Introduction: In Portugal, a colorectal cancer screening program based on faecal immunochemical test followed by colonoscopy was shown to be cost-effective for individuals between 50 and 74 years old. We report the first findings of the implementation of a population-based program In Northern Portugal. Material and Methods: In the pilot phase, eligible subjects were allocated either to a direct mailing invitation or to primary care centers. In the first year of program implementation, we assessed the uptake rate, the faecal immunochemical test -positivity rate, the diagnostic yield of advanced neoplasia, and the quality parameters for post-faecal immunochemical test + colonoscopy. Results: We invited 100 501 eligible subjects (49% male with a median age of 55 years). Of these, 5228 participated in the pilot phase and 95 273 participated in the first year of the program. In the first year of the program, the adherence was 29%, with a positivity rate of 5% and a 60% compliance to colonoscopy. The faecal immunochemical test-detection rate of advanced neoplasia was 0.35/1000 subjects, and the positive predictive value at post- faecal immunochemical test + colonoscopy was 44% and 2% for advanced adenoma and invasive cancer, respectively. No major adverse events were reported after colonoscopy. Discussion: The suboptimal adherence to faecal immunochemical test and post-faecal immunochemical test + colonoscopy remains the most urgent step to be addressed. Conclusion: A centralized invitation system based on direct mailing was feasible and both colonoscopy quality and diagnostic yield were adequate antecipating the success of the programme.

Abstract: In recent months, the euro area has been under fire from the financial markets, spe-culators being blamed for the mounting of a kind of conspiracy whose ultimate goal would be the disintegration of the project's Economic and Monetary Union (EMU). In this paper, we discuss the true reasons of this situation with reference to four issues which the debate remains open since the early 90s: nominal convergence versus real convergence; need -or not- for the coordination of fiscal policies; adjustment mechanisms to specific shocks and the theory of optimum currency areas; and need -or not- for further progress in political integra-tion. Using data on the Portuguese case, we argue that, on several of these topics, the present framework of the EMU still reveals many weaknesses, which would underpin the current unsta-ble situation. In this context, we present some suggestions for strengthening the euro area, par-ticularly in terms of its economic governance.

Nos últimos meses, a eurozona estivo baixo o fogo dos mercados financeiros, sendo os especuladores acusados dunha especie de conspiración cuxo último obxectivo sería a desin-tegración do proxecto europeo de Unión Económica e Monetaria (UEM).Neste artigo discútense as verdadeiras razóns que levaron a esta situación, con especial refe-rencia a catro aspectos cuxo debate se mantén aberto desde comezos dos anos noventa do século XX: a converxencia nominal versus a converxencia real; a necesidade –ou non– de coor-dinar as políticas orzamentarias nacionais; os mecanismos de axuste aos shocks específicos e a teoría das áreas monetarias óptimas; e a necesidade –ou non– para avanzar na consecución da integración política. Utilizando datos sobre o caso de Portugal, evidenciamos que en varios destes temas a construción da UME segue poñendo de manifesto varias deficiencias que están na base da actual situación de inestabilidade. Nese contexto, preséntanse varias suxestións pa-ra fortalecer a zona euro, sobre todo no que respecta ao seu goberno económico.

OBJETIVO: Determinar a validade do enxerto autólogo de crista ilíaca não vascularizado no tratamento cirúrgico do ameloblastoma de mandíbula. MÉTODO: Nos Serviços de Cirurgia de Cabeça e Pescoço e Semiologia Bucal do Complexo Hospitalar Heliópolis, de 1980 a 2000, foram tratados 31 pacientes com ameloblastoma de mandíbula, dos quais sete receberam enxerto de crista ilíaca autólogo, fixos com placa de titânio do sistema A-0 (quatro casos) e aço inox (três casos), sendo portadores da variedade folicular (seis casos) e plexiforme (um caso). RESULTADOS: Nesta análise, foi utilizado o Teste de Hipótese para a média populacional com a variança desconhecida, houve exposição da placa em três casos (40%) quando a neoplasia ultrapassava a linha média e em quatro casos (60%) não incidiu nenhuma complicação. CONCLUSÕES: Apesar da incidência de exposição de placa, o método é indicado na reconstrução da mandíbula de pacientes com ameloblastoma.
