Complex bimaxillary edentulous rehabilitations require careful and thorough planning of aesthetic and functional details. The main purpose of this case report is to highlight the stages of diagnostic, planning and treatment of a 62-year-old patient with multiple edentulous teeth, involving the following specialties: family medicine, implantology, prosthetics, periodontology and orthodontics. The role of multidisciplinary management and digital planning is highlighted as one of the most important requirements for an individualized edentulous treatment. The treatment included several stages (odontal, endodontic treatments, tooth extractions, insertion of implants and implant-prosthetic rehabilitations at the level of the upper jaw and prosthetic works at the level of the mandible). The complete rehabilitation determined major aesthetic changes from the point of view of macro-/mini-/micro-aesthetics, the patient’s quality of life being greatly improved. [ABSTRACT FROM AUTHOR]

Objectives. Orthodontic treatment intends to create a harmony between dental, muscular and osseous components. The purpose of this case presentation was to emphasize consequences of early loss of temporary teeth on dental-maxillary complex development. Material and methods. 10 years old patient with dental movements following the early extraction of temporary teeth. The patient has habitual oral breathing associated with lower lip interposition. Results. The functional treatment with twin-block appliance was chosen, taking into account the age-specific growth potential. Discussions. Premature extraction of temporary teeth engenders lee-way-space loss, significant shortening and transverse constriction of the dental arch, elements aggravated by dysfunctions and parafunctions. Conclusion. The early extractions performed during the growth period, through the associated dental migrations, determine growth changes with consequences on the functionality of the dento-maxillary apparatus. Purpose. The increasing frequency of using prosthetic restorations with implant aggregation confirms the great interest of patients in the application of modern dental therapies to restore the functions of the dento-maxillary apparatus, lost as a result of edentations. Case presentation. Patient D.D., aged 63, approached our clinic for consultation and the establishment of a specialized treatment. At the clinical and radiological examination we found the presence of a total maxillary edentation and a Kennedy class I edentation with a change in the lower arch, with all the remaining teeth with indication of extraction. Following the consultation carried out together with the dentist, we proposed to the patient a treatment plan that involved bimaxillary prosthesis on implants. Conclusions. Prosthetic restorations with implant aggregation offer patients with total edentations the possibility to recover the functions of the dento-maxillary apparatus at a higher level than traditional prostheses. Hybrid prosthesis with aggregation on zygomatic and pterygoid implants allows the reduction of the treatment duration and the decrease of the costs, in the conditions of maintaining a high therapeutic predictability. Performing alveolar corticotomies shortly before the application of orthodontic forces has been suggested as a method to improve the dental movement and, consequently, the orthodontic treatment as a whole. The study evaluate whether alveolar corticotomies can accelerate tooth movements in orthodontic treatment, if they can broaden the scope of treatment for different types of dento-maxillary abnormalities, reduce the need for dental extractions and sustain long-term stability. Several techniques have been proposed, although indications, type of surgery, possible postoperative complications, treatment objectives and patient satisfaction remain unclear. This study aims to show that a mandibular bone graft is a predictable therapeutic approach for bone regeneration after surgical removal of a tooth. The study was performed in a middle sized dog (Canis Familiaris); alveolectomy was used for extraction of upper second premolar. Bone augmentation with an autogenous graft was performed. After four months the fixation screws were removed and a single stage implant was placed. The implant was loaded after three months. The immediate outcome was excellent. The persistence of teeth on the arch for a long enough period of time represents an accomplished goal due to the existence and constant improvement of the conservative means of treatment. Most frequently, the natural teeth of the patient can satisfy or can reach the standards of function and esthetics, with minimum rehabilitation treatments. However, sometimes this goals can be achieved with great efforts due to a more serious pathology. In endodontic retreatment cases, the difficulty consists in aspects concerning: dental anathomy, lack of direct visibility, the technique used, the existing pathology and also the cases involved in retreatment due to primary endodontic failure. Introduction: The purpose of this study is to investigate the causes of tooth extraction and their correlation with patient age and gender in a sample of patients present in a private practice in Bucharest. Materials and methods: The study included 844 patients, 327 men and 517 women, aged between 18 and 84 years. Data were recorded about age, gender, number of teeth extracted and the reasons that led to the extraction, the study being realized over a period of five years (2006-2011). Results and discussion: During the study period, 1,570 permanent teeth were extracted from different reasons. The results showed that the main causes of extraction were represented by dental caries, endodontic treatment failure and periodontal disease. Dental caries was the main cause of tooth extraction for patients younger than 44 years, while periodontal disease was the main cause of tooth extraction in patients over 45 years. Endodontic treatment failure is also an important cause that led to tooth extraction, and in comparison with other studies, the percentage found in the present case is significantly higher. The other causes of tooth extraction like root fractures, accidents of eruption, prosthetic reasons, orthodontic reasons, recorded a relatively small percentage, yet these must be taken into account. Conclusions: The three main causes that led to tooth extraction were dental caries and their complications, periodontal disease and endodontic treatment failure. In conclusion, any effort to limit these cases is for the benefit of patients, thereby preventing loss of dental units that may have undesirable functional and psychological consequences. The purpose of the present study consisted in the application of laser treatment in several diseases in order to compare this method with the classical treatment one. Material and methods. The laser equipment used are: Therapeutic laser, diode BTL-5110 LASER with two probes in red spectrum (685nm/30mW) and infrared (830nm/400mW) as well as Surgery Laser Fotona Fidelis Plus 3 a laser system incorporating a Nd: YAG and Er: YAG for hard tissue.Treated dental diseases were dental caries, postextraction alveolitis and recent burns scars. Results and discution. Using laser in treatment of selected pathology has some advantages over traditional methods such as: elimination of vibration and pressure, high precision and elimination of pain and anesthesia, reducing the risk of infection, bleeding and healing time. The study regarding the dental implants, in the lateral maxillary zone, applied immediately after extraction or later. Materials and methods. We used for this study, 8 medium size dogs, common race (canis familiaris). Self conception implants (2-3 for each hemiarcade) were applied immediately after the extraction of the fi rst three molars or after 80 to 90 days after the extraction. Some implants were loaded immediately, and others were loaded after 75-80 days form insertion. After 3 months from loading, the animals were euthanized and samples were prepared for the histomorphometric study of the bone-implant-gum interface, with the aid of scanning electronic microscopy (SEM) and optical coherent tomography (OCT). The results. A light reduction of the bone`s height was observed, especially around the inserted implants immediately inserted after extraction . At the interface level, was observed the presence of collagen with a high level of fi brillary structure, and contact osteogenesis for the most of the length of implants, in almost all the cases. This fact was more obvious at the edge of spires rather than at the base of the implant spires. Discutions. The results obtained were commented in comparison with other similar studies. Conclusion. Issues raised by the last tooth on the arch, beyond the theoretical aspects are mostly practical and concern both the doctors in all branches of dentistry: general dental practitioners, oral surgeons, endodontists, periodontontist, orthodontists and prosthetics. Introduction: Bonding systems have revolutionized dental practice for over 50 years, increasing durability of different kind of restorations. These adhesive systems have been used in amalgam restorations too. However, most studies of amalgam bonding have used conventional three-bottle bonding agents or single solution adhesive systems in combination with one type of amalgam. Purpose: To compare SEM images of the amalgam-tooth structure interface in different types of classical and bonded amalgam restorations. Materials and method: On freshly extracted non-carious premolars and molars were performed ideal class I cavities. Two types of precapsulated alloys were used to fill this cavities: Lojic + (SDI Australia) - a single composition spherical alloy, and Ultracaps+(SDI Australia) - an admix alloy of spherical and irregular particles, with or without use of bonding agent. In this study, a single bottle universal bonding agent (OptiBond Solo Plus-Kerr Italia Spa-Italia) was used to bond amalgams based on different types of alloys to enamel and dentin. Then different areas of amalgam-tooth structure interface were observed under the scanning electron microscope (Philips L 20). Results and discussions: Amalgams based on single composition spherical alloy (Lojic+) have a higher rate of micro-leakage, which can lead to wider spaces between amalgam and tooth structure. It is interesting to point out that in case of amalgam based on admix alloy of spherical and irregular particles (Ultracaps+) this spaces are practically inexistent if proper trituration and condensation of amalgam was made. Conclusions: In case of bonded amalgams, we can come up with the conclusion that single bottle bonding agents can seal the interface between amalgam and tooth structure, thus preventing micro-leakage into the dentinal tubules. Early lesion diagnostic must be performed for a correct therapy of dental decay: invasive or non-invasive, by remineralisation due to reversibility of incipient decay. Paraclinic exams such as laserfluorescence and fi beroptic transillumination are complementary to clinical exam and must have sensibility and specificity. Extracted teeth with early decay have been studied using new devices (based on laserfluorescence and fiberoptic transillumination). First permanent molars extraction and the intervention of the orthodontist The purpose of this article is to describe one method of closing by orthodontic treatment the space that remains after the extraction of the permanent first molar. The purpose of this study is to determine the efficiency of the mechanical and manual endodontic treatment. Materials and method — The study was performed on 30 extracted teeth divided into 4 lots: the first lot has 6 teeths and is used only for comparison purposes, the other 3 lots have each 8 teeth that have been treated with mechanical ProTaper, manual ProTaper and with K-file. Prior to the experiment the teeth have been impregnated with methylic blue and have been weighted. After the experiment, the teeth have been once again weighted. Results and discussions — for the same number of rotations, the mechanical Protaper is more efficient than the manual ProTaper and the latter is more efficient than the K-file. The methylic blue impregnated dentine has been partially removed through the K-file technique, almost completely removed with the manual ProTaper and completely removed with the mechanical ProTaper. Teeth discolorations are changes in color caused by systemic, methabolic, pharmaceuticals, pathological, congenital factors. The purpose of this study is to highlight the therapeutical methods for curing teeth discolorations in order to determine the proper treatment for each patient. Material and method. The study was performed by applying treatment techniques such as: whitening teeth methods, ultrasonic scalling and root planing professional dental cleaning, direct and indirect veneers. The choice for each particular treatment was made according to the clinical requirements, local factors and patient financial means. Results and comments. Teeth discoloration tratment is general (according to the ethiology of the discoloration) and local (dental profilaxy and diet change, dental and professional cleaning, enamel abrasion, teeth bleaching, dental restaurations, dental extractions with applications of dental bridges or dental implant). The choice and application of treatment should be tailored for each particular patient. Good treatment results are assured through a proper analysis of each case, through a correct choice of treatment and through a good patient monitoring activity. [ABSTRACT FROM AUTHOR]

The present paper is the result of a study performed by sampling, transversally and retrospectively, of the X-rays, models, and photos of certain patients who came to the orthodontist for a specialty examination between 2000 and 2011. The patients included in the study were aged between 12 and 37 years old and had late mixed dentition and permanent dentition. Regarding the skeletal development, the patients belonged to stages CS4-CS6 (skeletal maturation). This study attempts to identify the characteristics of the palatine and the maxillary impacted canine, both dental and skeletal. The association of maxillary canine impaction and others anomalies were noted, such as microdontia or/and anodontia of the maxillary lateral incisor, the angle of impaction, the degree of overlapping of the canine on the root of the lateral incisor, the anteroposterior position of the apex of the canine, mandibular rotation, facial height - anterior and posterior, SNA-angle, the position of the maxillary central incisors. This can be useful to establish correlations between the etiology and treatment possibilities, as well as to determine the best moment to begin the treatment or the assessment of the average length of the active treatment. Implantology is an evolving discipline. Extraction with immediate implant helps to reduce the manipulation of the soft tissue and the limitation of bone resorption. It is known that the first 6 months after extraction, a loss of 40% to 60% of the height and thickness of the socket is to be noticed. The functional stimulus exercised on the natural tooth stimulates and supports the alveolar bone. In case of teeth loss, this physiological stimulus disappears in the advantage of a bone resorption, initially transversely and then axially that progresses quickly. In order to prevent this phenomenon, it seems legitimate that an intervention as early as possible will preserve the alveolar cortex. Immediate implant extraction has the following objectives: preservation of the initial bone volume; preservation of the adjacent soft tissue; non-invasive surgery, and if possible without flap; rendering the immediate and rapid function; aesthetic benefit. This technique can reduce the treatment time and the number of surgical interventions, compared with the conventional protocol.
