Medical corporations have secured their authority for professional self-regulation from the State and have responded throughout history by way of their scientifically grounded statements which confer rationality to State interventions in the field of health. During the pandemic in Brazil, there was a solid right-wing association between state governance and medical governance that supported a policy to address the pandemic based on scientific denialism, off-label use of ineffective drugs, and the disregard of certain preventive measures, which together were responsible for most of the more than 700,000 deaths that occurred in the country. This article examines the phenomenon of the interaction of these two forms of governance in a context of authoritarian neoliberalism and thus seeks to reflect on the necessary limits to medical autonomy at all its levels.

OBJECT relations , *INTERPERSONAL relations , *MOTHER-infant relationship , *CLINICAL medicine , *PERSONALITY - Abstract
Emotional development implies the need for communicational exchange from its base. In human relationships, implicit and explicit communications are at stake, extending from the !eld of mother-infant relationships to adult interpersonal relationships, including cultural experiences. Under the auspices of clinical care, the therapist deals with non-communicative aspects of human existence, re5ected both in non-communication present in pleasant states of stillness and relaxation and in defensive withdrawal in the face of the threat of annihilation of the central core of the personality. Also noteworthy is the state of permanent isolation and incommunicability that is characteristic of the true self, in favor of which the therapist is directed as a guardian and not as an interpreter, fundamentally. In this sense, this article presents a re5ection on the theory of communication and its opposites in the psychoanalytic work of Donald W. Winnicott, aiming to describe it in its semantic range and clinical applicability. It presents as a contribution the deepening of the theme from elucidative combinations that articulate communication to theoretical-experiential conjunctures, such as: mother--baby amalgam; inviolable core of self; object relations; transferential relationship in the analytic setting; non-defensive communication and the analyst's living presence. It is hoped that this study can contribute to future research on the subject. [ABSTRACT FROM AUTHOR]

Objective: To compare the results of hospital performance indicators of an urgency/emergency unit that has implemented the "Lean in Emergencies" project. Methods: A descriptive, retrospective, cross-sectional study with a quantitative approach, in two phases (before and after), whose population was composed of documents from 7,590 admissions in medical, surgical, and orthopedic clinics, seen in the emergency room. The data were submitted to the normality test and later to the Wilcoxon non-parametric test, considering a 5% significance level (p = 0.05). Results: The variables that showed significant difference before and after the implementation of the Project "Lean in Emergencies" were: Number of admissions (p = 0.03); Number of Exits (discharges + deaths) (p = 0.03); Bed Turnover (p = 0.03); Interval for bed replacement (p = 0.03) and Available operational beds (p = 0.04). The number of admissions; outputs and bed turnover increased by 13.2%, 20.6% and 11.7%, respectively, and the time to bed replacement interval decreased by 61.1%. Conclusion: The indicators that presented significant results demonstrate the project's effectiveness in important aspects for managing the number of beds in the emergency room, optimizing the patient's length of stay in the hospital environment. TEAMS in the workplace , *KEY performance indicators (Management) , *STRATEGIC planning , *TIME , *TRAVEL , *ACQUISITION of data , *SEASONS , *CLINICAL medicine , *ATHLETIC ability , *SPORTS events - Abstract
The present study presents the following objectives: i) verify the performance indicators of the teams of the Brazilian Championship A Series from the years 2015, 2016, 2017, 2018, and 2019 regarding the distances travelled to the matches; ii) compare the score achieved per match according to the quality of the opponent and distance travelled. All games of the Brazilian Championship Series A of 2015 until 2019 were considered for sample. The data were collected on the InStatScout website (InStat Limited, Limerick, Ireland); the CBF website was used to consult the match venue and the quality of the teams (www.cbf.com.br). The distances travelled in a straight line were calculated on a specialized website. The results showed that the teams that did not travel presented better offensive and defensive performance indexes (p<0,001) and higher scores per match (p<0,001). The teams that made long trips (>1000km) showed greater deficits in performance indices (p<0,001). Teams tended to earn fewer points per match when facing high-quality opponents (p<0,001), and the score decreased with long trips (p<0,001). Thus, travel logistics are fundamental and another factor to be considered by the technical committees when planning the training loads and strategies for the matches. [ABSTRACT FROM AUTHOR]

HUMAN beings , *POPULATION aging , *CLINICAL medicine , *PSYCHOANALYSTS , *PSYCHOANALYSIS , *PSYCHOTHERAPISTS - Abstract
One of the objectives of this article is to present and publicize the work of a group of psychoanalysts from the Sociedade Brasileira de Psicanálise de São Paulo (SBPSP), comprised of Affiliated Members and Associate Members who have been gathering for about 8 years to provide clinical care to patients aged 55 and older. Based on a sample of clinical data collected at the Centro de Atendimento Psicanalítico (CAP, Psychoanalytic Care Center), we encountered a picture of aging among the population that comes to us for help. Through clinical vignettes of some of the patients we also follow how analytical listening can benefit them at this specific moment of life, correlating the clinic with some psychoanalytic concepts that help us to understand aging, transience, and the finitude of human beings. We conclude with reflections on the need for a greater number of analysts interested in the subject and the service, as we perceive the contribution of psychoanalysis to people going through this period of existence, as well as the need to expand this type of activity in our society. To address clinical pharmacist importance for an intensive care unit, elucidating clinical pharmacy, as well as pharmacotherapeutic follow-up, through pharmaceutical interventions and pharmacotherapeutic follow-up in ICU. This is an integrative review built according to descriptors Pharmaceutical, Intensive Care Unit and Clinical Pharmacy. Manuscripts published between years 2008 and 2022, in English, Brazilian Portuguese and Spanish, in studies conducted in humans and in review scope were considered. Five scientific articles were included, in which relevance not only of clinical pharmacist presence was emphasized, but also of their attributions in health care. There is a greater cluster of journals that describe the importance of clinical pharmacy service in intensive care units. Thus, the clinical pharmacist role is fundamental in attention to clinical care and monitoring, as well as in the follow-up of the patient's pharmacotherapy. [ABSTRACT FROM AUTHOR]

CLINICAL psychologists , *CHILD care , *HUMAN beings , *CLINICAL medicine , *PHENOMENOLOGY - Abstract
In this article, we seek to reflect on clinical care for children based on Martin Heidegger’s Da-sein analytics and existentialphenomenological psychology. We begin with a brief historical review of the emergence of the notion of childhood. Then, we analyze existentially being-a-child from a philosophical point of view, taking as reference the hermeneutic phenomenology undertaken in Being and Time. The child, like every human being, has the way of being of Da-sein, being-there. Being in the world, it is always and from the beginning in the disposition and understanding of the orientations and interpretations already given and settled by tradition. In our time, the donation of meaning that already disposes of us as a usable entity is unveiled as the modern technique. In the horizon of technique, the child, like all of us, is always called upon to surrender, to produce results that are already defined a priori. By responding or not to this call, so-called adaptation or development problems may arise, considered failures in this epochal horizon. Faced with these problems, the clinical psychologist is called upon to adjust what “does not go well”, occupying a place that could be that of adapting to the calculating technical mode or of calmly meditating on it. Our bet is on this second option, in an anti-positive-liberating care practice that, with the child and his family, seeks to expand the understanding there at stake, allowing new meanings and possibilities of dealing with what exists at this stage of life to come to light without the rigidity of the identity frameworks that prevail in our time. We begin the article with a section that seeks to go beyond the historiographical record of the childhood phenomenon; we introduce an ontological-fundamental analysis of the phenomenon; we propose possible ways of caring for the child, highlighting the ones in the psychological clinic, which are not blindly given over to technical use; and, finally, we narrate a clinical situation. HOSPITAL care of children , *PSYCHOANALYTIC theory , *CLINICAL medicine , *CLINICAL psychology , *HEALTH care teams , *STUTTERING - Abstract
This experience report arises from the clinical practice of psychology in an Integrated and Multiprofessional Residency Program, with a focus area on children and adolescents health. From the study of the social construction of childhood, the meaning of infans (one who does not speak), and the context of a hospitalized child, the themes that permeated clinical care, were addressed. As an objective, we will seek to reflect on the importance of psychological care to hospitalized children, focusing on the following themes: silence, transference (patient x analyst relationship), and the experience of hospitalization. The clinical experience in the care of a child, who appears in front of all members of the health team refusing to speak, will be addressed, a child who “does not speak”. The theoretical discussions and constructions carried out, during clinical care and supervision, took place under the guidance of psychoanalytic theory. The theme of silence is discussed throughout the text, a factor present in this psychoanalytic treatment and extremely relevant to the reported experience. In the end, we talk about the importance of psychological listening for the child’s silence, the establishment of the transference relationship, the creation of a unique space of speech, and the changes that the treatment implied for the child. Background: Indicators of effectiveness in the dimension of patient-centered care. Objective: To validate indicators for determining attributes that make it possible to measure the effectiveness of hospital care in the dimension of patient-centered care. Methodology: Quantitative survey type study, carried out in 2017 to judge eight indicators, considering 11 attributes, by 52 specialists, using the Delphi technique and the electronic Survey Monkey® platform. The internal consistency was tested by Cronbach's Alpha 0,98. A valid indicator was considered when there was a minimum consensus of 70% between the responses of the participants, in all attributes. Results: Four indicators were validated: patient satisfaction; surgeries canceled on the scheduled day; hospital recommendation; and the patient's involvement in their own care, with high internal consistency (0.98). Conclusion: The validated indicators favor the evaluation of the effectiveness of hospital care oriented to the valorization of the person as a care center. Non-validated indicators are the subject of future studies. [ABSTRACT FROM AUTHOR]
