INTRODUÇÃO: Trata-se de estudo experimental com a finalidade de avaliar se o tempo de evolução de doença neoplásica pleural está associado ao grau de fibrose pleural na pleurodese com talco, descrever alterações nos níveis pleurais de mediadores inflamatórios ativados pela pleurodese em diferentes tempos de evolução da doença neoplásica pleural e analisar se o tempo de evolução da doença neoplásica pleural é o fator que influencia na segurança da pleurodese com talco em camundongos. MÉTODOS: Noventa e dois camundongos foram randomizados em 2 grupos Câncer(injetados com 50mil células de lewis intrapleural) e Salina(injetados com 0,5ml de soro fisiológico intrapleural), os quais foram subdivididos em Grupos Precoce(submetidos a pleurodese após 3 dias da injeção pleural) e Grupos tardios(submetidos a plerodese após 7 dias de injeção pleural), compondo 4 grupos de estudo. Parte dos animais foram sacrificados após 24hs da pleurodese (para obtermos os dados inflamatórios) e outra parte após 8 dias (para obtermos os índices macro e microscópico de fibrose pleural). RESULTADOS: Os Grupos Câncer tiveram escores menores de fibrose que os grupos salina, e essa diferença se intensificou quando comparadas as fases precoces com as fases tardias (p < 0,001). (Gráfico 6). Os escores de inflamação foram menores nos grupo Câncer, particularmente no grupo Câncer tardio(p < 0,001). Nos grupos Salina a inflamação foi intensa em 100% dos animais nos dois subgrupos (precoce e tardio) (p < 0,001).(Gráfico 7). As aderências não tiveram diferenças estatísticas nas fases precoces. Nos grupos tardios, o subgrupo Câncer tardio teve os escores mais baixos, e encontramos fibrose intensa somente no subgrupos Salina tardio(p < 0,001). (Gráfico 8). As analises dos marcadores inflamatórios pleurais mostraram resultados de variabilidade muito alta, não sendo possível identificação de diferenças significativas, a exceção do VEGF, que foi aumentado nos animais com câncer particularmente nos animais do subgrupo Câncer tardio(p=0,007), assim como o DHL sistêmico(p < 0,001). O grupo câncer tardio foi o único grupo onde ocorreu distribuição sistêmica do talco. (fig. 7). CONCLUSÃO: O tempo de evolução da neoplasia pleural é inversamente proporcional ao grau de fibrose pleural em camundongos submetidos a pleurodese com talco. Quanto mais precoce a pleurodese, melhores os resultados relacionados a fibrose, menor inflamação sistêmica, a circulação do talco é menor e consequentemente, maior a segurança da pleurodese PURPOSE: This is an experimental study aimed at evaluating whether the time course of pleural neoplastic disease is associated with the degree of pleural fibrosis in pleurodesis with talc slurry, describing changes in pleural levels of inflammatory mediators activated by pleurodesis at different times of the pleural neoplastic disease and determining whether the time course of neoplastic pleural disease is the factor that influences the safety of talc pleurodesis in mice.METHODS: Ninety-two mice were randomized in 2 groups with 50 mice each. Cancer group that received intrapleural injection ofLewis cells) and Salina group that received intrapleural injection of 0.5ml of saline solution. Both groups were subdivided into Early Groups (submitted to pleurodesis after 3 days of pleural injection) and Late Groups (submitted to pleurodesis after 7 days of pleural injection). Half of the animals in each group was sacrificed after 24 hours of pleurodesis (to obtain the inflammatory data) and another part after 8 days (to obtain the macro and microscopic indices of pleural fibrosis). RESULTS: Cancer Groups had lower fibrosis scores than Salina groups and this difference was more evident when comparing the early phases with the late phases (p < 0.001). Inflammation scores were lower in Cancer groups, particularly in the Late group (p < 0.001). In Salina groups the inflammation was intense in 100% of the animals in the two subgroups (early and late) (p < 0.001) . Pleural adhesions did not show statistical differences in the early stages. In the late groups Cancer roup had the lowest scores, and we found intense fibrosis only in t Salinegroups (p < 0.001). .The analysis of the pleural inflammatory markers showed high variability results and it was not possible to identify significant differences among groups. The exception was the level of , VEGF (p = 0.007) and LDH (p < 0.001) which were increased in animals with cancer particularly in Late Cancer subgroup . Systemic distribution of talc occurred only in the Late Cancer group. CONCLUSION: The time of evolution of pleural neoplasia is inversely proportional to the degree of pleural fibrosis in mice submitted to talc slurry pleurodesis. The earlier pleurodesis showed the best results related to fibrosis, with less systemic inflammation and lower systemic circulation of talc; consequently, the pleurodesis is better and safe
