatores de risco tais como IMC, CC e HDL-c e Apo-A1 mostraram uma associação significante com PCR-as. However, information is scant regarding the association between these inflammatory markers with other cardiovascular risk factors in Asian Indians, particularly in women. OBJECTIVE: To explore the association between inflammatory markers such as high-sensitivity C-reactive protein (hs-CRP) and white blood cell (WBC) count and cardiovascular risk factors such as overall and central adiposity, blood pressure, lipid and lipoprotein variables and fasting glucose. METHODS: We conducted a cross-sectional analysis on 100 women aged 35-80 years. Participants were selected following cluster sampling methodology from 12 different randomly selected urban wards of Kolkata Municipal Corporation. RESULTS: Hs-CRP has a significant association with body mass index (BMI) ( p < 0.001) and waist circumference (WC) (p = 0.002). Significant inverse associations were observed between high-density lipoprotein cholesterol (HDL-C) and both inflammatory markers, hs-CRP (p = 0.031) and WBC count, (p = 0.014). Apolipoprotein A1 (Apo A1) was also negatively associated with hs-CRP. WBC count has significant correlation with fasting glucose and total cholesterol (TC) /HDL-C ratio. Using logistic regression, adjusting for age, BMI (odds ratio/OR, 1.186; confidence interval/CI, 1.046-1.345; p=0.008) and WC (OR, 1.045; CI, 1.005-1.087; p=0.027) were the covariates significantly associated with hs-CRP. CONCLUSION: In the present study, risk factors like BMI, WC, and HDL-C and apo A1 show significant association with hs-CRP. WBC count was significantly correlated with HDL-C, fasting glucose, TC/HDL-C ratio in women.

eoenvironmental characteristics of the Peninsular drainage basins in Jharkhand State of India is the result of endogenetic, exogenetic and anthropogenetic mechanisms. As the Peninsular landmass is the oldest land mass in its geomorphic history. Since pre-cambrian times the landmass is experiencing phasesof aggradation and degradation. Recently, the man-induces mechanisms have also been dominating mechanisms in changing the surface morphology of the drainage basins. The role of weathering, mass movement and erosion is significant in reducing the strength of the landmass of such basins resulting environmental degradation in the basin landscape. Geoenvironmental factors are responsible in degradingthe landmass ultimately affects the sustainability of the region. Among the geoenvironmental factors the engineering properties of the landscape like topography, drainage network, soil erosion, rockmass strength are more significant in identifying the terrain stability of the basin which causes the degradation. In this paper, an attempt has been made to analyse the engineering properties of the basin along with the other geoenvironmental factors to find out the status of degradation in the basin with a view to suggest a sound environmental management for the future development of region.

Kinetics of thermal decomposition of Indian magnesite was studied by thermo-gravimetric analysis under non-isothermal condition. Coats and Redfern Integral approximation method was used to determine the kinetic parameters. Using the kinetic parameters different kinetic functions were analyzed with the experimental data to ascertain the decomposition mechanism of magnesium carbonate and it was observed that the decomposition reaction followed a contracting sphere kinetic mechanism.A cinética de decomposição térmica de magnesita indiana foi estudada por análise termogravimétrica sob condição não-isotérmica. O método da aproximação da integral de Coats e Redfern foi usado para determinar os parâmetros cinéticos. Usando estes parâmetros, foram analisadas diferentes funções cinéticas com os dados experimentais para avaliar o mecanismo de decomposição do carbonato de magnésio e foi observado que a reação de decomposição segue o mecanismo cinético de contração de esfera.

A aceleração do processo de mudança na produção de ciência e tecnologia, com a difusão de inovações radicais, tem sugerido a um grupo significativo de autores que estamos numa fase de passagem para uma era de informação e conhecimento, evidente, sobretudo, nos países que comandam e dão o ritmo da economia mundial (Lundvall, 2001, Cassiolato, 1999, Albagli, 1999, OCDE, 1992). Este complexo processo tem suscitado múltiplas interpretações, enfeixadas no termo globalização, que, malgrado a compreensão polissêmica, tem algumas características fortes já estabelecidas: em primeiro lugar, que se trata de uma nova fase de internacionalização do capital, iniciada nos anos 80 e resultante de políticas de liberalização e de desregulamentação do comércio mundial, das relações de trabalho e das finanças, sob a hegemonia do capital financeiro. The brain tissue classification from magnetic resonance images provides valuable insight in neurological research study. A significant number of computational methods have been developed for pixel classification of magnetic resonance brain images. Here, we have shown a comparative study of various machine learning methods for this. The results of the classifiers are evaluated through prediction error analysis and several other performance measures. It is noticed from the results that the Support Vector Machine outperformed other classifiers. The superiority of the results is also established through statistical tests called Friedman test.

In this paper, an ensemble classifier, namely RotaSVM, is proposed that uses recently developed rotational feature selection approach and Support Vector Machine classifier cohesively. The RotaSVM generates the number of predefined outputs of Support Vector Machines. For each Support Vector Machine, the training data is generated by splitting the feature set randomly into S subsets. Subsequently, principal component analysis is used for each subset to create new feature sets and all the principal components are retained to preserve the variability information in the training data. Thereafter, such features are used to train a Support Vector Machine. During the testing phase of RotaSVM, first the rotation specific Support Vector Machines are used to test and then average posterior probability is computed to classify sample data. The effectiveness of the RotaSVM is demonstrated quantitatively by comparing it with other widely used ensemble based classifiers such as Bagging, AdaBoost, MultiBoost and Rotation Forest for 10 real-life data sets. Finally, a statistical test has been conducted to establish the superiority of the result produced by proposed RotaSVM.

The crucial points in machine learning research are that how to develop new classification methods with strong mathematic background and/or to improve the performance of existing methods. Over the past few decades, researches have been working on these issues. Here, we emphasis the second point by improving the performance of well-known supervised classifiers like Naive Bayesian, Decision Tree and k-Nearest Neighbor. For this purpose, recently developed rotational feature selection scheme is used before performing the classification task. It splits the training data set into different number of rotational non-overlapping subsets. Subsequently, principal component analysis is used for each subset and all the principal components are retained to create an informative set that preserve the diversity of the original training data. Thereafter, such informative set is used to train and test the classifiers. Finally, posterior probability is computed to get the classification results. The effectiveness of the rotational feature selection integrated classifiers is demonstrated quantitatively by comparing with aforementioned classifiers for 10 real-life data sets. Finally, statistical test has been conducted to show the superiority of the results.
