ABSTRACT This report described a case of a 56-year-old female patient with a history of pterygium excision and use of mitomycin C eye drops during the postoperative period who developed necrotizing infectious scleritis caused by Pseudomonas aeruginosa. The patient was admitted for treatment with intravenous antibiotic therapy, topical fortified antibiotics, systemic corticotherapy, and debridement of the necrotic ocular tissue. After 3 weeks, she presented with good clinical evolution and resolution of the infection. Mitomycin eye drops should be used cautiously, particularly after surgery. If mitomycin is used, it should be applied at the lowest concentration and for the shortest duration possible and should be avoided postoperatively to prevent these complications. Although scleral infection is a serious condition with a high risk of permanent vision loss, early and appropriate treatment can be critical for good clinical outcomes.

Being an active older adult with functional independence is what everyone seeks, but one of the main causes of death and disability in the elderly is falls, associated with femur fractures. This fracture leaves the elderly person confined to bed for weeks, months or even death, which may be due to the extent of his injury, generalized weakness associated with immobility, comorbidities, or due to respiratory complications arising during the hospitalization process. The work is a quantitative descriptive study, in which 1161 medical records were analyzed and, after selection criteria, 414 were used. through the analysis of electronic medical records and associating them with variables such as gender, age, presence of comorbidities, type and duration of surgery. In the results, the highest incidence of femur fractures occurred in women, the age group most affected in terms of comorbidities and post-surgery was 80 years old, there was no significant difference between affected lower limbs. Regarding the respiratory part, pneumonia was the disease that most appeared in the medical records, corroborating the findings in other studies. The work adds current data to the literature of a hospital that receives a large public representative of the subject of this study, that is, elderly people with fractures of the femur, and thus reinforces the need for attention both for the prevention of fractures and for the prevention of complications associated with them and the importance of a respiratory evaluation, as well as the surgical risks, for example, cardiac ones, are evaluated. Spinal trauma is any injury of external cause in the spine, which results in spinal cord injury (SCI), leading to motor, sensory, autonomic and psycho-affective changes, which may be transient or irreversible. The objective study was to describe the sociodemographic and clinical aspects associated with the length of stay of patients suffering from spinal trauma undergoing surgery at a University Hospital. Analytical, observational, descriptive, retrospective study with a quantitative approach, including patients admitted to a University Hospital, between January and December 2017, with only the data referring to the first hospitalization being collected, medical records with missing or incomplete information were excluded. With a sample of 18 medical records. Male predominance mean, age 32.72 years, brown, resident in the interior of the state, single. The main cause was a motorcycle accident, the most affected cervical region, diagnosed with dislocation, as a surgical procedure they underwent arthrodesis, and presented paraplegia in the postoperative period. The median hospital stays of 9.5 days, waiting time for surgery obtained a median of 4 days. It was observed that patients with infection (p = 0.007) and the waiting time for surgery (p <0.001), correlated with a longer hospital stay. The length of hospital stay is linked to the appearance of complications. The most associated type of infection was urinary tract infection. It is concluded that the variables infection and waiting time for surgery showed a statistically significant association with the length of hospital stay of patients with spinal cord injury undergoing surgery. Introduction: The standardization of a weaning protocol and extubation is presented as the solution to eliminate the possiblefactors that would lead to complications andfailure of extubation. Objectives: To review the scientific literature to characterise the weaningprotocols and discontinuation of mechanical ventilation in adults in cardiac surgery postoperative. Methodology: Conducted via literature research in databases SciELO, Medline, Lilacs and PubMed, with the following search terms: weaning / weaning, mechanical ventilation / mechanical ventilation, cardiac surgery / cardiac surgery and "AND" to join words. It was implemented a descriptive analysis. Results: We selected 15 articles published between 1998 and 2016. The most used weaning protocols involve Adaptive Support Ventilation (ASV at 100% of the minute volume) and the Spontaneous / Support Pressure modality. The criteria analyzedfor interruption of mechanical ventilation and that ensure its success were: gasometry, consciousness recovery, hemodynamic stability and body temperature. Conclusion: Adaptive Support Ventilation (100%) and Spontaneous were the most used for weaning. The perception of gasometry, followed by hemodynamic stability, resumption of consciousness and normalization of body temperature were the most used criteria for the decision to interrupt mechanical ventilation. ARTERIAL catheterization , *VASCULAR catheters , *SURGICAL complications , *TEACHING hospitals , *CATHETERIZATION , *CATHETERS , *HOSPITAL patients , *CENTRAL venous catheters - Abstract
Background: Central venous catheters are essential for management of hospitalized patients, but their insertion is subject to complications that can make them unusable and/or cause patient morbidity. There are few data on the incidence of these complications and the variables associated with these outcomes in Brazil. objectives: To determine the incidence of mechanical complications and failures of short stay central venous catheters fitted by the vascular surgery service at a teaching hospital and identify variables associated with their occurrence. Methods: This was a prospective cohort of 73 attempts to fit patients with a central venous catheter performed by the vascular surgery service at a teaching hospital from July to October of 2022. results: Mechanical complications occurred in 12 cannulation attempts (16.44%) and there were 10 failures (13.70%). The factors associated with mechanical complications were less experienced operators (p < 0.001), less specialized operators (p = 0.014), a failed attempt prior to requesting help from the vascular surgery service (p = 0.008), and presence of at least two criteria for difficulty (p = 0.007). Conclusions: The local incidence of mechanical complications and central venous cannulation failures was similar to rates described in the international literature, but higher than rates in other Brazilian studies. The results suggest that the degree of experience of the person fitting the catheter, history of a failed prior attempt, and presence of at least two criteria for difficulty identified before the procedure were associated with worse outcomes. SURGICAL complications , *COMPUTED tomography , *INTRAMEDULLARY fracture fixation , *FRACTURE fixation , *TRAUMATOLOGY , *PATIENT monitoring - Abstract
Background: Posterior malleolus fractures (PMF), as well as other ankle joint injuries, are known to occur concomitantly with distal tibia diaphyseal fractures (DTDF), especially when there is a spiral or oblique pattern. If neglected, such fractures can lead to iatrogenic displacement during intramedullary nailing as well as unsatisfactory results even when complete union of the tibial shaft has been achieved. Furthermore, it is accepted that posterior and medial malleolar fractures require fixation in most cases, thus avoiding loss of postoperative reduction. Aim: To evaluate the epidemiological profile of DTDF patients with spiral line and PMF treated in our Hospital. Method: Patients treated at the Orthopedics and Traumatology Service of the Hospital Universitário São Francisco na Providência de Deus - HUSF, located in Bragança Paulista - SP, Brazil, from January to December 2022 were included in the study. Age at trauma, presence of any comorbidities, trauma mechanism, treatment, occurrence of effective consolidation after treatment, and clinical evolution were observed. Results: In our study, the ages of the 5 patients ranged from 26 to 75 years, and the two patients who remained followed up in our Service showed consolidation, one of which was only partial due to loss of follow-up. Neither of the two patients monitored presented post-surgical complications, and both began to walk without assistance and without complaints after fixation. Conclusion: We believe that many PMF were not included in our sample due to lack of use of the Computed Tomography (CT), available on our Hospital. Therefore, we highlight the importance of CT for the diagnosis of these cases, which will probably increase the accuracy of the diagnosis, thus allowing adequate care to be provided to these patients. ELECTRONIC health records , *SURGICAL complications , *BIRTH weight , *SYNDROMES in children , *TRACHEOTOMY , *CHILD patients - Abstract
Objective: to describe the profile of indications, comorbidities and early postoperative complications of children and adolescents undergoing tracheostomy in a Brazilian referral hospital. Method: retrospective, cross-sectional and quantitative study, with analysis of electronic medical records of children and adolescents who underwent tracheostomy in a hospital in Botucatu, São Paulo, Brazil, from 2013 to 2019. The sample was compared by dividing the presence and absence of post-operative complications. early operative procedures, with regard to sex, age, syndrome, death, elective or urgency/emergency character and birth weight. Results: among 100 records, 55% were male, with an average of 3.03±3.10 years. Twelve different syndromes were identified in 16 patients. With regard to early postoperative complications, cork was the most frequent (13%) and was associated with the presence of syndromes (p=0.01). The most identified reason for tracheostomy was prolonged orotracheal intubation, while 44% died due to the severity of the underlying disease. Conclusion: pediatric tracheostomy is a safe procedure, more frequently realized due to failure of extubation, in genetic syndrome children younger than one year old. Cork is a frequent complication, associated to syndromes. [ABSTRACT FROM AUTHOR]

LINGUAL nerve , *THIRD molars , *SURGICAL complications , *ORAL surgery , *NERVOUS system injuries , *DENTIST-patient relationship - Abstract
The lingual nerve is one of the most important nerves in the oral cavity and is often injured during certain oral surgery procedures. Due to its location and possible anatomical variations, some surgical procedures that encompass this region can become complex and may result in intra- and post-operative complications, such as nerve damage. Among the various causes of these injuries are incorrect anaesthetic techniques, trauma to hard and soft tissue, especially of the lingual flap, and implant-supported fixed rehabilitations. The main objective of this study is to help dentists to clarify how to avoid and/or resolve most of these complications. Based on exhaustive bibliographical research, this work focuses on aspects such as the frequency of occurrence and degree of severity of lingual nerve injuries, as well as the most common ways to treat them. The sensorineural changes resulting from complications related to dental treatments are a growing concern of dentists and, therefore, in this study, some examples will be focussed on, namely the extraction of third molars, as one of the procedures most frequently responsible for the high number of lesions of the lingual nerve. Given the above, good pre-surgical planning is essential, and the dentist should collect as much information as possible, given the inherent complications, including sensory changes in the orofacial region, which, in the last case, can irreversibly interfere with speech, chewing and consequently in the patient's social interactions. APPENDECTOMY , *SURGICAL complications , *OPERATIVE surgery , *APPENDICITIS , *LAPAROSCOPIC surgery , *REOPERATION , *PUBLIC hospitals - Abstract
Introduction: Appendectomy is the treatment of choice for acute appendicitis. Although the preference for minimally invasive techniques is a worldwide trend, open surgery remains a reality in most public hospitals. The rate of postoperative complications varies according to the surgical technique employed. The present study aimed to compare the incidence of postoperative complications between open and laparoscopic appendectomy. Methods: This retrospective cohort study included patients undergoing appendectomy at the Hospital de Pronto Socorro de Porto Alegre between November 2015 and November 2019. Demographic and clinical data, duration of symptoms, surgical technique, intraoperative findings, use of abdominal drains or stomas, operative time, length of stay, surgeon's experience, and outcomes were assessed. Results: Three hundred and fifty-eight patients were included, predominantly male (58.9%), with a mean age of 32 ± 13.8 years; 58.1% underwent open surgery, 41.9% underwent laparoscopic surgery, and 8% required conversion. One third of the cases were classified as complicated. The mean operative time was shorter for open surgery (79.3 ± 38.8 vs. 104 ± 35.2 minutes; p < 0.001). The rate of postoperative complications was 21.2%, with a significantly higher incidence in the open technique (26.4% vs. 13%; p = 0.003). Length of stay, reoperation rate, and mortality did not differ between the techniques. Conclusions: Although open appendectomy is a safe and efficient procedure, associated with low morbidity and mortality rates, laparoscopy provides potential clinically beneficial advantages in terms of postoperative outcomes, even in complicated cases. Therefore, it should be routinely performed where laparoscopic equipment and skillful staff are available. [ABSTRACT FROM AUTHOR]

Currently, the main form of malignancy among women is breast cancer. With the advent of radiotherapy, conservative breast surgery (lumpectomy) has become a possible technique. However, radical mastectomy still maintains its indications in the face of a more advanced pathology or in patients with deleterious BRCA 1 / BRCA 2 mutations. Unfortunately, complications of the surgical wound exist. Faced with this problem, PICO ™ Softpor - Negative pressure wound therapy system, is a unique device that has been used in the negative pressure wound therapy system to improve wound healing, this includes improved perfusion, removal of excess fluid and edema leading to less tension in the surgical wound. The study aimed to report the experience of using the Wound Negative Pressure Therapy System through PICO ™ Softport in the surgical treatment of breast cancer through a case report. Patient M. R. V., female, 57 years old, G2P2A0, asymptomatic with a diagnosis of T4BN0M0 in the right breast and T1N0M0 in the left breast was submitted to bilateral modified radical mastectomy. We used bilateral dressings with the Wound Negative Pressure Therapy System in the form of PICO ™ Softport, which made it possible to reduce the incision healing complications. After surgical recovery, the patient underwent adjuvant radiotherapy, presenting himself until the moment of this stable report using anastrozole, being completely satisfied with the dermal healing result that the device provided. Objective: To describe the clinical, surgical, and geographic profile of patients submitted to corneal transplantation and followed up at a teaching hospital, and, with a description of the indications for the procedure and mean waiting time. Methods: A cross-sectional and retrospective study, including 40 eyes of 40 patients who underwent penetrating keratoplasty, from January 1, 2018 to December 31, 2019, followed up at a teaching hospital in Alagoas State. Data were collected using the surgical records of corneal transplants, from the Central Reporting, Procurement and Distribution of Organs and Tissues of Alagoas, and adjusted to the research objectives. Results: Of the corneal transplants studied, 52.5% (n=21) were performed in female patients; 62.5% (n=25) in the age group over 60 years; with a mean age of 59.17 years (± 20.4). All transplanted patients lived in the state of Alagoas, 60% of them in the intermediate metropolitan region of Maceió. The main indications for corneal transplantation were pseudophakic bullous keratopathy (27.5%, n=11), descemetocele (22.5%, n=9) and late graft failure (15%, n=6). Seventy percent (n= 28) of procedures were performed on the left eye and 65% (n=26) for optical purposes. There was an association of cataract extraction in 22.5% (n=9), and 5% (n=2) had perioperative complications. The mean waiting list time was 332.3 days (11 months); in that, 486 days (2 years and 4 months) for elective surgeries and 12.8 days for emergency surgeries. Conclusion: The waiting time for elective surgeries was long and inappropriate. The major indication for corneal transplantation was bullous keratopathy. Knowledge of the clinical profile of corneal transplants can enable identifying the risk groups for prevention and implementation of care, resulting in better prognosis, fostering implementation of internal and external policies to improve the procurementdonation system. Background: The kidney transplant is a foreign body in the patient's organism, thus constituting a procedure that triggers an intense inflammatory reaction. Objective: To assess the clinical characteristics of people undergoing kidney transplantation with immune sensitization. Methodology: A descriptive, documentary, and retrospective study with a quantitative approach to data treatment and analysis was carried out between June and August 2019. Results: The association between donor-specific antibodies (DSA) and graft dysfunction (p < 0.03) and pregnancy (p < 0.04) was statistically significant. The occurrence of complications was also associated with the length of hospital stay (p < 0.00) and serum creatinine levels in the first week after transplant (p < 0.006). Conclusion: The clinical characterization showed that most patients had graft dysfunction, DSA, and complications, meaning that transplanted patients require a critical and individualized follow-up. Objective: To understand the processes or rationalities of excluding factors in breast reconstruction in mastectomized women. Methods: This is an exploratory and descriptive research with a qualitative approach, developed at Napoleão Laureano hospital in João Pessoa - Paraíba, with a sample of 10 women who had mastectomies in the immediate postoperative period. Results: After collecting the testimonies, the categories were established from the content analysis, which were ignorance about breast reconstruction after mastectomy; fear of further complications and lack of desire for breast implant placement. regarding the type of mastectomy, only 20% underwent immediate breast reconstruction. Conclusion: It was evidenced that there was a failure of professionals to communicate the rights guaranteed by law to these women, even if their clinical conditions are impeding the performance of the procedure. Despite the achievement of the federal law that guarantees breast reconstruction, there are women who choose not to perform, either for fear or for conformity of the cure. [ABSTRACT FROM AUTHOR]

Total hip arthroplasty, an orthopedic surgical procedure often performed by hip joint wear, can lead to pain, disability, proprioceptive disorders, a deficit in balance, and muscle strength. Thus, a systematic review study was carried out to verify if patients undergoing total hip arthroplasty had altered muscle strength and balance. Searches were carried out in the following databases: Lilacs, Pedro, and Pubmed, as well as in the online Scielo library and the CAPES portal from August 2017 to June 2018. Studies carried out on a population over 18 years of age who underwent total hip arthroplasty surgery. The search initially provided 595 articles, of which 15 were selected for complete reading, of which only seven were included. The studies evidenced alterations of decrease of muscular strength and deficits in the balance, resulting in worsening of the individual's functional capacity. Thus, it is concluded that patients who underwent total hip arthroplasty surgery have alterations in balance and mainly in peripheral muscle strength. However, due to the scarcity of studies related to the subject, new research is necessary. [ABSTRACT FROM AUTHOR]

The objective is to report the surgical reintervention for enlarging margins in a patient diagnosed with cutaneous mast cell tumor. A canine, male, French Bulldog, 7 years old, was seen with a history of skin nodule removal in the abdominal region 15 days ago, with a histopathological diagnosis compatible with grade I mastocytoma and compromised surgical margins. Then, it was decided to perform a second surgical procedure to expand the safety margins, which was preceded by planning 5 cm of cutaneous margins and a layer of fascia and a muscular plane as a deep margin, in addition to inguinal lymphadenectomy. During the procedure, synthesis was possible without the use of reconstructive techniques, from the subcutaneous divulsion and the use of the Walking Suture pattern. There were no transoperative complications, and hematoma formation was the only postoperative complication observed. The material was sent for histopathological analysis, confirming low-grade cutaneous mastocytoma with free margins and no evidence of lymph node metastasis. Thus, adjuvant therapies were not instituted, and after 320 postoperative days, the patient was in excellent general condition, with no evidence of recurrence or distant metastases on chest radiography and abdominal ultrasound. It is concluded that surgical reintervention can be an alternative for the treatment of low-grade mastocytoma with compromised margins, and that histopathological diagnosis and surgical planning are essential to reduce trans and postoperative complications, increasing the chances of success and cure of the patient. PRACTICE of dentistry , *SURGICAL complications , *SALINE solutions , *SALINE irrigation , *DENTAL extraction , *SALINE injections - Abstract
Introduction: Bucosinusal communications are relatively frequent surgical complications in dental practice. Although several conditions are related to this type of communication, the exodontia of upper posterior teeth presents as the main etiological factor. They are diagnosed through fistulas, which communicate the buccal and sinus cavities, thus allowing the passage of food and other substances into the maxillary sinus, thus providing infectious pictures known as Sinusites. Objective: through the report of a clinical case of buccosalusal fistula, we will focus on the success of the closure of buccomasinal communication with Bichat Adipose Body. Technique report: male patient, 59 years old, leucoderma, with no systemic alterations, who attended the University of the Sacred Heart Postgraduation outpatient clinic reporting as a main complaint the fact of "getting fluids through the nose" during feeding and that this had started after performing a residual root exodon of element 16. It also reported that after the exodontia, an attempt was made to close the buccosinusal fistula through the repositioning of the vestibular flap and no success was achieved. After 45 days of conditioning the maxillary sinus mucosa through irrigations with saline solution and 0.12% chlorhexidine, the patient was proposed and the buccosinusal fistula was closed using the Bichat Adipose Body. Conclusion: Today, in order to achieve the high success rate in the closure of buscossinusal communications, we use the Bichat Adipose Body, because it is a simple procedure and provides satisfactory results for the patient. Congenital heart defects are problems resulting from malformations in the structure of the heart during embryonic development. It affects about 1 in 100 children and approximately 20% of them need surgical intervention in the first year of life. This study aimed to describe the profile of children with congenital heart disease who underwent cardiac surgery in a referral hospital in the Northeast. This is a cross-sectional, quantitative and descriptive study, with data collected from medical records of children with congenital heart disease who underwent cardiac surgery at the University Hospital of São Luís-MA, from January to December 2017. Of the 31 cardiac surgeries in the period described, 58% were female, with a mean age of 4 years, 77% from the interior of the state and 64.5% of heart diseases were acyanogenic. The results showed the profile of patients with congenital heart disease, mostly female, preschool age, with acyanogenic heart disease, interatrial communication type, and most did not live in the capital. There were postoperative complications, the most frequent being cardiac and the association of heart disease with some syndromes or malformations, especially Down syndrome. Thus highlighting the need for studies on the characteristics of this population and the factors that may influence the success of cardiac surgery. [ABSTRACT FROM AUTHOR]

Introduction: Lymphedema is one of the post-surgical complications due to the treatment of breast cancer and is defined as excessive and persistent accumulation of extravascular and extracellular fluid and proteins in the tissue spaces due to the inefficiency of the lymphatic system. Due to non-standardization of lymphedema prevention guidelines on ipsilateral limb care this study was performed. Objective: To verify the care used to prevent lymphedema in women submitted to breast cancer treatment and their respective levels of evidence, and to subsequently construct an informative material for this population. Methods: This is a review of the systematic literature, and for the execution of this, a search was made in the databases, PubMed and SciELO, and in the website of the National Cancer Control Institute, using the descriptors: Prevention, lymphedema and risk factor, in Portuguese and English. Results: We selected 16 studies, in which there is controversy about the prevention of this complication, however, we observed a series of guidelines cited by authors such as avoiding weights, traumas, obesity, electrotherapeutic therapeutics and limb inactivity. However, it is worth mentioning that randomized clinical trials evaluating muscle strengthening and skin injuries were not associated with the appearance of this complication. Conclusion: A small number of studies related to the subject were observed, and the results are contradictory when compared to each other. In general, it can be concluded that the prevention of lymphedema occurs through a series of care that starts from the diagnosis of the tumor, but more studies of the type randomized clinical trial are necessary to justify these preventive measures, since these have a direct impact on the quality of the woman's life. Introduction: Although routinely performed, stoma closure cannot as yet be considered a simple procedure. We report here the morbidity, mortality and risk factors associated with this procedure over a 10-year period. Methods: The medical records of 252 patients (149 men; 103 women), with a mean age of 56 years (18 to 89 years), who underwent stoma closure were retrospectively reviewed for postoperative clinical and surgical complications, stoma-related features, among others. ICU admission, early surgical complications (within 30 days) according to the Clavien-Dindo classification, late surgical complications, and death were analyzed. Student's t-test, ANOVA, Pearson's chi-square test, Fisher's exact test, and Mann- Whitney U test were used for parametric and nonparametric data. Results: Colorectal tumors (64%) and diverticulitis (10%) were the main reasons for stoma surgery. Overall, 112 (44.4%) patients had at least one surgical complication. Early complications included surgical wound infection (13%), fistula and anastomotic/ intestinal dehiscence (9%), and abdominal wall or intra-abdominal abscesses (8.3%). Late complications included 36 (14.2%) cases of incisional hernia and one case (0.3%) of stricture. Patients with comorbidities were 10% more likely to have one or more surgical complications, and all 5 deaths occurred in these patients (2%). Large-bowel ostomies, longer operative time and ICU admission were significantly related to increased morbidity. Conclusion: Patients with an increased number of comorbidities and large-bowel ostomies are at higher risk for complications. Pre- and perioperative care and accurate patient selection are important to reduce morbidity and mortality. [ABSTRACT FROM AUTHOR]
