Currently, multi-slice computed tomography (MSCT) coronary angiography is a leader in the diagnosis of coronary artery disease in patients with non-ST elevation acute coronary syndrome of low or moderate risk. High coronary calcium score (CCS) obtained by MSCT indicate a high probability of obstructive coronary artery disease. In the presented case of an 83-year-old patient with unstable angina, the CCS was 1394, and hemodynamically significant stenoses were detected. However, according to selective coronary angiography, no hemodynamically significant coronary lesions were found. High CCS suggests poor image quality in MSCT coronary angiography. High CCS is detected in most people over 70 years of age. Obviously, in this patient, a high CCS is mainly determined by age. Most studies on CCS did not include patients over 80 years of age. When deciding whether to perform MSCT coronary angiography, it is necessary to take into account the individual characteristics of a particular patient, which may affect the interpretation of results.

Coronary heart disease (CHD) is one of the leading mortality causes. According to the latest guidelines, coronary computed tomography angiography (CCTA) is one of the main non-invasive methods for diagnosis of CHD, which allows for quantification of stenosis severity and the characterization of stenotic lesions. Aim of the study. It is to study the role, diagnostic value and dependence of coronary artery calcification on the degree of stenotic lesion of the coronary arteries and anthropometric parameters in patients with coronary artery disease examined by computed tomography. Aim. Coronary artery calcification is a characteristic of coronary atherosclerosis, which is often detected by chest computed tomography (CT). The aim of this study is to assess the efficiency of low-dose chest CT in calcium score (CS) determining and to compare it with conventional method and results of CT and selective coronary angiography.Material and methods. A total of 251 patients underwent a low-dose chest CT and a CS determining. Coincidence between the two methods was evaluated by the absolute CS values and by stratification of patients into five risk categories (“zero” CS values were excluded from the analysis). Eighty patients underwent CT or selective coronary angiography and the results were divided into two groups according to the degree of coronary stenosis — ˂50% and ≥50%. The CS values determined by chest CT are compared with the results of coronary angiography. Results. In 79 patients (31%), the CS was 0 and they were excluded from further analysis. Coronary artery calcification (CAC) is characteristic of coronary atherosclerosis, which is often detected by chest computed tomography. The standardized method for assessing CAC is the calcium score (CS) using the Agatston scoring system. A semi-quantitative ECG-synchronized assessment of coronary calcium score is considered a strong prognostic factor for coronary events in asymptomatic patients. Zero CS can serve as the strongest unfavorable risk factor for cardiovascular events in 10-15 years. According to international guidelines, CS should be used in patients of intermediate and low risks with a family history of cardiovascular diseases, as well as in patients with diabetes older than 40 years. The aim of this review was to summarize current data on the clinical significance of CS taking into account the results of international multicenter studies, current international guidelines and further prospects for its wider use in cardiology practice.

Introduction: Thanks to development of technology, new noninvasive techniques of coronary arteries evaluation, such as Multislice Computed Tomography (MSCT) were introduced. Nowadays tomographs are characterized by spatial and time resolution sufficient for reliable assessment of coronary arteries. Moreover, the dose of radiation taken by patient was substantially decreased. Two main methods of evaluation were introduced. Examination performed without contrast agent enables quantitive assessment of coronary arteries calcification by so called Agatston score. Another method performed after contrast injection is Computed Tomography Angiography (CTA) Goal: This studies purpose was to determine the role of Calcium Score in assessment of Coronary Arteries Disease (CAD) advancement Materials and methods: In this study the results of 100 consecutive patients (45% men, mean age 55 ± 8.9 years) were analyzed. All patients had a positive ECG stress test score. With a 64-row CT scan, coronary artery imaging of all patients was performed. The descriptive analysis of the group was based on information obtained from referrals issued by cardiologists to the MSCT. Analyzed parameters were: age, gender, indication for referral, Calcium Score and final diagnosis. Results: In most cases (84%) of patients with positive ECG stress test score, MSCT excluded significant changes in coronary arteries. Also, the correlation between Calcium Score and sex and diagnosis was proven. Conclusions: MSCT and Calcium Score enables to find patients without significant changes in coronary arteries and positive ECG stress test and to reduce the number of referrals to coronarography.
