Methodology: In a project named Carmen (after its initials in Spanish)a screening program was performed in twenty neighborhoodson women who do not have social security or insurance. We wantto show the results of the fi rst phase of this study made in 2005(clinical, mammographic and ultrasonographic findings) in Bucaramanga. Results: A total of 1.054 procedures were performed on527 women whose average age was 53,9. (SD: 11 years). Themain complaint was breast pain, 13,1% (138 patients), 1,6% (17patients) of the women surveyed reported to have a lump, aftermedical examination a lump was confirmed in 20% of the patients.Comparing de clinical findings, mammography and breast ultrasound211 lumps were diagnosticated by physical exam, mammographyonly shows 14 and breast ultrasound 52 of these masses 44masses in mammography and 152 masses in breast ultrasounddidn´t find at the physical exam. The calcifications were the mostfrequent find, 4% were classified malignant. Cysts were found in1,2% of the fatty breast, 14,5%, 37,7% and 41,8% in dense breast(mild, moderate and severe) respectively. Comparing the clinicalfindings, mammography and breast ultrasound the clinical examis not good for use in screening alone. Conclusion: The use ofbreast ultrasound with mammography increase the detection ofbreast lesions and provide a better description of masses.

La contribución del Doppler color en el estudio de la mama sigue siendo un tema en discusión. No obstante, en la práctica clínica diaria se ha convertido en un instrumento indispensable, formando parte integral del Ultrasonido (US) mamario. El objetivo de este artículo es demostrar su utilidad según la evidencia disponible y a través de nuestra experiencia. Se describen las consideraciones técnicas indispensables para realizar un buen estudio, los signos Doppler de benignidad / malignidad en lesiones focales de la mama y las ventajas de su uso rutinario en la práctica diaria. En nuestra experiencia es una herramienta útil para este propósito, sin embargo su impacto diagnóstico descrito en la literatura es variable. Para evaluar su real utilidad, se requiere de la realización de estudios prospectivos y de la estandarización la técnica. The contribution of color Doppler sonography in the study of breast cancer remains a topic of discussion. However, in the daily clinical practice it has become an indispensable instrument, and an integral part of the breast ultrasound (US). The aim of this paper is to demonstrate its utility based on the available evidence as well as on our experience. We describe the technical considerations necessary to conduct a good study, the Doppler signs of benignity/malignancy in focal lesions of the breast and the benefits of its routine use in day-to-day practice. In our experience, it is a useful tool for this purpose. Neverthless, its diagnostic impact as described in the literature is variable. To evaluate its real usefulness, prospective studies along with standardization of the evaluation technique would be required.

La Secretaría de Salud y del Ambiente de Bucaramanga viene desarrollando, como parte del proyecto Carmen, el Programa de Detección temprana de cáncer de mama, con el propósito de mejorar las condiciones de acceso de la población pobre no asegurada de los 20 barrios del área demostrativa. Se presentan los resultados del trabajo realizado en el 2005. Metodología: Estudio retrospectivo descriptivo de la prevalencia de hallazgos clínicos, mamográfica cos y ecográfica cos según la categorización Birads. El tamizaje incluyó interrogatorio, examen físico e imágenes en mujeres mayores de 40 años sin seguridad social, residentes en nueve barrios del área demostrativa del programa Carmen. Resultados: Se realizaron 1.054 procedimientos en 527 mujeres. El 53% de la población tuvo exámenes normales, mientras que el 40% tenían algún tipo de hallazgo benigno (cal-cifi caciones, lesiones nodulares sólidas o quistes). Las patologías benignas que se diagnosticaron con biopsia fueron fi broadeno-mas, cambios fi broquísticos, cambio fi broadenomatoide, mastitis crónica y adenoma tubular. Hubo lesiones diagnosticadas en la mamografía que no se observaron en la ecografía y viceversa, pero al combinar los dos métodos diagnósticos mejoraba la caracterización de las imágenes, la exactitud diagnóstica y los hallazgos. Conclusiones: el examen físico por sí solo, no es confiable para el tamizaje. La utilización de la ecografía en forma simultánea con la mamografía mejora la detección de lesiones mamarias y caracterización de las mismas. [Jaramillo N, Mantilla JC, Rey JJ. MedUNAB 2007 The Ministry of Health and Environment of Bucaramanga has been developing, as part of the Carmen project, the Breast Cancer Early Detection Program, with the purpose of improving access conditions for the uninsured poor population of the 20 neighborhoods of the demonstration area . The results of the work carried out in 2005 are presented. Methodology: Descriptive retrospective study of the prevalence of clinical findings, cos mammography and cos ultrasound according to the Birads categorization. The screening included questioning, physical examination and images in women over 40 years of age without social security, residing in nine neighborhoods of the Carmen program demonstration area. Results: 1,054 procedures were performed in 527 women. 53% of the population had normal examinations, while 40% had some type of benign finding (cal-ciptions, solid nodular lesions or cysts). The benign pathologies that were diagnosed with biopsy were fibroadenesmas, fibrocystic changes, fibroadenomatoid change, chronic mastitis and tubular adenoma. There were lesions diagnosed on mammography that were not seen on ultrasound and vice versa, but combining the two diagnostic methods improved image characterization, diagnostic accuracy, and findings. Conclusions: physical examination alone is not reliable for screening. The use of ultrasound simultaneously with mammography improves the detection of breast lesions and their characterization. El objetivo de nuestro estudio fue dar a conocer la experiencia en biopsias core de la Unidad de Imagenología del Hospital Padre Hurtado. Se revisaron retrospectivamente fichas de pacientes sometidas a core durante los años 2005 y primer semestre del 2006, registrando antecedentes importantes, resultados mamográficos, ultrasonográficos, de biopsias core y quirúrgicas, cuando existían. La población fue de 37 mujeres con edad promedio de 49,8 ± 11,0 años. La principal indicación de core fue la presencia de un nódulo sólido (91,7%); el 67,6% de estas biopsias resultaron benignas y 32,4% indicaron malignidad. 15 mujeres fueron sometidas a biopsias quirúrgicas, todas aquellas con core indicativas de malignidad (12), y 3 con core benignas. Sólo dos biopsias quirúrgicas no fueron concordantes con sus core, dando un coeficiente de correlación de Spearman de 0,72 (alta), p = 0,0001. La biopsia core en nuestro hospital demostró una alta correlación con la biopsia quirúrgica y fue de gran utilidad, evitando cirugías innecesarias de lesiones benignas The objective of our study was to describe the experience in US-guided core biopsy of the Radiology Department of Hospital Padre Hurtado. Case files of women that had undergone core biopsy during 2005 and the first semester of 2006 were retrospectively reviewed, obtaining basic demographic data, mammography and ultrasound reports, and results of core biopsy and surgical biopsy, when these were available. Our study population consisted of 37 women with a mean age of 49.8 ± 11.0 years. The main indication for core biopsy was breast nodule (91.7%). A 67.6% of core biopsies were benign and the remaining 32.4% were indicative of malignancy. 15 women had surgical excisions, all of those with indicative malignant core biopsies (12), and 3 with benign ones. Only two surgical biopsies were not consistent with their previous core, with a Spearman correlation coefficient of 0.72 (high), p = 0.0001. Core biopsy at Hospital Padre Hurtado showed high correlation with surgical biopsy, and, was useful avoiding unnecessary surgeries of benign lesions

Introducción. El control radiológico de la mama operada por cáncer y reconstruida tiene un doble objetivo: detectar precozmente una posible recurrencia y aportar datos sobre el estado de los mecanismos reconstructivos utilizados. Métodos. Deberemos conocer los antecedentes clínico-quirúrgicos de la paciente, en especial, la técnica quirúrgica, el modelo de implante colocado, su localización y la existencia o no de sintomatología.Las técnicas radiológicas empleadas son la mamografía, ecografía y resonancia magnética. La primera es útil para detectar roturas extracapsulares pero ineficaz para roturas intracapsulares. La ecografía es superior en la visualización de signos de rotura intracapsular y en la detección del carcinoma infiltrante, aunque no consigue visualizar con fiabilidad las calcificaciones. La resonancia magnética es la técnica más sensible y eficaz para detectar recidivas infiltrantes y también para roturas intra y extracapsulares. Si la naturaleza exacta de una lesión no puede ser determinada con fiabilidad con técnicas de imagen, el siguiente paso es una biopsia percutánea, extrayendo muestras aptas para análisis citohistológico. Los sistemas de guía para punción percutánea incluyen: palpación, mamografía-estereotaxia, ecografía y resonancia magnética. Resultados. Los hallazgos anormales en la mama reconstruída se clasifican en tres grupos, según su origen y localización: 1- dependientes de los implantes, 2- patología benigna extraprotésica y 3- patología maligna. Se repasan las diferentes situaciones patológicas que pueden aparecer. Conclusiones. El control radiológico anual de la paciente reconstruida tras un cáncer de mama es importante por el alto riesgo de recidiva y nuevos tumores. El control radiológico y el clínico son complementarios e incluyen el control local y regional. Mamografía, ecografía y resonancia magnética son las técnicas indicadas para el control radiológico. La sospecha de recidiva deberá ser confirmada mediante punción percutánea. El uso adecuado de estas técnicas requiere experiencia y dedicación preferente a la radiología mamaria. Introduction. Radiological control of the breast that has been operated because of cancer and reconstructed has a double aim: to provide early detection of any recurrence and data on the state of the reconstructive mechanisms employed. Methods. We must know the clinical-surgical antecedents of the patient, especially the surgical technique, the implant model used, its localisation and the existence of any symptomatology. The radiological techniques employed are the mammography, ultrasound and magnetic resonance. The first is useful in detecting extracapsular breast implant rupture but inefficient in intracapsular rupture. Ultrasound is superior in the visualisation of signs of intracapsular rupture and the detection of infiltrating carcinoma, although it does not manage to visualise calcifications with reliability. Magnetic resonance is the most sensitive and efficient technique for detecting infiltrating relapses and also for intra and extracapsular ruptures. If the exact nature of a lesion cannot be reliably determined with image techniques, the next step is a percutaneous biopsy, extracting samples that are suitable for cytohistological analysis. The guide systems for percutaneous puncture include: palpation, mammography-stereotaxy, ultrasound and magnetic resonance. Results. Abnormal findings in the reconstructed breast are classified in three groups, according to origin and localisation: 1.- dependent on the implants; 2.- extraprosthetic benign pathology; and 3.- malign pathology. The different pathological situations that might appear are reviewed. Conclusions. Yearly radiological control of the patient with reconstruction following breast cancer is important because of the high risk of relapse and new tumours. Radiological and clinical control are complementary and include local and regional control.Mammography, ultrasound and magnetic resonance are the most suitable techniques for radiological control. Suspicion of relapse should be confirmed by percutaneous puncture. The correct use of these techniques requires experience and a preferential dedication to breast radiology.
