Abstract Objective To evaluate the risk factors and outcomes in patients surgically treated for subaxial cervical spine injuries with respect of the timing of surgery and preoperative physiological parameters of the patient. Methods 26 patients with sub-axial cervical spine fractures and dislocations were enrolled. Demographic data of patients, appropriate radiological investigation, and physiological parameters like respiratory rate, blood pressure, heart rate, PaO2 and ASIA impairment scale were documented. They were divided pre-operatively into 2 groups. Group U with patients having abnormal physiological parameters and Group S including patients having physiological parameters within normal range. They were further subdivided into early and late groups according to the timing of surgery as Uearly, Ulate, Searly and Slate. All the patients were called for follow-up at 1, 6 and 12 months. Results 56 percent of patients in Group S had neurological improvement by one ASIA grade and a good outcome irrespective of the timing of surgery. Patients in Group U having unstable physiological parameters and undergoing early surgical intervention had poor outcomes. Conclusion This study concludes that early surgical intervention in physiologically unstable patients had a strong association as a risk factor in the final outcome of the patients in terms of mortality and morbidity. Also, no positive association of improvement in physiologically stable patients with respect to the timing of surgery could be established.

Abstract Objective To study the results of only posterior decompression and instrumentation in dorsal and dorsolumbar spine tuberculosis. Methods The patients (n = 30) who were included in this study had dorsal or dorsolumbar spine tuberculosis, with or without neurological deficit, and with or without deformity. All 30 patients were managed by only posterior approach decompression and instrumentation. We studied cases for correction and maintenance of deformity at dorsal and dorsolumbar spine, functional outcome by the Oswestry disability index (ODI) and visual analogue scale (VAS) scores, as well as neurological outcome by the Frankel grade. Results In the current series, 30 patients were operated with single stage posterior decompression and instrumentation, and showed significant improvement in neurological status and functional outcomes, which were accessed by the ODI score, VAS score, and Frankel grade. Conclusion The posterior (extracavitary) approach provides optimum access to the lateral and anterior aspects of the spinal cord for good decompression. It facilitates early mobilization and avoids problems of prolonged recumbency, provides better functional outcome, and significantly better sagittal plane kyphosis correction.

In isolated intersection’s actuated signal control, inductive loop detector layout plays a crucial role in providingthe vehicle information to the signal controller. Based on vehicle actuations at the detector, the green time is extended till a pre-defined threshold gap-out occurs. The Federal Highway Administration (FHWA) proposed various guidelines for detec-tor layouts on low-speed and high-speed approaches. This paper proposes single and multiple advance detection schemes for low-speed traffic movements, that utilizes vehicle actuations from advance detectors located upstream of the stop-line, which are able to detect spill-back queues. The proposed detection schemes operate with actuated signal control based on lane-by-lane gap-out criteria. The performance of the proposed schemes is compared with FHWA’s stop-line and single advance detection schemes in the VISSIM simulation tool. Results have shown that the proposed single advance detection schemes showed improved performance in reducing travel time delay and average number of stops per vehicle under low volumes while the multiple advance detection scheme performed well under high volumes.

In a time when many national governments are rhetorically, if not practically, turning away from global collaboration and emphasizing economic nationalisms, understood here as a plurality of historically and politically contextualized policies, the authors in this thematic issue explore the possible contributions of comparative ethnographic analyses of economic nationalisms to interdisciplinary, transnational analyses. In this introduction to the thematic section, the editors note the turn to economic nationalisms in the current context of global crises, review the literature on economic nationalisms, and illustrate the strong political and historical variations in economic nationalisms within even the same nation, using India as an example. The three ethnographic articles in this thematic section are discussed in relation to each other. They offer examples from Turkey, Italy, Guatemala and the U.S., with a shared focus on economic nationalisms and the reliance on the labor of transnational migrants displaced by war, land grabs, and climate change even as those vital contributors to national economies are denied cultural and national citizenship – an irony rendered all the more visible during the COVID-19 pandemic.

This essay reflects on the consequences for ethnography of the reversal of the direction of the anthropological encounter – what exactly happens when anthropologists from the Global South undertake fieldwork among populations in the Global North? I will argue that this does not merely correct historical imbalances of anthropological knowledge production, thereby changing the substance of that knowledge itself, although of course that is a goal worth pursuing, as has cogently been pointed out by the “world anthropologies” critique of hegemonic disciplinary practices. Using insights drawn from my own research site in the US heartland, where I conducted fieldwork initially in 1989-91, and then again during a “focused revisit” in 2015, I suggest that lasting misunderstandings between the anthropologist and the research participants are themselves a rich source of data and human possibility. My contention is that non-standard and long-term fieldwork of this kind may help reveal dimensions of the practice of ethnography that would otherwise remain obscured.

Objective: To identify the clinicopathological correlation of E-cadherin expression in metastatic and non-metastatic oral squamous cell carcinoma (OSCC). Material and Methods: A total of 90 paraffin-embedded tissue sections of OSCC were retrieved from the registry. The total selected samples were 45 cases each from the primary lesions of metastatic and non-metastatic OSCC. One section was subjected to routine Hematoxylin and eosin stain and another to immunohistochemical analysis for E-cadherin expression. Results: A non-significant (p˃0.05) increased expression is seen in the non-metastatic group compared to the metastatic group, with predominantly membrane as the staining site in either group. However, the expression of E-cadherin did not reveal any statistically significant association with independent variables such as age, gender, and adverse habits of the patients (p>0.05). ABSTRACT OBJECTIVE To assess the ownership and use of mosquito nets in 2014, in Mozambique. METHODS This observational and cross-sectional study assessed, in February and March 2015, 69 districts (nine of 11 provinces of Mozambique) that have benefited from the mass distribution of mosquito nets. The Lot Quality Assurance Sampling methodology was used. Each locality was denominated supervision area. The Lot Quality Assurance Sampling opts for a minimum of 19 households (in this case, we decided for a minimum of 100 households per district) from each supervision area to assess an indicator (in this case, two indicators were assessed: ownership and use of mosquito nets). Two questions guided the research: a) received a mosquito net; b) used a mosquito net the night before. RESULTS A total of 6,725 households were assessed. Eighty three percent of them had received mosquito nets in the campaign. Of the 6,232 respondents, 82.0% said they used mosquito nets the night before. The districts of the provinces with low coverage of ownership and use were Tete (69.5% and 60.0%, respectively), Zambezia (79.0% and 60.0%, respectively), and Gaza (81.6% and 70.7%, respectively). The largest coverage of ownership and use were observed in the districts of Nampula (96.7% and 93.8%, respectively) and Niassa (86.0% and 85.4% respectively). CONCLUSIONS In the districts assessed, the progression of ownership and use of mosquito nets is satisfactory. Nampula and Niassa are the only provinces where ownership and use are at desired levels.
