El síndrome de Marfan forma parte de los denominados trastornos aórticos torácicos hereditarios sindrómicos con una prevalencia estimada de 2-3 casos cada 10.000 habitantes. Las principales manifestaciones clínicas afectan a los sistemas cardiovascular, ocular y esquelético, siendo las primeras las de mayor relevancia por ser las condicionantes de la supervivencia en la evolución natural de la enfermedad. Entre las entidades de afectación cardiovascular destacan la dilatación de la aorta ascendente y el prolapso de la válvula mitral. El diagnóstico actual de la enfermedad se lleva a cabo mediante los criterios del grupo de expertos de Gante, revisados en 2010. El hallazgo de la asociación de este síndrome a mutaciones en el gen FBN1, codificante de una proteína microfibrilar de la matriz extracelular de la media aórtica, la fibrilina-1, motivó una teoría etiopatogénica basada en la debilidad estructural a pesar de que no lograba explicar el conjunto de las manifestaciones clínicas. Estudios posteriores vincularon el exceso de señalización en la vía de activación de la citoquina TGF-b como causante de la aortopatía. Sin embargo los datos experimentales contradictorios obtenidos en los últimos años, junto con el resultado negativo observado en ensayos clínicos aleatorizados dirigidos a prevenir la dilatación aórtica mediante la administración de fármacos bloqueantes de la producción y señalización de TGF-b, cuestionan estahipótesis. Las líneas de trabajo actuales tienen su origen en la identificación de una concentración disminuida de la metaloproteinasa ADAMTS-1 y elevada de la enzima NOS-2 en la pared aórtica de ratones y pacientes con síndrome de Marfan, por lo que la inhibición de esta última plantea una diana terapéutica prometedora. The Marfan syndrome is part of the so-called syndromic hereditary thoracic aortic disorders with an estimated prevalence of 2-3 cases per 10,000 inhabitants. The main clinical expression features of the disorder involve the cardiovascular, ocular and skelet systems, the former being the most clinically because it is the main limiting factor of patients survival. The most frequent forms of cardiovascular involvement include the dilation of the ascending aorta and the prolapse of the mitral valve. Currently, the diagnosis is carried out through the consensus Ghent criteria, revised in 2010. The earliest investigations trying to explain Marfan patients phenotype associated this syndrome with mutations in the gene FBN1, coding for a microfibrilar structural protein of the extracelular matrix in the aortic medial layer, fibrillin-1. The finding of a defective structural protein gave rise to a pathogenetic theory that could explain several of the features of the disease based on tissue weakness alone. However, there were quite a few other abnormalities (long bone elongation, craniofacial features, skeletal muscular changes) that seemed more related to an abnormal cell behaviour and could not be easily explained by the tissue weakness theory. Subsequent studies in fibrillin-KO mice, related the Marfan aortopathy with an excess of activation in the cytokyne Transforming Growth Factor-b (TGF-b) signalling pathway and paved the way for clinical trials aimed at the blunting of aortic dilation with TGF-b blocking drugs. The negative results of the latter trials, together with newer experimental evidence challenging the TGF-b theory, set the scene for novel mechanistic paradigms. Recent observations point to a decreased availability of the metalloproteinase ADAMTS-1 and subsequent overexpression of the enzyme NOS-2 in the aortic media of mice and human patients with SMF, as one of the main mechanisms initiating the disease. Palliation, or reversal, of aortic dilation by NOS-2 specific blockage is thus warranted. Grado en Medicina

El síndrome de Marfan (SM) es un trastorno sistémico causado por mutaciones en la proteína de la matriz extracelular fibrilina 1 (FBN1). Con un patrón de herencia autosómico dominante, los pacientes se caracterizan por presentar compromiso ocular, cardiovascular y esquelético dentro de un espectro clínico variable. Se ha sugerido que la variabilidad fenotípica intrafamiliar e interfamiliar característica del síndrome ocurre por la asociación de otras mutaciones denominadas modificadoras (driver mutations). Si bien hay claridad acerca de la causalidad genética clásica de la enfermedad, las mutaciones modificadoras descritas recientemente aún no están bien dilucidadas. Se presenta un caso de SM con una mutación no descrita previamente en el gen de la fibrilina 1; se aplica la nosología de Ghent revisada y se analiza el papel de esta mutación nueva y de las mutaciones modificadoras en la génesis de la enfermedad. Marfan syndrome (MS) is a systemic disorder caused by mutations in the extracellular matrix protein fibrillin 1 (FBN1). With a dominant autosomal pattern, MS patients are characterized by ocular, cardiovascular and skeletal involvement, all within a variable clinical spectrum. It has been suggested that the intrafamilial and interfamilial phenotypic variability, characteristic of the syndrome, occurs by the association of other mutations called driver mutations. Even though there is a clear genetic causation, the recently described driver mutations are not yet fully elucidated. We present a MS case with a mutation not previously described in the fibrilin 1 gene, applying the revised Ghent nosology and analyzing the role of this new mutation and of the driver mutations in the genesis of the disease.
