Resumen La deficiencia de hierro (DFe) es la patología nutrimental infantil de mayor prevalencia en el mundo y afecta la maduración cerebral y el consiguiente desarrollo cognitivo, lingüístico, motor y comportamental de quienes la padecen. El informe que se presenta tuvo como objetivo estudiar, en profundidad, literatura acerca del efecto de la DFe sobre el desarrollo neuropsicológico en lactantes. Para ello, se analizaron 51 estudios originales realizados en humanos, 49 de los cuales fueron obtenidos a través de Pubmed y 2, de Scielo. Cabe resaltar que, con el fin de discutir los hallazgos de estos artículos, se incluyeron investigaciones sobre el efecto de la DFe que utilizan modelo animal, así como estudios en humanos con desarrollo típico. Esta búsqueda se realizó a conveniencia. El reconocimiento de sus implicaciones facilitaría el trabajo terapéutico, así como la inclusión de programas de estimulación temprana junto con el manejo de la deficiencia nutrimental. Abstract Iron is involved in various aspects related to brain function, including oxygen transport, neurotransmitters metabolism, DNA synthesis, ATP production, dendritic growth, axonal development and transport, myelin production, glial development, in plasticity markers such as the brain-derived growth factor, and in synaptic plasticity. Human development (biological, cognitive, social) is associated with a bidirectional and dynamic interaction between gene activity, neural activity and environment. An environmental variable is nutrition, and it is known that the central nervous system is extremely vulnerable to nutritional deficiencies during pregnancy and in the first two years of life, period in which an accelerated maturational dynamism occurs. Therefore, a disturbance of these by an iron deficiency would result in neuropsychological alterations, with varied expressions depending on the age at which it occurs and the severity and duration of the nutritional disease. Iron deficiency is defined as the depletion of iron reserves in the body by various factors (nutritional, physiological, pathological, etc.). Three stages of the illness have been established: iron depletion, iron deficiency without anemia and iron deficiency anemia. The first is associated with a decrease in iron reserves without reaching the deficiency; it may be due to a reduction in iron intake and or absorption, excessive loss or an increase in iron requirements. At this stage, there are no functional consequences in the organism. The second stage, is characterized by biochemical changes that reflect a pathologically reduced concentration of serum ferritin with a normal hemoglobin concentration. Finally, iron deficiency anemia is defined by the combination of low concentration of serum ferritin and hemoglobin. Given that iron deficiency is the most prevalent childhood nutritional disorder in the world (affecting 43 % of children aged 6-59 months) and that it impacts brain maturation and the consequent cognitive, linguistic, motor and behavioral development of those who suffer from it, in this paper, the literature on the effect of iron deficiency on neuropsychological development in infants is analyzed in depth. The review was performed considering the short, medium and long lasting effects of iron deficiency without anemia, iron deficiency anemia, chronic iron deficiency during the first two years of life and the presence of treatment. To contextualize, the analysis of the relationship between iron and brain functioning is included and the variables that modulate the expression of its effect are addressed. To carry out the search of literature regarding the effect of iron deficiency on neuropsychological development in infants, Pubmed and SciELO were consulted. No date or language restriction was established. Different combinations of terms were used: “iron deficiency infant”, “iron deficiency fetal”, “iron deficiency neonatal”, “iron deficiency long lasting”. The analyzed reports fulfilled the following inclusion criteria: a) primary sources, b) establish a value of hemoglobin and at least one of iron (i.e, serum ferritin, mean corpuscular volume, free erythrocyte, protoporphyrin, transferrin saturation) to define the children of each group, c) human sample, d) description of the measures used for the evaluation, e) studies examined after this stage, had to determine that the sample evaluated presented iron deficiency during the first two years of life. In this paper, 51 original articles conducted in humans were analyzed, of which 49 were obtained through Pubmed and 2 from SciELO. It should be noted that, in order to discuss the findings of these reports, literature was included on the effect of iron deficiency using animal models, as well as studies in humans with typical development. The search for these was done at convenience. The recognition of its implications would facilitate the therapeutic work, as well as the inclusion of early stimulation programs together with the management of nutritional deficiency.

El síndrome de Devic, también conocido como trastorno del espectro de la neuromielitis óptica (NMOSD, por sus sigla en inglés), es considerado una enfermedad inflamatoria, desmielinizante y autoinmune del sistema nervioso central que afecta en su mayoría los nervios ópticos, el quiasma óptico y la médula espinal. Si bien en algunos casos se puede simular un cuadro clínico similar a esclerosis múltiple, hoy en día se conocen aspectos imagenológicos, inmunológicos y patológicos que permiten establecer las diferencias entre estas dos entidades. Se presenta el caso de una mujer adulta mayor con antecedente de síndrome de Sjögren en manejo ambulatorio con corticoide y azatioprina, quien ingresa por cuadro clínico de mes y medio de evolución consistente en disestesias de predominio en miembros inferiores, alteración de la marcha y compromiso visual. Ante el compromiso neurológico, se decide realizar imágenes diagnósticas y perfil inmunológico, con reportes de resonancia magnética nuclear cervical y torácica con contraste que evidencian mielitis multifocal por compromiso cervical y torácico a nivel de C4 en T7-T8 con hiperdensidad centromedular sin realce, además de autoanticuerpos séricos dirigidos contra el canal acuaporínico (AQP4) positivos, dando así el diagnóstico de NMOSD. Se inició manejo con pulsos de metilprednisolona concomitantemente con plasmaféresis completando cinco sesiones. Sin embargo, ante la persistencia del cuadro clínico se inició manejo con un agente biológico selectivo que bloquea la actividad de los linfocitos B tipo rituximab con resolución parcial de los síntomas. Se discute, además, la evolución clínica e imagenológica de este caso ejemplar, así como los avances más notables en el diagnóstico y manejo. Devic's Syndrome, known as Optic Neuromyelitis Spectrum Disorder (NMOSD), is considered an inflammatory, demyelinating and autoimmune disease of the central nervous system (CNS) that mainly affects the optic nerves, the optic chiasm and the spinal cord. Although the said syndrome can sometimes simulate multiple sclerosis (MS), nowadays there are imagining, immunological and pathological aspects that allow to establish the differences between these two entities. In the present paper, the case an older adult woman with a history of Sjögren's syndrome in ambulatory care with corticosteroid and azathioprine, who is admitted for a month and a half of evolution consisting of dysesthesias of predominance in the lower limbs, ambulation impairment and visual compromise is presented . In view of the neurological compromise, it was decided to perform diagnostic imaging and immunological profile. The imaging suggested a report of cervical and thoracic MRI with contrast with evidence of multifocal myelitis due to cervical and thoracic involvement at the level of C4 and T7-T8, with a report of serum autoantibodies directed against the positive aquaporic channel (AQP4). All of the above is consistent with the diagnosis of Devic Syndrome or NMOSD. Treatment was implemented through methylprednisolone pulses alternated with plasmapheresis. Five sessions of the said treatment were completed. Given the persistence of the clinical picture, treatment with rituximab was initiated resulting in partial improvement of the symptoms. The clinical and imaging evolution of this case is also discussed, as well as breakingthrough advances in its diagnosis and management.

La neuromielitis óptica, también conocida como síndrome de Devic, es una enfermedad que combina la neuritis óptica y la mielitis transversa. Hace unos anos era considerada una forma de esclerosis múltiple; sin embargo, realmente es una enfermedad diferente, con características clínicas, imágenes, serología e inmunopatologia propias. El presente artículo presenta el caso de una mujer de 29 anos con un cuadro clínico que inicia en el quinto mes de postparto, con pérdida progresiva de la fuerza en miembros inferiores, asociada a perdida de la agudeza visual bilateral. El objetivo es hacer, mediante el estudio de caso, una revisión amplia de esta enfermedad, poco común, mediada por un proceso inmune y desmielinizante. Para ello, se resumen los parámetros epidemiológicos mas importantes y se presentan las posibilidades diagnosticas y terapéuticas disponibles actualmente. Neuromyelitis Optical, also known as Devic’s Syndrome, is a disease which combines optic neuritis and transverse myelitis. Some years ago it was considered as a form of multiple sclerosis. Actually, it is consider as a different disease, on the basis of the clinical, imaging, serology and immunopatholoy profile. A case of 29 years old female patient is reported, based on her clinical findings which began in the fifth postpartum month, with progressive lower limb paresis, associated with bilateral vision loss. This paper attempts giving a synoptic overview of this uncommon immune mediated demyelinating condition; it summarises the most important epidemiological parameters and presents the diagnostic and therapeutic possibilities available today.

14 p., Los estudios en el encéfalo humano se han realizado con el fin de responder a varias preguntas de carácter científico relacionados con la neuroanatomía, neurofisiología, neurofarmacología, la neurología y la conducta. El encéfalo es el órgano en el que se encuentra la regulación de los reflejos y los mecanismos inconscientes como la transmisión del dolor, los cardiovasculares, los respiratorios, entre otros. Estos mecanismos están mediados por sustancias químicas tales como los neuropéptidos, que son cadenas cortas de aminoácidos que se han encontrado ampliamente distribuidos en el sistema nervioso central y periférico, además de ejercer acciones fisiológicas actuando como neurotransmisores, neuromoduladores (acciones paracrinas y autocrinas) y neurohormonas. En los últimos treinta años se ha incrementado el conocimiento sobre la distribución y función de los neuropéptidos en el sistema nervioso central de mamíferos (ratas, gatos, perros, alpacas, primates y humanos). Así, el objetivo de esta revisión se dirige a describir los datos más relevantes disponibles sobre los neuropéptidos en el encéfalo humano. Para ello se revisan aspectos importantes de los neuropéptidos en el encéfalo humano como: a) La distribución, b) Las relaciones anatómicas, c) Las funciones fisiológicas, d) La coexistencia, y e) Las investigaciones futuras a realizar., The human brain has been used in laboratory as an experimental model in order to answer several scientific questions related to neuroanatomy, neurophysiology, neuropharmacology, neurology and behavior. The brain is the organ in which the regulation of reflexes and «unconscious» mechanisms (transmission of pain, cardiovascular, respiratory, etc.) are carried out. These mechanisms are mediated by chemicals such as neuropeptides, which are shorter chains of amino acids that have been showed widely distributed in the central and peripheral nervous system and to exert physiological actions acting as neurotransmitters, neuromodulators (paracrine action and autocrine) and neurohormones. In the last thirty years, it has increased the knowledge on the distribution and function of neuropeptides in the central nervous system of mammals (rats, cats, dogs, alpaca, primates, and humans). Thus, the aim of this paper is to describe the most relevant information available about neuropeptides in the human brain. To do so will raise issues about neuropeptides in the human brain as: a) The distribution; b) The anatomical relationship; c) The physiological functions; d) The coexistence; and e) The related future research to make.

RESUMEN Los gangliogliomas (GG) se definen como tumores mixtos conformados por células de glía y neurona; son considerados como benignos, sin embargo hay reportes de GG que se diferencian por ser tumores de alto grado tipo glioblastoma; este cambio se observa más en el componente glial, pero el componente neuronal puede tenerlo. La patogénesis de los GG se conoce mejor, parte de esto esta representado por la identificación de cambios en los genes de p53 y la definición de marcadores de diferenciación tumoral como el Ki 67, ambos importantes para el pronóstico y las decisiones terapéuticas. El tratamiento se basa en una resección quirúrgica amplia considerando la morbilidad del paciente; la quimioterapia y la radioterapia son opciones que han mejorado la sobrevida de los pacientes con GG anaplásicos. Este manuscrito revisa los cambios en la clasificación histológica y los aportes hechos por la inmunohistoquímica, la genética y las opciones de tratamiento en los GG. Se presentan dos casos de pacientes, con diferentes edades, para ilustrar el GG benigno y el que se diferencia hacia un tumor de alto grado tipo glioblastoma multiforme, con una detallada revisión patológica. SUMMARY Gangliogliomas (GG) are defined as mixed tumors compose from glia and neurons usually with a benign biological behavior. However, there are reports of GG that differentiate toward high grade glioblastoma. This change is observed more frequently in the glial component but neuronal part could be also involved. Pathophysiology is better understood currently, part of this is represented by the identification of changes in genes at p53 and the definition of tumoral markers like Ki 67, both of them important at the moment of prognosis definition and therapeutic decisions. Treatment of these tumors implies broad surgical resection due to high morbidity of them. Chemotherapy and radiotherapy are options which improve survival of patients who suffer anaplastic varieties. This paper review recent changes in histological classification and recent contributions on histology, inmunochemistry, genetics and treatment. In addition we discuss two cases in two different ages and stages showing the usual benign behavior and the transformation to a multiform gliobastoma, along with a detailed pathological review.

La neuromielitis óptica, también conocida como síndrome de Devic, es una enfermedad que combina la neuritis óptica y la mielitis transversa. Hace unos años era considerada una forma de esclerosis múltiple; sin embargo, realmente es una enfermedad diferente, con características clínicas, imágenes, serología e inmunopatología propias. El presente artículo presenta el caso de una mujer de 29 años con un cuadro clínico que inicia en el quinto mes de postparto, con pérdida progresiva de la fuerza en miembros inferiores, asociada a pérdida de la agudeza visual bilateral. El objetivo es hacer, mediante el estudio de caso, una revisión amplia de esta enfermedad, poco común, mediada por un proceso inmune y desmielinizante. Con el término de zigomicosis (ZM) se designa un grupo de enfermedades causadas por hongos de la clase Zygomicetes, de los órdenes Mucorales y Entomophthorales, de varios géneros y especies. La presentación de ZM en animales ocurre esporádicamente en numerosas especies tanto en animales domésticos como silvestres (incluso en mamíferos marinos) y son pocos los casos comprobados de aislamiento e identificación del agente causal. Pocos hongos son considerados como neurotrópicos ya que tienden a localizarse en el SNC después de invadir el hospedador; sin embargo, la mayoría de los casos de meningitis o meningoencefalomielitis son parte de micosis sistémicas como la Candidiasis, la Criptococosis, la Aspergilosis o la ZM. El presente trabajo documenta el hallazgo de una meningoencefalitis zigomicotica secundaria a un trauma craneoencefálico en un puercoespín de cola prensil (Coendou prehensilis), siendo una enfermedad rara infrecuente en el sistema nervioso central tanto en humanos como en animales y adicionalmente sin reportes previos en esta especie. With the word Zygomycosis (ZM) are designating a group of diseases caused by fungi of the class Zygomicetes, the orders Mucorales and Entomophthorales, of several genera and species. The presentation of ZM in animals occurs sporadically in many species both in domestic and wild animals (including marine mammals), and are checked few cases of isolation and identification of the causative agent. Few fungi are considered to be neurotropic because they tend to be located in the CNS after invading the host; however, most cases of meningitis or meningoencephalomyelitis are part of systemic fungal infection such as Candidiasis, Cryptococcosis, Aspergillosis or ZM. This paper documents the discovery of a zigomicotic meningoencephalomyelitis secondary to a head trauma in a prehensile-tailed porcupine (Coendou prehensilis), being a rare disease infrequent in the central nervous system both in human and animals and additionally without previous reports in this specie.

La desnutrición afecta a millones de niños en el mundo y repercute considerablemente en el desarrollo del sistema nervioso, en el cual provoca alteraciones estructurales y funcionales. El presente estudio busca detallar el papel de la nutrición adecuada en el neurodesarrollo y describir algunas de las consecuencias de la desnutrición en ese proceso.Malnutrition affects to millions of children in world and has a significant repercussion on nervous system, where it provokes structural and functional alterations. Present paper tries to details the role of an appropriate nutrition in neurodevelopment, and to describe some of malnutrition consequences in this process.

A pesar de las grandes variaciones diarias en la ingesta de nutrientes, las reservas de energía del organismo permanecen constantes en el tiempo, lo que sugiere la existencia de numerosos mecanismos reguladores de la adiposidad, el gasto energético y el apetito. La distensión gástrica, la absorción intestinal de nutrientes, las reservas lipídicas y de glucógeno, la tasa de utilización de substratos energéticos a escala central y periférica, la palatabilidad del alimento y otros factores psicológicos influyen en la regulación a corto plazo de la oxidación y el almacenamiento de macronutrientes, así como del apetito. Además, hormonas como la insulina y la leptina actúan más a largo plazo a nivel del cerebro, reduciendo la ingesta y aumentando el gasto energético. En dicho órgano, regulan importantes circuitos neuronales, sobre todo a nivel hipotalámico, que inician señales efectoras que controlan la homeostasis energética. Este trabajo estudia el papel del sistema nervioso central en la regulación del apetito y del balance energético. In spite of the great daily variations in the ingestion of nutrients, the energy reserves of the body remain constant over time, which suggests the existence of numerous mechanisms regulating adiposity, energy expenditure and appetite. Gastric distention, the intestinal absorption of nutrients, lipid and glycogen reserves, the rate of utilisation of energy substrates on a central and peripheral scale, the palatability of the food and other psychological factors, influence the short term regulation of oxidation and the storage of macronutrients, as well as appetite. Besides, hormones such as insulin and leptin act over the longer term on the brain, reducing ingestion and increasing energy expenditure. Important neuronal circuits regulate this organ, above all at the hypothalamic level, which initiate effector signals that control energy homeostasis.
