The aim of the study was to present the first clinical experience of clinical trials of new methodology of the CI speech processor fitting with the new software Nucleus Fitting Software - NFS. At all 4 fittings sessions there were 15 children. During the fitting sessions 2 techniques were used - with the new NFS software and classical technique were used. The efficiency and need of collaboration of the speech therapist and the audiologist in the CI fitting sessions is proved. Results of inspection have shown that in 76.6% parents have established emotional cooperation with the children. Most of children have begun to seize the spontaneous speech in natural communication (60% - words, 23.3% - simple phrases). The results received by us allow to recommend the offered technique of the facilitated speech processor fitting technique for widespread introduction in clinical practice.

Endoprosthesis replacement is the leading surgical procedures among the whole range of procedures performed on hip joint. The optimal position of the acetabular component of prosthesis is an important factor determining the success of the operation. In the Center of Traumatology and Orthopedics of the Burdenko Main Military Clinical Hospital of the Russian Ministry of Defense was developed a hip replacement navigation system. In the given prospective and randomized clinical study acetabular component prosthesis was fixed with the help of "image-free" navigation system (the control group, n=56) and with the help of navigation system developed by the authors (experimental group, n=50). An evaluation of the cup position after the operation was carried out by means of computed tomography of the pelvis. An average value of anteversion in the experimental group was 14 +/- 3 degrees in the control group--13 +/- 4 degrees. The average value of the inclination of the experimental group was 44?5, in the control was 45 +/- 4 degrees. Developed navigation system allows us orienting acetabular component of prosthesis properly, reducing time of surgical procedure and obtaining good and excellent functional results after the surgery.

Complex examination and treatment of 120 patients with anatomo-functional insufficiency of the abdominal wall was made after operations on organs of the abdominal cavity and retroperitoneal space. In the 1st group (60 patients) the abdominal wall was sutured by traditional methods. In the 2nd group (60 patients) laparotomy was followed by implantation of polypropylene endoprosthesis by over-aponeurotic method, and suturing of the lateral wall--by sub-aponeurotic method. At the long-term postoperative period the postoperative ventral hernias were formed in 21.6% of patients of the 1st group, in the 2nd group of patients hernias were not detected. Physical component of quality of life of the 2nd group was 1.5 times, and mental component 1.7 times higher than in the 1st group.

The experimental study of liver cirrhosis with portal hypertension syndrome, modeled in 38 dogs, has been performed. Surgical treatment aimed on the portal decompression consisted of the implantation of the Celsite port-system. The reliable portal pressure decrease was observed in the first week of the experiment (from 269.3±17.8 to 157.4±26.5 millimeter of water). Portal decompression reasonably led to the improvement of the general state of the animals.

Impedance measurement as an objective method of hearing examination is widely used in cochlear implantation. Stapedial reflex is measured intraoperatively and during the procedure of cochlear implant fitting. It was found that reflex threshold levels and most comfortable levels vary significally in one patient and between all patients. Thus, reflexometry is a convenient method of fitting speech processors of cochlear implants in children but the subjective method of fitting is more effective.

The aim of the study was comparison of perception of pure tones and speech intelligibility in postlingually deaf patients using a cochlear implant before and after elective adjustment sessions. 10 patients were examined: 5 children (age 8-15.5 years) and 5 adults with cochlear implants NUCLEUS CI22 (6 patients) and NUCLEUS CI24 (4 patients) produced by COCHLEAR (Australia). It was found that between the adjustment sessions all the implanted patients showed deteriorated perception of pure tones throughout all the frequencies and therefore their speech intelligibility worsened. The recommended period between the adjustments is 6 months as acoustic perception degraded with time. However, such observations are not significant in view of insufficient material. Objective assessment of acoustic perception is necessary because subjective sensations are not always true.

Adjustment of a speech processor of the cochlear implant is the first procedure in rehabilitation of patients after cochlear implantation. Influence of setting understated and overstated threshold levels of current perception on processing of a speech signal was studied. The results of the study show that only precise estimation of current perception threshold and comfortable loudness allow transmitting maximal acoustic information for each patient.

Congenital anophthalmia and microphthalmia were responsible for 1.7-1.8% cases of all cases treated at laboratory of plastic surgery and ocular prostheses. Clinical picture of the condition is described. The philosophy of rehabilitation of patients with congenital anophthalmia and microphthalmia is as follows: 1) no operations during the first years of life; 2) early staged fitting with prostheses; 3) surgical treatment in older age, when the resources of conservative extension of the cavity are exhausted. The authors emphasize that active conservative treatment should be preferred. Method for nonsurgical extension of the conjunctival cavity by staged insertion of prostheses is described. Positive and negative aspects of surgical treatments are discussed. The authors emphasize that surgical activity in early age is extremely harmful. Results of surgical treatment of 27 patients are presented. The patients were divided into 2 groups differing by the tasks of surgery: repair of consequences of previous operations (59.2% cases) and typical correction of the eye lids (40.8%). Clinical examples are offered.
