Background: New chemotherapy regimens (gemcitabine / nab-paclitaxel, FOLFIRINOX) have improved overall survival rates in ductal carcinoma, which has renewed the interest in surgical treatment for isolated liver metastases. Aim: To identify the role of surgery in ductal carcinoma of the pancreatic head with liver oligometastases. Methods: We retrospectively analyzed the data from 490 patients with morphologically verified ductal carcinoma of the pancreatic head in two specialized centers, who had undergone pancreatoduodenectomy (PD) from January 2011 to April 2024. In 21 patients, PD was performed simultaneously with liver resection (liver oligometastases were found during the surgery). Results: In the postoperative period after PD with liver resection, compared to that after PD without liver resection, intra-abdominal abscesses were more frequent (4/21 (19.0%) versus 10/469 (2.2%), p = 0.002) and relaparotomy was performed more often by 9.8% [95 confidence interval (CI, Miettinen-Nurminen score method) 0.21–30.3] (3/21 (14.3%) versus 21/469 (4.5%), p = 0.077). The postoperative death rates were comparable (1/21 (4.8%) versus 19/469 (4.1%), p = 0.8). The median overall survival of the patients after PD with liver resection was 11 (95% CI: 6–16) months, and overall 1-, 2-, and 3-year survival rates were 39.2, 16.8 and 7.2%, respectively. In the patients after PD with liver resection without any adjuvant chemotherapy (n = 5), the median overall survival was 6 (95% CI: 5–7) months, and in those with chemotherapy, 13 (95% CI: 6–16) months (p = 0.006). The overall three-year survival of the patients after PD with liver resection and adjuvant chemotherapy was 11.5%. In the patients after PD with liver resection R0, the median overall survival was 13 (95% CI: 6–32) months, with R1 resection 6 (95% CI: 6–7), and with R2 resection 6 (95% CI: 6–7) months (p = 0.021). The median overall survival of patients after PD with liver resection for G1 tumors was 17 (95% CI: 6–32) months. Conclusion: Surgery as a component of combination therapy for oligometastatic pancreatic head cancer can be considered in a certain category of patients with high tumor differentiation grade and mandatory preliminary assessment of the feasibility of R0 surgery and systemic chemotherapy.

Aim. To identify population characteristics of the PIK3CA gene mutation among breast cancer patients in the Krasnoyarsk region.Materials and methods. The study included 140 patients with breast cancer with a certain mutation status of the PIK3CA gene, observed at the Krasnoyarsk Regional Clinical Oncology Center named after A.I. Kryzhanovsky. Determination of PIK3CA gene mutations (exons 2, 5, 8, 10 and 21) was carried out using commercial kits on equipment from Roche (Germany). Aim. To evaluate the efficacy and toxicity of second-line polychemotherapy according to FOLFOX or FOLFIRI regimen in patients with biliary tract tumours after progression during first-line chemotherapy (CT). Materials and methods. The study is based on the analysis of retrospective and prospective data on the examination and treatment of 94 patients with biliary cancer of grades T1-4N0-2M0-1 followed-up and treated at the N.N. Blokhin National Medical Research Center of Oncology from 2015 to 2023. All patients were divided into 2 groups: Group 1 (FOLFOX, n=47) and Group 2 (FOLFIRI, n=47). In Group 1, patients received the recommended FOLFOX second-line CT regimen. Patients in Group 2 received FOLFIRI regimen. The endpoints were overall survival (OS) and incidence of grade 3-4 adverse events. Results. The study included 94 patients. In the FOLFOX group, the median OS was 13.0 months (1-year OS was 57.8±7.4%); for the FOLFIRI group, the median OS was 12.3 months (1-year OS was 54.4±7.3%). The toxicity profiles of FOLFOX and FOLFIRI were acceptable and consistent with those reported for the regimens. According to the grade 3–4 toxicity data, diarrhea was significantly more common in the FOLFIRI group (p=0.014), and neurotoxicity was more common in the FOLFOX group (p=0.006). During the second-line CT, the frequency of grade 1–4 toxicities in the groups did not differ: 18 (38.3%) events in the FOLFOX group and 19 (40.0%) events in the FOLFIRI group. Conclusion. Our results of evaluating the efficacy and toxicity of the second-line polychemotherapy regimens show that the FOLFOX and FOLFIRI CT regimens have equal efficacy in patients with advanced biliary tract cancer with a good performance according to the ECOG scale, who previously received the first-line therapy with a combination of gemcitabine with a platinum agent (cisplatin or oxaliplatin); also, our data demonstrate similar toxicity profiles of these regimens.

Objective: to assess clinical and economic implications of adding daratumumab (Dara) to the “lenalidomide + dexamethasone” (Rd) treatment program in early lines of therapy for patients with multiple myeloma (MM) ineligible for high-dose chemotherapy (HDCT) with subsequent autologous hematopoietic stem cell transplantation (autoHPSCT).Material and methods. Partitioned survival model was developed to compare overall survival in the treatment programs of MM patients in the 1st to 3rd lines of therapy. The following treatment options were included in the analysis: 1st line – Dara+Rd; 2nd line – “carfilzomib + lenalidomide” (K+Rd) or “ixazomib + lenalidomide” (Ixa+Rd) triplets; 3rd line – pomalidomide (Pom+dex) and Rd+(Dara+Rd)+(Pom+dex) versus Rd+(K/Ixa+Rd)+(Pom+dex/Dara). Probabilities of transition between health states (progression-free, progressed disease, and death) were based on published clinical data. Lifetime direct healthcare costs and incremental cost-effectiveness ratios (ICERs) per 1 life year gained were calculated from a Russian health care perspective. The indicator of the difference in costs between current and expected practice was used in budget impact analysis.Results. Dara+Rd combination increased life expectancy by 1.04 years (+16.05%) and 0.95 years (+14.57%) per patient, and in the target population (n=955) by 995.48 and 903.40 years for (Dara+Rd)+(K+Rd)+(Pom+dex) and (Dara+Rd)+(Ixa+Rd)+(Pom+dex) treatment programs, respectively, compared to Rd use in the 1st line. The ICER per 1 life year gained for Dara+Rd in 1st line therapy was 8.95 mln rubles for (Dara+Rd)+(K+Rd)+(Pom+dex) and 8.41 mln rubles for (Dara+Rd)+(Ixa+Rd)+(Pom+dex). It's lower than the median ICER of 13.64 million per 1 life year gained for antitumor drugs used to treat cancer of lymphatic and hematopoietic tissues (reference ICER). Dara+Rd triplet is used in 2nd line therapy after Rd, the number of life years gained in the target population due to greater efficacy is higher by 291.50 years compared with K+Rd and by 203.93 years compared with Ixa+Rd. The ICER per 1 life year gained for Dara+Rd is also lower than the ICER reference value: 9.25 mln rubles versus 13.64 mln rubles. The budget impact analysis showed that using Dara in early stages of MM therapy does not increase drug provision costs in the target population.Conclusion. The obtained results allowed to conclude that it is economically feasible to add Dara to the 1st and 2nd lines of therapy in lenalidomide treatment programs for MM patients who are not candidates for HDCT with autoHPSCT.

In patients with lung metastases from renal cell cancer (RCC), it is necessary to search for new prognostic factors influencing survival rates. Aim: To analyze survival rates and identify prognostic factors in patients with lung metastases from RCC. Material and methods. We retrospectively analyzed the data from 655 patients with lung metastases from RCC, who were treated at Moscow city oncology Hospital No. 62 (Moscow) and city clinical oncology center (Saint Petersburg) from 2006 to 2022. Of these patients, 241 (36.8 %) were classified into the intermediate prognosis group, and 278 (42.2 %) into the unfavorable prognosis group. The study investigated clinical and morphologic prognostic factors influencing survival rates in patients with lung metastases from RCC. Statistical analysis was performed using Statistica 10.0 software packages (StatSoft, USA) by constructing Kaplan-Meier curves and survival tables, building a mathematical model of survival. Results. Background: Cutaneous melanoma is an extremely aggressive disease with an unpredictable prognosis. Even among patients with similar tumor parameters and extent of surgical treatment, survival may vary. The extent of surgical resection of primary cutaneous melanoma is well-established, and a BRAF mutation in the tumor is an unfavorable prognostic factor for patient survival. The mutation presence is mainly used as a marker for prescribing targeted therapy. The impact of the extent of surgery and the method for restoring a tissue defect on survival of patients with similar stages remains unexplored. The impact of the BRAF mutation in the tumor on survival of patients with cutaneous melanoma depending on the surgery method has also not been studied. We analyzed the surgical treatment outcomes of 221 patients with primary cutaneous melanoma (stages 0-IIA and IIB-IIC) who were found to have the BRAF mutation and underwent different extents of tumor excision with closure of the surgical tissue defect.Objective: To analyze the impact of the BRAF mutation in the tumor on 5-year survival of patients with primary cutaneous melanoma (stages 0-IIA and IIB-IIC), depending on the extent of surgery and the method of replacing the tissue defect.Materials and methods: We used data from 221 patients with primary cutaneous melanoma (stages 0-IIA and IIB-IIC) and assessed their BRAF mutation status, extent of tumor excision, and method of closing the tissue defect. The patients were divided into 2 groups: the main group (wide excision of the tumor and plastic replacement of the tissue defect) and the comparison group (standard margin and linear closure of the tissue defect). After surgery, all the patients underwent tumor analysis for the BRAF p.V600E/K mutation using real-time PCR. DNA was isolated using the cobas® DNA Sample Preparation Kit on a cobas® z 480 analyzer. We used the following statistical methods: frequency analysis, z test to check the equality, and Kaplan-Meier survival analysis and such software as MedCalc 12.5 (1993-2013, MedCalc Software) and SPSS 26 (IBM Corp, 2019, IBM SPSS Statistics for Windows, Armonk, NY, USA). Results: Wide excision of primary cutaneous melanoma (stages 0-IIA and IIB-IIC) was found to increase 5-year progression-free survival in patients with subsequent plastic replacement of the tissue defect compared with patients with standard margin and linear closure of the tissue defect and 5-year overall survival in patients with stage 0-IIA melanoma. In patients with 0-IIA stage melanoma and positive BRAF mutation test results, wide excision with reconstructive and plastic defect closure improved the 5-year progression-free survival. Conclusions: The 5-year progression-free survival and 5-year overall survival increased by 20.9% (P = .005) and 13.0% (P = .031), respectively, in patients who underwent wide excision of primary cutaneous melanoma followed by plastic replacement of the tissue defect compared with patients with standard excision of primary tumors and linear closure of the tissue defect. The 5-year progression-free survival increased by 23.7% (P = .017) in patients with stage IIB-IIC melanoma, regardless of the BRAF mutation. In patients with 0-IIa stage melanoma and positive BRAF mutation test results who underwent wide excision, the 5-year progression-free survival increased by 20.7% (P = .047) compared with patients who underwent tumor excision with a standard margin.

The article is devoted to the study of survival in colorectal cancer and the influence of some common clinical factors on the life expectancy. Colorectal cancer (CRC) is an urgent problem of today and modern oncology, as the incidence of CRC is constantly increasing. It is believed that the development of CRC in 50% of all cases is related to lifestyle and long-term exposure to carcinogens. Genetic factors play a decisive role in 5 to 20% of cases. It has been established that from 20 to 50% of patients with CRC already have distant metastases at the initial presentation. The understanding of the biology of colon cancer is based on the analysis of the degree of aggressiveness of its course, patient survival, the effect of performed operative and combined treatment methods on the course of the disease, and the presence of some other factors that are not yet completely understood by clinicians (the influence of hidden metastasis, circulating cancer cells, genetic causes, etc.). Proximal colon tumors are tumors that located in the cecum, ascending, and hepatic flexure of the colon, that is, in fact, tumors of the right half of the colon. The more proximally the tumor was located in the right half of the colon, the longer the patients lived. At a follow-up period of 24 months, the duration of follow-up for patients with cecum, ascending colon, and hepatic angle tumors was 71.43%, 63.64%, and 72.73%, respectively. For the observation period of 32 months: 42.86%, 36.36%, 18.18%. For the observation period of 55 months: 14.29%, 9.09%, 0%, respectively. Thus, biology was the key factor, as the groups were equivalent in terms of stage, frequency of lymph node involvement, and degree of tumor differentiation. Therefore, from our point of view, the separation of right-sided RTK, r-RTK into a apparant nosoform has a clinically significant character: it is in the right half of the colon that the toxicoanemic form occurs more often; and less often (almost never) – obturation, much more characteristic of left-sided cancer, l-RTK. Typically, right-sided colon tumors have the following genomic context: MLH1 hypermethylation, BRAF mutation, and high-grade MSI, while left-sided colon tumors are p53 and KRAS mutant.

Objective: evaluation of the comparative pharmacoeconomic effectiveness of treatment sequences with prolgolimab as the first line and combination therapy with BRAF/MEK inhibitors as the second line versus a regimen with BRAF/MEK inhibitors as the first line and prolgolimab as the second line in adult patients with metastatic or unresectable cutaneous melanoma.Material and methods. A detailed Markov and decision tree model was developed to allocate patients with metastatic cutaneous melanoma (mCM) with BRAF gene mutation (BRAF+) to treatment with prolgolimab or to targeted therapy with BRAF/MEK inhibitors (“dabrafenib + trametinib”, or “vemurafenib + cobimetinib” combinations). The costs of BRAF+ mCM therapy and the number of life years gained (LYGs) depending on the treatment regimen were calculated using approximated overall survival (OS) and progression-free survival (PFS) curves taken from relevant publications.Results. The treatment sequence for BRAF+ mCM had a significant impact on patient treatment outcomes: the median OS for the “prolgolimab → BRAF/MEK inhibitors” regimen was 41 months, while for the “BRAF/MEK inhibitors → prolgolimab” regimen it was 26 months; the median PFS was 11.5 months for the sequence starting with prolgolimab and 12.2 months for the strategy starting with “dabrafenib + trametinib” combination. The number of LYGs for a therapy regimen starting with prolgolimab and a regimen starting with “dabrafenib + trametinib” combination when modeling in the 1st year of therapy was 0.92 and 0.94 years, and at a 5-year horizon it was 3.19 and 2.75 years, respectively. At the same time, the cost of 1 LYG with a strategy starting with prolgolimab was 156 thousand rubles (5%) lower than the strategy starting with “dabrafenib + trametinib” combination.Conclusion. The developed pharmacoeconomic research model facilitated a clinical and economic analysis of using prolgolimab compared to targeted therapy with BRAF/MEK inhibitors across four lines of therapy, closely reflecting real clinical practice in the treatment of BRAF+ mCM patients.

Introduction. Malignant brain tumors, such as anaplastic astrocytomas and anaplastic oligodendrogliomas grade 3, are characterized by high aggressiveness and pose a serious clinical problem. This study focuses on assessing intratumoral heterogeneity in anaplastic astrocytomas and anaplastic oligodendrogliomas and its impact on disease prognosis.Aim. To study characteristics of intratumoral heterogeneity, in particular such morphological criteria as necrosis, vascular proliferation, mitoses, and mutations in the most significant for glioma progression genes in the groups of grade III astrocytomas and oligodendrogliomas, as well as analysis of prognostic significance of these parameters.Materials and methods. The study included 389 patients with IDH-mutant astrocytomas and 200 patients with oligodendrogliomas. The mean Ki-67 labeling index of astrocytomas was 12.78 %, while that of oligodendrogliomas was 8.54 %.Results. The presence of vascular proliferation, necrosis, of more than 20 % of the area of the specimen occupied by sarcomatous-like areas and the number of mitoses significantly affected not only disease-free survival but also overall survival of patients. In the clinical setting, mutations in the TERT promoter gene, amplification and mutation of the EGFR gene, deletion of the CDKN2A gene, and TP53 gene had a significant negative impact on recurrence-free and overall survival.Conclusion. The results of single-cell RNA sequencing showed additional factors, including sarcomatous-like areas, as well as TERT, EGFR, CDKN2A and TP53 mutations, in the progression of the tumors under consideration and in ensuring an increase in their malignant potential.

Background. Despite the progress in pelvic exenteration (PE) technique in international oncological practice, it is still debatable for cancer surgeons. The main reasons for its unpopularity are technical complexity, high rate of intra- and postoperative complications (60%), high postoperative mortality (30%), and challenging postoperative rehabilitation of patients. Aim. To evaluate long-term outcomes after pelvic exenteration in patients with locally advanced recurrent cervical cancer. Materials and methods. From September 2021 to December 2022, 23 PEs were performed, including 12 total and 11 anterior. The mean age was 52.2±10.0 years (range 39–83 years). All patients were diagnosed with locally advanced recurrent cervical cancer: 7 (30%) with stage II, 8 (35%) with stage III, and 8 (35%) with stage IV according to the TNM classification. Histologically, cervical cancer was represented by moderately differentiated squamous cell carcinoma. PE was preceded by the following treatments: neoadjuvant chemotherapy in 17 (74%) patients, surgical treatment in 4 (17%) patients, and chemoradiotherapy in 2 (9%) patients. More than half (56%) of patients achieved grade 2 therapeutic pathomorphism. The advanced disease involved the following adjacent organs: bladder (16; 70%), intestines (13; 56%), uterine endometrium (7; 30%), myometrium and parametrium (4; 17%), fallopian tubes, vagina, and ovary (2; 9%). Results. The average follow-up time for patients was 9.4±8.8 months. Median time to progression – 8.7 months [5.9; 10.8]. During the observation period, 78.3% (18/23) of patients died. Progression was recorded in 39.1% (9/23) of patients. The 1-year overall survival of patients was 38.7% (median 8.9 months). The 1-year tumor-specific survival rate was 60.6% (median 14.1 months). The 1- and 3-year progression-free survival (PFS) rates were 63.0 and 49.0% (median, 13.4 months), respectively. Conclusion. During the observation period, death occurred in 54.6% (12/23) of cases in the anterior PE group, and in 25% (6/23) in the total group. The median overall survival with anterior PE is 2 times higher compared to total PE (9.6 months versus 4.1 months). Median PFS was 13.4 months. For patients after anterior and total pelvic exenteration, 1-year PFS was 51.9±17.6 and 72.9±13.5; 3-year – 25.9±15.7 and 72.9±13.5 (p=0.178), respectively. Thus, PE is justified and significantly expands the possibilities of surgical treatment.

In case of locally advanced soft tissue sarcomas of the extremities (STS), the large size of which does not allow resection with preservation of the limb, isolated regional perfusion (IRP) of the limb is considered as one of the effective treatment options.Aim. To evaluate the long-term outcomes of IRP with melphalan under conditions of hyperthermia for treatment of locally advanced STS of the extremities.Materials and methods. We conducted a prospective cohort clinical study that included 42 patients with STS of the extremities. All patients underwent IRP with melphalan, in one case – in combination with tumor necrosis factor alpha. Additional lymph node dissection was performed in 8 cases (19%). The 5-year overall survival was estimated by the Kaplan-Meier method.Results. The median age of the patients was 50 years (from 24 to 80 years), women predominated – 32 (76%). STS of the upper limb was diagnosed in 14 patients (33%), of the lower limb in 28 (67%). G3 grade was verified in 34 (81%) patients, in the rest – G2. Most patients had a recurrent tumor – in 34 (81%) cases. All patients received previous treatment: surgery in combination with chemotherapy and / or radiation therapy – 36 (86%), chemotherapy alone – 2 (5%), surgery alone – 4 (9%) patients. Within 5 years, tumor recurrence developed in 20 (48%) patients, amputation was performed in 10 (24%) patients, distant metastases were diagnosed in 4 (10%) patients. The mean time to relapse after IPC was 9.3 ± 3.5 months. Overall survival at 1, 2, 3, 4, and 5 years was 98%, 88%, 86%, 74%, and 67%, respectively.Conclusion. IRP under conditions of hyperthermia with melphalan is an effective method for the treatment of STS of the extremities: the overall 5-year survival rate is 67% with a low rate of amputations and metastasis.

Aim. To study the effectiveness of modified subtotal laryngectomy with the formation of a tracheopharyngeal shunt in patients with locally advanced cancer of the larynx and laryngopharynx.Materials and methods. We conducted a cohort study on the use of combined treatment (surgical and radiation/ polychemotherapy) in 25 patients: 24 men, 1 woman; median age – 56 years. The subtotal laryngectomy technique that we developed and used included the preservation of one arytenoid cartilage, the posterior third of the vocal fold, and the recurrent nerve on the side not affected by the tumor. We studied postoperative complications, the length and diameter of the tracheopharyngeal shunt, the functioning of the arytenoid cartilage, the frequency of voice recovery and its quality, and overall and relapse-free survival.Results. In the postoperative period, 8 (32%) patients developed a pharyngocutaneous fistula; tracheal stenosis after tracheostomy was observed in 8 (32%) cases. The length of the tracheopharyngeal shunt varied from 16.4 to 25.6 mm, in all cases its course was tortuous, the diameter of the preserved part of the vocal fold varied from 1.1 to 1.5 mm. 24 (96%) patients demonstrated the functioning of the arytenoid cartilage and its work as a valve. Voice function recovered in 24 (96%) patients. Fundamental frequency was 155.3 ± 20.9 Hz, maximum phonation time of the vowel “A” – 3.5 ± 1.0 s, duration of pauses in phrase – 0.24 ± 0.03 s, speech rate – 99.8 ± 8.4 words/ min, number of words in phrase 6.5 ± 0.3. The average value of the Acoustic Voice Quality Index was 6.74, which indicates dysphonia (for the Russian language, the threshold value for dysphonia is 4.86), but this did not interfere with patients’ communication at home and on the phone. Overall and relapse-free survival at 1, 2, and 3 years were 100%, 89%, 78% and 86%, 78%, 72%, respectively.Conclusion. The method of operation proposed by us contributes to the restoration of voice function in the majority of patients with oncological outcomes comparable to total laryngectomy.

Introduction. Most patients with metastatic renal cell carcinoma (mRCC) who received systemic therapy are classified into as intermediate risk group according to the International mRCC Database Consortium (IMDC) assessment. However, survival differs in patients with one and two unfavourable prognostic risk factors.Objective. To analyze the impact of possible clinical and laboratory parameters on survival in mRCC patients with intermediate prognosis according to IMDC in the presence of one or two unfavourable prognostic risk factors.Materials & methods. A retrospective analysis of data from 316 mRCC patients with intermediate prognosis receiving systemic therapy was carried out. The presence and effect on survival of the following laboratory parameters were compared: hemoglobin, neutrophil count, LDH, platelet count, alkaline phosphatase, serum calcium level, ESR, and emphasis was also placed on the time of metastases appearance. Overall survival (OS), post-progression survival (PPS), and 3- and 5-year survival were evaluated.Results. The overall 3- and 5-year survival rate for subgroups with one and two unfavourable prognostic risk factors were 85.2% ± 1.8% and 58.1 ± 1.6%; 79.1% ± 1.7% and 35.6  1.6%, respectively (p < 0.001). Three- and 5-year PPS for both subgroups amounted to 66.1% ± 1.6% and 21.8% ± 1.4%; 45.2% ± 1.5% and 12.2% ± 1.3%, respectively (p < 0.001). Background. The results of several multicenter studies indicate a high risk of severe COVID-19 and fatal outcomes in immunocompromised patients, including those with cancer. Effective prevention is critical to saving cancer patients' lives during the pandemic. Additional passive immunization with a combination of monoclonal antibodies to the SARS-CoV-2 S protein in clinical studies showed a significant reduction in the risk of severe disease and death and a decrease in the frequency of hospitalizations. Real clinical practice shows the high efficiency of this approach in patients with oncological diseases receiving immunosuppressive therapy. Aim. To perform a comparative analysis of prevention effectiveness and COVID-19 severity in patients with solid malignant tumors receiving antitumor drug therapy. Materials and methods. The analysis included 100 vaccinated patients aged 22 to 84 with metastatic or inoperable solid tumors who received cytostatic therapy with or without a targeted agent. The median age was 56.5 years in Group 1 and 57.7 years in Group 2. In both groups, 32 (64%) patients had breast cancer, 10 (20%) had gastric, colon, and rectal cancers, 2 (4%) had lung cancer, 4 (8%), and 6 (12%) had reproductive cancers. In addition, Group 1 included 1 patient each with bladder and brain cancer. All were treated with antitumor drug therapy following clinical guidelines according to tumor localization. Results. The median number of received treatment lines of patients in Group 1 was 2.2, and 2.38 in Group 2. In Group 1, 42% of patients got infected, and 64% in Group 2. The combination of tixagevimab 150 mg + cilgavimab 150 mg monoclonal antibodies reduced the incidence of COVID-19 infection in any clinical form by 1.5-fold and hospitalizations by 1.3-fold. In Group 1, the rate of mild COVID-19 was higher; in Group 2, a higher risk of severe course was observed. In Group 1, viral pneumonia was 1.6-fold less common than in Group 2. Overall mortality in Group 1 was 6.5-fold lower than in Group 2. In Group 1, no COVID-19-related deaths were registered; in Group 2, the mortality rate was 8% (n=4). Mortality related to underlying disease in Group 2 was 3.5 times higher, and the risk of dying from the malignant tumor progression was 50% higher. In addition, in Group 2, 15% of deaths were related to cardiovascular diseases. Conclusion. Adding Evusheld to vaccinated patients significantly reduces the burden of COVID-19 infection in individuals with solid neoplasms who are receiving antitumor drug therapy. Patients receiving Evusheld at any stage of the underlying disease are less likely to have COVID-19, including severe infection, which requires hospitalization in an infectious hospital. The reduction in overall mortality in the Evusheld group suggests that COVID-19 affects overall survival in cancer patients. Evusheld reduced the risk of death in cancer patients from any causes: the progression of malignant tumors, COVID-19 infection, and other comorbidities.

Background. The development of myelofibrosis (MF) is driven by complex molecular genetic events that include driver somatic mutations responsible for the constitutive activation of the JAK/STAT signaling pathway (JAK2, CALR, and MPL), additional mutations affecting epigenetic regulators (TET2, ASXL1, IDH1/2, etc.) and RNA splicing (SRSF2, U2AF1, SF3B1, etc.), as well as genetic aberrations that contribute to genomic instability and disease progression.Aim. To analyze driver (JAK2, CALR, MPL) and prognostic (ASXL1) somatic mutations in patients with MF and evaluate their impact on survival.Materials and methods. The study included 29 patients diagnosed with MF, selected by hematologists from the City Clinical Hospital No. 7 and Regional Clinical Hospital (Krasnoyarsk).Results. 26 (89.6 %) out of 29 examined patients had some driver mutations in JAK2, CALR, MPL genes. The p.V617F mutation in the JAK2 gene was found in 20 (68.9 %) patients. Mutations in the CALR gene were detected in 4 (13.8 %) patients, mutations in the MPL gene were found in 3 patients (10.3 %). In 1 of 26 patients, 2 driver mutations were present simultaneously. 3 (10.3 %) patients were triple negative. Mutations in the ASXL1 gene were detected in 12 (41.4 %) out of 29 examined patients. Conducted targeted NGS (next generation sequencing) for 13 out of 29 patients revealed additional genetic variants that contribute to the understanding of the development mechanism and disease course. When evaluating the overall survival in the groups of patients diagnosed with MF examined by us, depending on the combination of driver (JAK2, CALR, MPL) and prognostic (ASXL1) mutations, no statistically significant differences were found (p = 0.12). This appears to be due to the small sample size. At the same time, assessment of patient survival depending on ASXL1 status showed that in the presence of mutations in the ASXL1 gene, the median survival was 45 months (range 7–120 months), while in the absence of mutations it was 48 months (range 21–359 months) (p = 0.03).Conclusion. The results obtained allow us to assume that the presence of mutations in the ASXL1 gene is an unfavorable factor in the course of the disease.

Background. Epithelial forms of ovarian cancer account for up to 90% of all ovarian malignancies. Epithelial forms of ovarian cancer are classified according to the WHO criteria of 2014 into several types: the most common are serous carcinomas (70%), mucinous carcinomas (3%), endometrioid tumors (10%), light cell cancer, transitional cell cancer, mixed and undifferentiated carcinomas are rare. Aim. To study the overall survival and progression-free survival depending on the stage of the disease, the pathological type of tumor and the degree of differentiation of tumor cells. Materials and methods. A retrospective analysis of the treatment of 467 patients with stage IIICIVB ovarian cancer in the Primorsky Regional Oncological Dispensary for the period from 2003 to 2021 was carried out. The obtained parameters were processed using standard statistical analysis methods using the IBM SPSS Statistics 26 program. Results. The overall results showed that the most favorable course of stage IIICIVB ovarian cancer has a serous type of ovarian cancer, probably due to its sensitivity to chemotherapy and aggressive surgical tactics. Patients with a mutation in the BRCA1/2 genes have a better prognosis in overall survival rates. BRCA1/2 mutations are associated with an improved response to chemotherapy with platinum-based drugs. Conclusion. The analysis made it possible to determine the most favorable prognostic factors for advanced stages of ovarian cancer, when comparing the groups of high-grade and low-grade IIIIV stage serous carcinoma, it was noted that the median overall survival, depending on the histological type, is better in the group of patients with low-grade IIIIV stage serous carcinoma. When comparing, depending on the timing of the operation performed, it can be noted that the median overall survival and median progression-free survival rates are better in the group of patients who received primary cytoreductive surgery.

Background: The PD-­L1 level is mainly used to predict the effect of drug treatment in patients with melanoma. Despite the radical nature of surgery in these patients, the rate of disease progression remains high. Therefore, it is important to study the relation between the PD-­L1 level in the tumor and surgical treatment options for cutaneous melanoma and patient survival.Objective: To analyze the effect of the PD-­L1 level in patients with primary cutaneous melanoma at different levels of tumor excision and options for closing the surgical defect.Materials and methods: We used data of 143 patients with stage 0­IIIC primary cutaneous melanoma who were randomized into 2 groups: the main group (wide excision of the tumor and plastic repair of the tissue defect) and the comparison group (standard tumor removal) with different PD-­L1 levels (IC [immune cells] %). The 5­-year survival of patients was estimated.Results: We found the mutual dependence of the PD­-L1 level and the survival of patients with stage 0-­IIC melanoma on the extent of the tissues removed and the option for closing the defect.Conclusions: The 5­year progression­free survival in patients with stage 0­IIA primary cutaneous melanoma was higher than that of those with stage IIB-­IIC melanoma regardless of the PD-­L1 level. In patients with stage 0-­IIA primary cutaneous melanoma and the low PD-­L1 level (˂5), progression­free survival was 26.3% higher (P = .013) than that of those with the high PD-­L1 level (≥5). In patients from the main group with stage 0-­IIA melanoma and the low PD­L1 level (˂5), progression­free survival was 25% higher (P = .017) and overall survival was 16.7% higher (P = .045) compared with patients from the control group with the same PD-­L1 level. In patients from the main group with stage 0­-IIC melanoma and the high PD­-L1 level (≥5), the 5-­year progression­free survival was 21.2% higher (P = .033) compared with patients from the comparison group.

Aim to study the frequency of BRCA1/2 gene mutations , the efficacy of ovarian cancer therapy depending on the presence of BRCA1/2 mutations as well as the efficacy of olaparib maintenance therapy in BRCAassociated ovarian cancer.Material and Methods. The retrospective analysis included 355 patients with high-grade, stage I–IV serous ovarian cancer. The examination for a mutation in the BRCA1/2 gene was carried out within the framework of the program “Improvement of molecular genetic diagnostics in the Russian Federation in order to increase the effectiveness of antitumor treatment”.Results. Mutations in the BRCA1/2 genes were detected in 98 out of 355 (27.6 %) patients. Mutations of the BRCA1+ gene were detected in 62 out of 230 patients with ovarian cancer of stages IIIC–IV (27.0 %), the BRCA2 gene – in 9 out of 230 (3.9 %). In ovarian cancer of stages III–IV, BRCA gene mutations were absent in 159 of 230 (69.1 %) patients. The median time to progression in stages III–IV of the disease with a mutation in the BRCA1 gene was 22.0 months, in the BRCA2 gene – 27.0 months, in patients without mutations in the BRCA1/2 genes – 17.0 months, median life expectancy – 70.0; 65.0 and 45.0 months, respectively. Patients with serous ovarian carcinoma of high-grade I–IV stages with the presence of mutations in the BRCA1/2 genes were divided into two groups. The first group (6 out of 26 patients, 23.1 %) consisted of patients with stage IIIC–IV high-grade serous ovarian carcinoma, who received olaparib as maintenance therapy after the 1st line of chemotherapy, the second group (20 out of 26 patients, 76.9 %) were patients with stage I–IV high-grade serous ovarian carcinoma, who received olaparib in maintenance mode after 2 or more lines of chemotherapy.Conclusion. The presence of BRCA1/2 gene mutations significantly increased the median life expectancy of patients with stage IIIC–IV serous ovarian cancer, and primary cytoreduction significantly improved both overall survival and survival to progression in this group of patients. Maintenance therapy with olaparib is more appropriate after the 1st line of treatment than after subsequent ones.

Aim. To assess the comparative clinical efficacy of Prolgolimab monotherapy versus combination therapy with BRAF/MEK inhibitors (Dabrafenib and Trametinib, Vemurafenib and Cobimetinb) in adult patients with metastatic or unresectable skin melanoma implementing a matching-adjusted indirect comparison (MAIC). Materials and methods. We conducted a systematic search for randomized clinical trials of Prolgolimab, combinations of Dabrafenib and Trametinib, Cobimetinib and Vemurafenib. Unanchored MAIC was applied due to the absence of common comparator between trials. We determined effect modifiers based on an expert survey. The population from Prolgolimab studies was weighted using defined effect modifiers, followed by the approximation of survival curves. Results. Systematic literature search revealed 4 RCTs that met the inclusion criteria: MIRACULUM, coBRIM, combi-v and combi-d. To increase the power of prolgolimab comparison, data from the observational study FORA were included in evidence synthesis and combined with data from MIRACULUM. We selected M staging and the proportion of patients with elevated LDH levels as effect modifiers. No significant differences (all p0.05) were established between Prolgolimab and combination therapy with BRAF/MEK inhibitors for both OS after 1 year and PFS outcomes after 2 years from initiation. Discussion. Despite the inclusion of observational data and the limitations of adjusted indirect comparison method, the results of this analyses are consistent with both other comparisons of anti-PD1 inhibitors with BRAF/MEK inhibitors, and with real world data. It is necessary to recompare targeted therapy and immunotherapy after five-year follow-up period due to peculiarities of time of onset of their effect with the presence of a primary failure with a gradual exit to a long plateau on anti-PD1 inhibitors therapy. Conclusion. In these unanchored MAICs, Prolgolimab monotherapy showed comparable efficacy with combinations of BRAF/MEK inhibitors (Dabrafenib + Trametinib, Vemurafenib + Cobimetinib) in first line therapy of patients with metastatic or unresectable melanoma. This analysis may be relevant for clinical decision-making about the choice of the first line therapy for patients with BRAF mutation.

Purpose of the study. Was to evaluate overall survival of CC patients after D2 and D3 lymph node dissections and assess the frequency of postoperative complications and dynamics of postoperative recovery.Materials and methods. The study included 50 CC patients aged 39–84 years.Results. The comparative assessment of the overall survival, frequency of postoperative complications and dynamics of postoperative recovery was performed in patients divided into 2 groups depending on the lymph node dissection extent. Group 1 included 23 patients (46 %) with D2 lymph node dissection, group 2–27 patients (54 %) with D3 lymph node dissection. 8 (16 %) patients developed complications in the early postoperative period. Patients in group 1 had grade I (5 patients) and grade IIIb (1 patient) surgical complications, according to the Clavien-Dindo classification. 2 patients in group 2 developed grade III b complications. No significant differences were found between groups in peristalsis, gases and bowel movement, starting eating and drinking. A significant increase in the number of postoperative bed-days was observed in patients who underwent open surgery, compared with the group with laparoscopic access. No statistically significant difference was found in the overall survival in patients of the two groups (p = 0.918).Conclusion. The rates of postoperative complications that required repeated surgery were comparable between the groups with D2 and D3 lymph node dissections. D3 lymph node dissection did not affect the dynamics of postoperative recovery and the overall survival of patients.

Introduction. Treatment of patients with brain metastases is an important problem that should be considered in the framework of combination approach. Introduction of new techniques of drug therapy as well as radiotherapy and neurosurgical treatment allows to significantly increase patient survival. Effective drug therapy and local control of brain metastases are of utmost importance in prediction of overall survival and patient quality of life.Aim. To investigate the prognostic factors for overall survival and intracranial progression (local recurrences, distant metastases) in patients with brain metastases of renal cancer after neurosurgical resection.Materials and methods. Retrospective analysis of the treatment results of 114 patients with metastatic brain lesions due to renal cancer who underwent neurosurgical resection (NSR) at the N. N. Blokhin National medical Research Center of Oncology was performed. Clinical data of 102 (89.5 %) of 114 patients for whom data on survival was available were evaluated. Among them, 80 (78.4 %) of patients died, 22 (21.5 %) are under observation. Extracranial disease status at the time of NSR was known in 82 (71.9 %) patients: 45 (54.8 %) patients had extracranial metastases, and 37 (45.1 %) did not. Total resection of brain metastases with perifocal and perivascular zones was performed in 92 (90.1 %) patients; in other cases, fragmental lesion resection was performed.Results. median overall survival after NSR was 13.8 months (95 % confidence interval 10.3–18.6). per study data, factors affecting overall survival of patients with brain metastases of renal cancer after neurosurgical resection were presence / absence of extracranial metastases and patient’s functional status. Local recurrences in the postoperative cavity after NSR were observed in 24 (21 %) of 114 patients. median time of local recurrence was not achieved. Statistically significant factor of high risk of recurrence in the postoperative cavity was presence of lesions with maximal diameter ≥2 cm. Development of new (distant) metastases was observed in 31 (27.2 %) of 114 patients. median survival without distant metastases in patients with brain metastases after NSR was not achieved. frequencies of distant metastases at 6, 12 and 24 months were 15.5; 24.1 and 35.8 % respectively. per multifactor analysis, factors affecting development of distant metastases in the brain after NSR are multiple metastatic brain lesions and presence of extracranial metastases.Conclusion. Neurosurgical resection in patients with cerebral metastases of renal cancer in the total group leads to median overall survival of 13.8 months. predictors of better overall survival are absence of extracranial metastases and high functional status.

We used the bortezomib (velcade) plus MP (VMP) in 24 previously untreated patients with multiple myeloma (MM). The patients had a median age of 73 years (range, 65–83 years). For patients who received velcade plus MP the overall response rate was 62.4 %. 33.2 % patients achieved complete remission (CR+n-CR). The median duration of the CR was 10.3 months. Side effects were predictable and manageable. The most common adverse events reported were asthenia, neuropathy, neutropenia and anemia. Serious adverse events were rare. These results establish VMP as a new standard of care in elderly untreated patients with multiple myeloma. VMP is highly active and well tolerated in elderly patients (> 75 years) with newly diagnosed multiple myeloma.

We used the bortezomib plus dexamethasone (VD) and bortezomib plus MP (VMP) in 56 previously untreated elderly patients with multiple myeloma. The patients were 65–89 years old. For patients who received bortezomib plus MP the overall response rate was 83%. 33.3% of patients achieved complete response (near-complete response + complete response) (based on EBMT criteria). For patients who received bortezomib plus dexamethasone the overall response rate was 73%. 34.6% of patients achieved complete response. Median overall survival in patients treated with bortezomib plus dexamethasone and bortezomib plus MP has not been reached. Median event-free survival was 15 and 17 months respectively. Side effects of bortezomib were predictable and manageable. The most common adverse events reported were asthenia, neuropathy, neutropenia and anemia. Serious adverse events were rare. These results establish VMP and VD as modern strategies in therapy of elderly untreated patients with multiple myeloma. VMP and VD is highly effective and well tolerated in elderly patients (> 65 years) with newly diagnosed multiple myeloma.

The purpose of the study was to assess the prognostic significance of a scaffolding NEDD9 protein in patients with non-small cell lung cancer (NSCLC). NEDD9 (Neural precursor cell expressed, developmentally down-regulated 9) is a scaffolding protein in many intracellular protein cascades, such as PI3K-AKT-mTOR, Ras-ERK, which play key roles in tumor progression. several studies define the regulatory role of NEDD9 in progression of several solid cancers, including non-small cell lung cancer (NSCLC), however its role as a prediction biomarker is minimally assessed. Materials and methods. In the current study quantitative immunohistochemical (IHC) approach was utilized to characterize NEDD9 expression in a cohort of NSCLC primary tumor samples (n=16), and correlative statistical analysis was performed between NEDD9 expression and a set of patients’ clinical and pathological characteristics. Hodgkin's lymphoma is a malignant disease of the lymphatic system. Hodgkin's lymphoma was first described by Dr. Thomas Hodgkin in 1832 and later named “Hodgkin's disease” by Samuel Wilkes. Hodgkin's lymphoma accounts for about 24 % of all lymphomas. Hodgkin's lymphoma is classified as classical and nodular lymphoid-predominant (Nodular type of lymphoidpredominant Hodgkin's lymphoma). Classical Hodgkin's lymphoma includes the following histologic variants: nodular sclerosis variant (types I and II), mixed cell variant, classic lymphocyte-rich variant, and rare lymphoid depletion variant. Epidemiological and serological studies showed the involvement of the Epstein-Barr virus into Hodgkin's lymphoma etiology, since its genome was found in the study of the biopsy material samples from patients with Hodgkin's lymphoma. A relationship with the human immunodeficiency virus (HIV) was revealed as well, and patients infected with HIV have a significantly increased risk of developing Hodgkin's lymphoma compared to healthy people. An in-depth study of the Hodgkin's lymphoma pathophysiology revealed new therapeutic targets in the treatment of this disease. All these discoveries changed the understanding of the Hodgkin's lymphoma pathogenesis, and were important for the development of new methods of treatment. The history of therapy begins on the cusp of the 19th and 20th centuries. Over the past four decades, achievements in radiation therapy and combined chemotherapy have significantly improved overall survival of patients with Hodgkin's lymphoma. Currently, more than 80 % of patients under 60 years old with first diagnosed Hodgkin's lymphoma can be cured from this disease after first-line chemotherapy.

Purpose. Evaluation of long-term results of uveal melanoma (UM) treatment in patients depending on clinical, hemodynamic and morphological parameters of the tumor. Material and methods. Long-term results of treatment were analyzed in 394 UM patients aged 17 to 82 (mean age 53.1 ± 13.2). The median follow-up was 56 months. Group A (n = 242, organ-preserving treatment) consisted of patients with a tumor height of 5.0 ± 1.8 mm and base diameter of 12.7 ± 2.9 mm. Group B (n = 152, enucleation) included patients with 8.3 ± 2.8 mm tumor height and 15.2 ± 4.1 mm base diameter. Overall survival and disease-dependent survival were determined. Results. The cumulative 3-, 5-, and 7-year survival rates of patients with choroidal melanoma were 91.6 ± 1.7, 84.0 ± 2.5, 74.0 ± 3.8 % , and those with ciliochoroidal melanoma were 84.2 ± 5.6, 79.9 ± 5.8, and 53.0 ± 14.7 %, respectively (p = 0.048). In group A, the 5-year survival rates of patients with mushroom-shaped and irregular forms were 80.6 ± 5.9 and 74.6 ± 10.9 %, respectively, and 96.0 ± 2.0 % for dome-shaped UM. The worst 5-year survival rates were shown by patients with tumor stabilization, and the best ones, by patients with complete tumor resorption (x2 = 6.880, p = 0.008). Patients with post-radiation optical neuropathy (n = 193) had better 5-year survival rates (90.3 ± 2.9 %) than patients (n = 49) without neuropathy (77.5 ± 8.0 %). In group B, cumulative 1-year survival was 95.2 ± 1.9 %, 2-year survival, 89.2 ± 2.8 % , 3-year survival, 84.9 ± 2.9 % , 4-year survival, 81.8 ± 3.6 %, 5-year survival, 77.5 ± 4.0 % , 6-year survival, 67.9 ± 4.8 % , and 7-year survival was 64.7 ± 5.2 %. In epithelioid-cell, mixed-cell, and spindle-cell types of UM, the 5-year survival rates were 46.3 ± 12.0, 70.7 ± 8.0 %, and 84.8 ± 5.4 %, respectively (x2 = 11.571, p = 0.004). The 5-year survival rate of patients with extrascleral growth was 56.0 ± 10.4 % and without extrascleral growth it was 81.7 ± 4.0 %. Conclusion. In organ-preserving treatment, the combination of prognostic factors allows us to stratify the patients based on the risk of early metastasis, which allows for personalized dynamic monitoring, taking into account the vital prognosis of a particular patient.

Abnormal gene copies, a special type of genetic polymorphism, is a hallmark of most solid tumors, including colorectal cancer. Abnormal copy number of genes leads to tumor-specific genomic imbalance, which manifests itself already in precancerous precursor lesions. The aim of this review was to systematize the scattered data on changes in gene copy number observed in colorectal cancer and their impact on the outcome of the disease and response to therapy. The data from 58 studies was analyzed on gene copy number changes and their expression in primary carcinomas, cell lines and experimental models. This review examines the spectrum of genetic changes that lead to colorectal cancer, describes the most frequent changes in the number of gene copies at different stages of the disease, and changes in the number of gene copies that can potentially affect the outcome of the disease of individual patients or their response to therapy. In fact, aberrant gene copy number as a form of chromosomal imbalance affects a number of genes that provide a metabolic selective advantage for a tumor cell. Changes in the genes copy number in colorectal cancer patients not only positively correlate with changes in their expression, but also affect the levels of gene transcription at the genome-wide scale. Aberrant gene copy numbers are closely related to disease outcome and response to treatment with 5 fluorouracil, irinotecan, cetuximab and bevacizumab. Nevertheless, the possibility of translating the genes copy number index into clinical practice requires further research.

Introduction. Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal tumors of the gastrointestinal tract the character diagnostic feature of which is CD117 (KIT) expression. GISTs are clinically diverse and have different genetic alterations that may have predictive and prognostic significance.Aim – the study of clinical, morphological and genetic features of GISTs to assess the overall survival (OS) of patients with various profiles of genetic disorders for elucidation the factors contributing to prognosis.Materials and methods. A total 244 GIST patients who received combined treatment were enrolled in the study and their clinical characteristics and mutational status of KIT, PDGFRA, BRAF were analyzed. SDH-deficient GISTs were detected using IHC-analysis of SDHB expression.Results. Stromal tumors developed in stomach (50 %), small intestine (37.7 %), colon or rectum (8.6 %), esophagus (0.4 %) and extraorganically (EGIST, 5.7 %). Background. Impaired immune response in patients with lymphoproliferative diseases (LPD) can lead to severe COVID-19 infection and significantly affect survival by increasing the risk of death. The effectiveness of vaccines may be reduced in people with compromised immune system function. Sequential vaccination does not ensure the production of sufficient antibodies in patients with hematological malignancies. Patients with LPD need additional measures to prevent COVID-19 infection.The high efficacy of monoclonal antibodies combinations to the SARS-CoV-2 S-protein for COVID-19 infection prevention and treatment has been shown in clinical trials. The complex use of active and passive immunization in immunocompromised patients requires confirmation in real clinical practice.Aim. A comparative analysis of COVID-19 infection severity and prevention effectiveness in patients with LPD.Materials and methods. The retrospective analysis included 200 patients with LPD who received induction therapy in R epublican Clinical Oncological Dispensary (Ufa) from 01.09.2021 to 01.09.2022. All patients received the Gam-COVID-Vac vaccine (Sputnik V, National Research Center for Epidemiology and Microbiology named after Honorary Academician N . F . Gamaleya, Ministry of Health of Russia). Patients were divided 1:1 into 2 groups matched by gender, age, LPD immunophenotype, history of previous treatment. In the 1st group, in order to pre-exposure prophylaxis of COVID-19 infection, in addition to the Gam-COVID-Vac vaccine, 2 recombinant monoclonal antibodies were administered – 150 mg tixagevimab + 150 mg cilgavimab intramuscularly. In both groups, the frequency of COVID-19 infection, the frequency of viral pneumonias identified and not identified as COVID-19 infection, the number of hospitalizations due to infection, and overall mortality were analyzed. SARS-CoV-2 positive tests results, symptoms of acute respiratory disease, the frequency of pneumonia, the number of hospitalizations for viral pneumonias, and the total mortality over a period of 4 months were recorded in the ProMed electronic medical system.Results. The patient groups were balanced by age (55 and 58 years, respectively), gender, pretreatment and use of anti-CD20 monoclonal antibodies (67 and 68 %), spectrum of nosologies: Hodgkin’s lymphoma in the 1st group was diagnosed in 21 %, in the 2nd – in 20 % of patients; diffuse large B-cell lymphoma – in 36 and 35 % of patients, respectively; follicular lymphoma – in 16 % of patients in each group; marginal zone lymphoma – in 14 % of patients in each group; mantle cell lymphoma – in 2 % of patients in each group; chronic lymphocytic leukemia – in 8 and 9 % of patients, respectively; peripheral T-cell lymphoma – in 3 % of patients in each group.The combination of tixagevimab 150 mg + cilgavimab 150 mg reduced the incidence of COVID-19 infection by almost 12 times: 59 % of patients in the 2nd group developed COVID-19 infection, while in the 1st group it was observed only in 5 % of patients, in addition, in patients of the 1st group, the infection was mild in more than half of the cases, while in the 2nd group, 2 / 3 of the patients developed viral pneumonia.The frequency of hospitalizations due to the severe course of COVID-19 infection in the 1st group was 9 times lower – 3 % versus 28 % in the 2nd group.The use of tixagevimab 150 mg + cilgavimab 150 mg combination reduced the frequency of deaths by 30 times: in the 1st group, 1 (1 %) patient died, in the 2nd group – 30 (30 %). No mortality from COVID-19 infection has been reported with the combination of tixagevimab 150 mg + cilgavimab 150 mg.The only lethal outcome in the 1st group was due to the progression of oncohematological disease. Among the 30 patients who died in 2nd group, almost half (46 %) died due to COVID-19 infection. In 2nd group, 3 (3 %) patients died from decompensation of concomitant diseases, which indirectly indicates a decrease in the risk of death with the use of additional prophylaxis in LPD patients.Conclusion. Additional prophylaxis of COVID-19 infection in oncohematological patients with the combination of monoclonal antibodies tixagevimab 150 mg + cilgavimab 150 mg (Evusheld) significantly improves outcomes by reducing the risk of infection, severe course and death from COVID-19. Reducing these risks allows patients to receive complete treatment course, without violation of the time intervals between courses, ensuring the expected overall survival.COVID-19 infection in any clinical form, including asymptomatic, delays antitumor treatment, which reduces overall survival. The use of Evusheld also reduces the risk of death from other comorbid conditions.

Background. Breast cancer is the most common cancer in women. Metastatic breast cancer is an incurable disease, the main treatment goals of which are to increase overall survival and improve quality of life. Aim. To assess the priorities of patients with breast cancer in the framework of a sociological study. Materials and methods. For the first time, the main results of a sociological study of patients diagnosed with breast cancer, conducted by the Interregional Public Movement "Movement Against Cancer" in OctoberNovember 2021. This analysis reflects the needs of patients with breast cancer in various regions of the Russian Federation. A total of 675 people were interviewed,662 questionnaires were allowed for analysis. Results. In the rating of patients' desires regarding the results of treatment, regardless of the region and the duration of the course of the disease, the leading positions were taken by "improved quality of life" (46.2%) and "increased life expectancy" (46.4%). In the rating of preferences regarding the therapy received, the first place was taken by the value It is important to maintain good health, working capacity and the ability to live a full life (39.1%). Conclusion. The priority for all patients diagnosed with breast cancer is an effective and safe treatment that will ensure a good health and performance and safe a high quality of life for a long time.

Background. According to randomized clinical trial SOLO1 olaparib statistically significantly improves progression-free survival versus placebo as a maintenance monotherapy in patients aged 18 and over with newly diagnosed advanced ovarian cancer with BRCA mutations, who had response to first-line chemotherapy. As the data on overall survival (OS) in this trial remains interim it is still uncertain whether treatment with olaparib can provide any benefits in terms of OS.Objective: to evaluate a long-term OS for olaparib versus placebo as a maintenance monotherapy in patients with newly diagnosed advanced ovarian cancer with BRCA mutations, who had response to first-line chemotherapy.Materials and methods. A 10-year mathematic model of disease progression and survival on olaparib versus placebo was developed. Modelling was based on data on progression-free survival from SOLO1 trial and data on OS after platinum-sensitive and platinum-resistant relapses from OCEANS and AURELIA trials. Additionally, patients who haven’t been treated with olaparib after first-line therapy in base-case scenario were assumed to get olaparib as a second-line treatment after platinum-sensitive relapse; mortality modelling for these patients was based on data from SOLO2 trial.Results. Median OS for olaparib was 107 months versus 66 months for placebo. 46 % of patients treated with olaparib were alive by the end of 10-year modelling period, but only 28 % patients from the placebo group. Hazard ratio of death for olaparib versus placebo was 0.64 (95 % confidence interval 0.49–0.84). Probabilistic sensitivity analysis showed robustness of these results.Conclusion. Using olaparib as a maintenance therapy in patients with newly diagnosed advanced ovarian cancer with BRCA mutations, who had response on first line chemotherapy, statistically significantly reduces risk of death by 36 %, compared to placebo.

Introduction. Neoadjuvant chemotherapy (nact) is a potential alternative to chemoradiation therapy (crt) for rectal cancer and may allow early prevention of distant metastasis.Objective: to study the safety and efficacy of nact for patients with rectal cancer without damage to the mesorectal fascia.Material and methods. From 2016 to 2019, patients with cancer of the upper ampullar (сmrt2-t4an+m0, cmrt4an0m0), medium ampullar (cmrt3сn0m0, cmrt2n+m0) rectal regions were included in the pilot prospective study. All patients underwent nact according to the capox 4 scheme. Evaluation of the effect was carried out on the basis of mri of the small pelvis. In the case of regression or stabilization, surgery was performed, and in the case of progression, crt was followed by surgery. After surgery, all patients were scheduled for adjuvant chemotherapy for a total duration of 6 months. The primary endpoint was the rate of pathological complete response (mandard trg 1). Secondary endpoints included disease progression, toxicity (nci-ctc v. 5.0), postoperative complications (clavien-dindo), chemotherapy regimen, and long-term treatment outcomes.Results. 136 patients were included into the study. Of 130 (11 %) patients, who underwent only nact prior to surgery, 15 had pathological complete response. 99 patients (72.8 %) received a full course of chemotherapy for 6 months (nact + adjuvant pct). 6 (4.3 %) patients after neoadjuvant chemotherapy were treated with crt, 5 (3.67 %) of them due to local progression according to mri data, 1 (0.7 %) due to grade 3 toxicity during 1 course of pct and inability to continue chemotherapy treatment plan. Radiation therapy resulted in partial tumor regression in all patients. Systemic progression was not observed in any patient. Grade iii–iv toxicity was observed in 7 (5.1 %) patients, including bronchospasm (n=2, 1.4 %), thrombocytopenia (n=1, 0.7 %), neutropenia (n=1, 0.7 %), peripheral neuropathy (n=1, 0.7 %), cardiotoxicity (n=1, 0.7 %), diarrhea (n=1, 0.7 %). Grade v toxicity (acute myocardial infarction) was observed in 1 (0.7 %) patient. R0 resection was performed in all cases. Grade iiia postoperative complications occurred in 6 (4.4 %) patients, iiib complications in 5 (3.7 %) patients, and death due to sepsis after postoperative pneumonia in 1 (0.7 %) patient. The frequency of anastomotic leak was 3.6 % (n=5). The median follow-up was 31.4 months, the overall survival (os) and disease-free survival (dfs) rates were 94 % and 92.8 %.Conclusion. Neoadjuvant chemotherapy is a promising treatment option for rectal cancer patients with negative prognostic factors.

Purpose: to study the expression of vascular endothelial growth factor (vegf-a) and its receptors (vegfr-1 and vegfr-2) in renal cell carcinoma (rcc) cells and assess the effect of the expression levels of these markers on the tumor characteristics and prognosis of patients with rcc.Material and methods. The study included 65 patients with rcc (pt1a-t4n0/+m0/+). All patients underwent radical surgery. Histological tumor tissue samples obtained during surgery were used for the study. Expression of vegfa, vegfr-1, -2 was studied by immunohistochemical staining using appropriate antibodies to receptors and growth factors.Results. Expression of vegf and vegfr-1 and vegfr-2 receptors was ound in the cytoplasm and on the membrane of primary tumor cells of patients with rcc. There was a significant direct correlation of overexpression of the markers with g 3-4 anaplasia (vegfr-1, -2) and signs of significant tumor extension, including high pt category (vegfr-1, -2), larger size of the primary tumor (vegfr-1 , -2), tumor invasion of paranephria (vegf, vegfr-1), tumor venous thrombosis (vegfr-1, -2), multiple metastases (vegf-2), metastases in the adrenal gland (vegf, vegfr-2) and liver (vegfr-1) (p0.05).Conclusion. Expression of vegfa, as well as vegfr-1 and vegfr-2 receptors, was found on the surface and in the cytoplasm of cells of the primary tumor of patients with rcc (pt1a-t4n0/+m0/+). There was a significant correlation between vegf/vegfr overexpression with a high grade (g3-4) tumor anaplasia and significant tumor extension. In univariate analysis, a significant adverse effect on specific survival of vegfr-2 overexpression was observed. In regression analysis, vegfr-2 overexpression tended to independently affect specific survival. These results show the importance of vegf/vegfr expression as biomarkers in renal cell carcinoma.

This article presents a systematic literature review to assess the survival outcomes of patients with bilateral renal cell carcinoma who underwent nephro-preserving surgery. Patients with bilateral renal neoplastic lesions represent a rare subpopulation, numbering, according to different authors, from 2 to 6 % of the total RCC group. Despite the increase in the number of cases of bilateral RCC, the number of works devoted to the study of the features of surgical intervention, prognosis factors, treatment outcomes and survival of this cohort of patients is limited, and a number of important questions regarding this problem remain unresolved. Currently, the only effective method of treating bilateral renal cancer is nephro-preserving surgery in the volume of bilateral renal resection / nephrectomy with resection of the contralateral kidney in case of synchronous lesion or resection of a single kidney in case of metachronous lesion. This strategy avoids or reduces the risk of developing renal failure and its consequences after surgery. Conclusion. Nephro-preserving surgery for bilateral renal cell carcinoma is the only effective method to achieve satisfactory oncological results with a low incidence of complications.

Introduction. mBRAF mCRC has the aggressive phenotype. The incidence of such mutation in Europe and the USA is around 8–14%, in Asian countries – 4–8%. The purpose of this population-based study was to determine the incidence and identifying prognostic factors in pts with mBRAF mCRC in Russia. Materials and methods. A multicenter retrospective analysis of clinical data and treatment results of pts with mBRAF mCRC was performed. The main method for determining mutations was a PCR. The main efficacy endpoint was progression free survival (PFS) at the 1 st line. Multivariate analysis was performed using Cox regression model. Results and discussion. 437 out of 8648 pts (5.17%) with a known mutational status had mBRAF (V600). Clinical data were collected from 131/437 (30%): the right-sided primary tumor – in 58,6%, left-sided – in 19%, rectum – in 21,4%. ORR in pts with mBRAF was 31%, median PFS was 6 months. We didn’t revealed any differences between FOLFOXIRI and doublets (XELOX/FOLFOX or XELIRI/FOLFIRI) in terms of PFS (HR 0.9, 95% CI 0.49–1.52, р = 0.6) and OS (HR 1.5, 95% CI 0.61–4.1, р = 0.35) in pts with mBRAF. Conclusions. The incidence of mBRAF gene in the population of pts with mCRC in the Russia is low and we found a high incidence of localization of the primary tumor in the rectum. We didn’t reveal any differences between the usual duplets and standard regimen for such mutation - FOLFOXIRI in term of 1 st line PFS.

Increasing life expectancy and maintaining its quality are the main goals of the hormone-dependent HER2-negative advanced breast cancer (HR+ HER2- аBC) therapy. A Satellite Symposium of Novartis Pharma was held on January 28th, 2021, within the framework of the RUSSCO Big Conference Breast Cancer. It was dedicated to the benefits of Risarg (ribociclib) therapy in combination with hormone therapy (HT). Experts-oncologists shared current data and their experience with ribociclib. The combination of CDK4/6 inhibitors with HT became a gold standard for the 1st line therapy for HR+ HER2- aBC. And the choice of a specific drug is based on the data of clinical studies and is made taking into account the individual characteristics of the patient. The results of ribociclib studies (MONALEESA-3 and MONALEESA-7) demonstrate a significant increase in the median overall survival and a decrease in the risk of death by 28 and 30%. The tolerance profile of ribociclib is well studied and controlled, therefore the risks of adverse events can be reduced by the competent monitoring, and the necessary dose modification can be made, which allows most patients to maintain highly effective therapy. The use of ribociclib allows to achieve the main goals of therapy to prolong the patients life and maintain or improve its quality.

Relevance. Breast cancer (BC) is among the most common cancers and the leading causes of cancer death in women worldwide. Much attention is paid to the problem of its hormoneresistance; however, the issues of using prognostic markers and predictors in routine cancer clinical practice remain unresolved. Aim. Study and analysis of prognostic significance of clinical and biological factors and parameters of the hormonal profile in patients with primary inoperable HER2-negative breast cancer receiving neoadjuvant chemotherapy. Materials and methods. The study included 162 patients with locally advanced primary inoperable HER2-negative breast cancer. Patients were divided into 2 groups. Group 1 included 58 patients with early disease progression within 6 to 12 months after radical surgical treatment. Group 2 included 104 patients with no disease progression within 2 years after radical surgical treatment. In all cases, diagnosis was verified histologically and immunohistochemically. Levels of prolactine, progesterone, estradiol, luteinizing hormone (LH), follicle-stimulating hormone, testosterone and cortisol were measured by RIA. The blood plasma values in 20 healthy donors were used as reference one. The data were processed using the Statistica 7.0 and MedCalc (version 9.3.5.0) programs. All patients received combination antitumor treatment according to clinical guidance. Results. An analysis of the overall (OS) and event-free (EFS) survival in group 1 showed that the median EFS in patients with luminal B BC was 9 months, with triple-negative BC (TNBC) 8 months. 6-month EFS in luminal B subtype was 87.5%, in TNBC 79.4%, p=0.37985. 1-year EFS was 1.721.7% regardless of the biological subtype. The median OS in luminal B BC was 25 months, in TNBC 26 months. 1-year OS in luminal B BC 100%, in TNBC 93.9%, p=0.138. 2-year OS in luminal B BC 54.2%, in TNBC 55.9%, p=0.697. 3-year survival in luminal B BC 37.5%, in TNBC 41.2%, p=0.639. An analysis of OS and EFS in group 2 showed that the median EFS was not reached for all biological subtypes. 3-year survival in the group was 100% regardless of the biological subtype. The median OS was not reached for all biological subtypes. 3-year OS in the group was 100%. An analysis of the hormonal profile in the treatment dynamics showed decreased levels of estradiol in all groups of patients (by 1.6 times). In group 1, progesterone was decreased by 2.1 times, testosterone by 2.4 times and LH by 2.1 times in all BC subtypes (p0.05). Patients of group 2 showed 2 times reduced cortisol and 3 times reduced prolactin in all BC subtypes, while LH levels were elevated by 1.6 times in luminal A and B BC. Conclusion. Aggressive course was observed similarly in triple-negative cancer as well as in luminal cancer with primary hormone resistance. Studying of pituitary and sex hormones and cortisol have a great clinical significance in patients with all biological subtypes of BC. This should be taken into account when predicting the course of the disease and developing further treatment options.

Lung cancer is one of the most common tumors in humans. The incidence is in one of the leading positions. In 2019 1327 patients with this pathology were identified in the Republic of Bashkortostan. Of these, with stage III of the disease 28.7%, with stage IV 45.8%, which is higher. Small cell lung cancer (SCLC) is one of the most prognostically unfavorable malignant tumors in humans. About 15% of lung tumors are SCLC. The first combination for the treatment of advanced SCLC after etoposide and platinum preparations was the combination of the etoposide + carboplatin (EP) regimen with atezolizumab. In the IMpower133 study, after a median follow-up of 13.9 months. median overall survival in the atezolizumab group was 12.3 months, and in the placebo group 10.3 months. In clinical observation, the effectiveness of the combination of the EP regimen with atezolizumab has been demonstrated. A patient with SCLC received 4 courses of EP therapy with atezolizumab and 14 injections of atezolizumab. The partial response achieved is retained. Case study efficacy and satisfactory safety profile of the combination of atezolizumab and the EP regimen.

The lack of trials directly comparing 2nd and 3rd generation EGFR tyrosine kinase inhibitors (TKI) do not allows to determine the optimal treatment approach. The main objective of this study was to compare the survival advantage achieved with these options. Materials and methods. As a result of a systematic search in the PubMed, ClinicalTrials.gov databases, as well as in the materials of international conferences American Society of Clinical Oncology and European Society for Medical Oncology data on the overall survival showed in 11 open randomized trials comparing different generation TKI monotherapy in TKI nave EGFR mutated non-small-cell lung carcinoma (NSCLC), both in the general population and in subgroups including sex, age, mutational status, smoking status. Using the package netmeta for the programming language R, a network meta-analysis of the obtained data was carried out using a frequency framework. Results. The 11 selected studies, including 3251 participants, had the following comparison groups: 6 studies (ENSURE, EURTAC, IPASS, NEJ002, OPTIMAL CTONG-0802, WJTOG3405) compared 1st generation TKIs and standard platinum-based drugs (CT); 2 studies (LUX-Lung 3, LUX-Lung 6) compared 2nd generation TKIs and CT; 2 studies (ARCHER 1050, LUX-Lung 7) compared 2nd generation TKIs and 1st generation TKIs; one study (FLAURA) compared 3rd generation TKI and the 1st generation TKIs. It was shown that for overall survival in comparison with CT, 3rd generation TKIs [hazard ratio HR 0.84 (0.64; 1.08) and 2nd generation (HR 0.86 (0.74; 1.00)] have the same effectiveness. Subgroup analysis did not reveal significant differences in OS except for the subgroup of patients with a deletion in exon 19 of the EGFR gene, where the use of 2nd generation TKIs in comparison with CT had a statistically significant difference HR 0.71 (0.37; 0.97). Conclusions. The network meta-analysis of the overall survival data of clinical trials of the use of different generations TKIs as the first line treatment in patients with EGFR-associated NSCLC showed that the use of TKIs of the 3rd and 2nd generation has the same efficacy, and the TKIs of the 2nd generation increase the total survival in a group of patients carrying a mutation in exon 19 of the EGFR gene.

Bone metastases are a common complication of cancer. Patients with bone metastases may have experienced skeletal-related events, such as hypercalcemia, pathological fractures, pain syndrome of varying intensity, spinal cord compression with negative effects on the quality of life. Current diagnostic tools have some limitations, such as high cost and limited availability in the distant areas, as well as falls negative and falls positive results. In this aspect, non-invasive sensitive markers of bone metabolism might give additional valuable information. Bone remodeling markers (N-terminal propeptide of type 1 collagen, osteocalcin, C-terminal telopeptide of type 1 collagen, etc.) have been used for a long time to predict the effectiveness of osteoporosis treatment; as additional risk factors for treatment initiation in patients with osteoporosis, in diagnostic search for secondary forms of osteoporosis; and as predictors of fracture in population studies. This review summarizes the clinically relevant biochemical markers of bone remodeling and the available evidence for their use in the metastatic bone disease in particularly in the diagnosis and prognosis of bone metastases risk and skeletal complications, predicting clinical outcomes, bone disease progression and overall survival. It has been shown that a sufficient suppression of bone remodeling biochemical markers while on treatment with bisphosphonates is associated with an improvement in survival and a decrease in the risk of skeletal complications in patients with bone metastases. New data may become a rational basis for wider use of bone metabolism markers in oncological practice. However, it is necessary to standardize and validate the determination of bone markers and verification of cut-off diagnostic values for their introduction into the routine clinical practice of patients with malignancy.

Aim. The main aim of this study was to model the effectiveness of multiple myeloma (MM) therapy using machine learning, which was based on the analysis of various methods of MM treatment, a number of prognostic factors and their results in the daily routine clinical practice of medical centers in European countries. Materials and methods. The present study was retrospective, non-interventional, multicenter. A structured database of MM patients provided by the Oncology Information service (O.I.s.) was used for the study. Registration took place in medical institutions in eight countries: Austria, Belgium, Switzerland, Germany, Spain, France, Greece and Great Britain. Results. In total, 57% of men and 43% of women were analyzed in the base of 6074 patients with MM. The median age was 71 years. The median follow-up time along the lines was 387 days. High-risk cytogenetics are represented in 15% of cases. The efficacy endpoint was the best response to each line of therapy, as measured by time to death (TTD) as an indirect indicator of overall survival and time to next treatment (TTNT) as an indirect indicator of progression-free survival. The median TTD and TTNT were 730 and 399 days respectively. After a multi-step selection process, characteristics with the greatest importance for the therapy prognosis were selected: age at the beginning of therapy, line of therapy, time after MM verification, ECOG (Eastern Cooperative Oncology Group), cytogenetic risk, transplant eligibible or not, TTNT after the previous line of therapy, therapy regimen. Discussion. To continue the study it is necessary to analyze literature data and compare with real practice. Also analysis and comparison with Russian data on the treatment of patients with MM is required. Conclusion. The analysis of the presented data provides a basis for modeling a tool for assessing the effectiveness of MM therapy (prognosis of TTD and TTNT) for each patient, based on a number of prognostic factors and the results of routine clinical practice in various medical centers in European countries.

Purpose. To evaluate the overall, disease-specific, event-free, relapse-free survival of retinoblastoma (RB) patients and cumulative probability of eye survival in such patients.Material and methods. The study included 223 children with RB (126 male, 97 female). Monolateral disease was diagnosed in 69.1 % of cases (n = 154), bilateral disease in 30.9 % (n = 69). The median follow-up was 40 months (interquartile range from 27 to 53 months). According to ABC classification, group A was diagnosed in 16 of 292 eyes (5.5 %), group B — in 53 (18.2 %), group C — in 41 (14.0 %), group D — 58 (19.9 %), and group E — in 124 (42.5 %). Group E was detected twice as often in patients with unilateral RB as in bilateral RB (56.5 % versus 26.8 %; p < 0.01). Stage T1 according to TNM classification was diagnosed in 69 cases (23.6 %), stage T2-T3 — in 213 cases (72.9 %). The extraocular form (stage T4) was detected in 3.4 % of cases (n = 10).Results. The overall five-year survival of patients treated for RB (n = 223) was 96.4 ± 1.4 %. The 5-year disease- specific survival (n = 222) was 96.8 ± 1.3 % in the general cohort, 96.5 ± 1.5 % in the group of children with monocular RB, 97.3 ± 2.7 % for binocular RB. Five-year event-free survival rate of children who received treatment (n = 223) was 66.7 ± 3.6 %, with monolateral lesion (n = 154) — 74.1 ± 4.2 %, with bilateral lesion — 50.6 ± 6.2 % (n = 69). Five-year relapse-free survival took place in 83.3 ± 3.1 %, with monolateral RB — 87.8 ± 3.3 %, with bilateral RB — 73.3 ± 6.4 %. Primary enucleation was performed in 121 of the 223 children treated for RB and included in the study. The frequency of primary enucleation with monocular RB was more than twice as high (55.2 %; 85 of 154 eyes) as with binocular RB (26.1 %; 36 of 138 eyes; p < 0.01). Organ-preserving treatment was carried out in 138 cases (171 eyes). It was effective in 123 (89.2 %) children (152 eyes; 88.9 %). 5-year ocular survival was 85.7 ± 3.7 %, with monolateral RB — 78.2 ± 6.6 %, with bilateral RB — 92.2 ± 2.8 %. Ocular survival counted separately for groups A, B, C, D, equaled 100 %, 100%, 76.6 ± 6.9 %, and 71.1 ± 11.9 %, respectively. The five-year disease-specific survival rate of children with monocular lesion given eye-preserving treatment was 100%, significantly higher than after primary enucleation — 93.4 ± 2.9 %.Conclusions. Modern combined retinoblastoma treatment is able to save the sick children’s lifes and and even their eyes and visual functions, which improves social adaptation and quality of life. It has been shown that, with a binocular form, it is possible to save more eyes than with a monocular form. Eye-preserving treatment administered to patients with intraocular RB under strictly observed criteria does not heighten the risk of metastasis. Yet timely enucleation in cases of absolute indications remains the method of choice in saving the life of a child.

Background. We previously found that a decrease in the number of NKT cells and activated CD 25+ peripheral blood lymphocytes (PBLs) before neoadjuvant chemotherapy was associated with an increased likelihood of disease progression in patients with locally advanced triple-negative breast cancer (TN BC).The purpose of this study was to determine the relationship between the initial number of NKT-and CD 25+ PBLs and relapsefree survival (RFS)/overall survival (OS ) in patients with TN BC who received neoadjuvant chemotherapy with cisplatin and paclitaxel followed by surgery.Material and Methods. The study included patients with stage II and III TN BC. The follow-up time was 36 and 66.9 months. Immediately before chemotherapy, the percentage of CD 3+CD 16+CD 56+ (NKT) -, CD 25+- and CD 8+ PBLs was determined by flow cytometry. Statistical analysis of the data was carried out using the Statistics 7 software package. The Kaplan-Meier method was used to determine the relationship between immunological parameters and RFS/ OS .Results. The decreased level of NKT cells before treatment was associated with a decrease in the 3-year RFS [Me: 20.1 (0.533 and 39.7) months] compared to that observed in patients with higher percentage of these cells than in the control (Me was not achieved). There were no statistically significant differences in the 3-year OS between the groups. The initially reduced number of CD 25+ lymphocytes in comparison with the control was associated with decreased rates of both RFS and OS . The difference in DFS and OS was more significant between the groups of patients who simultaneously had an increased initial number of both NKT and CD 25+ cells and patients in whom both cell populations were below normal levels.Conclusion. The initial (prior to chemotherapy) number of NKT and activated CD 25+ PBLs can apparently be a predictive factor in TN BC patients, who received neoadjuvant chemotherapy with cisplatin and paclitaxel.

Introduction. Nowadays the stereotactic radiotherapy (SRT) of patients with clinical stage I-II lung cancer is the choice of the treatment modality for functionally inoperable patients. It shows safety and high efficiency in reaching the local control. Though there is a range of unsolved issues connected with the prediction of treatment efficiency and frequency of complications, an integration of new technologies in the planning and treatment process allows to widen the search of the predictive factors.Materials and methods. Since 2014, 42 patients (T1N0M0 – 16 patients, T2N0M0 – 26 patients) with clinical stage I-IIa lung cancer have underwent SRT. The majority of patients (38) have been recognized as functionally inoperable due to the concurrent broncho-pulmonary pathology, 4 conditionally operable patients have refused an operation. 11 patients had the primary multiple tumors in their anamneses, 3 patients had a сentral tumor. Used dose fractionation options were: 10 Gy х 5 fractions (n = 29) and 7 Gy х 8 fractions (n = 13) – BED = 100 Gy.Resuts. The median follow-up was 32 months (range 6–56 months). The 3-year local control was 94%. The isolated local recurrences were not registered. Overall 3-year survival rate was 74% (95% CI, 60–90) and a 3-year tumor-specific survival rate – 84% (95% CI, 71–98). During one-factor analysis a reliable influence on the prognosis of the fractionation regimen (р = 0,04) and, close to reliability, the initial SUVmax level influence (р = 0,07) were revealed. Grade 3 pulmonary toxicity was observed in 4 (9%) patients, one patient with a Central tumor died from pulmonary hemorrhage (grade 5 toxicity). Grade 3 chest pain was observed in 3 (7%) patients, two of them had a rib fracture.Conclusions. With modern approaches to SRT treatment planning and delivery there should be a search for additional treatment efficiency and toxicity predictors. The total dose delivery regimen and initial tumor SUVmax can be predictive efficiency factors, while the pulmonary tissue volume can be a predictive toxicity factor.

Background. The prognosis of patients with multiple myeloma (MM) is significantly different depending on the biological characteristics of the tumor substrate, the microenvironment of the bone marrow, as well as factors associated with the patient’s body. Therefore, the search for new reliable and easily identifiable prognostic markers is relevant for the effective management of patients with this disease.The objective of the study was to assess the prognostic value of the study of serum free light chains (FLC) of immunoglobulins κ and λ and their ratio κ / λ FLC in the blood serum of patients with newly diagnosed MM in real clinical practice.Materials and methods. 369 patients with first diagnosed MM (134 men and 235 women) were examined who were hospitalized in the hematology department of the City Clinical Hospital No. 2 Novosibirsk in the period since January 2012 to December 2017. The median age of the patients was 67 (32–82) years. All patients received induction courses of chemotherapy based on bortezomib. The control group consisted of 56 conditionally healthy individuals: 34 women (60.7 %) and 22 (39.3 %) men with a median age of 62 (40–68) years. The concentration of FLC-κ and FLC-λ (mg / L) in blood serum was determined by immunoturbidimetric method on a Hitachi 911 automated biochemical analyzer using the Freelite Human Lambda and Freelite Human Kappa reagent kits (Binding Site, Great Britain).Results. It was found that in patients with MM, the concentration of serum FLC-κ or FLC-λ was statistically significantly higher compared to the control group and varied depending on the type of MM (p 65, as well as the concentration of FLC-κ and FLC-λ are higher than the median obtained in the whole group (FLC-κ ≥702 mg / L and FLC-λ ≥493.2 mg / L), correlate with known factors of poor prognosis for MM (with a high concentration of β2‑microglobulin (>3.5 mg / L) (r = 0.461; p 60 % (r = 0.420; p 177 μmol / L) (r = 0.380; p = 0.002), and also with high lactate dehydrogenase activity (>450 U / L) (r = 0.520; p 65 was 49 months compared to 76 months in the group with κ / λ FLC 0.04–65 (log-rank p = 0.012).Conclusion. The determination of free FLC in the blood serum of patients with MM can be used to assess the prognosis of their survival. The value of the κ / λ FLC ratio 65 allows us to divide patients with MM into risk groups with significantly different outcomes and can be used to identify patients at high risk who need more aggressive therapy and more detailed monitoring of the response.

Background. An increase in the number of patients with multiple myeloma (MM) necessitates the creation of reliable tools for assessing their somatic status (comorbidity) in order to personalize the optimal treatment regimen that helps to minimize its toxicity, improves survival and patients quality of life.The objective of this study was to modify the MM comorbidity index (MCI) by adding an additional variable reflecting the biological properties of the tumor, and to determine the informativeness of the new scale – a modified MM comorbidity index (M-MCI), to predict the outcome and select personalized therapy in patients with MM in real clinical practice.Materials and methods. From January 2012 to December 2017 the study included 369 patients with newly diagnosed MM (134 men and 235 women) who were hospitalized in the hematology department of the City Clinical Hospital No. 2, Novosibirsk. The median age of the patients was 67 (32–82) years. The prognostic value of concomitant diseases and individual prognostic factors in relation to the overall survival of patients with MM was evaluated.Results. Introduction. A number of studies show an obvious connection between erectile dysfunction (ED) and the development of cardiovascular diseases (CVD), which occupy one of the leading places in the structure of mortality in cancer patients. In these circumstances, it is potentially promising to use the erectile function (EF) status before surgery as an indicator of the general health of patients and a predictor of overall survival (OS).Purpose of the study. To study the causes of death of patients with prostate cancer (pca) after radical prostatectomy (RP) and evaluate OS depending on the preoperative EF.Materials and methods. The study is based on a series of 2642 Rp performed at one medical institution between January 2003 and December 2017. Total of 1203 patients met the inclusion criteria and were divided into two groups: 620 patients with preserved EF or mild preoperative erectile dysfunction (ED) according to the five-item International Index of Erectile Function (IIEF-5) score (group 1) and 583 patients with mild-to-moderate, moderate or severe preoperative ED (group 2). The Mann-Whitney U test and chi-square test were used for statistical analysis. Survival was assessed using the Kaplan-Meier method with a log-rank test.Results. A total of 101 deaths were observed in the study cohort; the median time to death was 72 months. Cardiovascular diseases (CVD) were the cause of death in 43.6% of patients, pc — 30.7%, other oncological diseases — 19.8%, other causes — 5.9%. There were no significant differences between the groups in age, body mass index, or degree of comorbidity. Purpose of the study. To analyze the treatment long-term results of patients with cervical cancer (CC) depending on the clinical and morphological characteristics of the disease.Patients and methods. 75 patients with stage I cervical cancer at the age of 21 to 76 years, were included in this retrospective study. All patients have been operated at the first stage of treatment in the Orenburg Regional Clinical Oncology Center from 2008 to 2013. Patient data was obtained from patient card and the "cancer-register". The statistical analis of the results was performed using the Statistica 6 software.Results. Recurrence of disease was detected in 7 patients and 7 patients died after treatment. 5-year relapse-free survival (RFS) and overall survival (OS) were 90,3%. 5-year RFS was: 95.2% and 66.0% — in depth of invasion of 10 mm or less and more than 10 mm (p=0.0004); 94.4% and 89.5% — in stages IA and IB (p>0.4) and 80.6% and 97.6% — in the presence and absence of adjuvant radiation therapy (p=0.002), respectively. 5-year OS was: 96.7% and 47.0% — in depth of invasion of 10 mm or less and more than 10 mm (p=0.00001); 97.0% and 84.2% — in stages IA and IB (p=0.07); 80.6% and 97.6% — in the presence and absence of adjuvant radiation therapy (p=0.02), respectively. There were no recurrence in 11 patients with IB stage, who did not receive adjuvant RT and, but ones were observed in 6 patients with stage IA — IB, who received the adjuvant RT.Conclusion. These results indicate that the use of only clinical and morphological factors is not sufficient to accurately assess of the CC prognosis and the search for new markers of the CC recurrence risk has not lost its relevance.

Aim. To assess the immediate and long-term results of treating patients with bilateral lung cancer (LC).Patients and methods. Radically operated 5 patients with primary multiple synchronous LC (PMSLC) and 5 patients with primary multiple metachronous LC (PMMLC) were included in the study. Clinical and morphological data, results of instrumental studies, immediate and long-term treatment results were analyzed. Statistical analysis was performed using the Statistica6 software.Results. Tumors corresponding to T1‑T2 prevailed in both groups. At the same time, both in the PMSLC and PMMLC group, only in one case both tumors corresponded to T1–2N0M0, in the others — one or both tumors were with metastases in lymph nodes of the root or mediastinum. In early postoperative period, the number of surgical complications was 10%, somatic — 20%. All complications were successfully treated conservatively. Postoperative mortality was not observed. In PMMLC and PMSLC cases, one-year relapse-free survival (RFS) was 100% and 80%; two-year — 80% and 60%; three-year — 60% and 60%; four-year — 60% and 20%, respectively. The five-year RFS in patients with PMMPC was 40%. One patient with PMSLC was alive without relapse of disease for 4.5 years. Accordingly, one-year overall survival (OS) was 100% and 100%; two-year — 80% and 80%; three-year — 80% and 60%; four-year — 80% and 40%. Five- and six-year OS in patients with PMMLC were 60% and 20%, respectively.Conclusion. The absence of postoperative mortality and low number of postoperative complications indicate the safety of bilateral operations in patients with bilateral LC. Low rates of patients’ 5‑year survival are connected with problems of primary diagnostics and low efficiency of existing algorithms of regular medical checkup of patients with this pathology. To improve long-term results of LC treatment, it is necessary to more actively introduce the modern programs of dynamic observation and complex examination of patients, undergone surgery for LC, using the annual chest CT and fibrobronchoscopy.

Objective. To study and analyze the significance of the degree and possibility of surgical resection of a brainstem tumor in children and adolescents on the results of treatment in relation to various prognostic factors.Material and methods. This investigation enrolled 102 children with a brainstem tumor. A morphological study was not performed because of the location of the tumor and the inability to perform a surgical intervention. The patients’ mean age was 7.1 ± 3.4 years. The results of the study were assessed in terms of overall cumulative survival rates.Results. Primary chemoradiotherapy was performed in 52 of the 102 patients; 50 children underwent repeat chemoradiotherapy. In the entire group, 89 (87%) patients received no surgical treatment; while subtotal resection or ventriculoperitoneal bypass surgery (VBS) could be done in 13 (13%) children; the median follow-up of the entire group was 19.2 weeks. In the study groups, the statistical differences in overall survival rates were significant (Log-Rank p = 0.044).Conclusion. Regardless of primary or repeat radiotherapy and chemotherapy, surgical intervention, including VBS in children and adolescents, significantly affects overall survival rates.
