The OBJECTIVE was to compare efficiency of the use of various types of conduits during coronary artery bypass grafting (CABG).METHODS AND MATERIALS. The retrospective study included 282 patients, who underwent CABG, performed with different types of conduits. The average age of the patients was 63.2±8.5 years; the majority of 231 patients (81.9%) were male. Patients were separated in 3groups depending on the use of conduits: the first one included patients after bimammary coronary artery bypass grafting (n=77); the second one involved coronary artery bypass grafting using radial artery(n=87); the third one included patients with venous conduits (n=77). The main endpoint of the study was the development of conduit dysfunctions in the long-term period according to diagnostic coronary artery bypass grafting.RESULTS. The average follow-up time in the long-term period was 44.9±39 months. During this period, 198 patients underwent diagnostic coronary artery bypass grafting. According to the results of the analysis, the greatest effectiveness was observed in groups using arterial conduits – the left internal thoracic artery and radial artery. For example, the incidence of dysfunction of the left internal thoracic artery in the anterior artery basin was 8.2%; the incidence of dysfunction of radial artery in the right artery basin was 12.5 %, and in the obtuse marginal branch basin – 11.5 %; while the incidence of venous conduits’ dysfunction varied, reaching its maximum of 42.8 %.CONCLUSIONS. Thus, the greatest efficiency was observed when using the left and the right internal thoracic arteries, thereafter by the radial artery. The results of using venous conduits were lower.

Aim. To compare the levels of novel biomarkers of systemic inflammation in pati­ents with and without atherosclerosis, as well as between groups of patients with atherosclerosis of different severity.Material and methods. Patients with suspected coronary artery disease after selective coronary angiography were included. The analysis included 901 patients with mean age of 61±10 years (men, 60% (n=549)). The patients were divided into two following groups: with and without atherosclerosis. The Propensity Score Matching method was used to adjust for baseline clinical differences.Results. In the group with atherosclerosis, the values of the systemic inflam­mation response index (SIRI), systemic immune-inflammation index (SII) and ag­gregate index of systemic inflammation (AISI) were significantly higher than in patients without atherosclerosis: 0,906 (0,632; 1,36) vs 0,745 (0,519; 1,02), p1,05, area under the curve (AUC) confidence interval (CI) — 0,615 (0,571-0,658), p368, AUC CI — 0,572 (0,528-0,616), p=0,004, sensitivity — 72,1%, specificity — 43,4%; for AISI >248, AUC CI — 0,604 (0,560-0,647), p1,05, for SII >368, for AISI >248.

Aim. The research on medical technologies for optimization of complication prevention and of post-surgery management of patients with coronary heart disease (CHD) with ejection fraction (EF) of the left ventricle (LV)

Aim. To assess safety and efficacy of minimally invasive myocardial revascularization (MIMR) in coronary heart disease (CHD) patients with left coronary artery (LCA) trunk pathology. Material and methods. Forty-six CHD patients (41 males, 5 females) with LCA trunk pathology, who underwent offpump coronary aortic bypass graft (CABG) surgery, were examined. Mean age of the patients was 58.7±2.5 years, mean CHD duration – 53.9±11.3 months. Results. Off-Pump Coronary Artery Bypass surgery is effective in CHD patients with LCA trunk pathology. It provides good results during in-hospital treatment, and is associated with low intra-operational risks. Conclusion. CHD patients with LCA trunk pathology require special attention at all levels of treatment. MIMR is effective and safe in this category of patients. It gives an opportunity to decrease CABG traumatic effects and is prospective for both patients and surgeons.
