The article focuses on the review of a pharmacotherapeutic approach to the therapy of hand osteoarthritis (HO). Apart from non-steroid antiinflammatory drugs (NSAID), promising therapeutic means of HO therapy include steroid drugs, chondroitin sulfate (CS), glucosamine sulfate (GS), and vitamin D. Transdermal application of NSAID and steroid drugs reduces the side effects without a decrease in the effectiveness of the therapy. Reconstructive therapy of cartilaginous and connective tissue of hands includes a long-term application of CS/GS, and in the acute period, injections of CS/GS.

Purpose of the research was studying circannual variation of parameters of hormonal regulation, lipids, oxidative systems of an organism of healthy men and women in conditions of Western Siberia.Monthly, within five years (2005–2009) on conditions of the voluntary informed consent 580 healthy donors (370 men and 210 women) in the age of from 25 up to 47 have been surveyed. In blood serum defined concentration of cortisol, insulin, thyroid hormones, concentration of cholesterol and other lipids, markers of stress and oxidative. Obtained data have been processed by adequate methods of the mathematical and statistical analysis.The analysis of results has shown that activity of functioning systems adaptation an of organism at all levels of the organization undergoes essential changes within a year. Revealed correlations are the certificate of cause and effect attitudes between the systems, providing adaptive mechanisms in constantly changing conditions of a living environment. The received results testify to an absolute must of revision of diagnostic criteria and expansions of concept of norm in view of circannual variation of functioning of physiological systems of an organism.

A clinical case of a sigmoid cancer patient with solitary lung metastasis is discussed in this article. Use of minimally invasive surgical technologies allowed to perform a simultaneous combined surgery on abdominal and thoracic cavities and improve rehabilitation time and time to systemic chemotherapy initiation. In presented clinical case patient was comorbid with stage III obesity, which was considered as a contraindication to laparoscopic surgery for a long time.

Авторы приходят к выводу, что муниципальные образования вправе устанавливать обязанность по уборке прилегающей территории для хозяйствующих субъектов, поскольку такая обязанность является составной частью благоустройства.

Many users use web maps not only in the traditional sense – ground navigation, but also as a background for their own services showing on the basemap different user-generated content, statistical information, media content. Today, the developers of such services have the option of using resources not only of many web portals with online maps, but also the option to select different basemaps such web portals have. Today is a rare user of mapping API or web-GIS aims to create own basemap. He rather connects to the existing one and «changes» it, applying own objects, thematic layers, and other information over the top of it. Therefore, he is interested in the mapping service that provides to him either a universal solution (one-map-fits-all), or the ability to choose a basemap, which will be well suited to the presentation of user's own data. The obvious question is – does it make sense to classify basemaps and limit their number on the web service, or allow any thematic map function as a basemap?

Variations of cerebral blood flow have been studied during a day with the use of rheoencephalography and Voll diagnostics in healthy teenagers and teenagers with consequences of natal trauma of cervical spine. The studies have proved that teenagers with trauma consequences had amplitude and phase desynchronosis in all REG parameters and electrical conductivity of some points of Voll meridians. The determination of the time, at which extreme REG values (acrophase and bathyphase) take place, with the aid of cosinor-analysis forms the basis for more correct scheduling of procedures during a day for teenagers with consequences of natal trauma of cervical spine.

The aim was to study the efficiency of group chronophysiotherapy of patients with motor defects of gall bladder living in the West-Siberian region.The analysis of the obtained results indicates that the combined physiotherapy of patients with gall-bladder dyskinesias with the use of the group chronobiological approach favors the improvement of the functional status of the biliary excretion system with normalization of indices of the contractile function of gall bladder and the increase of organism adaptability without their stress.

This article presents the comparative analysis of humoral and cellular immunity indicators under the influence of magneticlaser effect in 2 frequency ranges (150—1 5000 Hz) for treatment of pregnant women with mild anemia against the background of chronic gastritis, associated with contamination gastrointestinal tract Helicobacter pylori. The application of magnetic-laser effect in frequency 150 Hz allows to raise of hemoglobin lowers the titre of antibodies to Helicobacter pylori. The moderate stimulation of Th1 link withoit activation of humoral immunity is optimal in treatment of benign anemia of pregnancy.

We studied 57 patients with acute myocardial infarction (AMI) aged 20 to 63 years. Initially (day 25 from disease onset), Fibroblast growth factor (FGF), osteoprotegerin, vascular endothelial growth factor (VEGF-A), von Willebrand factor (vWF) were determined in patients’ blood by immunoenzyme method; hemostasis system was examined. The control group consisted of 15 healthy volunteers aged 19 to 50 years. The patients were divided into two groups: the major group was receiving Trombovazim in dosage 800 mg per day during 1 month together with the standard therapy of AMI, and the control group, which was receiving no Trombovazim. Control study of the dynamics of FGF, osteoprotegerin, VEGF-A, vWF, and hemostasis hemostasis parameters was performed after 1 month of treatment with Trombovazim. The inclusion of Trombovazim into the complex therapy of AMI was found to result in a decrease in FGF, osteoprotegerin, and VEGF-A levels due to the endothelial protection effect and apoptosis control.The parameters of vascular-platelet and coagulation hemostasis components had no significant changes.

At chronic opisthorchosis, disorders of venous blood flow in liver are observed in color Doppler liver mapping. To improve venous blood flow indices and the functional state of the hepatobiliary system as a whole, combined treatment should include factors not only ensuring the antiparasitic, antiphlogistic, and cholagogic effect, but also improving liver hemodynamics. This problem can be solved by combination of measures including morning exercises, pine baths, intake of cholagogic herbs with the following transversal galvanization in the epigastric region, and background x-ray radiation.

Chronic opisthorchiasis is human parasitologic disease with signs of decreased reactivity of the organism. One of the ways to assess the organism reactivity is qualitative and quantitative analysis of glycogen in hepatic cells. Decreased glycogen level is a sign of decreased adaptation possibilities and manifests in disregulation of adaptation reactions. Experimental studies in vivo (model of chronic opisthorchiasis) revealed favorable influence of the background resonance radiation on the glycogen level in hepatic cell which indirectly points to increased reactivity of the organism which in turn may contribute to the maintenance of hepatic morphologic-functional status of full value and to prevent possible overstrain of its reserve possibilities.

Изучение биоритмов при хроническом описторхозе имеет большое практическое значение. Электропунктурная диагностика по методу Фолля позволяет изучить суточную динамику функциональной активности органов гепатобилиарной системы и проанализировать полученные данные с использованием спектрального и косинор-анализа. Результаты показали, что у больных хроническим описторхозом на фоне перенапряжения адаптивных возможностей организма сохраняется степень корреляции между гепатобилиарной и вегетативной нервной системой и появляется корреляционная связь между вегетативной нервной системой и желчевыводящей системой, отсутствующая у здоровых лиц.

Objective: to analyze the results of treating viral-bacterial pneumonias in pregnant women and puerperas in the 2009 seasonal influenza period. Subjects and methods. Five hundred and seven pregnant women who were admitted to the clinics of three institutions for the diagnosis of pneumonia between October and December of 2009 were examined; of whom 52 subjects were hospitalized to the intensive care units for severe hypoxia and/or sepsis. The general principles of specific and symptomatic therapies, the efficiency of ventilation modes, organ protection methods, and morphological material were analyzed. Results. The total problems of the diagnosis and clinical course of the disease and the specific features of intensive care were identified. There were 2.4% deaths among all the pregnant women admitted for viral infection concurrent with pneumonia or 23.1% in all the patients admitted to the intensive care unit. The autopsy results were analyzed. Conclusion. The severest viral-bacterial pneumonias were noted in the third trimester of pregnancy. The later start of specific antiviral therapy was an independent predictor of death. Continuous veno-venous hemofiltration is inadequately effective in treating acute lung injury, producing a satisfactory organ-saving effect. The standard respiratory maneuvers showed a low effectiveness during mechanical ventilation. Key words: seasonal influenza A/H1N1, pneumonia, pregnancy, respiratory failure, intensive care.

Objective: to reveal lung morphological changes in preterm neonatal infants with hyaline membrane disease (HMD) in the use of exogenous surfactants and artificial ventilation. Materials and methods. Case histories and autopsy protocols were analyzed in 90 preterm neonates who had died from severe respiratory failure. All the neonates were divided into 4 groups: 1) 20 (22.2%) infants who had received the exogenous surfactant Curosurf in the combined therapy of HMD; 2) 19 (21.1%) babies with HMD who had taken Surfactant BL; 3) 25 (27.8%) surfactant-untreated infants who had died from HMD; 4) 26 (28.9%) very preterm neonates with extremely low birth weight who had died within the first hour of life. The lungs were histologically and morphometrically examined. Results. The study demonstrated the specific course of HMD when exogenous surfactants and artificial ventilation were used. The contributors to the development of the disease are intranatal amniotic fluid aspiration and intranatal fetal hypoxia. Conclusion. Artificial ventilation and the use of exogenous surfactants do not block the generation of hyaline membranes. The latter differ in formation time, form, and location. The differences in a cell response to hyaline membranes were found in the neonatal infants receiving exogenous surfactants. The characteristic morphological signs of the disease for all the neonates enrolled in the study are alveolar and bronchial epithelial damages and microcirculatory disorders. Objective: to optimize radiodiagnosis of neonatal lung diseases. Subjects and methods. The results of examinations were analyzed in 7 patients treated at a Kaliningrad regional children’s hospital. Along with physical examination and routine chest X-ray study, lung spiral computed tomography (CT) was made in all the babies on a Somatom Emotion spiral tomographic scanner (Siemens). The dose of irradiation was 4.1 mZV. The study was performed in the craniocaudal direction. The standard lung window mode was as follows: scanning time, 14.75 sec; scanning width, 3 mm; section width, 5 mm; 110 kV; 45 mA/sec. Results. CT established the pattern of a pathological process in the lung. It clarified the nature of the disease in all the cases. Conclusion. CT can provide an accurate topical diagnosis of a pathological process in the lung. This particularly applies to neonatal infants exposed to artificial ventilation. CT reveals the pattern and degree of evolved pulmonary complications and it is of high informative value. Key words: computed tomography, neonatal infants, lung.

Objective: to determine the causes of preterm labor and the factors contributing to the development of acute respiratory distress syndrome (ARDS) in the newborn. Subjects and methods. The paper presents the results of analyzing case histories and autopsy protocols of preterm neonates who died from ARDS. The study group comprised 20 (36.6%) very low-weight neonatal infants. Twelve (21.8%) neonates out of all the infants received the exogenous surfactants Curosurf and Surfactant BL. Lungs and placentas were histologically studied. Results. The study has demonstrated that various placen-tal abnormalities are one of the risk factors of preterm labor and ARDS. Intranatal amniotic fluid aspiration is a poor predictor. Conclusion. The efficiency of therapy with the exogenous surfactants depends on the quality of care delivered to a baby at birth, the time of switching the patient to artificial ventilation and the time of administration of a surfactant. Exogenous surfactants should be administered only in obstetric facilities that have all conveniences at their disposal to render a specialized aid to preterm infants. Tissue reactions to the grafting of samples of composite hydrogel based on polyacrylamide and cellulose of different origin (plant and bacterial) were studied in 48 laboratory outbred rats and 24 rabbits of chinchilla breed. The observations were carried out on Days 5, 14, 45 and 90 after placement of implants in the muscle, joint cavity, deep defects of the articular cartilage and subchondral bone. The study has revealed no migration and degradation of the samples. On Day 90, the signs of their biointegration (regardless of their nature) were detected in the muscles and in one case (the sample containing bacterial cellulose) in the cavity of the knee joint of the rat. The materials had good biocompatibility with muscle, cartilage and bone tissues, did not cause perifocal inflammation and effectively functioned as a prosthetic articular cartilage until the end of the study period.

Skin melanoma is believed to result from malignization of an inborn or acquired pigmented nevus under the action of a number of causative agents. With regard to this, the comparative analysis of skin melanoma tissue samples taken from patients of both sexes and pigmented nevus tissue samples was performed by polarization fluoroimmunoassay. The study involved analyzing the activity of plasmin, plasminogen, concentration and activity of urokinase type plasminogen activator, tissue type plasminogen activator, plasminogen activator inhibitor, as well as the level of growth factors--vasculoendothelial one and its receptor, epidermal one and its receptor, transforming one, fibroblasts growth factor, insulin-like 1 and 2 growth factors. The detected changes indicate possible mechanisms of malignant transformation of skin nevi. The obtained results can be used for developing the risk evaluation methods for neoplastic transformation of nevi as well as prevention methods.
