Situs inversus totalis is a rare congenital anomaly. We report a case of a 78-year-old woman with this condition and ischemic coronary artery disease who underwent myocardial revascularization. The cardiac catheterism showed severe proximal stenosis with aneurysms in the anterior interventricular branch, diagonal artery, and right coronary artery. The procedure was done with the surgeon positioned in the left hand side of the patient, with the right internal thoracic artery anastomosed to the anterior interventricular branch, and to two more vein grafts. Very few cases have been reported in the world and this is the third case in Brazil, and the first in the world with coronary aneurysms.

The authors report the clinical case of a 32 years old woman who suffered a spontaneous dissection of the common trunk, anterior descending, intermediary and circumflex coronary arteries, on the 6th post partum day. The diagnosis of acute myocardial infarction was made, through ECG and enzymatic studies and the patient was transferred to a Coronary Unit. Coronary angiography disclosed those lesions and the patient underwent a triple coronary bypass graft, under extracorporeal circulation. The post operative course was uneventful. Eight months after the operation she was found in good condition, free of symptoms and with a normal pattern of life.

The authors describe a case reported in a young female with antecedents of aortic pathology and renovascular hypertension submitted to renal artery surgery admitted to emergency with an extensive acute myocardial infarction, treated with thrombolysis. After coronariography we identify, as etiological factor, a spontaneous coronary dissection involving the descending anterior coronary artery and circumflex coronary artery with no affect on the left main coronary artery. About this case, the authors make a brief review of the literature emphasizing the therapeutic options.

A 57 year-old-man with acute aortic dissection (DeBakey type I) who developed right coronary artery dissection without acute myocardial infarction. He was successful surgically treated and became asymptomatic.

Ischemic cardiomyopathy in childhood is related with congenital and acquired pathologies which could lead to serious cardiac sequelae as myocardial infarction and sudden death. Unfortunately, when coronary artery malformations are excluded, it is difficult in some cases to be completely certain on the etiology of the arterial coronary disease, due the lack of pathognomonic diagnostic tests. Case report of 6 year old child, with severe coronary artery disease: aneurysm of main branch of left coronary artery; left anterior descending branch and right coronary artery occlusions with accentuated myocardial dysfunction. We believe that the diagnosis of Kawasaki disease is presumptive in this patient. The principal aspects to elucidate the etiology of coronary artery disease are analysed, as well as the importance of identifying patients without known previous illness, but with severe coronary artery lesions, as in this reported case.
