Objective: Evaluate the morphogenetic and pathogenetic features of hypertrophic and keloid scars of the head and neck., Material and Methods: The study included 286 patients, among them 176 (61.5%) patients with hypertrophic and 110 (38.5%) with keloid scars aged 18 to 65 years with a disease duration from 1 month to 2 years. Material for histological and immunohistochemical (IHC) studies of scar tissue was fixed in 10% buffered formalin. Serial paraffin sections were stained with H&E, according to Van Gieson and Weigert. IHC was performed using monoclonal mouse antibodies to collagen type I (clone 3G3, Santa Cruz, dilution 1:100), collagen type III (clone B-4, Santa Cruz, dilution 1:50), collagen type IV (clone COL-94, Santa Cruz, dilution 1:50), MMP-1 (clone 3B6, Santa Cruz, dilution 1:100), α-SMA1 (clone 1A4, Dako Agilent, dilution 1:100) and rabbit polyclonal anti-TGFβ antibodies (clone 3C11, Santa Cruz, 1:100 dilution)., Results: Pathogenetic, morphological and immunohistochemical differences in hypertrophic and keloid scars were established depending on their degree of maturity. In the formation of hypertrophic scars, the key factor in sclerotic processes is TGF-b on the background of low MMP1 activity. Keloid scars were distinguished not only by the accumulation of hard-to-degrade collagens, but also by the development of an osteoclast-like reaction with a high content of MMP1. Immature scar tissue was characterized by the presence of myofibroblastic α-SMA1 positive focus and center of inflammatory changes., Conclusions: The data obtained allow substantiating new approaches to the treatment of patients with hypertrophic and keloid scars.

Aim: Experimental evaluation of the efficacy and safety of lithotripsy using a new pulsed thulium fiber laser operating at a wavelength of 1.94 microns, peak power of 500 watts (maximum average power of 50 watts) by comparing it with a holmium laser operating at a wavelength of 2.1 microns (average maximum power 100 W and 120 W)., Relevance: The proportion of minimally invasive surgery in the management of urolithiasis, including retrograde intrarenal surgery, is steadily growing. The most appropriate tool for stone destruction is a laser. To date, the gold standard of minimally invasive surgical treatment of nephrolithiasis is holmium laser lithotripsy, which uses a laser on yttrium-aluminum-garnet activated by holmium ions (Ho: YAG) operating at a wavelength of 2.1 m. However, in recent years, lasers on a Tm-activated fiber with a wavelength of 1.94 m have become increasingly popular since the water absorption coefficient for the Tm radiation of a fiber laser is 5.5 times higher than for a Ho: YAG laser and in 2.2 times higher than for the Tm: YAG laser. This difference may translate into greater effectiveness and speed of stone crushing, which in turn leads to shorter operating time., Materials and Methods: This article describes physical foundations of holmium and thulium laser radiation, the mechanisms of stone fragmentation, data from a series of experiments comparing the efficiency and safety of ex-vivo lithotripsy using a holmium solid-state laser with a wavelength of 2.1 m and a thulium fiber laser with a wavelength of 1.94 m., Results: The study findings suggest that the STA IRE-Polyus thulium fiber laser operating at a wavelength of 1.94 microns and a maximum power of 500 watts has several advantages over the holmium laser in the stone fragmentation in urological practice., Conclusion: The results of the experimental work allow us to conclude that the use of a thulium fiber laser operating at a wavelength of 1.94 m and a maximum peak power of 500 W enables highly effective and safe lithotripsy.

The course of treatment with Alchemilla vulgaris extract increased the concentrations of lipids and phospholipids in erythrocyte membranes, decreased the number of abnormal erythrocytes, and improved deformability of red blood cells in rats with arterial hypertension.

Morphological organization of connections of ventro-lateral (nociceptive) and dorso-lateral (analgetic) midbrain central gray (vl SGC and dl SGC), as well as of dorsal raphe nucleus (analgetic zone, Rd), with different limbic structures, responsible for the formation of various emotional states, was studied in 26 cats. The methods of electrical destruction of brain areas were used that were followed by the light and electron microscopic study of degenerating fibers and synapses. Heterogeneity of connections of above mentioned formations with different limbic structures was demonstrated. Connections Rd and dl SGC with upstream limbic structures were found to be very similar in their organization and expression. Connections of vl SGC with the same structures were significantly different. It is suggested that similar (antinociceptive) function of dl SGC and Rd has determined the likeness of their connections. This, in combination with the heterogeneity of SGC in conduction of the pain and analgesia, supports the identification of two brain systems: nociceptive, conducting pain sensitivity, and antinociceptive, inhibiting its conduction. The nociceptive system includes the following structures: vl SGC, posterior and lateral hypothalamic nuclei, preoptic area. In the antinociceptive system two subsystems could be distinguished: midbrain units of these subsystems are localized in different structures (Rd and dl SGC), while the upstream ones are found in the same hypothalamic nuclei--ventromedial, dorsomedial, paraventricular. As far as septum, amigdala, hippocampus and cingular cortex are concerned, it was found impossible to refer them to any of these systems--either nociceptive or antinociceptive--basing solely on the findings of morphological studies because of approximately similar representation of axons of neurons in vl SGC, dl SGC, Rd in these structures.

The Japanese senescence accelerated mice (SAM) are a group of the low-longevity mouse lines and represent a new convenient model for studying the senescence process. We studied the proliferation of embryo fibroblasts of SAMP1 and SAMR1 mouse lines. It was shown that fibroblasts of the shortest longevity line SAMP1 have a markedly decreased proliferative potential of the mean 8.7 population doublings, whereas fibroblasts of a relatively high-longevity line SAMR1 have an average proliferative potential of 12.3 doublings. The fibroblast senescence in both lines is accompanied by a simultaneous lowering of the cell proliferative response to the blood serum, epidermal, fibroblast, and platelet-derived growth factors. At initial stages of the cell culture growth, lines SAMP1 and SAMR1 exhibit the same reactions to growth factors, but already beginning from the fifth doubling, the SAMP1 cell response is sharply decreased as compared with SAMR1. Lowering the proliferative reaction is accompanied by a decreased phosphorylation of tyrosine in the cell proteins responsible for mitogenic reaction. Thus, the parallel decrease of proliferative response to different growth factors during fibroblast senescence is most likely due the emergence of a regulatory block at common stages of the mitogenic signal transduction.

The Irkutsk province and the Republic of Buryatia which are a part of the Baikal Region (Siberia) were studied for environmental iodine balance. The water and local foodstuffs were found to show low iodine levels (they being 0.2 to 6.9 micrograms/l). In children, iodine deficiency was detected in 60-87% of the total number examined. There was a relationship between of the severity of neurological symptoms, the onset of thyroid diseases, general morbidity, physical development, iodine levels in the body. It is concluded that iodine deficiency has a great impact on the development of the regional pathology in children.

Clinical-experimental basis of the possibility of application of the method of indirect rheography of head (IRH) in wide clinical practice was presented. Estimation of the results reproduction and accuracy of the method was performed too. 40 healthy individuals of both sexes at the age of 18-50 and 102 patients with spinal osteochondrosis, arterial hypertension and neurocirculatory dystonia were examined. It was determined that IRH provided 93% of results reproduction and its precision was 90% and higher in comparison with ultrasound method. It was established that head circulation was 750-1200 ml/min or 12-24% of cardiac output in healthy individuals. The results of the study of head blood circulation in some groups of patients were also discussed.

Sixteen patients with hemangiomas of the ribs aged 18 to 51 were examined. Additional data on x-ray semeiotics of this condition are presented: the localization, structure, shape, and contours of these tumors are described in detail. Hemangiomas with an extraosseous component were detected, which should be differentiated from other pathological, mainly neoplastic malignant, processes in the ribs and lungs. The authors demonstrated the potentialities of the traditional x-ray method and computer-aided tomography in the diagnosis of hemangiomas of the ribs.

The article characterizes the mortality rates of nodular and mixed euthyroid goiter in the Ukraine Republic and discusses the results of surgical treatment of 681 patients with various forms of goiter. In view of the double increase of the number of cases of thyroid carcinoma in the last 4 years, the authors suggest resecting the thyroid gland more extensively than in the previously conducted economical resections. Radioimmunological study of the thyroid hormones and thyrotropin in dynamics showed activation of hypophyseal +thyrotropic function after the operation. The authors, therefore, recommend using thyroidin without fail. Postoperative recurrences-1.5%, stable hypothyroidism-1.8%.

Destructive changes in vertebral bodies, mainly in the thoracic part, zones of rearrangement and pseudoarthrosis are revealed in Bekhterev's disease. These changes develop in advanced stages of this disease in the form of subligamentous destruction of the anterior surface of vertebral bodies, limited destruction in the area of anterior parts in the zone of intervertebral disks, superficial destruction of cranial and caudal body plates with shortening of a height of an intervertebral disk, deep destruction of adjacent surfaces of vertebral bodies with expansion of intervertebral spaces.

The authors analyze 184 cases of AIDS in women treated from 1988 through 1990; 44.5% were 26-35 years old. The main diagnostic criteria of AIDS are described. The main neurological manifestations of a AIDS and their characterization in different concomitant infection. Treatment resulted in an improvement in 77% of cases. Fatal outcomes were in 29 patients (15.8%). The main causes of death were meningitides and pneumopathias of different etiology.

It has been found out, that under exposure to atmospheric discharge from chemical and oil-chemical enterprises school-age, children shown disorganization of the activity of nicotineamidadenindinucleotidediaphorase, cytochrome oxidase, acid and alkaline phosphatase in leucocytes, damage of lysosomal membranes of these cells, changes in erythrocytic system and in blood coagulation, shifts in total body sizes and subskin fat deposition in certain age-sex groups, disturbance in the structure of correlational interrelations of enzymocytochemical, hematological and anthropometric + indices etc. The observed deviations in school-age children health status are considered to be prepathological disturbances.

Quantitative assessment of spatial organization of cortical neurons forming descending fibre systems and hypothalamic structures has been made using technique of retrograde axonal transport of horseradish peroxidase. Heterogeneity of cortico-hypothalamic bonds is determined: the prefrontal cortex exerts a more pronounced effect on the hypothalamus as compared with other cortical areas of the limbic brain. Cortico-hypothalamic neurons form neuronal groups on different frontal levels of the prefrontal cortex and convolution of the cingulum, a form and orientation of the group being determined by the topography of dominating axonal systems running to the hypothalamus. There are various forms of cell associations in the frames of neuronal populations. Ensemble groupings of neurons may underlie the principle of organization of direct cortico-hypothalamic ways.

EEG activation can be produced by electrical stimulation of some cortical points with the same threshold current strength as by the midbrain RF and thalamic CM stimulation. Near-threshold stimulation of all these points acting simultaneously with inhibitory conditioned signals does not disturb the effector inhibition but displays an EEG difference between negative signals: the fine differentiation sound evokes considerable EEG desynchronization, while the rough one does not change the background rhythms. The same stimulation combined with a positive signal which has been made ineffective by successive inhibition or extinction, reestablishes the intensive EEG activation in response to this signal and the effector conditioned reflex. Therefore a mode-rate additional stimulation of the activating points in the cortex, RF and CM has a disinhibitory influence. When initiated in the cortex this influence may be transmitted from the cortical point to other parts of the brain along transcortical and corticofugal connections.

In chronic experiments of alert cats, moderate electrical stimulation of different points on the lateral cortical surface led to activation of different cerebral areas. Both the high-threshold and the low-threshold points were revealed in the sensorimotor cortex and in the Ep field of auditory area. The latter had as low a threshold of the activation response as the points within the mesencephalic RF and the thalamic CM, VPL and GM. In the morphological aspect, projections from the auditory Ep field to the thalamic intralaminar nuclei and to the brain stem RF, were revealed. Major projections spread from the dorsal part of the Ep field to the lateral tegmentum. The data obtained seem to provide better comprehending of the mechanisms for corticofugal activation. The functional role of the low-threshold loci in the cortex can involve triggering of unspecific apparatus of activation with a graduated volley which controls its excitation in accordance with biological significance of analysed signals.
