This article describes publication bias, its most frequent causes, its characteristics, the regulatory tools to avoid it, and some statistical techniques to analyze it. These techniques are explained and applied to three therapeutic interventions related to the 2019 coronavirus disease (COVID-19): corticosteroids, ivermectin, and tocilizumab. Risk of publication bias was detected for ivermectin and tocilizumab. Systematic reviews and meta-analyses are secondary research designs that provide a reference to guide decision-making. They are prone to different types of bias, i.e., a systematic deviation in the results. Even if carried out with methodological rigor, their validity can be threatened by publication bias. This is defined as the act of concealing or delaying publication, withholding data arising from research studies, or both. Up to half of controlled trials remain unpublished. During the H1N1 virus pandemic, publication bias from industry-funded studies led to the recommendation and large-scale procurement of oseltamivir, a drug that later proved to have no relevant beneficial effects. Two-thirds of clinical trial funding for COVID-19 comes from the pharmaceutical industry. In the context of the COVID-19 pandemic, studies are published at an accelerated pace, making it very important to understand and identify publication bias. To reduce publication bias it is necessary to regulate the registration and publication of clinical trials, but this requires coordination among countries and international bodies. It is important to suspect and attempt to identify publication bias for decision making.

ABSTRACT Objective: Bibliometrics is a group of statistical and mathematical methods employed to measure and analyze the quantity and the quality of scientific articles, books, and other forms of publications. The objective of the present study was to conduct a bibliometric analysis of the Jornal Brasileiro de Psiquiatria (JBP) by listing its 100 most highly cited articles in the scientific literature and identifying their main characteristics in terms of authorship and research topics and design. Methods: The 100 top-cited references in the JBP were identified through a search with Google Scholar. The main author, last author, corresponding author, the total number of authors, gender of the authors, year of publication, research institution, geographic origin, language, and the research design and subject of each reference were recorded and analyzed. Results: A marked increase in the number of citations in the last 15 years, a relatively balanced distribution of publications among the Brazilian states and research centers, absence of gender bias among authors, and a varied range of published topics suggest a good current editorial performance by the JBP. Relative lack of systematic reviews and longitudinal studies, dearth of articles published in the English language, and modest participation of foreign authors were points requiring improvement. Conclusion: The present study suggests that a survey of the 100 most cited articles in the JBP can provide a historical overview of the progress of this journal, as well as highlight the main obstacles, constraints, and challenges faced by its editors and authors.

Working memory is recognized as a cognitive skill that stores a limited amount of information, responsible for maintaining and operating this information in consciousness. The study aimed to identify the instrument and / or task most used by researchers to analyze working memory in children, and to identify, through a meta-analysis, their ability to discriminate performance between groups. To this end, a systematic review and meta-analysis was carried out focusing on the most used instrument. The searches were carried out in the databases PubMed, Elsevier, Wiley Online Library, Springer and SAGE Journals; the terms "working memory", "children" and "psychometrics" were used, and the PRISMA-P protocol for writing the systematic review. The study identified 421 articles and selected 19. The meta-analysis was conducted using the statistical software OpenMeta [Analyst]; and the assessment of publication bias using the Comprehensive Meta-Analysis (CMA) software. As a result, it was found that the digit extension task is the first choice among researchers for the analysis of working memory in children and it appears that the instrument has little capacity to distinguish participants in terms of individual performance, school grade and age group. Knowledge plays an important role in health care. The production and diffusion of health-related knowledge are increasingly under the control of private commercial interests, which are characterized by conflicts of interests that result in abuses of power. Considerable research has been done on the medical-industrial complex and its role in the production of power imbalances and the consequent abuses, but little attention has been dedicated to the role played by the publishing industry, which can be subject to the same problems. The widely diffused idea that "frequent and major changes" occur in medicine, albeit unsupported by clearcut evidence, is an effective marketing tool for both the pharmaceutical and publishing industries, who feed and thrive on physicians' insecurities. The production and distribution of knowledge should be addressed as a strategic component of public health.El conocimiento desempeña papel importante en el cuidado de la salud. La producción y difusión del conocimiento en salud están actualmente cada vez más bajo control de intereses comerciales privados, caracterizados por conflictos de intereses que resultan en abusos de poder. A pesar de que esfuerzos considerables hayan sido dedicados al estudio del complejo médico-industrial y su papel en la producción de tales desequilibrios de poder y abusos consecuentes, poca atención ha sido dada al papel desempeñado por la industria de publicación, que puede estar sujeta a los mismos problemas. Editors of scientific journals need to be conversant with the mechanisms by which scientific misconduct is amplified by publication practices. This paper provides definitions, ways to document the extent of the problem, and examples of editorial attempts to counter fraud. Fabrication, falsification, duplication, ghost authorship, gift authorship, lack of ethics approval, non-disclosure, 'salami' publication, conflicts of interest, auto-citation, duplicate submission, duplicate publications, and plagiarism are common problems. Editorial misconduct includes failure to observe due process, undue delay in reaching decisions and communicating these to authors, inappropriate review procedures, and confounding a journal's content with its advertising or promotional potential. Editors also can be admonished by their peers for failure to investigate suspected misconduct, failure to retract when indicated, and failure to abide voluntarily by the six main sources of relevant international guidelines on research, its reporting and editorial practice. Editors are in a good position to promulgate reasonable standards of practice, and can start by using consensus guidelines on publication ethics to state explicitly how their journals function. Reviewers, editors, authors and readers all then have a better chance to understand, and abide by, the rules of publishing.Editores de revistas científicas precisam estar atentos aos mecanismos de disseminação de condutas inadequadas no processo de publicação. Este artigo fornece definições, formas de documentar a extensão do problema e exemplos de iniciativas para conter fraudes editorias. Fabricação, falsificação, duplicação, autoria-fantasma, autoria concedida, falta de ética na aprovação de manuscritos, não-divulgação desses fatos, publicação "salami", conflitos de interesse, autocitação, submissão e publicação duplicadas, e plágio são problemas comuns. A conduta editorial inadequada inclui: falha em seguir o processo devido, atraso nas decisões e comunicação com os autores, falhas na revisão, e confundir o conteúdo de um periódico com seu potencial promocional e de propaganda. Os editores podem ser advertidos por seus pares por não investigar comportamento científico suspeito, por não se retratar quando indicado ou não obedecer as seis principais fontes internacionais de orientação em pesquisa, publicação e política editorial. Os editores estão em posição privilegiada para promover práticas adequadas, adotando orientações éticas e claras sobre os procedimentos adotados nos periódicos. Assim, revisores, editores, autores e leitores terão condições de compreender e seguir as normas de publicação.

Objective: To investigate whether Brazilian articles on cancer are published in journals with an impact factor and/or repercussion (measured by the number of citations) inferior to those that come from foreign organizations. Methods: A search was carried out in PubMed for the MeSH term "neoplasm" with the limits clinical trial, affiliation of the Brazilian author(s), and interval from July 1st, 2009 to June 30, 2010. Selected for matching were non-Brazilian related articles published from three months prior to three months after the date of publication of the Brazilian study. The numbers of citations were obtained from two databases, as well as the impact factor for the journals in which the articles were published. Results: Fortythree national and 876 related international articles were identified. The Brazilian publications had a mean impact factor of 3.000 versus 3.430 of the international ones (p=0.041). There was no statistically significant difference as to the number of citations between the two groups. The affiliation of the first author with a Brazilian or foreign organization did not significantly influence the number of citations or the impact factor. Conclusion: Brazilian articles are significantly less accepted in journals with higher impact factors, although it does not compromise its repercussion on the scientific community. O objetivo do estudo foi comentar a possível existência de preconceito editorial entre editores de revistas científicas de países do Norte contra autores do Sul. Destacou-se que em estudo por métodos bibliométricos ficou evidenciada a existência de um importante desequilíbrio entre a produção científica de pesquisadores de países de alta renda ("Norte") e daqueles trabalhando em instituições de países de renda média ou baixa ("Sul"). Há uma percepção generalizada entre autores brasileiros de que, em parte, isso seria devido a preconceito de editores de revistas internacionais contra autores do Sul - 75% de uma amostra de 244 autores que responderam a inquérito acreditam que exista preconceito. Essa impressão é reforçada pela observação de uma minoria dos membros de conselhos editoriais das principais revistas na área de saúde proveniente do Sul. Embora o preconceito possa explicar parte do problema, há também questões especificas e remediáveis que podem aumentar a probabilidade de publicar no exterior. Essas incluem investir na qualidade do texto e da redação, e mostrar empatia com editores e leitores, sinalizando claramente a contribuição que o artigo pode trazer para a literatura internacional. Finalmente, é abordada a questão de onde publicar: em periódicos nacionais ou internacionais. Foram propostos seis tópicos que devem ser levados em conta nessa opção: idioma e público-alvo; tipo de contribuição ao conhecimento; capacidade de generalização; índice de citações; velocidade de publicação; e acesso livre. O aumento rápido de publicações brasileiras em periódicos internacionais mostra que o preconceito editorial, embora existente, pode ser efetivamente vencido por trabalhos com metodologia sólida e apresentação de qualidade.

Objective: To evaluate the antidepressant effects of exercise in older adults, using randomized controlled trial (RCT) data. Methods: We conducted a meta-analysis of exercise in older adults, addressing limitations of previous works. RCTs of exercise interventions in older people with depression (≥ 60 years) comparing exercise vs. control were eligible. A random-effects meta-analysis calculating the standardized mean difference (SMD) (95% confidence interval [95%CI]), meta-regressions, and trim, fill, and fail-safe number analyses were conducted. Results: Eight RCTs were included, representing 138 participants in exercise arms and 129 controls. Exercise had a large and significant effect on depression (SMD = -0.90 [95%CI -0.29 to -1.51]), with a fail-safe number of 71 studies. Significant effects were found for 1) mixed aerobic and anaerobic interventions, 2) at moderate intensity, 3) that were group-based, 4) that utilized mixed supervised and unsupervised formats, and 5) in people without other clinical comorbidities. Conclusion: Adjusting for publication bias increased the beneficial effects of exercise in three subgroup analysis, suggesting that previous meta-analyses have underestimated the benefits of exercise due to publication bias. We advocate that exercise be considered as a routine component of the management of depression in older adults.

OBJETIVO: Investigar se artigos brasileiros sobre câncer são publicados em periódicos de fator impacto e/ou repercussão (medida pelo número de citações) inferiores aos oriundos de instituições estrangeiras MÉTODOS: Pesquisou-se, no PubMed, o MeSH Term "neoplasm" com os limitadores: clinical trial, afiliação de autor(es) brasileira e intervalo de 1º de julho de 2009 a 30 de junho de 2010. Foram selecionados para pareamento artigos relacionados, não brasileiros, publicados entre três meses antes e três meses depois da data de publicação do estudo brasileiro. Foram obtidos os números de citações, em duas bases de dados, assim como o fator de impacto para as revistas nas quais os artigos foram publicados. RESULTADOS: Identificaram-se 43 artigos nacionais e 876 internacionais relacionados. Os brasileiros apresentaram fator de impacto médio de 3.000 contra 3.430 dos internacionais (p=0,041). Não houve diferença estatisticamente significativa quanto ao número de citações entre os grupos. A afiliação do primeiro autor à instituição brasileira ou estrangeira também não influenciou significativamente no número de citações nem no fator de impacto. CONCLUSÃO: Artigos brasileiros são significativamente menos aceitos em revistas de maior impacto sem aparente comprometimento de sua repercussão na comunidade científica.

Abstract A collective obsession with the Impact Factor (IF) has led to major changes in political science in many countries, and in recent years, these changes have been especially visible in Brazil. Despite critiques of both this measure's ability to evaluate the scientific publications and its spread in the evaluation of researchers and institutions, Brazil has taken the IF as an important element in evaluating Brazilian graduate programs. In this article, we briefly try to demonstrate that the IF can be subject to many biases that seem to be completely ignored or unknown.

The purpose of this study was to evaluate the effects of neuromuscular electrical stimulation in patients submitted to total knee arthroplasty. This was a systematic review with no language or publication status restriction. Our search was made in Cochrane Library, MEDLINE, Embase and LILACS. Randomized or quasi-randomized clinical trials evaluating neuromuscular electrical stimulation after total knee arthroplasty were included. Four studies with moderate risk of bias and low statistical power were included, totalizing 376 participants. There was no statistically significant difference in knee function, pain and range of motion during 12 month follow-up. This review concluded that neuromuscular electrical stimulation was less effective than traditional rehabilitation in function, muscular strength and range of motion. However, this technique was useful for quadriceps activation during the first days after surgery.

Purpose: To estimate the occurrence of adverse effects associated with the use of botulinum toxin by performing a systematic review and meta-analysis., Methods: A systematic literature search of MEDLINE and Cochrane Library was conducted until September 2007. The occurrence of the adverse effects was estimated of the relative frequency for case report and of the relative risk for randomized trials. Heterogeneity was evaluated with the Q test and I(2) index. Egger's significance test was used to identify the publication bias. Sensitivity analysis was performed to evaluate the effect of the publication biases., Results: Eight randomized trials and thirteen case reports filled the inclusion criteria. In the systematic review of case reports, 1,003 subjects were studied and 182 (18.14%) showed adverse effects. Eyelid ptosis was the most frequent adverse effect, presenting in 34 (3.39%) patients of the case reports. In the meta-analysis of randomized trials, the overall relative risk for adverse effects as headache, eyelid ptosis, local reaction and infection was respectively: 1.07, 3.25, 0.99 and 0.94. For all comparisons, P values were greater than 0.05., Conclusion: Ptosis was the most frequent adverse effect and showed the higher relative risk associated with botulinum toxin. It is necessary to perform a pattern of adverse effects related for better understanding the relationship between adverse effects and use of botulinum toxin.

A systematic review was conducted to identify studies assessing the effect of food fortification with iron on childhood anemia. The MEDLINE, LILACS, and PubMed databases and WHO and PAHO sites were searched with no time limitation, including articles published in Portuguese, English, or Spanish, using the following key words and their combination: food fortification, iron, effectiveness, efficacy, anemia, flour, staple foods, interventions, and children. Of 21 studies reviewed, only one failed to report a positive, favorable effect of iron fortification, indicating the possibility of publication bias. The studies showed important methodological limitations. The two studies with the best methodological scores showed opposite results, highlighting the need for larger trials with better planning to explore this hypothesis.
