Introduction: Patients with chronic wounds have multiple factors that slow down the healing process and create widespread discomfort. Chronic wounds are considered wounds whose debut was more than 3 months ago. Developing an algorithm involves synthesizing knowledge and developing specific recommendations for clinical practice based on scientific evidence. Purpose: To develop a tool to support professionals. Material and method: Literature review related to chronic wound care guides. Meeting with the nursing team from HOSPICE Casa Speranței Brașov, in which the relevant articles for the elaboration of a wound care instrument were analyzed. Development of a wound care algorithm. Results and conclusions: The care algorithm is divided into 5 sections: 1. Patient (diagnostic; age; nutritional status; medication; affecting daily activities; alteration of body image and wound assessment) and wound evaluation (description, the type and proportion of the tissue present, the odor, the presence of the exudate/hemorrhage, the surrounding tissue appearance. 2. Establishing the goals of care: preventing the aggravation or occurrence of other wounds; controlling the symptoms related to the wound, maintaining the quality of life, healing the wound. 3. Interventions: choosing the solution of irrigation of the wound, choosing the type of dressings, determining the frequency of changing the dressing. 4. Re-evaluation. 5. Results. The implementation of a wound care algorithm has the benefit of the complex and unitary evaluation of the patients presenting with chronic wounds and the establishment of the objectives and the individualized interventions without omitting one of the sections included in the algorithm. Statistics show that end-of-life wounds are a huge problem and are often found in palliative patients. Prevalence rates range between 30% and 47%, and incidence rates range from 8% to 17%. Wounds have different etiologies and characteristics, and in order to improve or heal, a clear and logical step-wise care plan is required. The first and the most important stage in this plan is the assessment of the wound: the location and size of the wound, the type of lesion, the existence of necrosis, infection and hemorrhage, the appearance of the perilesional skin and, last but not least, the type of the dressing used. Life in the century of speed has shown that we lose a lot of beautiful and interesting experiences that we have no time to reflect on, so we cannot consolidate them through the emotions we are experiencing and then they become just happenings in a simple list. This article makes a journey through history of wound healing to see how people have used their knowledge, imagination, and intuition to solve these health problems and it highlights how much they have evolved medicine today. We present the people who contributed to the development of the wound care system over time through their scientific contribution and ingenuity, in the order in which these people appeared in history - the Mesopotamians, the Egyptians, the Greeks, the Romans, the Indians, the Chinese and the Africans. Along with these, some historical data and aspects will be presented on what Romanians have practiced in this context. In the native medical history, it was not necessary to acquire a long-term diploma or training to be a physician, i.e. to apply treatment or to execute surgical interventions or to give medical recommendations to a patient. In Romania, centuries ago, besides law-recognized physicians, medicine was practiced by several categories of professionals, the most representative of which were: barber, midwife, confessor, witches. Era antiviralelor cu acţiune directă a schimbat complet tabloul infecţiei cronice cu virusul hepatitei C. Ratele de răspuns virusologic susţinut au depăşit procentul de 90%, dar au apărut alte provocări: persistenţa bolii hepatice, complicaţiile pe termen lung, monitorizarea după vindecare. Noile tratamente au adus avantajul posibilităţii tratării unui număr mai mare de pacienţi (pacienţii cirotici, inclusiv cei cu boală decompensată, cei coinfectaţi HCV-HIV şi cei cu boală renală cronică dializaţi). Experienţa utilizării noilor tratamente este încă scurtă, dar există unele recomandări generale în ceea ce priveşte supravegherea pe termen lung. Odată cu introducerea terapiei cu antivirale directe în opţiunile terapeutice ale hepatitei C, s-au stabilit şi criterii de eligibilitate pentru tratament. În România, în anul 2016, s-a aprobat, în cadrul unui program naţional, tratamentul cu ombitasvir / paritaprevir / ritonavir şi dasabuvir pentru pacienţii cu fibroză stadiu F4 dovedită prin Fibromax sau biopsie hepatică. Scopul acestui studiu a fost evaluarea eligibilităţii pacienţilor pentru terapie şi a concordanţei între testele serologice, elastometrice şi imagistice pentru evaluarea fibrozei într-o cohortă de pacienţi cu hepatită C. Au fost evaluaţi atât pacienţi nou diagnosticaţi, cât şi pacienţi cunoscuţi cu hepatită cronică C cu fibroză avansată, pe durata a 7 luni, între 1 noiembrie 2015 şi 31 mai 2016. S-au colectat date clinice, biologice, ultrasonografice, elastografice şi endoscopice. Au fost evaluaţi 146 de pacienţi pentru eligibilitatea la terapia antivirală fără interferon. Dintre aceştia, 89 (61%) au fost femei şi 57 (39%) bărbaţi, cu o medie de vârstă de 60 ± 8 ani. În ceea ce priveşte terapiile antivirale anterioare, 58 (52,5%) au fost naivi, 30 (27,4%) nonresponderi, 19 (17,81%) relapseri si 3 (2,74%) netoleranţi la biterapie. 80 (54,8%) au prezentat valori crescute ale alfa-fetoproteinei, fiind evaluaţi imagistic pentru excluderea unui hepatocarcinom - dintre aceştia, 8 pacienţi au fost diagnosticaţi cu hepatocarcinom. Ecografic, apoape jumătate dintre pacienţi aveau dilataţie de ax spleno-port, iar la endoscopia digestivă superioară 42% aveau varice esofagiene. Elastografia Fibroscan a fost efectuată la 88 / 146 de pacienţi: 82,95% aveau fibroză F4, 1,14% F3-F4, 9,09% F3 şi 7,2%

In this study, the author examines by comparison the legal regulations concerning the enforcement treatment applicable to minors in different states; the analysis is carried out starting with the different systems of legal enforcement - special regulations or rules included in a general regulation, like the Penal Code, continuing with the limitations of the penal responsibility of minors, and finally - the presentation of the sanctions applicable to minors in 17 different states. In the end, the author examines the opportunity for new legal provisions to be included in the future Penal Code, in order to reduce the applicability of the sanctions in favor of the educational measures, some of which being successfully applied in other european penal regulations. In this study, the author examines the possibility of enforcing precautionary measures on the assets of third parties for the offense of money laundering; the analysis is carried out from the point of view of the doctrine, comparative law and the case law of the courts and the European Court of Human Rights. Gastrointestinal hemorrhage remains an important problem for all emergency departments. Endoscopy is the primary diagnostic and therapeutic tool for gastrointestinal bleeding. The performance of endoscopic therapy depends on findings of stigmata of recent hemorrhage. Endoscopic therapies include injection using sclerosing agents, mechanical and coagulation hemostasis. Endoscopic hemostasis by an experienced endoscopist reduces bleeding, the risk of hemorrhage recurrence and surgical recommendation. [ABSTRACT FROM AUTHOR]

Gastrointestinal bleeding is one of the diseases which often require emergency medical assistance, both for adults and children. This pathology also requires a well determined diagnosis and therapeutical strategy. Successful hemostasis is directly dependent on the applied method of treatment. The patient comfort and the quality of life after the medical care are directly dependent on the method of hemostasis, mininvasive or not. The hemostasis endoscopic access is effective in the majority of cases. The procedure success depends on bleeding characteristics, location and its degree of visibility in the endoscopic placement. We present a prospective study which included patients diagnosed with various causes of gastrointestinal bleeding and solved by endoscopic hemostasis. The most common method was the injection one, although the best definitive efficiency has been found by mechanical method (p 0,000**), by band ligation, respectively. It is known that immune function decreases with age. There is the possibility that changes in the immune system to be a critical predisposing factor in the aging process; defining the immunological dysfunctionality in the elderly can add a new insight into the aging mechanisms. Getting older, there are alterations in the acquired immunity, especially in cells of the immune system T and B lymphocytes. Less scientifically proved are alterations in the innate immunity (dendritic cells, macrophages, polymorphonuclear leucocytes and complement) and in the mucosal immunity. [ABSTRACT FROM AUTHOR]
