2. Outcomes of Extended Curettage with and without Bone Allograft for Grade II Giant Cell Tumors around the Knee. A Retrospective Comparative Study

Abstract Objective This is the first study to establish the utility of extended curettage with or without bone allograft for Grade II giant cell tumors GCTs around the knee joint with the aim of exploring postoperative functional outcomes. Methods We retrospectively reviewed 25 cases of Campanacci grade II GCTs undergoing extended curettage between January 2014 and December 2019. The participants were divided into two groups: one group of 12 patients underwent extended curettage with bone allograft and bone cement, while the other group of 13 patients underwent extended curettage with bone cement only. Quality of life was assessed by the Revised Musculoskeletal Tumor Society Score and by the Knee score of the Knee Society; recurrence and complications were assessed for each cohort at the last follow-up. The Fisher test and two-sample t-tests were used to compare the categorical and continuous outcomes, respectively. Results The mean age was 28.09 (7.44) years old, with 10 (40%) males and 15 females (60%). The distal femur and the proximal tibia were involved in 13 (52%) and in 12 (48%) patients, respectively. There was no significant difference in the musculoskeletal tumor society score (25.75 versus 27.41; p= 0.178), in the knee society score (78.67 versus 81.46; p= 0.33), recurrence (0 versus 0%; p= 1), and complications (25 versus 7.69%; p= 0.21). Conclusions Extended curettage with or without bone allograft have similar functional outcomes for the knee without any major difference in the incidence of recurrence and of complications for Grade II GCTs. However, surgical convenience and cost-effectiveness might favor the bone cement only, while long-term osteoarthritis prevention needs to be investigated to favor bone allograft.

The mandibular torus can be defined as a normal exostosis that does not present symptoms nor require treatment, except in specific conditions. Note that one of the indications for removal of the mandibular torus is the installation of removable prostheses, since in these cases surgery is performed to ensure better stability and function of the prosthesis that will be adapted in the future. The causes for its origination remain uncertain and studies indicate that it might present hereditary and environmental issues such as: quality of daily diet, nutritional status and drugs involved in calcium homeostasis such as phenytoin, in addition to being associated with chewing and occlusal trauma. Imaging exams are essential to establish the diagnosis, along with a thorough clinical examination. Thus, the research herewith presented has the following problematic: could the extension of mandibular torus constitute a challenge to rehabilitative prosthetic treatment? This article was composed based on the analysis of the results from a clinical case, in which the removal of a bilateral mandibular torus appeared as evidence to make possible the fabrication of an inferior removable partial prosthesis. The anatomical study describes the normal aspects of the conformation of a certain species, that is, the characteristics that occur with high frequency in this. In this way, when an event occurs in most individuals of a species, it is considered as normal. Although anatomical variations are also observed, characterized by the alteration in the normal pattern in anatomy, it is defined as a small deviation from the normal morphological aspect of an organ or the deviation from the general plane of organization of an individual, events that in animals and humans is well studied. In the present case, craniofacial fissure is considered a rare congenital anomaly of unknown etiology. The location is usually in certain lines of eyelids, eyebrows, nostrils, lips and jaws, reaching soft and bony parts. In veterinary medicine it is little studied and its classification is based on the changes observed in humans based on the anatomical relations. The cracks may present with the characteristics of excess tissue or agenesis of the same. In humans, depending on the case, the treatment is based on suture of the soft tissues, in three planes: cutaneous, muscular and mucous; zetaplasty; orthodontic treatment; bone grafts, when necessary already in animals in the consulted bibliography there were no cases with treatment. The present report describes the changes related to a craniofacial fissure associated to palatoschisis, cheiloschisis, macroglossia, absence of the ocular orbit and consequently the eyeball in a newborn of dog. 8. Volumetric quantification of the alveolar ridge after reanatomization of the maxillary sinus floor for implant installation

Background: The loss of upper posterior teeth leads to bone resorption and maxillary sinus pneumatization, often preventing immediate insertion of dental implants and leading to the need for sinus floor augmentation (SFA) and bone graft. Objective: To evaluate, in cone-beam computed tomography (CBCT) images, the volumetric changes in the alveolar ridge and maxillary sinus after bone grafting for SFA and dental implants placement. Materials and methods: Bone and sinus volume of 16 maxillary sinuses of 12 patients submitted to SFA, bone graft, and implant placement were evaluated. CBCT exams were performed three times: evaluation of the residual bone (T0), evaluation of the graft after SFA before implant surgical planning (T1), and evaluation after implants healing (T2). All DICOM formatted images were evaluated by two calibrated examiners (ICC > 0.9) using ITK-SNAP software. The volume measured at each time point was compared using the ANOVA test. Clinical and demographic variables were collected and the percentage of bone gain and resorption were compared using the t-test. The significance level considered was P < 0.05. Results: The bone volume showed a significant increase after SFA (T1), with also significant resorption (T2); the sinus volume showed a significant reduction after SFA (T1) but with volume maintenance over time (T2). Sites with volumetric variation at T1 greater than 200% had lower longitudinal resorption (p=0.036), with no association with clinical or demographic variables (p > 0.05). Conclusion: The present study found an average increase of 200% in the bone ridge after SFA and an average resorption of 11% over four years, after implant placement, concentrated in the cervical region of the implants. Bone gain and resorption of the graft volume were not associated with the clinical or demographic variables of the patients. Introdução: A ausência dos dentes superiores posteriores leva à reabsorção óssea e pneumatização do seio maxilar, muitas vezes impedindo a inserção imediata do implante e levando à necessidade da realização de enxerto para levantamento do seio maxilar (LSM). Objetivo: Avaliar, em imagens de tomografia computadorizada de feixe cônico (TCFC), as alterações volumétricas no rebordo alveolar e seio maxilar após cirurgia de LSM, enxerto com substituto ósseo bovino e instalação de implantes dentários. Materiais e métodos: Foram avaliados o volume ósseo e sinusal de 16 seios maxilares de 12 pacientes submetidos ao LSM, enxerto ósseo e instalação de implantes. Exames de TCFC foram realizados em três momentos: avaliação do leito ósseo remanescente (T0), avaliação do enxerto após LSM para planejamento cirúrgico do implante (T1) e avaliação após cicatrização dos implantes (T2). As imagens em formato DICOM foram avaliadas por dois examinadores calibrados (ICC > 0,9) no software ITK-SNAP. O volume medido em cada tempo foi comparado utilizando o teste ANOVA. Variáveis clínicas e demográficas foram coletadas e a porcentagem de ganho e de reabsorção ósseas foram comparadas utilizando o teste t. O nível de significância considerado foi P < 0,05. Resultados: O rebordo ósseo apresentou aumento significativo após LSM (T1), com reabsorção óssea também significativa (T2); o volume sinusal apresentou redução significativa após LSM (T1) e manutenção deste volume ao longo do tempo (T2). Locais com variação volumétrica em T1 superior a 200% apresentaram menor reabsorção longitudinal (p=0,036), sem associação com variáveis clínicas ou demográficas (p > 0,05). Conclusão: O presente estudo encontrou aumento médio de 200% do rebordo ósseo após LSM e reabsorção média de 11% ao longo de quatro anos, após a instalação dos implantes, concentrada na região cervical dos implantes. O ganho ósseo e a reabsorção do volume enxertado não tiveram associação com variáveis clínicas ou demográficas dos pacientes.

This study evaluated the risk of neurovascular injury in surgical techniques of the anterior region of the mandible. 250 cone beam computed tomography scans of dentate adults were analyzed. It was observed that the most frequent distance between the base of the mandible and the mental foramen was 8mm. Additionally, 20.4% of the scans showed 6mm from the vestibular cortex to the lingual foramen. The study found safety margins for surgical techniques of mentoplasty and chin bone grafting that adopt a cutoff point of 5mm. [Extracted from the article]

This article reports a successful case of palatal fissure closure using the rotation of a vascularized myocutaneous flap from the frontal region. The patient, a 52-year-old woman, had undergone several unsuccessful surgeries before. After the removal of a devitalized bone graft, the procedure of rotating the vascularized myocutaneous flap from the frontal region was performed. After 11 months of follow-up, the patient showed stability in the surgical closure of the fissure, without aesthetic complications. It is concluded that this approach can be an effective option for refractory cases of palatal fissure. [Extracted from the article]

This article summarizes a literature review on the applicability of trans-sinus implants (TSIs) in the oral rehabilitation of severely atrophic maxillae. Severe maxillary atrophy and maxillary sinus pneumatization make it difficult to fix conventional implants, leading to the need for bone grafts or other fixation modalities. The literature review showed that the use of TSIs allowed for the fixation of longer implants with better retention, enabling immediate loading. Furthermore, in cases where bone grafting procedures were not used, implant survival rates were high. Therefore, the use of TSIs in the oral rehabilitation of atrophic maxillae can be a viable alternative when conventional treatment methods are contraindicated. [Extracted from the article]

Due to the need for rehabilitation with dental implants, maxillary sinus lifting surgery is described as a safe technique, in this case report we demonstrate a modified maxillary sinus lifting technique, the pillow technique, in which a pillow is made with a absorbable membrane filled with bone graft. At the time of surgery, the side window technique with displacement of the Schneider membrane and insertion of the pillow is used. As a clinical result, a satisfactory amount of bone gain was observed in the 11-month postoperative period. With this technique, it is possible to obtain a shorter surgical time due to the need only for the preparation of the receiving bed, since the pillow is made prior to the surgical act, in addition to a new maneuver for the protection of the biomaterial in cases of perforations of the Schneider membrane decreasing the chance of postoperative complications. Debido a la necesidad de rehabilitación con implantes dentales, la cirugía de elevación del seno maxilar se describe como una técnica segura, en este reporte de caso demostramos una técnica de elevación del seno maxilar modificada, la técnica de la almohada, en la cual se hace una almohada con una membrana absorbible rellena de injerto óseo. En el momento de la cirugía se utiliza la técnica de ventana lateral con desplazamiento de la membrana de Schneider e inserción de la almohada. Como resultado clínico, se observó una cantidad satisfactoria de ganancia ósea en el período posoperatorio de 11 meses. Con esta técnica, es posible obtener un menor tiempo quirúrgico debido a la necesidad solo de la preparación de la cama receptora, ya que la almohada se realiza antes de la cirugía, además de una nueva maniobra para la protección del biomaterial en casos de perforaciones de la membrana de Schneider que disminuyen la posibilidad de complicaciones posoperatorias. Devido à necessidade da reabilitação com implantes dentários, a cirurgia de levantamento do seio maxilar é descrita como uma técnica segura, neste relato de caso demonstramos uma técnica modificada de levantamento de seio maxilar, a técnica do travesseiro, na qual é confeccionado um travesseiro com uma membrana absorvível preenchida com enxerto ósseo. No momento cirúrgico é utilizada a técnica da janela lateral com deslocamento da membrana de Schneider e inserção do travesseiro. Como resultado clínico foi observado uma quantidade satisfatória de ganho ósseo no pós-operatório de 11 meses. Com essa técnica é possível obter um menor tempo cirúrgico devido à necessidade apenas da preparação do leito receptor, visto que o travesseiro é confeccionado previamente ao ato cirúrgico, além de uma nova manobra para a proteção do biomaterial em casos de perfurações da membrana de Schneider diminuindo a chance de complicações pós-operatórias.

The development of new devices using available and low cost materials may be an useful alternative for the treatment of long bone fractures in large animals. The aim of this study was to evaluate the mechanical strength of young bovine femur with diaphyseal fracture fixed with different polymeric intramedullary nails. Bending and compression tests using a universal machine were carried out in 4 distinct groups of 6 femurs from young calves. In one of the groups bones were intact while in the other three fractured bones were fixed using an intramedullary nail made of polypropylene, polyacetal or polyamide (one for each group). Considering bending and compression tests together, none of the used polymers offered strength to the fractured bones similar to the intact ones. The agreement of this findings with those from previous published in vivo results indicates that the employed ex vivo methods for mechanical assays may be useful in the search of stronger materials to be used in development of new devices [ABSTRACT FROM AUTHOR]

The present study evaluated autogenous platelet rich plasma's (PRP) influence on the reparation process of four bone defects made on rabbit's skull, associated or not to autogenous bone graft (EOE). Defect I received PRP only; defect II received 3mg of EOE only; defect III received EOE associated to PRP; defect IV was left to heal naturally, serving as control. After each surgery the animals were randomly divided into three groups that were euthanized at 30, 60 and 90 days. In the mesoscopic evaluation bone ingrowth started from the defect's borders to the center and from the bottom to the surface for all observation times on the control (VI) and PRP only (I) groups. In the groups treated with EOE only (II) and EOE associated to PRP (III) new bone was observed in the center of the defects. Radiographic analysis showed greater central radiopacity for groups treated with EOE only (II) and EOE associated to PRP (III) at all observation times. Microscopically in the group treated with EOE associated to PRP (III) at 30 days the graft was indistinguishable from new bone present on the border of the defect, associated to a moderate quantity of a very vascularized and cellular fibrous connective tissue. This tissue showed an extracelular eosinophilic amorphous foamy material, associated to an inflammatory process constituted by lymphocytes and in less number by macrophages and multinucleated giant cells that may have negatively influenced early bone formation. At 60 and 90 days occasional spots of lymphocytic inflammation were observed. Both treatments, PRP associated or not to EOE, were similar for the bone ingrowth at the final time of observation; the graft used alone determined early bone reparation and thromboplastine used for the platelet gel formation incited a foreign body-like reaction that acted negatively on the initial reparation. The ostectomia is a basic surgical maneuver that can be to interfere in bone repair. This work aimed at achieving histological analysis qualitative of bone repair after removal bone graft from the rabbit tíbia using two different rotative instruments: pneumatic micromotor lower rotation and electric motor lower rotation. Were utilized nine rabbit and making standart cavity (6 x 12mm). In right tibia were used pneumatic micromotor lower rotation (Group I); in left tibia were used electric motor lower rotation (Group II). Tree animals were sacrifice in period of 2, 14 and 30 days after surgery. The histological specimens were submitted at microscopy optical. The result showed that bone repair in the period of 2 and 30 days was the same, independence at instrument used. In the period of 14 days at verifying discreet evolution a bone repair of cavity made with electric motor lower rotation. [ABSTRACT FROM AUTHOR]

Essa revis?o sistem?tica foi conduzida para avaliar a estabilidade e complica??es p?s-operat?rias da osteotomia Le Fort I (OLFI) com uso de enxertos ?sseos com intuito de melhorar o entendimento dessa t?cnica e averiguar a necessidade de novas metodologias sobre o assunto. A busca sitem?tica foi efetuada nas bases de dados PubMed, EMBASE, Biblioteca Cochrane, literatura cinza (OpenGrey, registros de ensaios cl?nicos and Google Scholar) e busca manual, obtendo 1748 artigos publicados at? 2019. Destes, 72 foram selecionados para leitura na ?ntegra, mas apenas 23 foram inclu?dos nessa revis?o. O n?vel de concord?ncia entre os autores para sele??o dos estudos e para elegibilidade foi considerado excelente (k= 0.891 e k= 0.907 respectivamente). A qualidade metodol?gica foi considerada baixa, tendo todos estudos inclu?dos alto potencial de vieses. Mesmo assim, o estudo destaca que o enxerto ?sseo reduz a taxa de recidiva fornecendo resultado das medidas est?veis nos planos sagital e vertical da maxila. Esses resultados de estabilidade permitiram organizar uma categoriza??o estratificada dos movimentos cir?rgicos em ambos os planos da maxila, afirmando que o enxerto ?sseo pode estar bem indicado para avan?o maxilar > 5 mm e reposicionamento inferior ? 3 mm. As complica??es p?s-operat?rias mais freq?entes foram o deslocamento do bloco HA, propaga??o da cicatriz do local doador e sinusite. Os enxertos alog?nicos e alopl?sticos apresentaram maior preval?ncia em rela??o ?s complica??es, com taxas de 6.1% e 5% respectivamente. Portanto essa t?cnica de enxertia apresenta baixa taxa de complica??o sendo considerada segura. Apesar das diretrizes propostas nesta revis?o sistem?tica, houve amostras pequenas de alguns estudos, heterogeneidade em m?todos e resultados, poucos ensaios cl?nicos com grupo controlado e uso de sobreposi??o 2D das altera??es cir?rgicas. Isso indica a necessidade de pesquisas mais s?lidas baseada em evid?ncias para dar suporte a esses resultados. This review systematic was conducted to evaluate the stability and postoperative complications of Le Fort I osteotomy with the use of bone grafting in order to improve the understanding of this technique and to investigate the need for new methodologies on the subject. The systemmatic search was carried out in PubMed, EMBASE, Cochrane Library databases, grey literature (Opengrey, Register of clinical trials and Google scholar) and manual search, yielding 1748 articles published up until 2019. Of these, 72 articles were selected for full-text screening, but only 23 articles were included. The level of agreement between the authors for study selection and for eligibility was considered excellent (k = 0.891 and k = 0.907 respectively). Methodological quality was considered low, with all studies including high bias potential. Even so, the study highlights that the bone grafting reduces the rate of relapse by providing results of stable measurements in the sagittal and vertical planes of the maxilla. These stability results allowed us to organize a stratified categorization of the surgical movements in both planes of the maxilla, stating that the bone graft may be well indicated for maxillary advancement > 5 mm and inferior repositioning ? 3 mm. The most frequent postoperative complications were HA block displacement, donor site scarring and sinusitis. The allogeneic and alloplastic grafts presented a higher prevalence regarding complications with rate of 6.1% and 5% respectively. Therefore, this grafting technique presents a low complication rate and is considered safe. Despite the guidelines proposed in this systematic review, there were small samples in some studies, heterogeneity in methods and results, few clinical trials with xi controlled group and use of 2D overlap of surgical changes. Esta pesquisa avaliou a estabilidade primária dos implantes instalados em regiões de enxerto heterógeno bovino utilizado no levantamento do soalho seio maxilar nos períodos de dois e seis meses após a enxertia, a fim de verificar a possibilidade de instalação de implantes no período de reparo ósseo primário. O estudo foi composto por sete pacientes. O mesmo paciente fez parte dos dois grupos analisados, sendo considerados os lados direito ou esquerdo (GT = grupo teste; avaliação do enxerto com dois meses) e esquerdo ou direito (GC = grupo controle; avaliação do enxerto com seis meses). O trabalho foi descrito nos artigos a seguir: ARTIGO I: foi realizada análise clínica e histopatológica nos períodos de incorporação óssea (dois meses) e de maturação óssea (seis meses). A análise histopatológica mostrou neoformação óssea com presença de osteoblastos, osteócitos e neovascularização tanto no GT como no GC. A análise estatística apontou diferença significativa (p = 0,007) entre a quantidade de osso neoformado nos dois grupos. Clinicamente, não houve prejuízo na estabilidade primária dos implantes instalados no GT. Concluiu-se que houve diferença estatística na taxa de osso neoformado nos períodos de dois e seis meses após o procedimento de enxertia, porém não houve prejuízo na estabilidade primária dos implantes instalados no GT. ARTIGO II: foi realizada análise clínica e histoquímica nos períodos de incorporação óssea (dois meses) e de maturação óssea (seis meses). Por meio da coloração picrosirius red, constatou-se a presença de colágeno tipo I (maduro) e tipo III (imaturo) em ambos os grupos, porém a análise estatística apontou diferença significativa entre o tipo I (p = 0,002) e o tipo III (p = 0,014) no GT e no GC. Clinicamente, não houve prejuízo na estabilidade primária dos implantes instalados no GT. Diante desses resultados, foi possível concluir que, apesar de o osso neoformado se mostrar mais maduro aos seis meses, não houve prejuízo na estabilidade primária dos implantes instalados no GT. This study evaluated the primary stability of implants installed in areas of heterogenous bovine grafting used in lifting the maxillary sinus floor two and six months after grafting, in order to verify the possibility of implant installation during the primary bone repair stage. This study consisted of seven patients. The same patient was part of both analysis groups, where the right or left sides (TG = test group, two-month graft evaluation) and left or right sides (CG = control group, six-month graft evaluation) were considered. The work was described in the following articles: ARTICLE I: clinical and histopathological analysis was performed in periods of bone incorporation (two months) and bone maturation (six months).The histopathological analysis showed new bone formation with osteoblasts, osteocytes and neovascularization in both groups. The statistical analysis showed a significant difference (p = 0.007) between the amount of newly formed bone in the TG and CG. Clinically, there was no impairment in the primary stability of implants installed in the TG (two months). We concluded that there was a statistical difference in the rate of newly formed bone in the periods of two and six months, but there was no impairment in the primary stability of the implants installed in the TG. ARTICLE II: clinical and histochemical analysis were performed in periods of bone incorporation (two months) and bone maturation (six months). The presence of collagen type I (mature) and type III (immature) in both groups was observed through picrosirius red staining, but the statistical analysis indicated a significant difference between type I (p = 0.002) and type III (p = 0.014) in TG and CG. Clinically, there was no impairment in the primary stability of the implants installed in the TG. We conclude that, although the newly formed bone showed to be more mature at six months, there was no impairment in the primary stability of the implants installed in the TG.

Introdução: Frequentemente após extrações ou pneumatização do seio maxilar, é frequente a perda de rebordo alveolar devido à sua reabsorção ficando assim impossibilitados de realizar uma correta reabilitação (Monje et al. 2013). Materiais e métodos: Foi realizada uma pesquisa bibliográfica na base de dados b-on (biblioteca do conhecimento), no repositório institucional da Universidade Fernando Pessoa e biblioteca Ricardo Reis. Das 57 referências bibliográficas pesquisadas, incluindo livros, artigos, revistas cuja recolha foi feita entre Janeiro 2017 e Maio 2017, tiveram como limite temporal 2005-2017 no idioma inglês. Destas foram selecionadas 12 sendo que foram aplicados como critérios de exclusão o limite temporal e a pertinência/enquadramento da informação recolhida. Discussão: Nos casos de atrofia severa da maxila o osso autólogo é o que proporciona maior capacidade osteoindutiva e osteogénica (Schmitt et al., 2012). Da pesquisa realizada, os enxertos ósseos inlay demonstraram ser a opção de maior sucesso; quando utilizados conjuntamente com a colocação de implantes. Conclusões: Existem as mais diversas opções de tratamento para os aumentos ósseos verticais e horizontais, destacando as técnicas de enxerto: Onlay, Inlay (sinus lift). O gold standard a nível de materiais, preconizado por toda a literatura trata-se de enxerto autólogo, idealmente osso intramembranoso. Em todos os casos a escolha do material deve ser feita pelos doentes e a escolha da técnica deve ser feita pelo profissional atendendo às características físicas do caso clínico e dos diversos materiais a usar (Park et al., 2009 e Wauf, 2002). Introduction: Maxillary bone resorption following tooth extraction or due to pneumatization of maxillary sinus often results in inadequate residual ridges for ideal implant placement (Monje et al. 2013). Materials and Methods: A research bibliography was made at b-on data base, Universidade Fernando Pessoa-institutional repository and Ricardo Reis library. From 57 references researched including books and articles from 2005 – 2017 in English were selected 12. The criteria used to select the chosen resources was based on timeline frame and content relevance. Discussion: In cases with severe resorption of maxillary alveolar ridge, the autogenous bone is the one that provides the best osteoinductive and osteogenic capacities (Schmitt et al., 2012). From the gathered research, the inlay bone graft showed the most success from the other options, while simultaneously using implants. Conclusions: There is a large diversity of options for the treatment of vertical and horizontal bone increase, especially when using inlay/onlay grafting techniques. The gold standard of materials, as proclaimed throughout all the scientific arcticles is the autologous graft. In all cases the material choice should be made by the patient, while the technique applied should be chosen by the professional according to the characteristics of the clinical case and the various materials available (Park et al., 2009 e Wauf, 2002).

A Implantodontia, afim de, proporcionar melhores resultados estéticos e funcionais, tem apresentado inúmeras inovações e tecnologias visando alcançar estes objetivos associado a redução do tempo de tratamento. Contudo pacientes que apresentam a necessidade de cirurgias de reconstruções associadas a enxertos ósseos visando à instalação adequada destes implantes permanecem como desafios. Deste modo, o presente trabalho analisou o enxerto ósseo autógeno em dois diferentes períodos e avaliou os implantes instalados nestes períodos analisados, a pesquisa foi subdividida e descrita nos Capítulos à seguir. CAPITULO I: Análise histológica das características existentes no processo de incorporação e maturação óssea em áreas submetidas à cirurgia de enxerto ósseo em elevação de seio maxilar. Observou-se nos dois períodos analisados, correspondentes do processo de reparo ósseo, que foram semelhantes em ambos períodos. Deste modo, pode-se concluir que o enxerto ósseo autógeno apresenta características semelhantes na fase de incorporação (2 meses) e maturação (6 meses) o que possibilita a manipulação deste tecido na fase mais inicial sem o seu comprometimento. CAPITULO II: O propósito deste capítulo foi avaliar a osseointegração em dois períodos diferentes sendo, incorporação (2 meses) e maturação (6 meses) óssea em áreas submetidas a cirurgia de enxerto ósseo em elevação de seio maxilar e a estabilidade da prótese instalada. Observou-se nos dois períodos analisados, a incorporação óssea (2 meses) e maturação óssea (6 meses), que embora divergentes nas etapas de reparo ósseo, apresentaram resultados semelhantes para a osseointegração e estabilidade da prótese. Deste modo, pode-se concluir que a inserção de implantes osseointegráveis nestas regiões em menor período de reparo, ou seja, na fase de incorporação óssea (2 meses) não compromete a osseointegração nem tão pouco a estabilidade da prótese a longo prazo. A redução do tempo de tratamento de pacientes submetidos a cirurgias de reconstruções com enxerto ósseo em elevação de seio maxilar é possível realizando a instalação dos implantes no período de incorporação óssea. The implantology in order to provide better aesthetic and functional results, presented numerous innovations and technologies to achieve these goals associated with reduced treatment time. However patients who have the need for reconstructive surgery associated with bone grafts seeking the proper installation of these implants remain as challenges. Thus, this work presents two studies described in the following chapters. Article I: Histological analysis of existing resources in the process of development and maturation in areas prone to bone graft surgery in sinus floor elevation. It was observed in both periods analyzed, bone repair, which were similar in both the periods. Thus, it can be concluded that autogenous grafts has similar characteristics in the incorporation phase (2 months) and mature (6 months), allowing the tissue manipulation in the initial stage without their involvement. Article II: The objective of this chapter was to evaluate the osseointegration in two different periods being, incorporation (2 months) and maturity (6 months) bone in areas subjected to bone graft surgery in sinus floor elevation and stability of the installed prosthesis. It was observed in both study periods, the bone ingrowth (2 months) and maturation (6 months), although at different stages of bone healing that showed similar results for bone integration and stability of the prosthesis. Thus, it can be concluded that the insertion of dental implants in these regions in the shortest repair, namely on bone growth phase (2 months) does not jeopardize the osseointegration or long-term stability of the prosthesis. The reduction in the treatment of patients undergoing reconstructive surgery with bone grafting time in sinus floor elevation is possible to perform the installation of implants in bone growth period.

Introdução: Determinar as causas da perda óssea nos maxilares, compreender os mecanismos biológicos desencadeados após a perda dental e criar recursos técnicos no intúito de prevenir e/ou minimizar as sequélas decorrentes, tem sido ao longo dos anos uma das vertentes de maior pesquisa e desenvolvimento na medicina dental . Objetivo: Assim, o objetivo desta dissertação é realizar uma revisão da literatura sobre os avanços nos biomateriais e técnicas na correção dos defeitos ósseos maxilares, para que seja possível, futuramente, ampliar as suas aplicações em Medicina Dentária, ultrapassando as limitações das técnicas e materiais existentes atualmente. Metodologia: Para isso, foi realizada uma pesquisa de artigos na base de dados PubMed, Bireme, Lilacs, Medline, revistas e periódicos nos idiomas: português, inglês e espanhol; assim como livros consagrados na literatura médico-odontológica,com o recurso a limites e palavras-chave de forma a refinar essa pesquisa. Desenvolvimento: Os avanços nos biomateriais e técnicas na correção dos defeitos ósseos maxilares tem seguido, assim como os implantes dentais, dois principais eixos de pesquisa, primeiro no que diz respeito aos biomateriais empregados pós-exodontia, a prevenir a reabsorção osséa e aqueles utilizados à fim de corrigir defeitos já existentes; a aplicação destes materiais recai sobre fatores que são decisivos na escolha do cirurgião, tais como: disponibilidade, necessidade de procedimento cirúrgico adicional, compatibilidade, morbidade do enxerto, qualidade do osso resultante e tempo de neo-formação. Segundo, as técnicas e recursos desenvolvidos para garantir a eficáz correção do defeito, assim como proporcionar procedimentos menos traumaticos ao organismo e de maior simplicidade e previsibilidade na sua execução e reprodução pelos profissionais. Discussão: Os trabalhos desenvolvidos em volta dos biomateriais atualmente buscam não só o substituto ideal, mas sim a melhoria na interação entre o osso hospedeiro e o biomaterial enxertado perante os recursos já utilizados e consagrados pelas literaturas; assim como a utilização das técnicas já protocoladas de forma associada a estes novos materiais. Conclusão: As pesquisas sobre o aperfeiçoamento do processo de regeneração óssea nos defeitos maxilares avançam na questão de promover a rápida e eficaz interação entre organismo e biomaterial, a fim de trazer soluções para problemas como a anti-genicidade, previsibilidade dimensional, menor tempo entre a enxertia e a reabilitação protética e agrega-se a utilização de recursos técnicos práticos, uma vez que o planejamento da reabilitação inicia-se nas decisões pré-exodontias e pré-implantares. Introduction: To determine the causes of jaw bone loss, understand the biological mechanisms triggered after tooth loss and create technical resources in order to prevent and/or minimize sequelae, has been over the years one of the largest research and development aspects in dental medicine. Objective: The objective of this work is to conduct a literature review on the advances in biomaterials and techniques for correction of maxillary bone defects, to be able, in the future, expand its applications in dentistry, overcoming the limitations of techniques and materials currently exist. Methodology: For this, an article survey was conducted in the PubMed, database, Bireme, Lilacs, Medline, magazines and journals in languages: Portuguese, English and Spanish; as well as books contained in the medical and dental literature, the use of limits and keywords in order to refine this search. Development: Advances in Biomaterials and techniques in correcting the jaw bone defects is followed, as well as dental implants, two main lines of research, first in respect of biomaterials post-extraction employed to prevent bone resorption and those used for to correct existing shortcomings; the application of these materials lies on factors which are decisive in the choice of the surgeon, such as availability, requiring additional surgical procedure, compatibility of the graft morbidity and quality of the resulting bone neo-formation time. Second, the techniques and features designed to ensure the effective correction of the defect, as well as providing less traumatic procedures to the body and greater simplicity and predictability in implementation and reproduction by professionals. Discussion: The work developed around the biomaterials currently seeking not only the ideal replacement, but the improvement in the interaction between the host bone and biomaterial graft before the resources already used and consecrated by literature; as well as the use of techniques already filed in association with these new materials. Conclusion: The research on the improvement of the process of bone regeneration in the jaw defects advance on the issue of promoting the rapid and efficient interaction between organism and biomaterials, to provide solutions to problems such as anti-nicity, dimensional predictability, less between grafting and prosthetic rehabilitation and adds to the practical use of technical, since the rehabilitation planning starts in decisions pre-extractions and pre-implants.

INTRODUÇÃO: O planejamento cirúrgico nas rinoplastias para tratamento do "dorso em sela" apresentou grande avanço nas últimas décadas, em decorrência do melhor manejo de enxertos e implantes. Porém, a escolha desses materiais é tema de controvérsia e debates. As forças da cicatriz no nariz com deformidades tendem a subjugar qualquer tipo de reconstrução que não seja rígida ou semirrígida. Assim, o enxerto ósseo é uma boa opção, pois é estável, apresentando boa disponibilidade e confiabilidade nos resultados estéticos. O enxerto ósseo de olécrano é de fácil coleta, tem uma única espessura do córtex, característica importante para resistir à reabsorção, além de proporcionar resultado estético estável. Este trabalho tem como objetivo discutir as vantagens da utilização do enxerto ósseo de olécrano em rinoplastias e demonstrar a experiência com esse tipo de procedimento. MÉTODO: Estudo descritivo e retrospectivo, sendo realizada revisão dos prontuários de pacientes submetidos a enxerto ósseo de olécrano para aumento de dorso nasal, no período de janeiro de 2000 a janeiro de 2010, com acompanhamento por igual período, no Serviço de Cirurgia Plástica do Hospital Daher Lago Sul (Brasília, DF, Brasil). O controle de possível reabsorção do enxerto foi realizado com medidas antropométricas do nariz em documentação fotográfica, além de controle radiológico do enxerto. RESULTADOS: Foram operados 9 pacientes, sendo obtidos bons resultados estéticos. Em nenhum dos métodos para controle de possível reabsorção do enxerto ficou evidenciada alguma perda de projeção ou sinais de reabsorção óssea no acompanhamento com até 6 anos de pós-operatório. CONCLUSÕES: O enxerto de olécrano demonstrou ser uma boa opção para rinoplastias primárias ou secundárias, no tratamento do "dorso em sela", não tendo sido demonstrada reabsorção a longo prazo ou morbidade da área doadora, com bons resultados estéticos, previsíveis e duradouros. BACKGROUND: Rhinoplasty surgery, performed for the treatment of "saddle-back" nose, has significantly improved in the last decades due to better management of grafts and implants. However, the choice of these materials is controversial and remains a subject of discussion. Scar contracture forces in nasal deformities prevent any type of reconstruction that is not rigid or semi-rigid. Therefore, the bone graft is an appropriate choice because it is stable, available, and reliable with regard to aesthetic outcomes. The olecranon bone graft is easily harvested and has a single cortical thickness, which is essential for resisting bone resorption and for providing long-lasting cosmetic results. The aim of this study is to discuss the advantages of the olecranon bone graft in rhinoplasty and to demonstrate our experience with this procedure. METHODS: In this descriptive and retrospective study, we reviewed the medical records of patients who underwent olecranon bone grafting for nasal dorsal augmentation between January 2000 and January 2010 and were consequently followed-up for an equivalent period, at the Plastic Surgery Service of Hospital Daher Lago Sul (Daher Lago Sul Hospital, Brasília, DF, Brazil). Possible graft resorption was monitored by anthropometric measurements of the nose with photographic documentation and graft radiological control. RESULTS: Satisfactory aesthetic results were achieved for the 9 patients who underwent surgery. According to the examinations performed to monitor possible graft resorption, projection loss or signs of bone resorption were not observed for up to 6 years after surgery. CONCLUSIONS: The olecranon bone graft proved to be a suitable choice for primary or secondary rhinoplasty to treat "saddle-back" nose, with no long-term resorption or morbidity of the donor area observed up to this point. Moreover, the olecranon bone graft provides satisfactory, predictable, and long-lasting aesthetic results.

Orientador: Frederico Andrade e Silva Dissertação (mestrado) - Universidade Estadual de Campinas, Faculdade de Odontologia de Piracicaba Resumo: Varias pesquisas tem mostrado que aumento de energia na superfície e a fabricação do implante na ausência de ar, aumenta a hidrofilia da superfície reduzindo o tempo da osseointegração. O objetivo desse estudo foi a avaliar o tempo de vida útil e o Nível de Reabsorção Óssea (NRO) entre os implantes com superfícies química (SLActive) e fisicamente (SLA) tratadas, instalados em maxilas previamente enxertadas com osso autógeno em bloco, e carregados com coroas unitárias parafusadas. Foram utilizados 17 voluntários e 20 implantes (10 com superfície SLA e 10 com superfície SLActive). Dez semanas após a instalação dos implantes no grupo controle (SLA) e 4 semanas no grupo teste (SLActive), foram iniciados os procedimentos para a confecção das próteses parafusadas. Os pilares foram apertados com força de 35Ncm, 12 semanas após a colocação dos implantes com superfície SLA e após 6 semanas com a superfície SLActive. As radiografias periapicais foram tomadas e avaliadas no ato da colocação dos implantes, no dia de aperto do pilar, e, 1 e 3 meses com a coroa definitiva em função. Foi realizada a comparação do NRO entre dois tipos de superfícies. Dois implantes SLActive foram perdidos durante o aperto dos pilares. Os resultados mostraram que NRO ao redor dos implantes com 3 meses em função foi menor no grupo teste quando comparado com grupo controle. Os implantes com superfície SLActive instalados na maxila, na área tratada com enxerto ósseo em bloco, e carregados com coroas unitárias 6 semanas após a instalação, tiveram menor vida útil quando comparados com implantes com superfície SLA. Abstract: Many researches have shown that higher energy of dental implants and production in chamber without air, enlarge surface hidrofilicity and reduce osseointegration time. The aim of this study was to evaluate the survival rate and the Level of Bone Resorption (LBR) between the implants with chemically and physically treated surfaces, placed in maxillae previously treated with bone block grafts, and loaded with single screwed crowns. Seventeen patients were treated with 20 implants (10 with SLA and 10 with SLActive surface). Ten weeks after implant placement in control group (SLA) and four weeks in test group (SLActive) we began with prosthodontical treatment. The abutments were tightened with 35Ncm, 12 weeks after placement of implants with SLA surface, and after 6 weeks with SLActive surface. Periapical radiographies were made and evaluated after implant placement, after abutment tightening, 1 and 3 months with crown in function. We have done a comparison of LBR between two surfaces. The results have shown that LBR around the implants in function was smaller in the test group comparing to the control group, while implants with SLActive surfaces placed in maxillae, previously treated with bone block graft, and loaded with single screwed crowns 6 weeks after, have smaller survival rate comparing to SLA implants. 34. Consolidação da osteotomia valgizante proximal da tíbia com cunha de abertura fixada com placa 'calço' de Anthony®

OBJETIVO: Este estudo tem por finalidade verificar a consolidação da osteotomia valgizante da tíbia com cunha de abertura fixada com placa tipo calço de Anthony® (OVT), no tratamento da osteoartrose medial do joelho varo, a correção da deformidade e a resposta clínica ao tratamento cirúrgico. MÉTODOS: Vinte pacientes (vinte joelhos) com osteoartrose do compartimento medial do joelho, com idade média de 48,4 ± 9,9, foram avaliados por um período mínimo de um ano. Os pacientes foram submetidos a avaliação radiográfica da consolidação e do eixo mecânico no pré e pós operatório, além da avaliação dos critérios de LYSHOLM. RESULTADOS: A consolidação da osteotomia ocorreu após 12 semanas em 100% dos casos sem complicações. A avaliação do LYSHOLM no pós operatório apresentou 80% de excelentes e bons resultados. A correção final média do eixo mecânico foi de 3,4 ± 3,3 graus de valgo. CONCLUSÃO: Concluímos que a consolidação da osteotomia supra-tuberositária da tíbia com cunha de abertura fixada com placa calço de Anthony® e com enxertia óssea tricortical ocorre num intervalo de três meses. A cirurgia é eficaz para a correção da deformidade em varo do joelho, e propicia melhora clínica significante para o paciente.
