Abstract

OBJETIVO: Estudar as dificuldades apresentadas pelos pacientes deficientes auditivos idosos relacionadas ao uso da amplificação, e elaborar um material didático em DVD, contendo informações sobre o uso e o manuseio do aparelho de amplificação sonora individual (AASI). MÉTODOS: Participaram do estudo dez indivíduos idosos, com idade entre 65 a 80 anos, com deficiência auditiva do tipo neurossensorial de grau leve a moderado, novos usuários de AASI. Após o período de adaptação foi aplicado um questionário de múltipla escolha para avaliar as dificuldades encontradas. RESULTADOS: Os resultados demonstraram que no processo de adaptação do AASI o individuo idoso apresenta maior dificuldade no uso e manuseio do aparelho e, principalmente, na inserção e remoção do molde auricular. A partir dos resultados, foi elaborado um material didático em DVD contendo informações importantes sobre o uso e manuseio do AASI. CONCLUSÃO: Analisando as dificuldades apresentadas pelos pacientes deficientes auditivos idosos é importante enfatizar a necessidade da elaboração de um material o didático sobre a orientação e a adaptação do aparelho objetivando o melhor aproveitamento da amplificação.PURPOSE: To study the difficulties faced by hearing impaired elderly patients related to the use of hearing aids, and to elaborate a didactic material on DVD, containing information about the use and handling of the hearing aid. METHODS: Ten elderly subjects, with ages ranging from 65 to 80 years, new users of hearing aids with mild to moderate sensorineural hearing loss participated in the study. After the adjustment period, a multiple choice questionnaire was applied to assess the difficulties they faced. RESULTS: Results showed that in the adaptation process with their hearing aids, elderly subjects have more difficulty to use and handle the apparatus and, mainly, to insert and remove the earmolds. Based on these results, a didactic material containing important information about using and handling hearing aids were elaborated on DVD. CONCLUSION: After considering the difficulties presented by hearing impaired elderly patients, it is important to emphasize the need for didactic material regarding orientation and adaptation of the hearing aids, in order to improve the use of amplification.

Abstract

Mitochondrial myopathy is caused by the absence and/or insufficiency of L-carnitine, a quaternary enzyme responsible for transporting free fatty acids into the mitochondria. The primary function of the mitochondria is to produce energy, contributing to proper cell functioning. Muscular lipidosis causes abnormalities in enzymes that metabolize fat, resulting in the accumulation of harmful amounts of fats in tissues. The aim of this study was to present the case study of patient B.D., a 37-year-old woman diagnosed with muscular lipidosis with L-carnitine deficiency at 6 years old, and describe the speech-language follow-up performed at a hearing care clinic. The first entry in the patient's medical chart was on 03/05/1989, with continuous use of 2g/day of L-carnitine prescribed by a neurologist. The mother reported that B.D. had difficulty hearing and was inattentive, which became more evident when she started school. In 1988 the patient was diagnosed with moderate bilateral sensorineural hearing loss and began using behind-the-ear (BTE) hearing aids in 1989, after which her academic performance and communication improved. In 1998 she switched to Completely in Canal (CIC) hearing aids, which are more discreet, provided better sound localization and greater high frequency gain, although her hearing thresholds worsened slightly. She completed her graduate studies and currently works at a large financial institution. It was concluded that early neurological diagnosis and speech-language intervention enabled adequate language development in the patient.

Abstract

O implante coclear é um grande avanço no tratamento da surdez. Há grandes evidências que a audição bilateral traz diversas vantagens ao ser humano e muito mais quando se trata de audição binaural. OBJETIVO: Este artigo tem por objetivo descrever o primeiro caso operado no Brasil, sendo o terceiro país das Américas (México e Colômbia) a realizar, com implante binaural Neurelec Digisonic SP® demonstrando a técnica cirúrgica. MÉTODO: Descrição da técnica cirúrgica. RESULTADOS: Ver técnica cirúrgica. Procedimento ocorreu sem intercorrências. DISCUSSÃO: O efeito "squelch", a somação binaural, a localização da fonte sonora e o efeito sombra da cabeça são umas das principais razões que justificam a superioridade da reabilitação binaural. O custo do tratamento deve ser levado em conta em políticas de saúde pública. CONCLUSÃO: Custo do implante coclear é um dos grandes limitadores da bilateralidade na reabilitação desse grupo de pacientes, porém, com essa tecnologia, é possível solucionar esse problema, expondo o paciente a poucos riscos, com o uso de uma técnica cirúrgica pouco invasiva. Cochlear implants represent a significant breakthrough in the treatment of hearing loss. Evidence indicates bilateral hearing brings significant benefits to patients, particularly when binaural hearing is offered. OBJECTIVE: To describe the first case of implantation of a Digisonic SP® Binaural Neurelec device in Brazil (the third implant placed in the Americas, after Mexico and Colombia) and the chosen surgical approach. METHOD: Description of a surgical approach. RESULTS: The procedure was successfully completed. DISCUSSION: The squelch effect, binaural summation, location of the sound source, and the shadow effect of the head are listed among the reasons to explain the superiority of binaural rehabilitation. Cost of treatment must be considered in the development of public health policies. CONCLUSION: The cost of cochlear implants has been one of the main impediments to bilateral rehabilitation. The Digisonic SP® Binaural Neurelec device addresses this issue and exposes patients to less risk through a minimally invasive implantation procedure.

5. The influence of cognitive aspects and auditory processes on the hearing aid acclimatization in the elderly.

Abstract

Purpose: To verify the speech recognition processes in a monoaural task and in a binaural integration task with dichotic listening, and to compare them to the cognitive processing in elderly subjects pre and post hearing aid acclimatization., Methods: Participants were 60 elderly subjects of both genders, ranging in age from 61 to 85 years. They were evaluated for cognitive aspects by means of the Mini-Mental State Examination (MMSE) and the Alzheimer's Disease Assessment Scale (ADAS-Cog). To evaluate speech processes in a monoaural task and in a binaural integration task with dichotic listening, the Percentage Index of Speech Recognition Test (PISR) and the Dichotic Digits Test (DDT) were completed before and after using the hearing aid. Data was descriptively presented and compared with non-parametric tests., Results: Speech recognition at the PISR was low (mean 69.6%) and the DDT presented better digit recognition on the right ear (mean 74.1%) than on the left ear (mean 61.1%). The sample presented improved performance on speech tests after the use of hearing aids, with relevant difference between the means of the first and the second evaluations. The PISR was not correlated with cognitive aspects. The DDT was associated with both cognitive tests, especially for the left ear., Conclusion: All subjects were acclimatized, because there was improvement in speech recognition following the use of hearing aids. Subjects with cognitive problems present poorer performance in speech recognition processes with dichotic listening.
