Objective: To validate a multiprofessional protocol for the care of patients with delirium admitted to an intensive care unit., Method: Methodological study with the purpose of confirming with experts the care recommendations proposed in the protocol. For the content validation process, the content validity index of ≥ 0.90 was considered., Results: Of the 48 recommendations submitted to content validation, only four did not reach consensus through the content validity index. The multiprofessional protocol for patients with delirium in the intensive care unit included care related to the diagnosis of delirium, pause in sedation, early mobilization, pain management, agitation and delirium, cognitive guidance, sleep promotion, environmental interventions, and family participation., Conclusion: The multiprofessional protocol qualifies the care provided to critically ill patients with delirium, improving clinical outcomes.

Objective: To describe the multiprofessional care for the management of critical patients in delirium in the ICU from the evidences found in the literature., Methods: This integrative review was carried out in the period from February 1 to June 30, 2016 through searches on PubMed, Scopus, Web of Science, and CINAHL, with the following descriptors: delirium, critical care e intensive care units, which brought up 17 original papers., Results: A bundle and a guideline, two systematic reviews, evidence 1a and four clinical trials, evidence 1b and 2b, cohort and observational studies were found. The multiprofessional care was presented to better understand the diagnosis of delirium, sedation pause, early mobilization, pain, agitation and delirium guidelines, psychomotor agitation, cognitive orientation, sleep promotion, environment and family participation., Conclusion: The care for delirium is wide and not specific, which determines its multifactorial aspect.

Introduction: Delirium is an acute, transient and fluctuating neuropsychiatric syndrome that is common in medical wards, particularly in the geriatric and palliative care population., Material and Methods: We present a brief literature review of the definition, pathophysiology, aetiology, diagnosis, prevention and treatment of delirium and its social and economic impact., Results and Discussion: Delirium is under-recognized, especially by health professionals, and is associated with higher morbidity, mortality and economic burden. Moreover, the presence of delirium interferes with the evaluation and approach to other symptoms. Furthermore, it causes significant distress in patient's families and health professionals. The best treatment for delirium is prevention which is based on multidisciplinary interventions that addresses the main risk factors. The scientific evidence for the treatment of delirium is scarce. Non-pharmacological approaches are usually the first choice, and includes environmental, behavioural and social strategies. Pharmacological options, mainly antipsychotics, are a second-line treatment used essentially to prevent self harm., Conclusion: The recognition and prevention of delirium are crucial. Health professional education and training, patient clinical monitoring and families support are mandatory. Considering the impact of delirium on patients, relatives, health services and professionals we must be more aware of delirium and, why not, make it the 7th vital sign.

Delirium is the more common form of acute cerebral dysfunction in Intensive Care Unit (ICU), characterized by transitory and alternate changes in conscience and cognitive impairment that occur with physical disease. The incidence in ICU is high, reaching 89% of the patients. The diagnosis is exclusively clinic and could be used any one of the validated scales: Confusion Assessment Method for the ICU (CAM-ICU) or Intensive Care Delirium Screening Check-list (ICDSC), which should be applied, at least, once per shift by trained professionals. Its treatment and, more important, its prevention are known for several years. Currently there are new, safer and more effective therapeutic strategies for Haloperidol, such as Dexmedetomidine. Two new definitions of Delirium exist, besides the usual (hiperactive, hipoactive, mixed), which are: Subsyndromal Delirium and Delirium Superimposed on Dementia. Although they don't have all the same risks, the occurrence of Delirium in general leads to a decrease in cognitive function at long term and to a higher mortality. In spite of that, only 33% of the intensivists actively seek diagnoses of Delirium.

Objective: To conduct a multinational survey of intensive care unit professionals to determine the practices on delirium assessment and management, in addition to their perceptions and attitudes toward the evaluation and impact of delirium in patients requiring noninvasive ventilation., Methods: An electronic questionnaire was created to evaluate the profiles of the respondents and their related intensive care units, the systematic delirium assessment and management and the respondents' perceptions and attitudes regarding delirium in patients requiring noninvasive ventilation. The questionnaire was distributed to the cooperative network for research of the Associação de Medicina Intensiva Brasileira (AMIB-Net) mailing list and to researchers in different centers in Latin America and Europe., Results: Four hundred thirty-six questionnaires were available for analysis; the majority of the questionnaires were from Brazil (61.9%), followed by Turkey (8.7%) and Italy (4.8%). Approximately 61% of the respondents reported no delirium assessment in the intensive care unit, and 31% evaluated delirium in patients under noninvasive ventilation. The Confusion Assessment Method for the intensive care unit was the most reported validated diagnostic tool (66.9%). Concerning the indication of noninvasive ventilation in patients already presenting with delirium, 16.3% of respondents never allow the use of noninvasive ventilation in this clinical context., Conclusion: This survey provides data that strongly reemphasizes poor efforts toward delirium assessment and management in the intensive care unit setting, especially regarding patients requiring noninvasive ventilation.

Background and Objectives: Postoperative delirium is a common complication in the elderly whose pathophysiology is not well known. It seems to be related to central cholinergic activity. A literature review was undertaken to describe the etiology, diagnosis, prophylactic strategies, and treatment of postoperative delirium., Contents: Probable etiologies, diagnosis, prophylaxis, and treatment of postoperative delirium in the elderly were described., Conclusions: The incidence of postoperative delirium is higher in the elderly, and prophylactic measures should be taken to reduce the mortality associated with this complication.
