Background: Periampullary duodenal diverticula are rare and pancreaticobiliary complications infrequent, however, when they are diagnosed and associated with symptoms, they warrant urgent intervention. The aim of this article is to present a clinical case of severe cholangitis secondary to the presence of a periampullary diverticulum successfully treated endoscopically., Clinical Case: A 68-year-old man with a history of diabetes and hypertension, was admitted to the emergency room with symptoms of abdominal pain, fever, and tachycardia. With acute kidney injury and alterations in liver function tests, ultrasound with dilated common bile duct and gallstones. Magnetic resonance cholangiography is performed, showing duodenal diverticulum and choledocholithiasis. Antibiotic management is given, and endoscopic retrograde cholangiopancreatography is decided, finding a duodenal diverticulum with stones and pus inside, sphincterotomy, transpapillary dilation and multiple sweeps are performed. Cholecystectomy was performed 7 days later, and the patient was discharged without complications., Conclusions: In patients with signs of severe cholangitis, it is important not to delay endoscopic retrograde cholangiopancreatography, even when infrequent associated pathologies are evidenced, such as a periampullary duodenal diverticulum, since this represents the diagnostic and therapeutic method of choice with high rates of resolution in the case of an obstructive pathology of the bile duct., (© 2023 Revista Médica del Instituto Mexicano del Seguro Social.)

We present an obese patient who underwent high digestive endoscopy due to dysphagia after accidental ingestion of chicken bone. There is mild reflux oesophagitis and duodenal atrophy. Intestinal biopsy reports severe villous atrophy with lymphoplasmocytic infiltrate. Autoantibodies for celiac disease return positive. The patient never presented digestive symptoms.

In this case we present a patient with chronic abdominal pain of a year of evolution, three months before being admitted to hospital the patient presented melaena and anaemia. Within the surgery antecedents it appears a caesarean 2 years before and a cholecystectomy a year before. In the endoscopy study a textiloma was observed coming up from the posterosior side of the antrum which, after its endoscopic removal, showed a gastro-duodenal fistula with a non-patent pylorus.

Background: the presence of duodenal diverticula was first described in 1710 by Chromel. Duodenal diverticulum is the second most common site of diverticula in the digestive tract. Anatomically duodenal diverticula are located in 10 to 67% in the second portion of duodenum, and its finding in most cases incidental. About 90% of patients appear asymptomatic, manifesting symptoms mostly once established complications such as: gastrointestinal bleeding and perforation., Clinical Case: 78-years-old woman who attended our Emergency department with dyspnea, moderate epigastralgia, abdominal bloating, constipation and difficulty to pass gas; Laparotomy was performed to identify duodenal diverticulum in the third portion of the duodenum with a perforation of 5 mm in its cupula. It proceeds with diverticulectomy., Conclusions: The diagnosis of duodenal diverticulum as a cause of acute abdomen must be considered in our differential diagnosis in acute abdomen supported by imaging and endoscopy. The surgical management of duodenal diverticulum, in particular the resection of the diverticulum, remains as the recommendation for treatment with less morbidity and a good recovery.

A 85-year-old woman was seen for a 7 day history of abdominal pain and hematemesis the day of her admission. Endoscopic and radiologic tests revealed cholecystoduodenalfistula and stones (Bouveret syndrome).

Alimentary tract duplications are very rare, especially among adults, and only in 4-12% of the cases the duodenum is the site of origin. The main symptoms are usually epigastric pain, weight loss and vomiting. We report a case of a duodenal duplication cyst filled with enteroliths in a 23-year-old female patient presenting these symptons. The computed tomography showed a mesenteric cyst and the patient underwent a surgical procedure during which the diagnosis of duplication cyst with enteroliths was made and the cyst was excised with success. The diagnosis before surgery in these cases is extremely difficult especially if one considers how rare they are. The most helpful exam is the computed to- mography and the treatment is mainly surgical, although there are recent case reports showing the possibility of endoscopic resection (still being discussed). The presence of enteroliths can be explained by stasis and food alkalinity inside the cyst.

Bouveret's syndrome is a rare type of gallstone ileus in which a gallstone enters the intestinal tract via a cholecystoenteric fistula and is lodged in the duodenum or the stomach. Since the first description by León Bouveret in 1896, fewer than 200 cases have been described in the worldwide literature. Mortality is high, at 25%, but may be related to the advanced age of the typical patient and comorbidities, as well as diagnostic delay. Diagnosis may be made with radiological (abdominal X-ray, ultrasound, computed tomography or magnetic resonance imaging) and endoscopic techniques. Endoscopy is preferred as the first therapeutic option but is frequently unsuccessful and surgery is often required. We present the case of a patient admitted to hospital with a history of vomiting after eating and epigastric pain. The management of this rare cause of gastric outlet obstruction is discussed.

Introduction: End Stage Renal Disease (ESRD) causes changes that affect every organ system including the digestive tract. These changes are manifested by a variety of symptoms and are responsible for a high degree of morbidity and mortality. The main objective of this study is to determine the most common endoscopic lesions of the upper gastrointestinal tract seen in patients with a diagnosis of ESRD., Material and Methods: This is a descriptive and retrospective study that included patients (age > 18 yrs.) with a known diagnosis of ESRD. The study was done at Hospital Central FAP - Hemodialysis Program from January, 2000 to February, 2007. Patients had an esophagogastroduodenoscopy (EGD) during that period of time. We conducted a chart review to evaluate medical history, demographics and EGD results., Results: The study included 54 patients with a diagnosis of ESRD; mean age was 73.2 years and the male to female ratio was 1.45:1. The average time from diagnosis was 32 months and the mean blood urea nitrogen and creatinine were 94.4 and 3.9 mg/dL respectively. Symptoms were present in 37.1% of the patients and the most common cause of endoscopic evaluation was upper gastrointestinal bleed (29.6%). The most prevalent endoscopic findings included gastric erosions (35.2%), patchy antral erythema (27.8%), gastric ulcer (24.1%), and duodenal erosions (18.5%). The most common histopathologic diagnosis was superficial chronic gastritis (65.8%). Helicobacter pylori was found in 38.6% of the studied individuals., Conclusions: The most common endoscopic changes seen in patients with ESRD were gastric erosions, patchy antral erythema, gastric ulcers, and duodenal erosions.

We present a case of obstructive jaundice due to duodenal amyloidosis presenting as a bulbar pseudotumor (amyloidoma). The duodenal and biliary obstruction were treated by double bypass, hepatojejunostomy and gastroenterostomy. Our case suggests that surgical palliative treatment may be effective, and that anastomosis is probably safe, allowing the patient a good quality of life.

Diverticular disease of the duodenum is uncommon. Perforation is the least frequent complication. Diagnosis and treatment are not well defined as the presentation and symptomatology are nonspecific. Diagnostic delay carries high rates of postoperative mortality (30%). Early diagnosis is essential to reduce morbidity and mortality. We present the case of a male patient with perforated duodenal diverticulum. This case suggests that computed tomography can be highly useful in the preoperative diagnosis of this entity. Treatment consisted of duodenal exclusion and retroperitoneal drainage.

Cystic dystrophy of the duodenal wall is an uncommon complication of aberrant pancreas characterized by increased duodenal wall thickness associated with intraparietal cystic lesions. We present the case of a male patient with cystic dystrophy of the duodenal wall, which posed major diagnostic problems due to the difficulty of distinguishing this entity from tumors of the head of the pancreas. Echoendoscopy was useful in establishing the definitive diagnosis, allowing puncture-evacuation of the intracystic contents with resolution of obstructive symptoms.

Congenital extraluminal duodenal diverticula are extremely rare. They usually present as repeated crisis of abdominal pain and most of the times their diagnosis is accidental because laboratory test performed by other cause. We report a girl with relapse crisis of abdominal pain since 2 years, the laboratory and radiographic findings are displayed. It is noteworthy that this disease can be life threatening sometime, when an acute abdomen is developed. Conversely of criteria followed in adults, surgery is adviced in children even without symptoms, to avoid further risks.

Objectives: Digestive anisakiasis is a parasitic disease whose clinical manifestations depend on the effect of Anisakis simplex on the digestive tract wall. Larvae are acquired by eating raw or poorly cooked fish. It is estimated that this entity is currently under-diagnosed, although publications are becoming increasingly common. We analyse our series of digestive anisakiasis checking localization and studying its relationship with symptoms and method of diagnosis., Patients and Method: We review 23 cases of digestive anisakiasis registered between 1989 and 2001, and confirmed by the measurement of specific serum Ig E antibodies. We analyse clinical symptoms and method of diagnosis according to whether localization was gastro-duodenal or intestinal, evaluating whether surgical intervention was needed for a correct diagnosis. The statistical analysis is made using Fisher's test., Results: 23 patients were included between 1989 and 2001, 8 with gastro-duodenal localization and 15 with intestinal localization. All patients with intestinal localization had abdominal pain. Symptoms were less severe for gastro-duodenal cases, and diagnosis was made by clinical suspicion and subsequent gastroscopy, whereas more than a half of intestinal cases required histological examination of a surgical specimen for correct diagnosis. In the remaining half, diagnosis was made by abdominal ultrasonography. We also observed that the need for surgery has decreased with time from 6/6 cases in the 1989-1996 period of time to 2/9 in the 1997-2001 period of time., Conclusions: Clinical manifestations of anisakiasis vary depending on localization, symptoms being more severe in intestinal forms. The diagnosis of gastro-duodenal anisakiasis did not need surgery and was based mainly on gastroscopy findings, whereas intestinal forms frequently required histological examination of the surgical specimen. In our hospital, a higher index of clinical suspicion allowed us to diagnose intestinal anisakiasis without examination of surgical specimens in the last years.

Intraluminal duodenal diverticulum (IDD) is a rare congenital anomaly, which usually causes symptoms in adulthood. We report a 39-year-old woman, who presented several episodes of upper gastrointestinal bleeding and who was successfully treated with surgery. The clinical presentation, diagnosis and treatment of the 56 cases published in the English and Spanish literature from 1975 to 1998 are reviewed.

We report the case of a previously well 89-years-old-healthy man who presented at least four episodes of intermittent obstructive jaundice during the eight months prior to admission in our Hospital. Studies revealed a duodenal diverticulum arising near of the ampulla of Vater. We believed the diverticulum was responsible for the intermittent obstructive jaundice and we performed a choledochoduodenostomy. He had no postoperative complications and was discharged from the hospital asymptomatic. This case documents an uncommon presentation of this disease generally asymptomatic with intermittent obstructive jaundice episodes.

Crohn's disease may involve any area of the digestive tract, but its localization in the duodenum is rare. The clinical, radiological or endoscopic findings may simulate those of peptic ulcer and histologic examination is not diagnostic. In contrast with Crohn's disease of any other localization, the formation of fistulas is exceptional. The authors present one case of duodenal Crohn's disease the unspecific symptomatology of which deviated the original orientation towards the more common diseases. Steroid treatment was not sufficient to control the disease which led to complications with the formation of an enterocutaneous fistula. The association of azathioprine was very effective not only in achieving remission of the process, but also in resolving the complications.

The authors report an exceptional case of acute abdomen caused by the necrosis and perforation of a duodenal duplication cyst in a 23 year-old-patient. The cyst was located at the anterior and lateral surface of the duodenum, between its first and second portion. The patient underwent a cephalic duodenopancreatectomy (Whipple's procedure) because of the intimate adherence of the necrotic and inflammatory mass to duodenum and head of the pancreas. The histological examination of surgical specimen confirmed the diagnosis, showing a cystic structure lined with an epithelium and a common muscle wall shared by the cyst and the duodenum. There was no communication between the cyst and duodenal lumen.

Involvement of the duodenum in Crohn's disease is uncommon, and the pathomechanism of the associated acute pancreatitis remains controversial. We describe a case of Crohn's disease with duodenal involvement associated with hyperamylasemia and malabsorption showing a favorable response to steroid treatment.

A case of digestive hemorrhage due to duodenal angiodysplasia at the level of the Vater papilla is presented. Diagnosis was achieved by analysis of the surgical resection sample.

Jejunal diverticulosis is an unusual finding. We present the case of 70-year-old man with bacterial overgrowth secondary to multiple diverticula of the small intestine. The diagnosis was based on clinical events, x-ray films and mainly on the optimal answer to antibiotic treatment.

Biopsy samples obtained by endoscopy to diagnose infection by H. pylori usually come from the antral region. Nevertheless, there are few reports documenting the prevalence of infection at duodenal level. OBJECTIVE. To investigate the prevalence of H. pylori infection in duodenal bulb in different endoscopic diagnosis. METHODS. 331 patients with the following endoscopic diagnosis were studied: normal appearance (n = 55), gastritis (87), gastric ulcer (49), duodenal ulcer (120), and duodenitis (20). At endoscopy, different samples from duodenal bulb were obtained (apart from gastric fundus, body and antrum), which were processed for microbiology (Gram stain and culture) and histology (hematoxilin-eosin). RESULTS. Patients with duodenal ulcer or duodenitis had the highest H. pylori infection rate at duodenal bulb (47.7% and 65%, respectively). Differences were significant when compared with normal appearance (14.5%; p < 0.001) and gastritis (25%; p < 0.05). In all patients with H. pylori at duodenal bulb this organism was also detected at antral region. CONCLUSIONS. 1) The highest prevalence rates for H. pylori infection in duodenal bulb were found in patients with duodenal ulcer or duodenitis. 2) Diagnosis of H. pylori infection should not be based on the duodenal bulb samples alone, as this sample yields high false-negative results.

A twenty six year old woman was admitted suffering an anemia syndrome, postprandial vomiting and intermittent melena of six months evolution. The hemoglobin was 3.5 g/dL: an endoscopy detected a large tumor in the duodenum with a 90% obstruction of the lumen. A biopsy reported an erosive, acute and chronic duodenitis. Subsequently a surgical exploration with duodenotomy showed a large nodular polyp attached to a stalk that was removed: a hamartomatous polyp of Brunner's glands was reported. It coursed with gastrointestinal bleeding and symptoms of duodenal obstruction, which are two of the most common symptoms of this rare tumor.

Primary aortoduodenal fistula is an uncommon complication of abdominal aortic aneurysm and is a rare cause of gastrointestinal bleeding. A case of primary arterioenteric fistula due to the rupture of an abdominal aortic aneurysm into the third portion of the duodenum is presented. Aneurysmectomy, duodenal repair, and extra-anatomic revascularization of the legs was performed. Diagnosis of primary aortoenteric fistula is difficult. The usual explorations are not demonstrative, and exploratory laparotomy is often mandatory.

The aortoduodeonal fistula is a rare condition and has high mortality. We report five retrospective cases of digestive hemorrhage due to an aortoduodenal fistula (4 secondary and 1 primary) between 1986 and 1990. Three of them died. Attention is called to the importance of clinical suspicion and the main diagnostic methods in early diagnosis such as oral endoscopy. We also enhance the importance of emergency surgery for survival.
