Background: Apocrine hidrocystomas, also known as apocrine cystadenomas, are bening cystic tumours derived from the secretory portion of apocrine sweat glands., Clinical Case: A 78-year old female was referred to our division for assesment an asymptomatic translucent, well-defined cystic lesion located on the upper helix. The histological features were consistent with apocrine hidrocystoma coexisting with gouty tophi., Conclusions: We report the second case of apocrine hydrocystoma located in the pinna, outside of ear canal and the first case of its association with gouty tophi described. It is difficult to know which lesion was first established and if the tophi may lead to ductal obstruction and subsequent cystic retention.

Tympanic paragangliomas usually present as a vascular middle ear mass, with the most common presenting symptoms being pulsatile tinnitus and hearing loss. Introduction: Papilar adenocarcinoma of endolymphatic sac is related with Von Hippel Lindau disease at 15% of cases, has a slow growing with a high local aggressiveness, and doesn't metastasize. It causes symptoms of Meniere's syndrome due to the compression that produces at endolymphatic duct. When it presents with hearing loss is usually sudden and irreversible manner. The diagnostic is made with image tests and analysis of its structure with immunohistochemical tests. The elective treatment is surgical remove, and its main complication the perioperative bleeding it can be avoided with preoperative embolization or stereotactic radiation., Case Report: A case of endolymphatic sac tumour is presented, in a 17-years-old male with unilateral deafness and crisis of rotate vertigo, with family history of Von Hippel-Lindau disease. Perceptive deafness and right vestibular arreflexia are detected at technical exploration. In a petrous bone computer tomography appears a mass at vestibular aqueduct. We performed a petrosectomy with presigmoidal approach and saving of inner ear. Pathological analysis revealed an endolymphatic sac tumour., Discussion: In patients with a family history of Von Hippel Lindau disease and clinical symptoms of vertigo and normal hearing or with slight hearing loss we should suspect the presence of endolymphatic sac tumor. The clinical presentation of hearing loss can be sudden and irreversible even with negative or inconclusive images. Therefore, a quick action is important for the preservation of this function.

Endolymphatic sac tumours are uncommon. They have been classified as adenocarcinomas with a low degree of malignancy and no metastases have yet been documented. We report on a female patient with Von Hippel-Lindau disease and Ménière's syndrome suffering from an endolymphatic sac tumour. Tumors originating in the endolymphatic system are extremely rare, and very few articles have been published about this type of tumors. Up to 15% of cases are associated to von Hippel-Lindau disease. This article describes the case of a patient previously diagnosed with von Hippel-Landau disease that presented a sudden loss of hearing in the left ear. Specific imaging tests (computed tomography [CT] and magnetic resonance imaging [MRI]) showed the presence of a neoplasm originating in the left endolymphatic sac. Histological analysis of the surgical specimen determined it corresponded to an adenocarcinoma of the endolymphatic sac.

Tumours of the middle ear are rare. Among them glomus tympanicum is the most frequent, followed by adenoma of the middle ear (AME). This insidious pathology entity displays unspecific clinical and audiological features. Advances in histology and immunohistochemistry have greatly contributed to an accurate diagnosis. We report our experience with two patients, a middle-aged man and a woman, diagnosed in our hospital. Both patients underwent surgical treatment and have shown no signs of recurrence after a 27- and 28-month follow-up. In both cases an accurate diagnosis was confirmed by histological and immunohistochemical analysis.

A retrospective study of 50 patients with an aural polyp located in the external auditory canal was made. Our main objective was to identify predictive factors for underlying cholesteatoma, as well as to determine our management strategy for aural polyps. We analyzed the most important findings of otological imaging, hearing loss, radiological examination, and medical or surgical treatment. The anatomopathological examination in some cases showed a rare pathology that should be considered in our diagnostic evaluation. The presence of cholesteatoma underlying the polyp in 52% of cases indicates the importance of the polyp location and radiological signs as predictive factors for the diagnosis of cholesteatoma.

Recent applications of magnetic resonance in the ear pathology are described. Attention is drawn to the new magnetic resonance sequences in two and three dimensions and their contribution to the understanding of anatomy and pathology of the inner ear, especially the membranous labyrinth.

Seventh nerve involvement for tumors of the neck of the face is an event not too much infrequent. The paralysis can present due to direct propagation or for not close vicinity. Seventeen (17) patients with facial palsies in association with neoplasties seen in a four years term (1989 - 1992) are contemplated. This figure represents a 6.6 percent of the whole number studied (N. = 270). The paper deals with clinical, etio-pathogenic features and especially with the so-called "alarm signals".

We make a review of facial palsy of tumoral etiology, including the diagnosis, evolution and treatment. We also present a case of facial palsy caused by retroauricular basal cell carcinoma.

A case of florid papillomatosis lesions overlying manifestations of chronic disseminated lupus erythematosus located on the external ear is reported. Besides, the patient presented an infiltrating semi-differentiated squamous cell epithelioma in the lower lip semimucosa. A description and evaluation are made of the therapeutic plan instituted for both conditions. Besides the infrequent skin localization of florid papillomatosis another point worthy of note is its manifestation on lupus erythematosus lesions. The authors remark that they have found no record of the association reported upon in the literature at their disposal.
