The effect of the monthly intake of Cognifen, added to antihypertensive therapy (combined medication Triplexam) on the psychophysiological conditions, the dynamics of blood flow in the brachiocephalic arteries in 66 patients with Arterial hypertension with uncomplicated cerebral hypertensive crises associated with hypertensive remodulation of brachiocephalic arteries, in comparison with Fesam and control group (33 students) has been studied. Cognitive disorder was diagnosed by the following techniques: SAN, method of Spielberg-Hanin, method of Munsterberg, assessment of personal and situational anxiety. Remodulation of brachiocephalic arteries and blood flow level were determined using duplex dopplerography. BP was measured by the «office method». The main hemodynamic, electrophysiological indicators of central hemodynamics, clinical and biochemical parameters of homeostasis were studied. It was found that Cognifen normalizes the parameters of extracranial blood flow by reconstructing turbulent blood flow into laminar flow, which was clinically manifested in cessation of cephalalgia in all subjects by the end of the first week of observation. Against the background of a decrease in personal and situational anxiety the general state of health and sleep normalized. Short-term and long-term memory, concentration of attention was improved, general and daily activity and mood increased and improved. There were no side effects suggesting discontinuation of treatment observed. In the dynamics of general clinical and biochemical studies, there were no dysmetabolic effects found. In 5 patients taking this medication, a minor side effect in the form of a moderate dyspeptic syndrome was noted, which did not require drug discontinuation, whereas in the Fesam group there was a tendency to an increase of the total bilirubin level in 14 patients. 28 patients in the Fesam group had daytime drowsiness., Изучено и проведено сравнение влияния месячного приёма препаратов когнифен (33 пациента) и фезам (33 пациента) на фоне антигипертензивной терапии (комбинированный препарат триплексам) на психофизиологическое состояние, когнитивные функции и динамику кровотока в брахиоцефальных артериях у больных АГ с неосложненными церебральными гипертензивными кризами, ассоциированными с гипертензивной ремодуляцией брахиоцефальных артерий, по сравнению контролем (33 студента). Когнитивное расстройство диагностировали с помощью методик САН, Спилбергера–Ханина, Мюнстерберга, определения личностной и ситуационной тревожности. Ремодуляцию брахиоцефальних артерий и состояние кровотока определяли с помощью дуплексной допплерографии. АД измеряли «офисным методом». Исследованы основные гемодинамические, электрофизиологические показатели центральной гемодинамики. Установлено, что когнифен нормализует показатели экстракраниального кровотока, перестраивая турбулентный кровоток в ламинарный, что клинически проявлялось в прекращении цефалгии у всех обследуемых к концу первой недели наблюдения. На фоне снижения личностной и ситуационной тревожности нормализовались самочувствие и сон, улучшились кратковременная и долгосрочная память, концентрация внимания, повысилась общая и дневная активность и настроение. Побочных эффектов, предполагающих прекращение лечения, не выявлено. Встановлено, що когніфен нормалізує показники екстракраніального кровотоку, перебудовуючи турбулентний кровотік в ламінарний, що клінічно проявлялося в припиненні цефалгії у всіх обстежуваних до кінця першого тижня спостереження. На тлі зниження особистісної та ситуаційної тривожності нормалізувалися самопочуття і сон, покращилися короткочасна і довгострокова пам’ять, концентрація уваги, підвищилися загальна і денна активність та настрій. Побічних ефектів, які б призвели припинення лікування, не виявлено. У 28 пацієнтів, що приймали фезам, відзначалася денна сонливість.

The paper reviews the literature related to the investigations of neck pain (cervicalgia) in patients with headache (cephalalgia). Neck pain is second to lower back pain as a reason for considerable socioeconomic damage to society. The prevalence of cervicalgia in the population ranges from 5.9 to 38%; the annual incidence is 10.4–21.3%; 14.2 to 71% of people report to have neck pain at some time in their lifetime. Neck pain is concurrent with cephalalgia in 70% of cases. In patients with cervicalgia, the prevalence of headache is 20–40% higher than in those with musculoskeletal pain at another site. Neck pain is as a major risk factor for migraine and tension headache (TH). Neck pain in TH progresses with the increased intensity, frequency, and strength of headache. There is a direct relationship of the quality of life worsening associated with cervicalgia to the frequency of migraine attacks and the risk of its chronization. Neck pain is noted in cervicogenic headache belonging to secondary headaches. The identification of mixed headache in a patient with cervicalgia allows the prescription of a treatment option that may be effective in relieving both headache and neck pain. The paper discusses the causes and pathogenesis of cervicalgia in patients with headache, examination methods, and main approaches to drug and nondrug therapies in relation to the leading pathophysiological mechanism, as well as new possibilities for the effective and safe relief of pain syndrome in this category of patients. Nonsteroidal anti-inflammatory drugs, myorelaxants, and their combination are observed to be effective in treating patients with cervicalgia and cephalalgia.

Idiopathic occipital epilepsy is a rare epileptic syndrome. Its incidence in a Novosibirsk cohort of patients with idiopathic focal epilepsy is 0.9%. Objective : to present a clinical description of new cases of Gastaut syndrome, the types of its course, and treatment options in these patients. Patients and methods. The study covers 17 cases of Gastaut type idiopathic occipital epilepsy in 13 women and 4 men aged 11–53 years. Results . Among 17 cases we present 4 family cases with the disease. Three generations in 2 families were observed to have epilepsy, including Gastaut syndrome concurrent with childhood absence epilepsy. The adolescent onset of the disease was seen in most cases. Its main symptoms were focal visual seizures (100%), focal sensory seizures (58.9%), cephalalgia (47.1%), speech disorders (41.2%), and secondarily generalized convulsive seizures (35.3%). According to the frequency of seizures, the investigators identified 5 types of the course: single focal seizures, rare focal seizures with or without convulsions, frequent focal seizures with or without convulsions. The identity of the course of epilepsy was found in familial cases. 76.5% of the patients had a good quality of life: 41.2% of them were untreated while 35.3% were treated; no seizures were noted.
