Electrical shock can cause a direct myocardial damage and different types of arrhythmias, which are uncommon and occur more often when there is a high voltage exposure. We report a 19-year-old male that received a high voltage shock, falling thereafter from an altitude of four meters. On admission to the emergency room, he had second and third degree burns in the right hand and the left thigh. The electrocardiogram showed a nodal rhythm of 72 beats per minute. After four hours of monitoring, sinus rhythm returned spontaneously.

Introduction: Electrocution is the cause of a large number of accidents and, of these, a considerable percentage result in death. Several factors affect the severity and distribution of the injuries. It is known that for low voltages the characteristics of alternating current make it three times more dangerous than continuous current. The high percentage of neurological sequelae can be accounted for by the fact that nerves are the tissue with the lowest resistance in the body and electricity tends to follow the path that offers the least resistance., Case Report: A 16-year-old male who was accidentally electrocuted by an electric guitar and later suffered a cardiorespiratory arrest; the patient required intubation. After withdrawing sedation, the patient was found to be blind in both eyes and could not remember what had happened. FLAIR MR imaging revealed oedema in both occipital lobes and, to a lesser extent, in the junction between the parietooccipital and temporooccipital lobes. Treatment with steroids was established for three days. After approximately one month, the patient had recovered his sight and the brain injuries had disappeared., Conclusions: The neurological manifestations secondary to accidents caused by electricity are usually divided into two types -immediate and delayed. Blindness has rarely been reported as a sequela in those who have survived fulguration due to a lightning strike. The patient's recovery after administering treatment with steroids and the reversibility of the oedema could lead us to think that the mechanism producing this sequela was the damage to the nervous system caused directly by the electricity, which is known as the 'phenomenon of electroporation'.

We present a 56-year-old man who was admitted to an emergency service after receiving an electric shock. The ECG showed a J point and ST segment elevation of up to 5 mm in leads V1 to V3, which normalized in 24 hours. The ajmaline test caused elevation of the J point and of the ST segment up to 12 mm in leads V1 to V3, QTc lengthening, and QTc and T wave alternans. These results denoted alterations in the duration of myocardial action potentials, a common finding in patients with Brugada syndrome and long QT syndrome.

Objective: To report the clinical and tomographic findings in a case of maculopathy secondary to high-tension electric current strike., Case Report: A 27-year-old male refers a reduction in his visual acuity after electric strike. Visual acuity was 0.05 in both eyes. Circular reddish macular lesions measuring 400 micro m in RE and 200 micro m in LE were evident. Optical coherence tomography (OCT) showed intraretinal foveal cysts. At the third month follow-up visit, visual acuity had improved to 0.4 in RE and 0.5 in LE. Clinical and tomographic evaluation showed a reduction of the foveal cysts., Discussion: Electricity affects retinal pigment epithelium and retina by thermal denaturation. OCT provides a conclusive diagnosis.

Electrical injury can affect many organ systems, it seems that the damage results from the conversion of electrical energy into heat. Electrical injuries to the heart are far less frequent and thus, less known. Additionally, it is very difficult to interpret electrocardiographic and enzymatic changes, which are, in other situations, diagnostic of a myocardial infarction. We report two cases of young men without coronary risk factors. They present a rise in enzyme levels and electrocardiographic changes suggestive of acute myocardial infarction after alternating current electrocution. The difficulties in evaluating the myocardial damage in these situations are also discussed. We recommend ECG monitoring and long-term cardiac evaluation of these patients.

We present a case of accidental electrocution in a 62-years-old male. After the accident he referred a non-specific thoracic pain that disappeared in a week, remaining asymptomatic since then. On the other hand, he shows a permanent electrocardiographic pattern of anterolateral subepicardial ischaemia. Myocardial function revealed by echocardiography and pertechnectate gammagraphy, showed an early primary damage, with a posterior slow tendency to recovery. Exercise treadmill test and Tallium-Dipyridamole test were normal, so an ischaemic background could be rejected in this case. No other diseases were presented, so the changes observed could be exclusively connected with the electrical accident.
