CONTEXTUALIZAÇÃO: A análise cinemática é um método de avaliação quantitativa empregada em diferentes áreas de estudo. Na área do desenvolvimento motor, essa análise pode proporcionar uma melhor compreensão da aquisição e do desenvolvimento das habilidades motoras. OBJETIVOS: Desenvolver e comparar dois arranjos experimentais para análise cinemática dos movimentos de cabeça durante a coordenação viso-cefálica (CVC) em lactentes. MATERIAIS E MÉTODOS: Foram testados dois arranjos experimentais (A e B) que diferiam quanto ao número e posicionamento das câmeras, bem como quanto ao volume do sistema de calibração. RESULTADOS: A acurácia dos dois arranjos experimentais foi de 2,47mm, indicando que ambos podem fornecer uma reconstrução verossímil do movimento. As três câmeras usadas no arranjo B favoreceram a visualização de toda a amplitude do movimento por pelo menos uma das câmeras. Isso levou à melhora da análise qualitativa e à redução do tempo de processamento dos dados quantitativos, reduzindo-o em 33% quando comparado ao arranjo A. Além disso, o arranjo B apresentou melhor relação custo-benefício. CONCLUSÕES: Ambos os arranjos são adequados para a análise cinemática dos movimentos de cabeça durante a CVC de lactentes, entretanto, o arranjo B é mais vantajoso. A metodologia do arranjo B pode ser empregada em estudos que investigam o movimento de cabeça de lactentes, sejam eles típicos ou atípicos. Os resultados de tais estudos poderão ser empregados para complementar a avaliação de lactentes de risco e, conseqüentemente, auxiliar na intervenção precoce destes. The methodology for set-up B can be used in studies investigating head movements in either typical or atypical infants. The results from such studies could be used to complement assessments on at-risk infants and consequently could assist in implementing early interventions. Contextualização: Para que sejam realizadas atividades funcionais da vida diária é primordial a manutenção do equilíbrio estático, possibilitando posturas, movimentos e respostas adequadas. Objetivos: O presente trabalho teve como finalidade avaliar e comparar, através do teste de Romberg adaptado, as oscilações do equilíbrio estático de amputados protetizados de membros inferiores com indivíduos não amputados. Método: Foram avaliados 33 indivíduos do sexo masculino com idade média de 33,38 anos, divididos em três diferentes grupos. O grupo A foi composto por 12 indivíduos amputados que utilizavam prótese transfemural. O grupo B foi formado por 9 indivíduos amputados transtibiais. O grupo controle C continha 12 indivíduos não amputados. Cada indivíduo foi filmado nos planos frontal e sagital, avaliado quadro a quadro para a seleção de momentos de maiores oscilações. As imagens selecionadas foram quantificadas através da Biofotogrametria Computadorizada, utilizando-se o programa ALCimage 2.1®. Resultados: Os resultados mostraram diferenças significativas entre as oscilações anteriores para o grupo A (transfemural) e entre as oscilações para o lado esquerdo, no grupo B. Conclusão: A partir destes resultados, conclui-se que amputados de membros inferiores apresentam uma tendência de oscilação mais significante em direção anterior e para o lado contra-lateral à prótese. Com o avanço da idade, nota-se uma menor tendência à oscilação posterior. Background: To accomplish day-to-day functional activities, it is fundamentally important to maintain static balance, thereby enabling appropriate postures, movements and responses. Objective: The purpose of this study was to evaluate oscillations in static balance among amputees with lower limb prostheses, by means of an adapted Romberg test, and to compare such oscillations with those among individuals without amputations. Method: Thirty-three male individuals of mean age 33.38 years were evaluated, in three different groups. Group A was composed of 12 amputees who were utilizing a transfemoral prosthesis. Group B was formed by 9 individuals with a transtibial amputation. Group C (control) contained 12 normal individuals. All subjects were filmed in the frontal and sagittal planes, and their movements were evaluated frame by frame to select the moments of greatest oscillation. The images selected were quantified by means of computerized biophotogrammetry using the program ALCimage 2.1®. Results: The results showed significant differences among the anterior oscillations for the group A and among the left-side oscillations for group B. Conclusion: From these results, it was concluded that lower-limb amputees present a tendency towards more significant oscillations in the anterior direction and contralaterally to the prosthesis. With advancing age, a lesser tendency towards posterior oscillation was noted.

21. Histological analysis of healthy epithelium of Wistar rats in vivo irradiated with different intensities of therapeutic ultrasound.

BACKGROUND: Ultrasound (US) is a widely used and studied resource for physical therapy treatments. Given the scarcity of studies on the effects of US on healthy tissue, many physical therapy professionals make unfounded decisions regarding its methods and parameters of application. OBJECTIVES: The possible histological and morphometric changes in the healthy tissue of Wistar rats in vivo irradiated with different intensities of US were evaluated. METHODS: Thirty Wistar rats, randomly distributed among five groups of six animals each, were treated on the right side of the dorsal region, over an area of 4 cm2. The left side served as a control. The treatment was applied over a four-day period, with two mins. of daily irradiation. The output intensity was checked using a precision dosimeter before the applications. Histological and morphometric analyses were performed using the Image Tool software. RESULTS: There were slight inflammatory infiltration and thinning of the dermis fibers, particularly in the groups irradiated with 1.5 and 2 W/cm2. There was also thickening of the epidermis in the samples from the irradiated animals. To evaluate the quantitative results, the statistical analyses consisted of one-way ANOVAs with the post-hoc Tukey tests. There were significant differences in epidermis thicknesses between the control group and the groups irradiated with 1.0, 1.5 and 2.0 W/cm2. CONCLUSIONS: Higher doses of US produced changes in the epidermis and dermis, i.e. increased thickness and collagen fiber thinning and proliferation, respectively. These results serve as a warning of the possible implications of therapeutic ultrasound use in esthetics. [ABSTRACT FROM AUTHOR]

O crescimento de muitas espécies vegetais é negativamente afetado pela ocorrência de alumínio em solos ácidos. O estresse oxidativo é um importante componente da reação das plantas a níveis tóxicos de alumínio, causando a ativação de enzimas antioxidantes. A pêra (Pyrus communis L.) é uma importante espécie frutífera que recentemente vem suscitando interesse no Brasil. O marmelo (Cydonia oblonga L.) é usado na Europa como porta-enxerto ananizante para a pêra. O presente experimento estudou o efeito do aluminio e pH ácido sobre crescimento e enraizamento de brotações de marmeleiro BA 29 in vitro, com particular atenção a ativação das enzimas SOD e CAT, e do metabólito MDA, envolvidos no estresse oxidativo dos tecidos vegetais. Na fase de multiplicação, Al reduziu o peso e a proliferação a partir, respectivamente, de Al 15 e 30 mg/L. No enraizamento, o peso das brotações foi reduzido com Al 60 mg/L (Tab.2). Em ambas as fases, foi verificado amarelecimento das folhas, sobretudo na concentração de 60 mg/L. A ativação de SOD nas brotações em multiplicação foi maior com Al 60 mg L/1, enquanto CAT não mostrou variação. Tanto SOD quanto CAT não foram influenciadas pelo Al na fase de enraizamento. MDA aumentou particularmente nas brotações em multiplicação (70% e 100%, respectivamente com Al 45 e 60 mg/L). No enraizamento, MDA aumentou com Al nas brotações (de 30 a 60 mg L/1) e nas raízes (45 mg/L), embora de maneira menos significativa. O presente resultado demonstra a sensibilidade das brotações cultivadas in vitro de marmeleiro ‘BA 29’ ao alumínio, evidenciada, sobretudo, na fase de multiplicação pela ativação de MDA.

- Abstract
Prevention is universally advocated, especially in the case of noncommunicable diseases. However, given the proliferation of preventive technologies, it does not seem defensible to generically encourage preventive behaviors and tests for healthcare professionals and users. In this essay, we articulate concepts, ideas and criteria for considering preventive measures, providing a minimum guide to be used by professionals (especially in primary healthcare) and managers. The concepts of primary, secondary and quaternary prevention are explored, as well as those of reductive and additive prevention, high-risk and population-based preventive strategies; evidence-based medicine and its contemporary crisis; the precautionary principle; health promotion, an expanded, person-centered approach and shared decision-making. This discussion was designed to improve competence in the evaluation of preventive measures, making clinical and health decisions more judicious and less iatrogenic regarding primary and secondary prevention.

In the municipality of Rio de Janeiro, Brazil, the incorporation of the concept of epidemic intelligence and technological resources has supported new perspectives for the use of data by health surveillance, since the COVID-19 pandemic. This article presents the Epidemiological Intelligence Center (CIE) and the tools and products developed in its coordination. The CIE was inaugurated in March 2022, with a multiprofessional team, supported by the premises of transparency and integration of various data sources for early detection of changes in the trends of events of importance in Public Health. The initial acquisition of a data lake favored changes in the consumption, management and security processes for the data processed. This data lake currently stores the Carioca Base - a retrospective cohort of individuals with a history of COVID-19 vaccination and/or events related to the disease. Descriptive and analytical dashboards have been developed and made available, respectively for public use and for health surveillance administrators. An alert panel, aimed at monitoring trends in care in the urgency and emergency network, was implemented and subsidizes rapid response actions in the city's territories. The CIE developed the concept of epidemiological intelligence in the Brazilian Unified National Health System and this paradigm shift was made possible by investments in physical/human resources, the integration of epidemiological, statistical and data science methods, as well as the incorporation of different data sources in data analysis.

- Abstract
This article aimed to know a Shelter for women in situations of intimate partner violence at imminent risk of death located in the state of Rio de Janeiro from the perception of its professionals. To this end, this qualitative research adopted semi-structured interviews with seven professionals directly involved with the care and assistance to the Shelter's user population. The findings were interpreted through Bardin's thematic content analysis. Moreover, the profile of the women and children sheltered in 2021 was drawn based on the data provided by the institution. In conceptual terms, this work focused on intersectional feminist theoretical references. Among the results, we argue that the shelter is permeated by contradictions, from its normative idealizations to institutional practices: on the one hand, the shelter represents the possibility of salvation, that is, of interrupting the escalation of violence and, therefore, preventing femicide. However, on the other hand, it appears as an upside-down prison, which "incarcerates" the victims. We highlight the importance of thinking about new ways to ensure protection for women who need this shelter.

This paper describe a successful experience of promotion, prevention and support for breastfeeding developed by professionals from a basic health unit. This is a Breastfeeding Outpatient Clinic, implemented in a health center in Belo Horizonte, in August 2019. The Outpatient Clinic was established based on the perception of the unit's team that many mothers had difficulty breastfeeding, however, due to the work overload of this team, this assistance did not occur in a timely manner, resulting in early weaning. Initially, a meeting was held to sensitize the team on the breastfeeding indicators of the unit. Based on this knowledge, the implementation of a breastfeeding Outpatient clinic was proposed, aimed not only at dyad with difficulties in managing breastfeeding, but at all postpartum women in the area covered by that health center. A flow was created, through which it was established that all postpartum women who brought their children to carry out the heel prick test at the unit would be referred to the Breastfeeding Outpatient Clinic to perform this service. With the improvement of care, the users of the health center started to breastfeed for longer, which reflected in the improvement of the unit's indicators.

- Abstract
The COVID-19 pandemic generated a large volume of scientific productions with different quality levels. The speed with which knowledge was produced and shared worldwide imposed on health management the challenge of seeking ways to identify the best available evidence to support its decisions. In response to this challenge, the Department of Science and Technology of the Brazilian Ministry of Health started offering a service to produce and provide scientific knowledge addressing priority public health issues in the pandemic scenario. Drug treatments, non-pharmacological measures, testing, reinfection and immunological response, immunization, pathophysiology, post-COVID syndrome and adverse events are among the topics covered. In this article, we discuss the strengths and lessons learned, as well as the challenges and perspectives that present a real example of how to offer the best scientific evidence in a timely manner in order to assist the decision-making process during a public health emergency.

Objective: To analyze the association between intimate partner violence during pregnancy (IPVP) and quality of life (QOL)., Methods: A cross-sectional study was conducted with pregnant women receiving care in Primary Health Care in the municipality of Criciúma, Santa Catarina state, Brazil, in 2022; QOL was assessed in the physical, psychological, social relationship and environmental domains using WHOQOL-Bref instrument; IPVP was evaluated by means of the World Health Organization Violence Against Women; Crude and adjusted linear regression analyses were performed., Results: A total of 389 pregnant women were evaluated; IPVP was observed in 13.6% of cases; in the adjusted analysis, IPVP remained associated with physical, psychological and social relationship domains; pregnant women who experienced IPVP had a reduction in their QOL score by 9.77, 11.07 and 8.95 points, respectively, when compared to those who did not experience IPVP., Conclusion: IPVP was associated with poorer QOL in the physical, psychological and social relationships domains. Health services equipped to address and prevent violence against pregnant women are essential., Main Results: Intimate partner violence (IPV) was observed in 13.6% of pregnant women and was associated with poorer quality of life in the physical, psychological and social relationship domains., Implications for Services: The results emphasize the need for an intersectoral approach in addressing the issue, with specialized healthcare centers for situations of violence integrated with social assistance and public security., Perspectives: Development of intersectoral policies and actions that strengthen existing ones and ensure social and healthcare assistance to pregnant women victims of violence and their children, given the negative impact of IPVP on quality of life.

Objective: To evaluate the combined impact of videofluoroscopic swallow study (VFSS) and therapeutic feeding and swallowing interventions on clinical outcomes in children with oropharyngeal dysphagia (OPD)., Methods: This was an uncontrolled longitudinal analytical study in which OPD patients were evaluated before and after VFSS. Children ≤ 24 months of age diagnosed with OPD in a clinical setting and undergoing VFSS for investigation and management of OPD were included in the study. The study participants received therapeutic feeding and swallowing interventions after having undergone VFSS, being followed at an outpatient clinic for pediatric dysphagia in order to monitor feeding and swallowing difficulties. Respiratory and feeding outcomes were compared before and after VFSS., Results: Penetration/aspiration events were observed in 61% of the VFSSs (n = 72), and therapeutic feeding and swallowing interventions were recommended for 97% of the study participants. After the VFSS, there was a reduction in the odds of receiving antibiotic therapy (OR = 0.007) and in the duration of antibiotic therapy (p = 0.014), as well as in the odds of being admitted to hospital (p = 0.024) and in the length of hospital stay (p = 0.025). A combination of oral and enteral feeding became more common than oral or enteral feeding alone (p = 0.002)., Conclusions: A high proportion of participants exhibited penetration/aspiration on VFSS. Therapeutic feeding and swallowing interventions following a VFSS appear to be associated with reduced respiratory morbidity in this population.

Introduction: Reduced muscle strength and low-exercise capacity are well documented in adults, but there are few studies examining those impairments in children and adolescents after kidney transplantation. The objective of this study was to evaluate peripheral and respiratory muscle strength and the association with submaximal exercise capacity in children and adolescents after kidney transplant., Methods: Forty-seven patients between six and 18 years of age clinically stable after transplantation were included. Peripheral muscle strength (isokinetic and hand-grip dynamometry), respiratory muscle strength (maximal inspiratory and expiratory pressure), and submaximal exercise capacity (six-minute walk test - 6MWT) were assessed., Results: Patients had a mean age of 13.1 ± 2.7 years and an average of 34 months had elapsed since the transplantation. Flexors of the knee showed a significant reduction in muscle strength (77.3% of predicted) and knee extensors had normal values (105.4% of predicted). Hand-grip strength and maximal respiratory pressures (inspiratory and expiratory) also were significantly lower than expected (p < 0.001). Although distance walked in the 6MWT was significantly lower than predicted (p < 0.001), no significant correlation was found with peripheral and respiratory muscle strength., Conclusion: Children and adolescents after kidney transplantation have reduced peripheral muscle strength of knee flexors, hand-grip, and maximal respiratory pressures. No associations were found between peripheral and respiratory muscle strength and submaximal exercise capacity.

Objective: To identify strategies to increase adherence to systemic arterial hypertension (SAH) treatment and describe the barriers and facilitators for implementing these strategies in primary health care (PHC)., Method: A rapid evidence review was performed. We included systematic reviews with or without meta-analyses, published in English, Spanish or Portuguese, covering adults (age 18 to ≤ 60 years) with SAH followed in the PHC. Searches were performed in nine databases in December 2020 and updated in April 2022. The systematic reviews were assessed for methodological quality using the AMSTAR 2 tool., Results: Fourteen systematic reviews on treatment adherence strategies and three on barriers and facilitators for implementation were included. Regarding methodological quality, one systematic review was classified as moderate, four as low, and the others as critically low. Four strategies were identified as options for health policies: actions performed by pharmacists; actions performed by non-pharmaceutical health professionals; self-monitoring, use of mobile apps, and text messages; and subsidies for the purchase of medicines. Low digital literacy, limited access to the internet, work process and incipient training were barriers for professionals. The users' educational and health literacy levels, accessibility to health services and good relationships with professionals were facilitators., Conclusions: Positive effects of strategies related to pharmaceutical care, self-monitoring, and the use of cell phone applications and text messages were identified to increase adherence to the treatment of SAH in the context of PHC. However, for implementation purposes, it is necessary to consider barriers and facilitators, in addition to the methodological limitations of the analyzed systematic reviews.

- Abstract
The covid-19 vaccine confers direct protection and reduces transmission rates of the virus and new variants. Vaccines from Pfizer/BioNTech and CoronaVac have been cleared for children in Brazil. They are safe, effective, and immunogenic. There are no known complications associated with the use of steroids or vaccines in pediatric patients with covid-19 and nephrotic syndrome. With or without immunosuppression, these patients are not at increased risk of severe covid-19, and steroids are safe for them. A milder form of covid-19 occurs in patients with chronic kidney disease without the need for hospitalization. The vaccine response may be reduced and/or the duration of antibodies after vaccination may be shorter than in the general population. However, considering risk of exposure, vaccination against covid-19 is recommended. It is believed that patients with hemolytic-uremic syndrome are at higher risk of severe covid-19. Vaccination is recommended, although specific data on the safety and efficacy of the covid-19 vaccine are limited. There is agreement that the benefits of induced immunity outweigh the risks of immunization. Vaccination against covid-19 is recommended for children and adolescents needing kidney transplantation or who have undergone transplantation. These patients present decreased immune response after vaccination, but immunization is recommended because the benefits outweigh the risks of vaccination. Current recommendations in Brazil stipulate the use of the messenger RNA vaccine. This paper aims to provide pediatric nephrologists with the latest knowledge about vaccination against covid-19 for children with kidney disease.

- Abstract
SARS-CoV-2 infection and its clinical manifestations (COVID-19) quickly evolved to a pandemic and a global public health emergency. The limited effectivity of available treatments aimed at reducing virus replication and the lessons learned from other coronavirus infections (SARS-CoV-1 or NL63) that share the internalization process of SARS-CoV-2, led us to revisit the COVID-19 pathogenesis and potential treatments. Virus protein S binds to the angiotensin-converting enzyme 2 (ACE2) initiating the internalization process. Endosome formation removes ACE2 from the cellular membrane preventing its counter-regulative effect mediated by the metabolism of angiotensin II to angiotensin (1-7). Internalized virus-ACE2 complexes have been identified for these coronaviruses. SARS-CoV-2 presents the highest affinity for ACE2 and produces the most severe symptoms. Assuming ACE2 internalization is the trigger for COVID-19 pathogenesis, accumulation of angiotensin II can be viewed as the potential cause of symptoms. Angiotensin II is a strong vasoconstrictor, but has also important roles in hypertrophy, inflammation, remodeling, and apoptosis. Higher levels of ACE2 in the lungs explain the acute respiratory distress syndrome as primary symptoms. Most of the described findings and clinical manifestations of COVID-19, including increased interleukin levels, endothelial inflammation, hypercoagulability, myocarditis, dysgeusia, inflammatory neuropathies, epileptic seizures and memory disorders can be explained by excessive angiotensin II levels. Several meta-analyses have demonstrated that previous use of angiotensin-converting enzyme inhibitors or angiotensin receptor blockers were associated with better prognosis for COVID-19. Therefore, pragmatic trials to assess the potential therapeutic benefits of renin-angiotensin-aldosterone system inhibitors should be urgently promoted by health authorities to widen the therapeutic options for COVID-19., (Copyright © 2023 Sociedade Portuguesa de Cardiologia. Objective: To synthesize the evidence about the effects of telehealth interventions provided through smart-phone apps and text messages on the behavior of adults regarding healthy food consumption., Method: A rapid systematic review of the literature was performed through searches in nine electronic databases to identify systematic reviews published in English, Portuguese, and Spanish that evaluated telehealth strategies compared to face-to-face interventions to improve dietary intake in the adult population (18 to 59 years old). Searches were performed in November 2020 and updated in April 2022. The included systematic reviews were assessed for methodological quality using the AMSTAR 2 tool., Results: Five systematic reviews were included. Methodological quality was moderate in one review and critically low in four. There was a dearth of studies comparing the use of telehealth strategies with face-to-face interventions for the promotion of healthy eating in adults. The most consistent results refer to an increase in the consumption of fruits and vegetables with the use of an app or text messages, in addition to improvement in the dietary habits of people with diabetes or glucose intolerance with the use of text messages., Conclusion: Positive effects were observed on healthy eating outcomes for most interventions using mobile apps or text messages; however, the findings refer to a few clinical trials with small samples that were analyzed in the systematic reviews covered in the present rapid review, most of which had low methodological quality. Thus, the current knowledge gap warrants the performance of further methodologically robust studies.

- Abstract
Objective: To analyze the spatiotemporal distribution of the burden of occupational cancer in Brazil and federative units between 1990 and 2019., Methods: Data were extracted from the Global Burden of Disease (GBD) study. Deaths from cancer whose attributable risk factor was occupational carcinogens were considered. Spatial analysis was performed with the first and last years of the series (1990 and 2019). Age-adjusted mortality rates were used to estimate the global Moran's Index (Moran's I), and the local indicator of spatial association (LISA) to identify clusters in the country with the respective statistical significance. The occupational cancer mortality rate, adjusted for age, was analyzed based on its trend for Brazil and federative units, in the period between 1990 and 2019., Results: Between 1990 and 2019, occupational cancer mortality rate showed a decreasing trend (R2=0.62; p<0.001) as well as the burden of disease indicator - DALY (R2=0.84; p<0.001). However, mortality is increasing in most states, suggesting that a minority of federative units induce the country's global trend. There is also the development of a spatial pattern of autocorrelation, indicating clusters of states with low mortality and DALY rates in the Northeast and high values in the South of the country., Conclusion: The overall decreasing trend in the trend of occupational cancer masks the heterogeneity across states. This scenario may be associated with the diversity of economic activities, and suggests a decentralized and equitable plan for occupational cancer surveillance.

The Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) statement, published in 2009, was designed to help systematic reviewers transparently report why the review was done, what the authors did, and what they found. Over the past decade, advances in systematic review methodology and terminology have necessitated an update to the guideline. The PRISMA 2020 statement replaces the 2009 statement and includes new reporting guidance that reflects advances in methods to identify, select, appraise, and synthesise studies. The structure and presentation of the items have been modified to facilitate implementation. In this article, we present the PRISMA 2020 27-item checklist, an expanded checklist that details reporting recommendations for each item, the PRISMA 2020 abstract checklist, and the revised flow diagrams for original and updated reviews.

Objective: To describe the IMPACTO-MR, a Brazilian nationwide intensive care unit platform study focused on the impact of health care-associated infections due to multidrug-resistant bacteria., Methods: We described the IMPACTO-MR platform, its development, criteria for intensive care unit selection, characterization of core data collection, objectives, and future research projects to be held within the platform., Results: The core data were collected using the Epimed Monitor System® and consisted of demographic data, comorbidity data, functional status, clinical scores, admission diagnosis and secondary diagnoses, laboratory, clinical, and microbiological data, and organ support during intensive care unit stay, among others. From October 2019 to December 2020, 33,983 patients from 51 intensive care units were included in the core database., Conclusion: The IMPACTO-MR platform is a nationwide Brazilian intensive care unit clinical database focused on researching the impact of health care-associated infections due to multidrug-resistant bacteria. This platform provides data for individual intensive care unit development and research and multicenter observational and prospective trials.

Introdução: A diálise peritoneal (DP) é importante para a pediatria. Este estudo mostrou dados de centros brasileiros que utilizam DP pediátrica., Método: Estudo transversal, observacional, descritivo com questionário eletrônico. Incluiu-se pacientes de 0-18 anos em DP cadastrados nos bancos de dados dos diversos centros. Questionário preenchido anonimamente, sem dados de identificação. Foi adotada metodologia quantitativa., Resultados: 212 pacientes estão em DP no Brasil (agosto, 2021). 80% têm menos de 12 anos de idade. A maioria realiza DP automatizada e 74% são dependentes do Sistema Único de Saúde. Em 25% dos centros faltou material de DP e em 51% os pacientes pediátricos foram convertidos de DP para HD., Conclusão: A maioria dos pacientes tinha menos de 12 anos e era dependente do SUS. A escassez de insumos aconteceu em 25% dos centros. Esses dados apontam para o problema da sustentabilidade de DP, única alternativa de TRS em crianças muito pequenas.

The scope of this article is to analyze vaccination coverage rates in children under one year of age during the period from 2015 to 2020 in the state of Minas Gerais (MG). It involved an ecological, time-series study on vaccination coverage in children under 1 year of age, considering the 28 Regional Health Management/Superintendencies (GRS/SRS) of MG as the unit of analysis. The following immunobiological vaccine coverage was analyzed: Bacillus Calmette and Guérin, human rotavirus, pneumococcal 10, pentavalent, meningococcus C, yellow fever, and polio vaccines. The Prais-Winsten autoregressive model was employed for trend analysis. The year 2020 stands out, as it presented the lowest proportion of GRS and SRS that reached the recommended vaccine coverage goals for the immunobiologicals analyzed. Regarding the analysis of the coverage trend, 8 of the 28 GRS/SRS showed a decreasing trend for at least 5 of the 7 immunobiologicals evaluated. We observed a downward trend in the vaccination coverage of at least five immunobiologicals in eight of the GRS/SRS, with emphasis on the Pentavalent vaccine, which showed a downward trend in vaccination coverage in 60.71% of the GRS and SRS.

Objective: To evaluate social inequalities of Brazilians in alcohol consumption and cell phone use while driving motor vehicles., Methods: Cross-sectional study conducted with people who drive (n=23,474) in 2019. The outcomes adopted were cell phone use and alcohol consumption while driving, associated with the variables gender, age group, skin color, education and macro-region of housing and analyzed using the slope index of inequality using logistic regression., Results: The inequalities related to alcohol consumption and driving were identified in adults with brown skin color (7.8) linked to the North region (6.8). As for cell phone use while driving, they were higher for the younger age group (19.4) and individuals with higher education (27.1)., Conclusion: Cell phone use and alcohol consumption while driving motor vehicles have social inequalities regarding the age group and education, and skin color and macro-region respectively.

- Abstract
This article analyzes the discourses within Health Promotion (HP) practices in Primary Health Care (PHC) that constitute actions of resistance and counter-conduct to the neoliberal governmentality inscribed in these tools. Drawing upon descriptive and exploratory field research, together with a qualitative approach, we interviewed 23 PHC workers in a municipality in southern Brazil. The results point out possibilities to enhance HP in PHC: strengthening training activities within health units; problematizing Social Determinants of Health (SDH); methods to encourage participation; collectivization of health demands; appreciation and reinforcement of achievements and collective actions; recovery of the local community's culture and habits; and HP advocacy in PHC.

This study analyzes the economic, racial, and geographic inequalities in risk behaviors for chronic non-communicable diseases of Brazilian adults. This is a cross-sectional study conducted with data from the 2019 Vigitel (Risk and Protective Factors Surveillance System for Chronic Noncomunicable Diseases Through Telephone Interview). The analyzed risk behaviors were smoking, alcohol abuse, physical inactivity, overweight, regular consumption of soft drinks or artificial juice drinks, and non-regular consumption of fruits, legumes, and vegetables. Inequalities in risk behaviors were assessed considering Brazilian's schooling level and their dwelling region, via the slope index of inequality (SII). Equiplots graphs were also built to better illustrate the inequalities. Stata svy command was used for all analyses due to the complexity of the sampling process. In total, 52,395 patients were evaluated. Significant inequalities in risk behaviors for chronic non-communicable diseases were observed: most risk behaviors were concentrated in those with low schooling. Smoking and soft drinks consumption were more observed in the Southern region of Brazil. Public policies are necessary to reduce the inequalities found, allowing for improvement in health indicators of the Brazilian population.

49. Factors associated with umbilical cord clamping in term newborns.

Objective: To identify factors associated with umbilical cord clamping in term newborns and to compare the recording of clamping time in the medical record with what was observed., Method: Cross-sectional study, with 300 mothers-infants, in a university hospital. Clamping time and medical records were observed, and a structured questionnaire was applied to postpartum women for sociodemographic variables. Bivariate analysis, multivariate Poisson Regression model, and Kappa concordance test were performed., Results: The percentage of late/optimal clamping observed was 53.7%. The associated factors were skin-to-skin contact in the delivery room (PR = 0.76; 0.61-0.95; p = 0.014), position of the newborn below the vaginal canal (PR = 2.6; CI95%: 1.66-4.07; p < 0.001), position of the newborn at the vaginal level (PR = 2.03; CI95%: 1.5-2.75; p < 0.001), and need for newborn resuscitation in the delivery room (PR = 1.42; CI95%; 1.16-1.73; p = 0.001). Kappa concordance level of the professionals, records compared to the observation was: nurse 0.47, obstetrician 0.59, and pediatrician 0.86., Conclusion: the identification of associated factors and the comparison between recording and observing the clamping time can help in the planning and implementation of improvements for adherence to good practices at birth.

50. [Continuous support during childbirth to reduce cesarean sections: evidence brief for policy].

- Abstract
Cesarean section rates have increased globally in the last 30 years, representing more than 56% of total births in Brazil. Considering the impacts of elective cesarean sections in maternal and child health, this review aimed to present an option to reduce these rates. We used the SUPPORT tools to summarize the best evidence. We included systematic reviews published in English, Portuguese, or Spanish, which focused on non-clinical interventions. Continuous support during labor and birth was described in four systematic reviews as an option to reduce cesarean rates (down to -51%). It also decreased unnecessary clinical interventions during labor, the duration of labor, and the likelihood of delivering a baby with a low Apgar score at five minutes. Moreover, continuous support improved breastfeeding, postpartum recovery of women during the puerperium period, and increased women's satisfaction with the birth experience and the possibility of spontaneous vaginal deliveries. The continuous support was beneficial for women and newborns. The intervention fosters respect, safety, and quality during birth, besides reducing hospital costs. Therefore, it is an effective strategy that should be adopted in hospitals and other health services to improve women's access to their rights.
