ABSTRACT Objective: To verify the temporal trend of neonatal mortality in the health regions of Pernambuco between 2000 and 2020. Method: A time-series ecological study was conducted, analyzing the total neonatal mortality rate and its early and late components. For regression analysis, Joinpoint Regression was applied, trends were classified, and annual and average percentage changes were calculated for the period, with a significance level of 95%. Results: The average neonatal mortality rate in Pernambuco was 11.5 during the studied period. A decreasing trend in neonatal mortality rate was observed, especially in the early component. The region where the state capital is located showed the fastest decrease across all components. Conclusion: The temporal trend of neonatal mortality was decreasing; however, the rate of reduction was not uniform across the health regions of the state, and the implementation of the Mãe Coruja Pernambucana Program did not impact the trend in neonatal mortality.

Introduction: One of the health indicators of the newborn children is low birth weight (LBW) - weight less than 2,500g - which can be determined by the influence of several reasons, such as, maternal, fetal, placental and environmental factors. Objective: To determine the prevalence and factors associated with LBW in the state of Minas Gerais (MG), Brazil. Method: Cross-sectional study which analyzed data from 259,863 live births, in MG, from Brazilian Live Birth Information System (SINASC), in 2011. Of these, 242,487 records met the inclusion criteria: birth weight equal to or greater than 500g, no malformations, 22 weeks or more of gestational age. The prevalence of the LBW was determined. The crude and adjusted prevalence ratios were determined by Poisson regression. Results: The prevalence of LBW was 9.2%. The prevalence was higher in mothers with lower educational level, primiparity, dead children from previous pregnancies, prenatal care with less than 5 visits to doctor, gestational age less than 37 weeks, multiple pregnancy, and female newborn. The prevalence was lower in the North, Northeast, Northwest, Eastern of South, Southeast, Southern Triangle, East and Jequitinhonha health macro-regions. Conclusion: The LBW prevalence was high, and the variables associated with higher LBW prevalence were gestational age, multiple pregnancy, educational level, number of prenatal visits to doctor, parity, gender, place of birth. The lower prevalence was observed in the health macro-regions in which the socioeconomic and sanitary conditions are worst, characterizing the LBW paradox.

ABSTRACT This study evaluated whether there were differences in vaccination coverage rates between municipalities with different vaccination strategies regarding the species and the coverage rates during different day periods. The vaccination period was categorized as M1 (morning; 09h00 am - 11h00 am), M2 (morning/afternoon; 11h00 am - 01h00 pm), M3 (afternoon; 01h00 pm - 03h00 pm), and M4 (afternoon; 03h00 pm - 05h00 pm). A repeated measures model compared the vaccination rate between periods and municipalities. The interaction between time and municipality was statistically significant (P < 0.01), indicating that the vaccination rate difference between periods depended on the municipality analyzed. Results of the difference between proportions analysis revealed that municipality B vaccinated 3.3% (2.2%-4.4%, 95% confidence limits) less dogs and 20.1% (17.9%-22.4%, 95% confidence limits) less cats than the municipality A. In municipality A, the vaccination rate in period M1 was higher than in periods M3 (P = 0.07) and M4 (P < 0.01). The vaccination rate was higher in M2 than in M4 (P < 0.01). In municipality B, the vaccination rate in period M1 was higher than in M2 (P = 0.01). The vaccination rate in M2 was lower than that observed in M3 (P = 0.01) and M4 (P = 0.01). Based on these results, mass vaccination campaigns have better results during the week, with the highest vaccination rate at lunchtime.

This article aims to describe the activities conducted by the National Childhood Cancer Plan Working Group to support the development of national childhood cancer plans in Latin America and the Caribbean in the period 2019–2022, and to present the stage of plan development. The Working Group activities were supported by the Pan American Health Organization and St. Jude Children’s Research Hospital, which is the World Health Organization (WHO) Collaborating Centre for Childhood Cancer. Year after year, the workshops and activities developed with the Working Group mobilized key stakeholders: pediatric oncologists, representatives of the Ministry of Health, foundations supporting childhood cancer initiatives, and hospital administrators. As of February 2023, one regional framework is in place, approved by the Council of Ministries of Health of Central America and the Dominican Republic, nine countries are currently implementing national plans or laws that include childhood cancer, and ten countries are writing new plans. The WHO three-step framework helped to guide the Working Group activities. All plans were supported by a situational analysis, which highlighted the importance of having systematized data for evidence-based policies. To increase implementation success, an accompanying budget and timeline help to ensure the adequate implementation of the interventions. More than anything, committed stakeholders remain the most fundamental element to successfully write and approve a national childhood cancer plan. This is an opportunity to share these countries’ experience so the strategy can be adapted to support other countries developing a childhood cancer plan and extended to other public health areas.

ABSTRACT Objective: This work aimed to estimate the avoidable COVID-19 cases and deaths with the anticipation of vaccination, additional doses, and effective non-pharmacological interventions in Brazil. Methods: We developed a susceptible-exposed-infectious-recovered-susceptible model based on epidemiological indicators of morbidity and mortality derived from data obtained from the Health Information System of the Ministry of Health of Brazil. The number of cases and deaths was estimated for different scenarios of vaccination programs and non-pharmacological interventions in the states of Brazil (from March 8, 2020, to June 5, 2022). Results: The model-based estimate showed that 40 days of vaccination anticipation, additional vaccine doses, and a higher level the nonpharmacological interventions would reduce and delay the pandemic peak. The country would have 17,121,749 fewer COVID-19 cases and 391,647 avoidable deaths Conclusion: The results suggest that if 80% of the Brazilian population had been vaccinated by May 2021, 59.83% of deaths would have been avoided in Brazil.

Objective : Identify challenges in the applicability of ontological models that represent relationships and elements of health management competencies. Methods: Scoping Review, according to Joanna Briggs Institute (JBI) and the guiding question: "What are the challenges in the applicability of ontological models that represent the health management competencies described in health in the literature?". Included national and international studies, English, Portuguese and Spanish, in the time span of 2015 to 2020. Results : The search strategy was applied in eight databases; 2024 articles were found and eight were selected as the final sample. The studies highlighted the following difficulties in this theoretical framework: interpretation of complex, heterogeneous and dispersed data by health managers, lack of automation in patient monitoring, lack of training of health managers, inadequate task distribution and workload of the nursing team and also, deficiency in decision-making shared between health professionals and the patient. Conclusions: The application of ontological models in health management systems resulted in time optimization, data integration and better management decision-making in order to positively help the work of health professionals and managers. The so-called health maps are an instrument for the promotion, democratization and right to public information in health. They have also been a tool for contextualizing and better targeting inequalities in the public health sector and other related sectors. This article aims to explain the elaboration of the health map of Mauritania, and its value in the planning and reordering of the supply of health services at all levels of the country's health pyramid in order to respond to the growing requests for information generated by various organizations, but mainly from the Mauritanian health sector. The methodology used to develop a participatory map of health assets in a specific African country is presented. The survey technique was used as a research procedure, as it allows data to be collected and processed quickly and efficiently. Geographic information systems were an important support to the work. The health map drawn up was a fundamental tool for the management and organization of the territory in terms of health planning for the following five years. It has enabled health resources to be organized in order to achieve a supply of services more suited to the needs of the population, avoiding duplication of care, while at the same time becoming a powerful instrument to support decision-making. This territorial knowledge of the country allowed and made possible the subsequent implementation of Health Development Plans in the regions with the greatest health inequalities. The Region of the Americas is facing accelerated demographic and epidemiological changes. As these trends will continue in future years, long-term care needs are expected to rise. How can countries respond to these challenges? We propose that countries in the Region should invest in the implementation of long-term care systems. Considering the heterogeneity in the Region, we propose a strategy based on three components: (i) understanding the problem; (ii) thinking about solutions; and (iii) building support and consensus. Depending on each country’s needs and capacities, these three elements suggest short-term and long-term actions and goals, from generating better information on long-term care needs to the implementation of long-term care systems. Longterm care is a relevant issue for the Region today. The task is challenging, but countries need to embrace it and move forward before it is too late.

Introduction: The dissemination of the COVID-19 pandemic in Europe, namely in Portugal, demanded an organizational and clinical reaction from the Portuguese National Health Service. With the unpredictable impact of COVID-19 infected patients redefining hospital logistics, reducing non-priority elective care and extending the hospital capacity for critical care patients made mobilizing a significant part of human resources a priority. We conducted a national survey to monitor the contribution and the role of anaesthesiologists belonging to the 53 Portuguese National Health Service hospitals in the first wave fight against the pandemic. Material and Methods: This prospective cross-sectional observational study used a weekly survey sent to the Directors of the Anaesthesiology Departments of all Portuguese National Health Service hospitals, between the period of 13th April and 21st June 2020. Directors were asked about human resources, hospital logistics, anaesthetic activity and residency programs in their departments as well as contingency plans facing the impact of the pandemic growth in the PNHS. Results: Contingency strategy for all Portuguese National Health Service hospitals planned for a total of 1524 level III critical care beds during the initial phases of the pandemic, an increase of 151% from the existing 607 level III critical care beds in Portugal in January 2020. This re-configuration effort of the Portuguese National Health Service was only possible due to the partial or total suspension of non-urgent elective activity that reached over 90% of these institutions in the first pandemic months (March and April) and the deployment of anaesthesiologists from their normal activities to the treatment of critical care patients. During the peak of the first pandemic wave, 209 anaesthesiology specialists and 170 trainees (22.9% of the total anaesthesiologist’s staff in the Portuguese National Health Service) were deployed in critical care. There was an almost complete interruption of the residency program rotation in 70.4% of hospitals with anaesthesiology residents, between March and April 2020. Conclusion: During the first pandemic wave there was an effective and fast reorganisation of the Portuguese National Health Service in order to increase level III critical care beds, which might have contributed to the low mortality rates in Portugal. We believe that this could have also been a result of the contribution given by all public anaesthesiology departments.

Abstract Objectives: to analyze how the disseminationof COVID-19 occurred in the cities of Pernambuco State, Northeast in Brazil. Methods: descriptive, exploratory and quantitative study whose units of analysis were the 184 cities and Fernando de Noronha Archipelago which constitutes the state of Pernambuco. Geoprocessing techniques used QGis 3.14.16 and were presented in figures. Results: the first city to register a case of COVID-19 was Recife, in 129 days there were already confirmed cases of the disease in all the cities in the state and including Fernando de Noronha Archipelago. Only 117 cities informed the patients’ sexin public reports since the first case and only 88 cities mentioned the patients ’ age. Conclusion: there was a fast spread of COVID-19 in the state of Pernambuco, showing the inability of the Health Surveillance services to control the transmission, especially in smalltowns.

ABSTRACT Objective: To describe the reorganization of nursing work in an intensive care unit of a public hospital due to the COVID-19 pandemic. Method: Report of the experience from February to April 2020, about the reorganization of a unit. Results: The description of the experience was divided into four moments: Definition of the cohort isolation; Reorganization of the intensive care units as General and Respiratory; Health care teams and work shifts; and Wearing and removing protective clothing by the teams. Conclusion: The COVID -19 pandemic brought numerous challenges to the management of intensive care units. The socialization of management experiences can contribute to the definition of new strategies, including in the post-pandemic period.

Objective: analyze the hospitalizations prevalence for Ambulatory Care Sensitive Conditions (ACSCs) and its main causes in elderly according to the healthy macroregions in Santa Catarina State. Methods: this is a descriptive ecological study where were picked Hospitalizations System data (SIH - DATASUS). Results: in the study period, there were 262.104 hospitalizations of elderly for ambulatory Care Sensitive Conditions in Santa Catarina. The most frequent diseases were Heart Failure Lung Diseases, Cerebrovascular Diseases, Angina and Diabetes Mellitus. The regions from the state that had more hospitalizations were the South and West. However, it was found that there was a decrease in the total number of elderly hospitalizations during the study period, suggesting a good resolution of primary health care in the State. Conclusion: the ACSCs in elderly is relevant when it is compared to the total number of hospitalizations made by the Unified Health System, in this population, in the state of Santa Catarina. The Emergency Medical System of Ouro Preto and Mariana operates together, continuously, 24 hours a day, seven days a week, using four ambulances fixed located in three bases scattered throughout the region. Given the degree of uncertainty of the system coupled to the number of parameters capable of interfering in its efficiency, it is interesting to determine and evaluate indicators to make a diagnosis of the complete situation of the emergency system in the region in line with the service quality analysis. In this sense, this work proposes discrete event simulation models that, after validation, were able to analyze (1) the location of ambulance bases in centralized and decentralized configurations, (2) the behavior of the system in meeting the estimated real demand, (3) the performance of the system after the removal of an ambulance using two ambulance dispatch policies, and (4) the changes in system demand throughout the day. Objective. Patient safety is challenging for health systems around the world, particularly in low- and middleincome countries such as Guatemala. The goal of this report is to summarize a strategic planning process for a national patient safety plan in Guatemala. Methods. This strategic planning process involved multiple stakeholders, including representatives of the Guatemala Ministry of Health and Social Assistance, medical leadership from across the public health system, and academic experts from Guatemala and the United States of America. We used mixed methods (quantitative and qualitative surveys) and a nominal group technique at a national symposium to prioritize patient safety challenges across Guatemala, and subsequent meetings to develop a national patient safety plan. Results. This national patient safety plan outlines four domains to advance patient safety across the public hospital system over a five-year period in Guatemala: leadership and governance, training and awareness, safety culture, and outcome metrics. For each domain, we developed a set of goals, activities, outputs, and benchmarks to be overseen by the Ministry of Health. Conclusions. With this national patient safety plan, Guatemala has made a long-term commitment to improving patient safety across the public hospital system of Guatemala. Future efforts will require its extension to all levels of the Guatemalan health system.

RESUMO A partir da descentralização da gestão Sistema Único de Saúde (SUS), surge a necessidade de se discutir a capacidade de governo, ou de gestão, no âmbito municipal, para a implementação dessa política. Este estudo objetiva analisar a capacidade de governo de Secretarias Municipais de Saúde em municípios baianos, tendo por base a experiência dos gestores. Foi aplicado questionário on-line, subdividido em 03 dimensões, a 15 gestores representantes de Secretarias de municípios de pequeno (porte I) e grande porte (porte II), nas 09 macrorregiões de saúde do estado. Foi utilizada uma matriz de pontuação, que permitiu a quantificação e a classificação dos municípios em cada uma das dimensões e quanto à sua capacidade de governo. Obteve-se que 12 dos municípios apresentaram capacidade moderada, e 03 apresentaram baixa capacidade de governo. A dimensão relacionada ao desenho organizativo das Secretarias foi aquela em que os municípios apresentaram melhor desempenho, seguida por sistemas de trabalho nessas organizações e trajetória profissional dos gestores. Objective: to evaluate how an intelligent tool can contribute to the monitoring of municipal health indicators. Method: descriptive study, carried out in the city of São José, Santa Catarina, Brazil. Data collection took place in January 2021. For data analysis, descriptive statistics were performed. The data were organized according to the national pact health indicators achieved in the years 2017-2019, made available by the State of Santa Catarina. All ethical aspects were respected. Results: the development and application of the smart tool enabled trend analysis, the achievement of goals and indicators, of the national agreement, enabling directors to have a systemic view of health planning at the municipal level. Conclusion: strategic planning and local health programming through the development of an intelligent tool, allowed the transformation of the relationships between the supply and demand of services, redirecting the reorganization of health practices, directing them to the solution of problems. Objective: Evaluate the implementation of the Ministry of Health's "Action Plan: Border Vaccination Strategy - Agenda 2022" in the Brazil's 33 twin cities and evaluate the increase in the country's vaccination coverage (VC)., Methodology: Pre-post community clinical trial. Implementation of the strategy was analyzed, and pre- and post-intervention VC were compared in two stages: P1 (pre-intervention) and P2 (post-intervention). Based on statistical analyses of P1 and P2 coverage, calculations were made of municipal averages, standard deviation, and difference in VC between the two periods., Results: Integration was observed between the primary health care (PHC), surveillance, immunization, and special indigenous health district (DSEI) teams, although there were difficulties, for example, in relation to migratory flows. While immigration flows present challenges in the areas of immunization, PHC, and DSEI, the difficulties are compounded by the polarization of these services, which hinders intersectoral integration. After carrying out the workshops, a total of 50 977 doses were administered in the general population in the 33 twin cities. There was an increase in vaccination coverage in children up to 1 year of age in the locations evaluated after the intervention, which may be relevant in terms of increasing VC in Brazil., Conclusion: There was an increase in vaccination coverage in children up to 1 year of age in the locations evaluated after the intervention, helping to increase VC in Brazil., Competing Interests: Conflito de interesse.
