Background: Primary gastric lymphomas account for less than 5% of non-Hodgkin lymphomas (NHL). The vast majority of primary gastric lymphomas are high-grade B-cell lymphomas. Primary gastric T-cell lymphomas are very rare and are usually associated with HTLV-1 infection in endemic regions. Material and methods. We describe the case of a middle-aged female patient who presented with pneumoperitoneum due to a perforated gastric ulcer, wasting syndrome, and skin and oral lesions. Results. Histopathology and immunohistochemistry confirmed primary gastric T-cell lymphoma with skin involvement. The serology for HTLV-1, Epstein-Barr virus and HIV were negative. Conclusion. The aim of this report is to present this extremely rare presentation of primary gastric lymphoma. Introducción: Los linfomas gástricos primarios representan menos del 5% de los linfomas no Hodgkin (LNH). La gran mayoría de linfomas gástricos primarios son linfomas de células B de alto grado. Los linfomas gástricos primarios de células T son muy raros y usualmente se asocian a infección por HTLV-1 en regiones endémicas. Material y método. Describimos el caso de una paciente mujer de mediana edad que se presentó con neumoperitoneo debido a ulcera gástrica perforada, síndrome consuntivo y lesiones cutáneas y orales. Resultados. La histopatología e inmunohistoquímica confirmaron linfoma gástrico primario de células T con compromiso cutáneo. La serología para HTLV-1, virus del Epstein-Barr y VIH fueron negativas. Conclusión. La finalidad de este reporte es dar a conocer esta forma de presentación sumamente rara de linfoma gástrico primario.

The objective of the study was to determine the diagnostic yield of the indirect immunofluorescence (IFI) test for the detection of antibodies against HTLV-1. A diagnostic test evaluation study was performed. HTLV-1-infected MT2 cells and HTLV-1-uninfected K-562 cells were cultured; then these cells were impregnated and fixed in sheets for immunofluorescence and faced to Peruvian sera. A total of 155 sera (80 HTLV-1-positive sera and 75 sera positive for other diseases) from the Peruvian Instituto Nacional de Salud were used. In addition, the parameters of repeatability (intra-laboratory) and reproducibility (in laboratories of the Peruvian coast, mountains and jungle) of the test were evaluated. The IFI test detected the presence of antibodies against HTLV-1 reaching a sensitivity of 98.75% (95% CI: 95.69 – 100.00%), a specificity of 98.67% (95% CI: 95.40 – 100.00%) and the Kappa index was 0.975. There were no false positives or false negatives; however, one undetermined result and one non-specific result were obtained. The test showed 100% qualitative agreement when performing the repeatability and reproducibility. The results obtained are comparable to the reference test. Therefore, the IFI test had a good diagnostic performance and would be useful for the confirmation of HTLV-1. El objetivo del estudio fue determinar el rendimiento diagnóstico de la prueba de inmunofluorescencia indirecta (IFI) para la detección de anticuerpos contra HTLV-1. Se realizó un estudio de evaluación de prueba diagnóstica. Se usaron cultivos celulares MT2 infectados con HTLV-1 y K-562 sin infección, luego fueron sembrados, fijados en láminas para inmunofluorescencia y enfrentados a sueros. Se usaron 155 sueros (80 positivos para HTLV-1 y 75 positivos para otras enfermedades) procedentes de la seroteca del Instituto Nacional de Salud del Perú. Adicionalmente, se evaluó la repetibilidad (en el laboratorio) y reproducibilidad (en laboratorios de costa, sierra y selva) de la prueba. La prueba IFI para la detección de anticuerpos contra HTLV-1 tuvo una sensibilidad de 98,75% (IC 95%: 95,69-100%), una especificidad de 98,67% (IC 95%: 95,40-100%) y el índice de kappa de 0,975. No hubo falsos positivos ni falsos negativos; sin embargo, sí se obtuvo un resultado indeterminado y uno inespecífico. La prueba mostró 100% de concordancia en la repetibilidad y reproductibilidad. Concluimos que los resultados obtenidos son comparables a la prueba de referencia. La prueba de IFI presenta un buen rendimiento diagnóstico y sería de utilidad para la confirmación de HTLV-1.

La infección por el virus linfotrópico humano de células T tipo 1 (HTLV-1) ha sido descrita en muchas áreas del mundo, como en los países del Caribe, Japón, África, Oceanía y en Sudamérica. En la presente revisión definimos la endemicidad del HTLV-1 en el país, planteando cuatro criterios epidemiológicos. Luego discutimos el tema central de la revisión: la transmisión vertical del HTLV-1, que en nuestro país sería uno de los principales mecanismos de transmisión. Dentro del desarrollo de este aspecto en particular, presentamos una estimación de la tasa de transmisión vertical y los factores de riesgo asociados con la transmisión vertical sobre la base de una revisión exhaustiva de estudios nacionales y extranjeros. Con esta revisión pretendemos dar una primera aproximación al estudio de la trasmisión vertical de HTLV-1, un aspecto poco estudiado en nuestro medio. Human type 1 T-cell lymphotropic virus (HTLV-1) infection has been described in many areas of the world, including Caribbean countries, Japan, Africa, Oceania and South America. In this review we define the endemicity of HTLV-1 in the country proposing four epidemiological criteria. Then we discuss the core subject of the review, which is the vertical transmission of HTLV-1. This mechanism is one of the main forms of transmission in our country. Within the development of this particular topic, we present an estimated rate of vertical transmission and the risk factors associated to vertical transmission based on an exhaustive review of the national and international literature. This review pretends to provide a first approach to the vertical transmission of HTLV-1, an aspect poorly studied in our country.
