2. Perinatal outcomes in a hospital from southern Brazil: Predictors of low APGAR in the first minute of birth

Introduction: The Apgar Index (AI), is a method that assesses the physical conditions of the newborn (NB) in the period immediately after birth. The index proved to be a predictor of neonatal mortality in several clinical and population-based studies and has been used as a health assessment parameter in the immediate neonatal period for over 60 years. Objective: To evaluate obstetric history and perinatal outcomes in newborns (NB) Apgar score equal zero in the first minute of life. Data from births in a public hospital in southern Brazil were evaluated. The studied population was divided into two groups: cases (AI equal zero) and controls (NB with AI ≥8). The cases were composed of 219 NB and the controls of 657 NB. Results: In the multivariate analysis, the maternal variables predictors of AI zero in the first minute of life were: mother’s age >25 years [odds ratio (OR): 1.8; confidence interval (95%CI): 1.1 – 2.8; p = 0.020] and absence of prenatal care (OR: 2.8; 95%CI: 1.5 – 5.5; p = 0.010). Low birth weight (LBW) is defined by a weight lower than 2,500 grams in newborns. It is associated with prematurity and/or restriction of intrauterine growth, being a determining factor for neonatal complications. Thus, the present research evaluated the obstetric and neonatal profile of newborns with low birth weight and their parents attended at the Maternity Santa Casa de Misericórdia de Sobral (SCMS). The research was carried out through a documentary, retrospective, quantitative and descriptive study, conducted in a reference public maternity in the North of the state of Ceará. The research was approved by the ethics committee of the State University of Vale do Acaraú. From this, 1,101 declarations of live births registered in 2015 were evaluated. Of this total, 300 medical records (n=300) of newborns with LBW were included, seeking obstetric and neonatal variables such as maternal age, gestational age at birth, number of prenatal visits and Apgar index. After data collection, the information was aswers in tables for further analysis. Through the interpretation of the results, it was evidenced that the northern region of Ceará has a high rate of newborns with LBW, and may be associated with prematurity, teenage pregnancy and lower than recommended number of prenatal consultations. As a complication, they present low Apgar indices. Therefore, it is necessary government measures of family planning, better prenatal care and greater availability of inums for the prevention and treatment of newborns with LBW. [ABSTRACT FROM AUTHOR]

The objective was to analyze the spatial distribution of pregnancy in children under 14 years and six months by Brazilian region and municipality and sociodemographic and health characteristics of pregnant women and live births. Ecological study analyzing the Live Birth Information System (SINASC) from 2011 to 2021 in three age groups (< 14 years and six months, 15-19 years, and 20 years and above) by demographic and birth variables. We applied the Global and Local Moran. A total of 127,022 live births to girls aged 10-14 years were identified during the period, most of whom were Black, 21.1% in common-law or married relationships, with a lower proportion of seven prenatal care appointments and enrollment in the first trimester, a higher proportion of low birth weight and low Apgar score, residing in the North and Northeast. The mean live birth rate for 10-to-14-year-old girls was significantly autocorrelated with space, especially in municipalities of the Midwest and North. Pregnancy from 10 to 14 years of age reveals several vulnerabilities suffered by these girls due to pregnancy at an early age, which is more common among Black women, with implications for morbimortality for them and their children and the presumed violence in these cases, including denied access to legal abortion.

Abstract Objective To compare the effects of expectant versus interventionist care in the management of pregnant women with severe preeclampsia remote from term. Data sources An electronic search was conducted in the Medical Literature Analysis and Retrieval System Online (MEDLINE), Excerpta Medica Database (EMBASE), Cochrane Central Register of Controlled Trials (CENTRAL), Latin American and Caribbean Health Sciences Literature (LILACS, for its Spanish acronym), World Health Organization’s International Clinical Trials Registry Platform (WHO-ICTRP), and Open- Grey databases. The International Federation of Gynecology and Obstetrics (FIGO, for its French acronym), Royal College of Obstetricians and Gynaecologists (RCOG), American College of Obstetricians and Gynecologists (ACOG), and Colombian Journal of Obstetrics and Gynecology (CJOG) websites were searched for conference proceedings, without language restrictions, up to March 25, 2020. Selection of studies Randomized clinical trials (RCTs), and non-randomized controlled studies (NRSs) were included. The Grading of Recommendations, Assessment, Development and Evaluation (GRADE) approach was used to evaluate the quality of the evidence. Data collection Studies were independently assessed for inclusion criteria, data extraction, and risk of bias. Disagreements were resolved by consensus. Data synthesis Four RCTs and six NRS were included. Objective: to evaluate the factors associated with low Apgar in newborns in Angola. Method: analytical and cross-sectional, quali-quantitative study, in a General Hospital in Luanda, Angola, between March and May 2021. Interviews were carried out in an observation room and consultation of the parturients' medical records. Data were analyzed using the chi-square test (X2) and logistic regression. Results: pregnant women aged 19-35 years (70.0%), 7th-9th year (57.5%), multiparous (75%), multiparous (75.0%), without a history of abortion (72.5%), prevailed. gestational age of 40 weeks (60.0%), users of alcoholic beverages (77.5%), low frequency of prenatal consultations (52.5%), with natural childbirth (82.5%) and newborns with normal weight (52.5%). Women from peri-urban regions [OR:6.85 (95% CI:0.65-71.2), p=0.108] and rural [OR:4.47 (95% CI:0.47-48.4), p=0.184] were more likely to have low Apgar newborns, as well as alcohol users [OR:3.28 (95% CI:0.58-18.3), p=0.176] and newborns who were born with normal [OR:1.75 (95% CI:0.49-6.22), p=0.387]. There was no statistical association between sociodemographic and maternal data. Conclusion: age group, place of residence, maternal alcohol consumption and newborn weight may imply low Apgar. ABSTRACT Objective: to analyze factors associated with Apgar of 5 minutes less than 7 of newborns of women selected for care at the Center for Normal Birth (ANC). Method: a descriptive cross-sectional study with data from 9,135 newborns collected between July 2001 and December 2012. The analysis used absolute and relative frequency frequencies and bivariate analysis using Pearson’s chi-square test or the exact Fisher. Results: fifty-three newborns (0.6%) had Apgar less than 7 in the 5th minute. The multivariate analysis found a positive association between low Apgar and gestational age less than 37 weeks, gestational pathologies and intercurrences in labor. The presence of the companion was a protective factor. Conclusion: the Normal Birth Center is a viable option for newborns of low risk women as long as the protocol for screening low-risk women is followed.

Objective: To know the profile of newborns hospitalized in the Intermediate Neonatal Care Unit and associate maternal and neonatal factors with the length of hospitalization. Methods: Cross-sectional, documentary research, carried out in a university hospital in the South Region of Brazil, with 384 newborns hospitalized. A descriptive analysis and the chi-square test were performed. Results: Most hospitalizations occurred in the early neonatal period, of male babies, white ethnicity, gestational age 36 weeks, weight 2,690g, Apgar score 6.77 in the first and 8.49 in the fifth minute, and average days of hospitalization 8.49. Prolonged period of hospitalization was associated with maternal intercurrence, prematurity, low birth weight, male gender, use of antibiotics and weight gain. Conclusion: Reaffirms the importance of knowing the causes of hospitalization of newborns and understanding the profile of illness in the neonatal period, to subsidize public policies and quality assistance to reduce hospitalization and perinatal morbidities. 13. Apgar Scoring System in Brazil’s Live Births Records: Differences between Home and Hospital Births

Abstract Objective To promote informed choice for women and to compare home andhospital births in relation to the Apgar score. Methods Mother’s profile and Apgar score of naturally born infants (without forceps assistance) in Brazil between 2011 and 2015, in both settings-hospital or home-were collected from live birth records provided by the Informatics Department of the Unified Health System (DATASUS, in the Portuguese acronym). For the analysis, were included only data fromlow-riskdeliveries, including gestational time between 37 and 41weeks, singleton pregnancy, at least four visits of prenatal care, infants weighing between 2,500 g, and 4,000 g, mother age between 20-40 years old, and absence of congenital anomalies. Results Home birth infants presented significantly higher risk of 0-5 Apgar scores, both in 1 minute (6.4% versus 3%, odds ratio [OR] = 2.2, confidence interval [CI] IC 2-2.4) and in 5 minutes (4.8% versus0.4%,OR = 11.5,CI 10.5-12.7). Another findingis related to recovery estimateswhen from an initially bad 1-minute Apgar ( 6). In this scenario, home infants had poorer recovery, Apgar scorewas persistently < 6 throughout the fifth minute in most cases (71% versus 10.7%, OR 20.4, CI 17-24.6). Conclusion The results show worse Apgar scores for babies born at home, compared with those born at the hospital setting. This is a pioneer and preliminary study that brings attention concerning differences in Apgar score related to home versus hospital place of birth in Brazil.

14. The effects of crystalloid warming on maternal body temperature and fetal outcomes: a randomized controlled trial

Abstract Background and objectives: Hypothermia occurs in about 60% of patients under anesthesia and is generally not managed properly during short lasting surgical procedures. Hypothermia is associated with adverse clinical outcomes. The current study is designed to assess the effects of crystalloid warming on maternal and fetal outcomes in patients undergoing elective cesarean section with spinal anesthesia. Methods: In this prospective randomized controlled trial, sixty parturients scheduled for elective cesarean section with spinal anesthesia were randomly allocated to receive crystalloid at room temperature or warmed at 37 °C. Spinal anesthesia was performed at L3-L4 interspace with 10 mg of hyperbaric bupivacaine without adding opioids. Core temperature, shivering, and hemodynamic parameters were measured every minute until 10th minute and 5-min intervals until the end of operation. The primary outcome was maternal core temperature at the end of cesarean section. Results: There was no difference for baseline tympanic temperature measurements but the difference was significant at the end of the operation (p = 0.004). Core temperature was 36.8 ± 0.5 °C at baseline and decreased to 36.3 ± 0.5 °C for isothermic warmed crystalloid group and baseline tympanic core temperature was 36.9 ± 0.4 °C and decreased to 35.8 ± 0.7 °C for room temperature group at the end of the operation. Shivering was observed in 43.3% in the control group. Hemodynamic parameter changes and demographic data were not significant between groups. Conclusions: Isothermic warming crystalloid prevents the decrease in core temperature during cesarean section with spinal anesthesia in full-term parturients. Fetal Apgar scores at first and fifth minute are higher with isothermic warming.

Objective: Find out the evidences about the effect of the use of cocaine and crack during the privacy for the newborn. Methods: It is a integrative literature review, based on the Medical Literature on Line - MEDLINE (by PubMed), Latina American literature and Caribbean in Heath Science- LILACS (at Virtual lLibrary in Heath) and the Nursing dates (BDENF). For search strategy, the following descriptors (DeCS) in English and Portuguese were used: Pregnancy, Complications, Fetus, Street, Drugs, Infant, Newborn. Original articles available in full, published between 2014 and 2019 were included. Results: A total of 9 articles were included in this study. The main effects of crack or cocaine use during the pregnancy for the newborn were low APGAR, delayed neuropsychomotor, low birth weight, changes in primitive reflexes, congenital malformation and microcephaly. Conclusions: Nine articles were found that comprised the final sample of this study. This number reaffirm the need to carry out original studies on the biological effects of crack and cocaine use during pregnancy for the newborn. [ABSTRACT FROM AUTHOR]

This article presents a literature review on the epidemiological profile of newborns with congenital heart disease in Brazil. Eight articles published between 2007 and 2018 were selected, which addressed factors related to maternal characteristics, clinical characteristics of newborns, types of treatments performed, and main outcomes in seven Brazilian states. The main factors related to congenital heart disease in Brazilian newborns were maternal multiparity, prematurity, low APGAR score, low birth weight, length of hospital stay, and risk of mortality. Resumo Apesar da variação entre observadores na avaliação do escore de Apgar, ele permanece um indicador útil das condições gerais do recém-nascido. O presente trabalho é um estudo de corte transversal baseado na população de nascidos vivos no Brasil no ano de 1999 e no biênio 2018-2019. Foram avaliadas todas as declarações de nascidos vivos (DNV) obtidas a partir do banco de dados do Sistema de Informações sobre Nascidos Vivos. As frequências foram comparadas entre os grupos por meio do teste qui-quadrado de Pearson e foi realizada análise de regressão logística multivariada. Adotou-se nível de significância estatística de 0,05. Foram analisadas 9.050.521 DNVs em nossa pesquisa. Constatamos que 2,1% dos recém-nascidos tiveram Apgar de 5º minuto < 7 em 1999, em comparação com 0,9% em 2018-2019. A análise multivariada indicou que gemelidade e gravidez na adolescência deixaram de ser fatores de risco para Apgar de 5º minuto < 7. Entre os fatores de risco, nota-se aumento da prematuridade, baixo peso ao nascer e anomalias congênitas. Observou-se melhoria de marcadores maternos, em especial o aumento do número de consultas pré-natais e escolaridade. Tais achados mostram a importância de acesso e seguimento pré-natal adequado e investimento em melhores condições socioeconômicas como estratégia eficaz para redução de morbimortalidade neonatal. Abstract Although variation between observers in the assessment of the Apgar score, it remains a useful indicator of the general conditions of the newborn. This is a cross-sectional study based on population of live births in Brazil in 1999 and biennium 2018-2019. All declarations of live births (DNV) obtained from the Live Births System database were accessed. Frequencies were compared between groups using Pearson’s chi-square test and multivariate logistic regression analysis was performed. A statistical significance level of 0.05 was considered. We included 9.050.521 DNVs in our research. We found that 2,1% of newborns had 5th minute Apgar < 7 in 1999 compared with 0,9% in 2018-2019. Multivariate analysis shows that twins and teenage pregnancy are no longer risk factors. Among risk factors, we observed an increase in prematurity, low birth weight and congenital anomalies. An improvement in maternal markers was observed, especially increase in the number of prenatal consultations and schooling. Such findings demonstrate the importance access and adequate prenatal care and improved socioeconomic conditions as effective strategy to reduce neonatal morbidity and mortality.

Dissertação de Mestrado Integrado em Medicina Veterinária, na área científica de Clínica Na prática clínica dos centros de atendimento médico-veterinários (CAMVs) é frequente o aparecimento de cadelas com distócia ou com historial de distócia, constituindo uma situação que pode colocar tanto os cachorros como a cadela em risco de vida, representado um grande desafio para o bem-estar animal, especialmente no contexto da canicultura. Na maioria das vezes, a abordagem a esta situação passa pela cesariana. Este estudo teve por objetivo a caracterização de uma amostra de cadelas submetidas a cesariana eletiva e de urgência e dos respetivos neonatos, bem como a avaliação da contribuição de fatores maternos, fetais e relativos à duração anestésica e cirúrgica no índice de Apgar neonatal. Neste estudo, foram incluídas as 19 cadelas submetidas a cesariana (8/19 submetidas ao procedimento de urgência e 11/19 submetidas ao procedimento eletivo) que se apresentaram ao CAMV entre 24 de dezembro de 2021 e 28 de fevereiro de 2022 e os respetivos 103 neonatos. A maioria das cesarianas realizadas foram eletivas, sendo que o Bouledogue Francês foi a principal raça submetida a cesariana (36,8%), seguida do Retriever do Labrador (21,1%), Spitz Alemão Anão (15,8%), Chihuahua (10,5%) e em último lugar, com a mesma percentagem, encontram-se o Bichon Maltês, Pinscher Miniatura e Pug (5,3%). As raças pequenas (5-9,9 kg), médias (10-24,9 kg) e grandes (25-40 kg) constituíram 42%, 37% e 21% dos casos apresentados, respetivamente. A idade média das cadelas foi de 3,26 ± 1,52 anos. Do número total de cadelas sujeitas à intervenção cirúrgica, 63,2% não apresentaram história de cesariana prévia. A sobrevivência materna correspondeu a 100%. Da totalidade dos neonatos nascidos por cesariana, apenas 1,96% constituíram nados-mortos e 2,94% apresentaram malformações congénitas. A sobrevivência neonatal correspondeu a 98%. A mortalidade perinatal correspondeu a 11,22%. O índice de Apgar médio, avaliado em 96 neonatos, apresentou um valor de 7,96 ± 1,321. O intervalo de tempo entre o início de cirurgia e extração do último feto, o tempo de cirurgia e o tempo de anestesia fetal apresentaram influência no índice de Apgar, sendo que o aumento dos referidos intervalos resultou em índices de Apgar superiores. Também a raça e o sexo constituíram variáveis que influenciaram significativamente o índice de Apgar, sendo o Retriever do Labrador a raça com a melhor classificação de índice de Apgar e o Bichon Maltês a raça com pior classificação. Os machos apresentaram piores índices de Apgar. ABSTRACT - Characterization of emergency and elective cesarean sections in the canine species - In clinical practice it is relatively common the appearance of bitches with dystocia, which is a life-threatening situation, not only for the bitch but for the fetus too. The dystocia constitutes a big challenge for animal wellfare and in the majority of times, the clinical aproach is the cesarean section. The aim of this study was the characterization of a sample of bitches who underwent urgency and elective cesarian section as well as the respective neonates. The contribution of maternal and fetal factors as well as factors associated with anesthetic and surgical duration on neonatal Apgar score were also evaluated. The sample of this study included 19 bitches who underwent cesarean section (8/19 underwent the urgency procedure and 11/19 underwent the elective procedure) as well as the 103 neonates born by it between the 24th of December of 2021 and the 28th of February of 2022. The majority of cesarean sections perfomed were elective. The French Buldogue was the main breed submitted to cesarean section (36,8%), followed by the Labrador Retriever (21,1%), German Spitz (15,8%), Chihuahua (10,5%) and lastly, with the same percentage, the Maltese Bichon, Pinscher and Pug (5,3%). Small breed (5-9,9 kg), medium breed (10-24,9 kg) and large breed (25-40 kg) made 42%, 37% and 21% of the cases, respectively. The average age was 3,26 ± 1,52 years. Of the total number of bitches submitted to the surgical intervention, 63,2% didn’t have a history of cesarean section before and 36,8% had the surgery before. The maternal survival was 100%. Only 1,96% of the neonates were stillborn and 2,94% had congenital malformations. Neonatal survival was 98%. Perinatal mortality was 11,22%. The Apgar score average was 7,96 ± 1,321. The time between beginning of the surgery and extraction of the last fetus, the time of surgery and the time of anesthesia were variables that influenced the neonatal Apgar score. The increase of the referred times resulted in better Apgar scores. The breed and the sex were variables that influenced the neonatal Apgar score. The Labrador Retriever was the breed with better Apgar scores and the Maltese Bichon the breed with worse Apgar scores. Males presented worse Apgar scores than females. N/A

Objective: to describe the maternal and neonatal outcomes of the care provided at the In-hospital birth center. Method: an exploratory, descriptive study with quantitative approach where the analysis of 378 (74,85%) birth assisted medical records was performed from September 2014 to September 2016. Results: the majority of parturients (50,79%) was young, between 18 and 25 years old and multiparas (74,86%). The result obtained was an episiotomy rate of 8,46% and 52,37% of perineal laceration, predominating the first degree lacerations. The majority of newborns remains in skin-to-kin (86,60%) and was made the late clamping umbilical cord (77,78%). The average of Apgar's rate was 8,72 in the first minute and 9,76 in the fifth. Conclusion: The results meet the guidelines adopted by the Ministry of Health and the World Health Organization. [ABSTRACT FROM AUTHOR]

The article entitled "First minutes of life: registering the Apgar Score in the Child Health Booklet for the follow-up of child neuropsychomotor development" discusses the importance of registering the Apgar Score in the Child Health Booklet for the monitoring of child neuropsychomotor development. The study was conducted in health units and a reference center in Santarém, in the Amazon, and involved the participation of 222 parents. The results showed that 52.5% of the participants did not have the Apgar Score registered in the Booklet, which limits the understanding of the newborn's initial conditions. O Brasil é o primeiro mercado mundial consumidor de agrotóxicos. Evidências epidemiológicas apontam associação entre exposição materna a agrotóxicos e eventos adversos na gravidez. Realizou-se um estudo ecológico para investigar possíveis associações entre o consumo per capita de agrotóxicos por microrregiões e eventos adversos em nascidos vivos na Região Sul do Brasil no período 1996-2000. Dados foram obtidos junto ao Instituto Brasileiro de Geografia e Estatística (IBGE) e ao Departamento de Informática do SUS (DATASUS). Microrregiões foram agrupadas segundo quartis de consumo de agrotóxicos e foram calculadas razões de prevalência (RP). O teste qui-quadrado foi usado para calcular o valor de p de tendência lineal. Nas microrregiões de maior consumo de agrotóxicos nascimentos prematuros (< 22 semanas) e de índice de Apgar 1º e 5º minuto insatisfatório (< 8) apresentaram RP significativamente maiores, tanto em meninos como em meninas. Não foi observado um padrão similar em relação a baixo peso ao nascer. Estes achados sugerem a exposição intraútero a agrotóxicos como possível fator de risco para eventos adversos na gravidez, como parto prematuro e maturação inadequada.Brazil is the world's largest consumer of pesticides. Epidemiological studies have shown an association between maternal exposure to pesticides and adverse pregnancy events. An ecological study was conducted to investigate potential relations between per capita pesticide consumption and adverse events in live born infants in micro-regions in the South of Brazil (1996-2000). The data were obtained from the Brazilian Institute of Geography and Statistics (IBGE) and the Health Information Department of the Unified National Health System (DATASUS). Micro-regions were grouped into quartiles of pesticide consumption, and prevalence ratios (PR) were calculated. Linear trend p-values were obtained with the chi-square test. Premature birth (gestational age < 22 weeks) and low 1 and 5-minute Apgar score (< 8) in both boys and girls showed a significantly higher PR in the upper quartile of pesticide consumption. No significant differences were observed for low birth weight. The findings suggest that prenatal pesticide exposure is a risk factor for adverse pregnancy events such as premature birth and inadequate maturation.

Este trabalho analisa a vitalidade do recém-nascido por tipo de parto para os nascidos vivos do Estado de São Paulo, Brasil, em 2003. Com base nos dados das declarações de nascimento processadas pela Fundação Sistema Estadual de Análise de Dados (Fundação SEADE) e na regressão logística, foram analisadas as odds ratio (OR) para nascidos vivos com baixo índice de Apgar (0 a 6) no quinto minuto de vida por tipo de parto, controlados os fatores de ordem obstétrica, demográfica e social. Tanto na regressão simples, quanto na regressão múltipla, o peso da criança ao nascer; duração da gestação; número de consultas pré-natal; idade, estado civil e anos de estudo da mãe resultaram ser todos significativos e, conforme esperado, todas as categorias de exposição apresentaram-se diretamente relacionadas à baixa vitalidade do recém-nascido. Na regressão logística simples para baixa vitalidade do recém-nascido, o parto cesáreo, em relação ao parto vaginal, registrou OR de 0,890 (IC95%: 0,836-0,948). Entretanto, na regressão logística múltipla, o parto cesáreo registrou OR de 1,045 (IC95%: 0,977-1,117). Portanto, a variável tipo de parto não resultou ser estatisticamente significativa para baixa vitalidade do recém-nascido.This paper analyzes Apgar score associated with mode of delivery for live births in São Paulo State, Brazil, 2003. Based on data from the State Data Analysis System Foundation (SEADE), logistic regression was analyzed for live births with low Apgar score (0 to 6) at the fifth minute of life per mode of delivery, adjusted for obstetric, demographic, and social factors. Birth weight, gestational age at delivery, number of prenatal visits, maternal age, marital status, and years of schooling were statistically significant, and (as expected) all exposure categories were associated with low Apgar score. In the unadjusted model, the odds ratio (OR) for low Apgar was 0.890 (95%CI: 0.836-0.948) for cesarean section compared to vaginal delivery. Nevertheless, in the adjusted model the OR was 1.045 (95%CI: 0.977-1.117) for cesarean section compared to vaginal delivery. Adjusted for obstetric, demographic, and social factors, mode of delivery was not statistically significant for low Apgar.

OBJECTIVES: to describe hospital lethality rates and factors correlated to death in neonates with brain white matter lesions. METHODS: a retrospective study was performed from January 1994 to December 2001. Neonates with white brain matter lesions were divided into survival and death groups and their medical files reviewed through the single blind method to determine evolution. Death certificates provided the cause of death. The groups were compared through correlation coefficients. Hospital lethality rate was calculated. RESULTS: ninety three cases of white brain matter lesions and seven deaths were determined. Hospital lethality rate was of 8.2.% (95%CI: 2.4-14.0) independently from lesion occurrence time, and of 10.3% (95%CI: 3.3-17.3) for deaths occurred during prenatal and perinatal periods. Death was correlated to: Apgar score, non-cephalic presentation, gestational age, hyperglicemia, hypercalcemia, convulsion, respiratory insufficiency and atelectasy. CONCLUSIONS: hospital lethality was of 10.3% generating the following hypothesis: perinatal asphyxia must be the principal direct and indirect etiologic factor (aggravating the expression of prematurity and infection diseases), of prenatal and perinatal mortality among newborns with white brain matter lesions; and OBJETIVOS: descrever a taxa de letalidade hospitalar e fatores correlacionados com o óbito em crianças com lesão da substância branca cerebral (LSB). MÉTODOS: estudo retrospectivo realizado de janeiro de 1994 a dezembro de 2001. Os neonatos com LSB foram divididos em sobreviventes ou óbito, e seus prontuários revisados de forma cega para a evolução. Dos atestados de óbito, a causa de morte. Os grupos foram comparados por coeficientes de correlação. Calculada a taxa de letalidade hospitalar. RESULTADOS: foram encontrados 93 casos de LSB e sete óbitos. A taxa de letalidade hospitalar foi de 8,2%, (IC95%: 2,4-14,0), independentemente da época de instalação da lesão, e de 10,3% (IC95%: 3,3-17,3) para aqueles de ocorrência pré/perinatal. O óbito correlacionou-se com: escore de Apgar, apresentação não-cefálica, idade gestacional, hiperglicemia, hipercalcemia, convulsão, insuficiência respiratória e atelectasia. CONCLUSÕES: a letalidade hospitalar foi de 10,3% e as seguintes hipóteses foram geradas: a asfixia perinatal deve ser o principal fator etiológico, direto e indireto (agravando a expressão das doenças da prematuridade e da infecção), da mortalidade pré/perinatal entre neonatos com LSB; e o escore de Apgar do 5o minuto

O índice de Apgar menor que sete foi o parâmetro utilizado para se diagnosticar hipóxia fetal. RESULTADOS: foram encontradas alterações placentárias compatíveis com hipertensão e infiltrado inflamatório. As placentas com alterações compatíveis com baixo fluxo sanguíneo cursaram mais freqüentemente com fetos com índice de Apgar OBJECTIVES: to describe possible placental anatomopathological alterations associated to fetal hypoxia as evaluated by the Apgar score. METHODS: one hundred sixty seven placentas of children delivered at the Hospital Escola da Universidade Federal do Triângulo Mineiro, Uberaba city were analyzed both macro and microscopically; their clinical records were reviewed as well. Apgar Scores below 7 were considered consistent with fetal hypoxia. RESULTS: placental alterations compatible with inflammatory infiltrate and arterial hypertension were found. The latter was more often diagnosed in fetuses with 5 minutes Apgar scores below 7 (p=0.017). CONCLUSIONS: there may be a relationship between placental alterations and fetal hypoxia as diagnosed by the Apgar score. Therefore, the exam of the placenta by the pathologist could be a mean to elucidate a plausible cause of clinically undetected perinatal hypoxia.

In view of social and lifestyle changes, the incidence of advanced maternal age pregnancy, i.e. in women over 35 years, has markedly increased in recent decades. Such surge contributed to higher rates of obstetric and fetal complications. A critical review of the literature was conducted with the objective of assessing the available evidence on the frequency of the main pregnancy complications in advanced maternal age as well as possible explanations and controversies about the matter. A detailed search of medical literature was performed including the databases: Medline (via PubMed), SciELO and LILACS. Gestational and perinatal outcomes assessed were: gestational age at delivery, low birth weight (LBW < 2.5 kg), Apgar score < 7 at 1 and 5 minutes, increase in the incidence of cesarean sections and stillbirth. The results retrieved were: gestational age (< 37 weeks = 3.4 to 51.4%; > 42 weeks = 0.4 to 6.9%), stillbirth (0 to 3.2%), LBW (0 to 26.8%), Apgar score < 7 (1st min = 5.6 to 22.9; 5th min = 0.9 to 5%), cesarean section (31.9 to 72.4%). The relative impact of maternal ageing and chronic diseases on pregnancy outcomes requires further research. Health professionals must be alert to the peculiarities of advanced maternal age pregnancy in order to timely identify potential complications. A specialized multiprofessional team should be available to warrant specific needs regarding exams and admission. Thus, advanced maternal age pregnancy can be more safely managed, reducing the rate of complications and maternal and perinatal adverse outcomes. [ABSTRACT FROM AUTHOR]

The aim of the study was to evaluate the influence of glucocorticoids on vital parameters of full-term and preterm lambs from birth to 48 hours of life. Four experimental groups were formed: NDG (normal delivery group - lambs vaginally delivered, n=15, average of 146-day gestation); NDEXG (normal delivery with dexamethasone group - lambs vaginally delivered whose mothers received 16mg of dexamethasone at 141 days of gestation, n=8, average of 143-day gestation); PRE (premature lambs born by cesarean section at 138 days of gestation, n=10) and PREDEX (premature lambs born by cesarean section at 138 days gestation, whose mothers received 16mg of dexamethasone two days before, n=9). Heart and respiratory rates had variations during the observation period, with the highest mean values in the groups of normal deliveries (NDG and NDEXG). Rectal temperature decreased in all groups in the fi rst 60 minutes of life, with the lowest mean values in premature lambs (PRE and PREDEX) and the Apgar score was higher in animals delivered at normal gestational time. Preterm lambs had lower vitality and chance of survival, however, lower mortality rate was observed in offspring of ewes that received dexamethasone two days before surgery. [ABSTRACT FROM AUTHOR]

OBJECTIVE: To analyze the influence of maternal, obstetrical and neonatal factors on the outcome of the evaluation of the Apgar Index of newborns at the Family Health Center in the neighborhood of Dom Expedito Lopes in the city of Sobral/CE. METHODS: It is a retrospective, quantitative and descriptive study, having as a data source the charts of pregnant women attended in the years 2016 and 2017. The variables analyzed were those that allowed to correlate the Apgar Index with maternal, obstetric and neonatal factors: maternal age, type of delivery, number of prenatal consultations, gestational age and birth weight. The present work was submitted to the local ethics committee and approved with the number of opinion 1.878.614, it remained anonymous and followed the recommendations of the Ordinance of the National Health Council/MS - CNS, Resolution 466/12, adopting the four basic principles of bioethics: autonomy, beneficence, non-maleficence and justice. RESULTS: In the majority of pregnant women, the percentage of the age group was from 21 to 30 years old (56.25%) with a satisfactory Apgar score, that is, above seven. Regarding the type of delivery, it can be seen that the Cesário delivery was well present with 62.5% with a score of 7-10. Regarding the number of prenatal consultations, the majority performed 07 or more consultations (82.81%) with an Apgar score above 7. For gestational age the majority were between 37 and 41 weeks, being considered at term and that they obtained the weight at birth greater than or equal to 2,500g, presenting Apgar above 7. CONCLUSION: These findings make it possible to visualize the importance and adherence of pregnant women to a quality prenatal follow-up, resulting in a reduction in neonatal mortality and a satisfactory Apgar score. OBJETIVO: Analisar a influência dos fatores Maternos, obstétricos e neonatais no resultado da avaliação do Índice de Apgar de neonatos no Centro de Saúde da Família do bairro Dom Expedito Lopes na cidade de Sobral/CE. MÉTODOS: Trata-se de uma pesquisa retrospectiva, quantitativa e descritiva, tendo como fonte de dados os prontuários de gestantes atendidas nos anos de 2016 e 2017. As variáveis analisadas foram aquelas que permitissem correlacionar o Índice de Apgar com os fatores maternos, obstétricos e neonatais: idade materna, tipo de parto, número de consultas pré-natal, idade gestacional e peso ao nascer. O presente trabalho foi submetido ao comitê de ética local tendo sido aprovado com número de parecer 1.878.614, manteve o anonimato e seguiu as recomendações da Portaria do Conselho Nacional de Saúde/MS – CNS, Resolução 466/12, adotando os quatro princípios básicos da bioética: autonomia, beneficência, não maleficência e justiça. RESULTADOS: Na maioria das gestantes o percentual da faixa etária foi dos 21 aos 30 anos (56,25%) com índice de Apgar satisfatório, ou seja, acima de seteEm relação ao tipo de parto pode-se constatar que o parto Cesário esteve bem presente com 62,5% com escore de 7-10. Sobre o número de consultas pré-natal a maioria realizou 07 ou mais consultas (82,81%) com escore Apgar acima de 7. Para a Idade gestacional a maioria esteve entre 37 a 41 semanas, sendo considerada a termo e que obtiveram o peso ao nascer maior ou igual a 2.500 g , apresentando Apgar acima de 7. CONCLUSÃO: Estes achados possibilitam visualizar a importância e a adesão das gestantes à um acompanhamento de pré-natal de qualidade, resultando numa redução na mortalidade neonatal e um índice de Apgar satisfatório.

Introdução: O período neonatal, compreendido entre 0 e 28 dias de vida, é responsável por grande parcela da mortalidade infantil no Brasil, a qual, apesar de ter apresentado redução, ainda se mantém elevada. Nessa fase, pode-se relacionar alguns fatores à maior prevalência de óbito, como características socioeconômicas, enfatizando as desigualdades sociais, fatores associados à mãe (como morbidades durante a gestação e realização ou não de pré-natal adequado), além de fatores associados ao recém-nascido, como peso ao nascer, idade gestacional, boletim de APGAR e ocorrência de infecção grave. Objetivo: O presente trabalho teve por objetivo encontrar a prevalência da mortalidade neonatal no Estado da Paraíba, no período de 2011 a 2015, e os fatores a ela associados. Método: Trata-se de um estudo observacional e transversal, cujos dados foram levantados por meio de análise das informações provenientes de bases de dados de registro contínuo, contidas no DATA SUS, oriundas do Sistema de Informações sobre Mortalidade (SIM), bem como de dados do Sistema de Informações sobre Nascidos Vivos (SINASC), além da avaliação das Declarações de Óbito (DO) e Declaração de Nascidos Vivos (DNV). Resultados: A prevalência da mortalidade neonatal no período estudado foi de 9,6 óbitos por mil nascidos vivos, sendo 74,5% deles no período neonatal precoce (de 0 a 6 dias de vida) e 25,5% no período neonatal tardio (7 a 28 dias), a maioria foi do sexo masculino, as mães tinham entre 20 e 29 anos, pardas, solteiras, com nível de escolaridade de ensino médio e começaram o pré-natal no segundo mês de gestação, porém houve divergência entre os dois grupos (precoce e tardio) em relação à via de parto, idade gestacional e boletim de APGAR (no 1º e 5º minutos de vida). Conclusão: Após esse estudo inicial, deve-se considerar a necessidade de adoção de políticas públicas eficazes, no intuito de melhorar a atenção ao RN, desde o pré-natal à capacitação de profissionais e assistência hospitalar, visando à redução da prevalência da mortalidade neonatal, e, com isso, também a redução da mortalidade infantil. Introduction: The neonatal period, between 0 and 28 days of life, is responsible for a large proportion of infant mortality in Brazil, that even presenting reduction, is still high. In this phase some factors can be related to the greater prevalence of death, such as socioeconomic characteristics, emphasizing the social inequalities, some factors associated with the mother, such as morbidities in gestation and whether or not adequate prenatal care is performed, in addition to factors associated with the newborn, such as birth weight, gestational age, APGAR score, and occurrence of severe infection. Objective: The objective of this study was to find the prevalence of neonatal mortality in the State of Paraiba, in the period from 2011 to 2015 and the factors associated. Method: This is an observational and cross-sectional study whose data were collected through the analysis of information from records, contained in the DATASUS, originated from the Information System on Mortality (SIM), as well as data from the Birth Information System (SINASC), in addition to the evaluation of the Death Certificates (DO) and the Declaration of Live Births (DNV). Results: The prevalence of neonatal mortality in the studied period was 9.6 deaths per thousand live births, 74,5% of them in the early neonatal period (from 0 to 6 days of life) and 25,5% in the late neonatal period (7 to 28 days of life), the majority were male, which mothers were between 20 and 29 years old, brown, single, graduated from high school and started prenatal care at the second month of gestation, however there was divergence between the two groups (early and late neonatal period) in relation to the type of delivery, gestational age and APGAR score (in the 1st and 5th minutes of life). Conclusion: After this initial study, consideration should be given to the need to adopt effective public policies in order to improve care for the newborn, from prenatal care to professional training and hospital care, aiming at reducing the prevalence of neonatal mortality, and with this, reducing child mortality.

This is a study of quantitative nature with the aim of establishing the relationship between maternal position in the second period of labor and the condition of the newborn, infant as assessed by Apgar index. A descriptive cross-sectional study was designed, with an analysis of 8,538 live-birth deliveries occurring between 1996 and 2005 at a maternity school in southern Brazil. The analysis included data on all vaginal deliveries with the woman in a vertical (squatting) or horizontal (supine or semi-sitting) position, together with data on their unborn children and their rates of Apgar. There were 5,002 (58.6%) deliveries in the upright position, and 3,536 (41.4%) in the horizontal position. The mean Apgar rate at 1 minute was 8.11 for deliveries in the vertical position, and for the horizontal position it was 8.04 (p=0.009). Objective: To correlate the Apgar score, and neonatal mortality and its causes at a hospital located in the southern area of São Paulo City. Methods: A retrospective study performed by analysis of medical charts (n=7,094) of all live newborns during the period of 2005 to 2009, with data up to 28 days of life in reference to weight, Apgar score, survival and cause of mortality. Cases were analyzed by the X2 test (p < 0.05). Results: In 7,094 births, there were 139 deaths, 58.3% during the first week, and 3.6% of them with Apgar < 4 in the 1st minute. A positive association was found between mortality and this variable, with significantly declining values up to 2,000 g in weight. In the group with weight < 1,000 g, the association with Apgar < 4 in the 1st minute with mortality was three-fold greater than in the 1,000-1,500 g weight group, and 35-fold greater than in the ⩾ 3,000 g group. Among newborns with Apgar 8-10, the rate of mortality and low weight was two times greater than in those with weight > 2,499g. Fetal distress and prematurity were associated with early neonatal death; malformations and fetal distress to late mortality. The predictive value of death with Apgar < 4 varied, according to weight, from 62.74% in the < 1,000 g group to 5.5%, in the > 3,000 g group. Conclusions: The Apgar score proved linked to factors both epidemiological and related to attention given to the birth and neonatal mortality, and was associated with extremely low birth weight.
