RESUMEN: El objetivo de este estudio fue reportar un caso clínico donde se realizó el manejo quirúrgico de múltiples condilomas de la mucosa oral en un paciente infectado por Virus de Inmunodeficiencia Humana (VIH) bajo Terapia Antirretroviral de Gran Actividad (TARGA). Hombre de 58 años en tratamiento por infección con VIH en TARGA hace 17 años, que acude al Servicio de Cirugía Maxilofacial del Hospital Barros Luco-Trudeau con múltiples lesiones verruciformes ubicadas en margen y cara dorsal de lengua, cara interna de ambas mejillas y labio inferior. Se realizó escisión quirúrgica de las lesiones de labio, cara dorsal de lengua y cara interna de mejilla del lado derecho, obteniéndose el diagnóstico histopatológico de condiloma. Tras 2 meses de realizar la cirugía se obtuvo recurrencia. La recurrencia de las lesiones puede originarse por la recrudescencia del virus latente adyacente al lecho quirúrgico y, por ello, deben considerarse otras alternativas de tratamiento. Por el impacto en la función, estética, potencial de contagio y malignización, es necesario su tratamiento. ABSTRACT: The objective of this study was to report a clinical case in which surgical management of multiple condylomas in the oral mucosa was performed in a patient infected by Human Immunodeficiency Virus (HIV) who is under highly active antiretroviral therapy (HAART). A 58-yearold man, under HAART for 17 years for HIV infection, was admitted at the Maxillofacial Surgery Service at the Hospital Barros Luco-Trudeau as he was experiencing multiple verrucous lesions located on the lateral margin and dorsum of the tongue, as well as on the inner face of both cheeks and the lower lip. A surgical excision of the lesions on the lip, dorsum of the tongue and inner face of the right cheek was performed, where the histopathological diagnosis of condyloma was obtained. There was a recurrence two months after surgery. Recurrence of the lesions may be due to the recrudescence of the latent virus adjacent to the surgical bed and, therefore, other treatment alternatives should be considered. Treatment is necessary due to the impact on the function, aesthetics, and the potential to become contagious and malignant.

El objetivo del estudio fue determinar la frecuencia y características de la dislipidemia en pacientes con VIH en terapia antirretroviral de gran actividad (TARGA) en un hospital público peruano. Se realizó un estudio transversal en pacientes que tuvieran un perfil lipídico completo luego de recibir al menos seis meses de TARGA. La dislipidemia se definió según los criterios NCEP-ATP III. Se revisaron 2975 historias clínicas, 538 (18.1%) fueron incluidas en el análisis. La frecuencia de dislipidemia fue 74.7%. Los esquemas de TARGA que incluían inhibidores de la proteasa (IP) (OR 1.22; IC95% 1,11-1,34) y la edad mayor de 40 años (OR 1.17; IC95% 1,06-1,29) mostraron asociación con dislipidemia, ajustado por carga viral, nivel de células CD4 y sexo. En conclusión, la dislipidemia fue muy frecuente en la muestra estudiada y estuvo asociada principalmente al uso de IP. Es necesario promover el control de la dislipidemia como parte de la atención integral del paciente con infección por VIH. The objective of the study was to determine the frequency and characteristics of dyslipidemia in patients with HIV in highly active antiretroviral therapy (HAART) in a Peruvian public hospital. A cross-sectional study was carried out in patients with complete lipid profile after receiving at least six months of HAART. Dyslipidemia was defined according to the criteria of the NCEP-ATP III. We reviewed 2 975 clinical histories, and included 538 (18.1%) in the analysis. The frequency of dyslipidemia was 74.7%. HAART regimens which include protease inhibitors (PI) (odds ratio [OR]: 1.22; confidence interval at 95% [CI 95%]: 1.11-1.33) and to be older than 40 years (OR: 1.17; CI 95%: 1.05-1.28) were associated with dyslipidemia, adjusted by viral load, CD4 lymphocyte level and gender. In conclusion, dyslipidemia was very common in our sample and was mainly associated with the use of PI. It is necessary to promote the dyslipidemia control as part of the comprehensive care of the patient with HIV.

Introduction: Pharmaceutical care improves medication adherence that is why is important that the pharmacist uses instruments to evaluate and improves it through pharmaceutical intervention at pharmaceutical care. Objective: To evaluate the impact of the pharmaceutical intervention in the improvement of the medication adherence of the patients with HIV and AIDS, and to identify the factors that infl uence on medication adherence and which one could be modifi ed by the pharmaceutical intervention during pharmaceutical care. Methods: Pharmacotherapeutic follow-up was realized for 23 months to 52 patients, older than 18 years, with antiretroviral treatment for up to three months, consent informed was obtained from patients. Objetivo: Analizar la adherencia al terapia antirretroviral, a partir de la determinación de las variables de clasificación de los no adherentes, según las variables de comportamiento, y de la definición de la prevalencia de no adhesión en los adultos mayores de 50 años, atendidos en el Hospital Universitario Santa Maria / RS. Método: Investigación de enfoque cuantitativo, tipo descriptivo y transversal. La población de estudio estaba formado por pacientes mayores de 50 años que tienen el HIV / AIDS, y que estaban bajo tratamiento en HUSM, por un total de 72 pacientes. La recolección de datos se desarrolló en el periodo de abril de 2009 hasta octubre de 2010. Se realizaron análisis univariado (porcentajes de frecuencia) y bivariado, cruzando el resultado (cumplimiento del TARV) y las variables de comportamiento de la clasificación de no adherentes, por medio del test de chi-cuadrado. Resultados: La población de pacientes no adherentes fue de 21, con una prevalencia de incumplimiento del 29,2%. A partir del análisis bivariado fue posible determinar las variables de comportamiento para la clasificación de no adherentes: por alguna razón no tomó alguna de las dosis, deje de tomar los medicamentos por el consumo de alcohol, los efectos secundarios impidieron tomar alguna dosis del medicamento, no tomó el medicamento durante el tiempo que estaba trabajando. Conclusiones: Después de identificar la prevalencia de no adhesión al TARV, y la determinación de las variables que determinan la clasificación de los no adherentes, se pudo conocer la población de no adherentes y las variables de comportamiento que indican esta capacidad para la no ingestión del 100% de las dosis prescritas. Objetivo: analisar a aderência a terapia antirretroviral, a partir da determinação das variáveis de classificação de não aderentes, segundo variáveis comportamentais, e da definição da prevalência de não adesão em adultos acima de 50 anos, atendidos no Hospital Universitário de Santa Maria/RS. Método: pesquisa de abordagem quantitativa, do tipo descritivo e de delineamento transversal. A população do estudo se formou de pacientes com idade acima de 50 anos que têm HIV/aids, e que estavam em tratamento no HUSM, totalizando 72 pacientes. A coleta de dados se desenvolveu no período de abril de 2009 até outubro de 2010. Foram realizadas análises univariada (frequência percentual) e bivariada, cruzando o desfecho (adesão ao TARV) com as variáveis comportamentais de classificação de não aderentes, por meio do teste do qui-quadrado. Resultados: a população de não aderentes foi de 21 pacientes, com prevalência de não aderência de 29,2%. A partir da análise bivariada foi possível determinar as variáveis comportamentais de classificação de não aderentes: por algum motivo deixou de tomar alguma das doses; deixou de tomar os medicamentos por uso de bebida alcoólica; os efeitos colaterais impediram de tomar alguma dose do medicamento; não tomou o medicamento durante o horário em que estava trabalhando. Conclusões: após definir a prevalência da não adesão ao TARV e a determinação das variáveis de classificação de não aderentes, foi possível conhecer a população de não aderentes e as variáveis comportamentais que indicam esta potencialidade para não ingestão dos 100% das doses prescritas. Objective: To analyze the antiretroviral therapy adherence, from the determination of non-adherents' classification variables, according behavior variables, and over 50 year old adults' non-adherence prevalence definition, treated at Santa Maria University Hospital (SMUH), RS. Method: This research presents a quantitative approach, descriptive type and cross-sectional design. This study's population was patients with age above 50 years old that have aids that were treated in SMUH, total of 72 patients. Data collection was developed from April 2009 to October 2010. We performed univariate (frequency percentages) and bivariate analysis, crossing the outcome (ART adherence) and behavioral variables for classification of non-adherents through the chi-square. Results: Non-adherents' population was 21 patients, with non-adherence prevalence of 29,2%. From bivariate analysis, it was possible to determinate non-adherents' behavior variables classification: for some reason didn't take any of the doses; stopped taking medicines for alcoholic use; side effects prevented from taking any of the medicine doses; didn't take this medicine during working hours. Conclusions: After defining ART prevalence and non-adherents' behavior variables classifications determinations, it was possible to know the non adherents' population and behavior variables that indicates this potentiality to non ingestion of 100% prescribed doses.

La Cystoisospora belli, antes denominada Isospora belli, es el agente etiológico de la cystoisosporiasis, una infección oportunista que afecta a pacientes inmunodeprimidos, caracterizada por diarrea crónica y pérdida ponderal. La incidencia de diarrea crónica por este agente, en pacientes infectados por el VIH, ha disminuido considerablemente. Ello gracias al advenimiento de la terapia antirretroviral de gran actividad (TARGA), con la que se ha logrado mejorar la respuesta inmunológica del paciente y disminuir su carga viral. Se presentan seis casos de cystoisosporiasis recurrente y refractaria en pacientes con infección por VIH, en quienes se diagnosticó cystoisosporiasis pese a que previamente se encontraban recibiendo profilaxis con trimetropin/sulfametoxazol (TMP/SMX). Cinco de ellos evolucionaron de manera tórpida y fallecieron, a pesar de una buena respuesta al TARGA (adecuado incremento de CD4 y disminución de la carga viral hasta rangos indetectables), y de tratamiento con TMP/SMX por vía oral y otros medicamentos de segunda línea. The Cystoisospora belli, before denominated as Isospora belli, is the etiologic agent of cystoisosoporiasis, an opportunistic infection affecting immunocompromised patients, characterized by chronic diarrhea and weight loss. The incidence of chronic diarrhea for this agent, in HIV patients, has decreased considerably. This thanks to the advent of highly active antiretroviral therapy (HAART), which has improved the patient’s immune response and decrease viral load. We present six cases of cystoisosoporiasis recurrent and refractory to treatment in HIV patients, who was being treated with with trimethoprim / sulfamethoxazole (TMP / SMX) orally as a prophylaxis. Five of these patients passed away due to the infection, despite of the fact that they had a good response to HAART (adequate increase in CD4 and viral load undetectable) and they had been treated with second line drugs.

The objective of this study is to describe the clinical profile of the immune reconstitution syndrome due to Mycobacterium bovis Bacillus Calmette-Guérin (IRS-BCG) in children with HIV infection who receive highly active antiretroviral treatment (HAART) at Instituto Nacional de Salud del Niño de Lima (National Children's Health Institute of Lima), Peru. A case study was conducted, including 8 children with IRS-BCG, defined as the presence of regional lymphadenopathy or inflammation on the BCG vaccination site with at least one less logarithm in the viral load or immune improvement. All patients had AIDS (C3). The starting median age in HAART was 7.2 months and the event occurred 3 to 11 weeks after the treatment was started. 7 cases showed axillary adenitis. When compared with the Non IRS-BCG group, a significant association between the age at which HAART was started at one year, severe immunodepression, and increased viral load was found. It is concluded that IRS-BCG was related to a rapid clinical progression of the mother-to-child transmitted HIV/AIDS infection, severe immunosuppression and high viral load when the HAART began. El objetivo del estudio fue describir el perfil clínico del síndrome de reconstitución inmune por Mycobacterium bovis Bacillus Calmette-Guérin (SIRI-BCG) en niños con infección VIH que reciben tratamiento antirretroviral de gran actividad (TARGA) en el Instituto Nacional de Salud del Niño de Lima, Perú. Se realizó un estudio de serie de casos, que incluyó ocho niños con SIRI-BCG, definido como la presencia de linfadenopatía regional o inflamación en sitio de inoculación de BCG con disminución de al menos un logaritmo en la carga viral o mejoría inmunológica. Todos los pacientes tenían estadio SIDA (C3). La mediana de edad de inicio del TARGA fue de 7,2 meses y el evento se produjo entre 3 a 11 semanas luego de haberlo iniciado. En siete casos se produjo adenitis axilar. Al comparar con el grupo sin SIRI-BCG se encontró asociación significativa con la edad de inicio del TARGA de un año, estado de inmunodepresión severa, y carga viral incrementada. Se concluye que el SIRI-BCG está relacionado con una rápida progresión clínica de la infección VIH/SIDA de trasmisión vertical, estadio de inmunosupresión severa, y carga viral alta al momento del inicio del TARGA.

Objetivos: Evaluar la frecuencia de alteración tiroidea y los factores asociados en los pacientes con VIH/SIDA de un hospital universitario en Colombia. Pacientes y Métodos: Estudio tipo corte transversal de pacientes con VIH/SIDA durante el periodo de 2007 a 2008. Se registró niveles hormonales, inmunológicos, carga viral y tratamiento anti-retroviral. Resultados: En 636 pacientes la prevalencia de hipotiroidismo (TSH > 4,6 μUI/mL) fue de 15,5% (100/636). El análisis independiente demostró relación significativa para el uso de nevirapina (RR 1,6; IC 95% 1,1 - 2,3) y estavudina (RR 1,5; IC 95%o 1 - 2,3). Conclusiones: La prevalencia de hipotiroidismo fue alta y se relacionó con el uso de nevirapina. Introduction: The objective of this study was to evaluate the frequency of thyroid function alterations and its associated factors in a group of patients from a university hospital in Colombia. Methods: From June 2007 through June 2008, 636 HIV patients were followed in order to assess the relation of thyroid function with the use of HAART. Results: The overall prevalence of hypothyroidism (TSH > 4.6 μUI/mL) was 15.5% (100/636). The association of hypothyroidism in the independent analysis showed significant relation only for the use of nevirapine (RR 1.6; CI 95% 1.1- 2.34) and stavudine (RR 1.5; CI 95%, 1 - 2.3). Conclusions: The prevalence of hypothyroidism was surprisingly high among the studied population.

El síndrome de reconstitución inmunológica inflamatorio (SRII) en pacientes con infección por el virus de inmunodeficiencia humana (VIH), es una entidad clínica rara caracterizada por un aumento de linfocitos T CD4+ (CLTCD4+) y disminución de la carga viral servirá del VIH, que aparece corto tiempo después de iniciar terapia antirretroviral de gran actividad (TARGA). Usualmente se manifiesta con un deterioro paradójico del estado del paciente secundario a una reactivación de procesos infecciosos oportunistas. Caso clínico: hombre con diagnóstico de toxoplasmosis cerebral en el curso de infección por VIH, quien posterior al inicio de la TARGA desarrollo coriorretinitis. El paciente experimento mejoría poco tiempo después de haber suspendido la TARGA y de administrar esteroides. Conclusión: La incidencia del SRII ha aumentado en los pacientes VIH que cursan con alta carga antigénica y CLTCD4+ bajo. Su patofisiología genera incertidumbre debido a que los biomarcadores diagnósticos y pronósticos no se conocen del todo. El tratamiento propuesto lo constituye en algunos casos la suspensión temporal de la TARGA con administración de esteroides, no obstante es necesaria la realización de más estudios. Immune reconstitution inflammatory syndrome (IRIS) in context of severe immunosupression secondary to human immunodeficiency virus (HIV), is characterized by a dramatic rise of T lymphocyte CD4+ count in addition to a HIV viral load drop in, typically it is manifested by a paradoxical clinical impairment due to reactivation opportunistic of infectious processes previously under immune surveillance. A 36 years old male with cerebral toxoplasmosis in context of HIV infection developed chorioretinitis 8 weeks after commencing HAART. Workup laboratories only showed a marked T Lymphocyte CD4+ count increase with a HIV viral load lower than 40 copies/ml and other possible causes of chorioretinitis were excluded. After cessation of HAART and establishing corticosteroid therapy clinical improvement of this condition was documented. IRIS incidence has increased among AIDS patients especially with high antigenic burden and very low CD4 + T lymphocyte count. However its mechanism is still unclear due to biomarkers for diagnosis and prognosis are not completely known at all. In order to validate and refine diagnostic criteria and therapeutic approaches of IRIS more studies are needed.

Objetivos: La adherencia al tratamiento antirretroviral de alta eficacia (TARV) se considera el gold standard para el éxito del mismo. Para mejorar la adherencia en persona privadas de libertad viviendo con VIH-Sida, se propuso la utilización del tratamiento directamente observado tomando el modelo para el tratamiento de la tuberculosis. Material y métodos: se incluyeron las mujeres VIH positivas con criterios de TARV que voluntariamente decidieron participar. Los datos principales a evaluar fueron control inicial y final de CD4 y carga viral para VIH. Resultados: estudiamos 52 mujeres, edad promedio 34 años, entre 1 y 20 años de infección VIH. CD4 menor a 100 cél/ml inicial en 16 pacientes (30.7%) y final en 4 pacientes (7.6%). Carga viral indetectable inicial en ninguna paciente y final en 33 pacientes (63.4%) 21 % con infecciones oportunistas, principal tuberculosis. 17% coinfectadas con HCV. El esquema de TARV más utilizado fue 2 INTR más 1 INNTR. Conclusiones: La estrategia DOT aplicada al TARV fue efectiva en nuestras pacientes, evidenciado por el aumento de los CD4 y el mayor número de pacientes con disminución de la carga viral hasta permanecer indetectables. Si bien es difícil de implementar en tratamientos crónicos consideramos que es una herramienta útil para personas privadas de libertad. Objectives: It is considered that the gold standard for success in HAART is adherence. To improve adherence amongst inmates with HIV-AIDS, the use of directly observed treatment (DOT) is proposed using the tuberculosis treatment model. Material and methods: HIV positive female patients with ARVT criteria who voluntarily participated were used for the study. The initial and final CD4 cell count and HIV viral load were the principal data used for assessment purposes. Se presentan los resultados del estudio realizado por el Grupo de Enfermedades Infecciosas de la Sociedad Española de Sanidad Penitenciaria (GEISESP) sobre la opinión de los facultativos penitenciarios en cuanto a la conveniencia o no de realizar el control de la infección por el VIH en el medio carcelario y sobre las dificultades, esfuerzos, necesidades y responsabilidades que esta actividad puede suponer. Este estudio se efectuó en el año 2007 utilizando una encuesta diseñada ad hoc remitida a todos los facultativos de 73 centros penitenciarios españoles. Se recibieron 116 cuestionarios válidos, estimándose que los datos obtenidos podían aplicarse al total de la población con un nivel de confianza del 95% y un margen de error del 8%. La mitad de los médicos refirieron prescribir tratamientos antirretrovirales, aunque sólo el 10% decían prescribir combinaciones de rescate. Es de destacar también que sólo la mitad de los médicos se consideran formados adecuadamente para atender pacientes infectados por el VIH, aunque la mayoría afirma haber hecho formación específica de postgrado en esta materia. En general, los médicos consideran que la calidad de la asistencia recibida por los infectados por el VIH en prisión es buena. We present the results of the study carried out by The Spanish Society of Penitentiary Health Working Group on Infectious Diseases (GEISESP) on the opinion of prison doctors concerning the suitability of undertaking or not undertaking controls of HIV infection in prisons and about the difficulties, efforts, needs and responsibilities that this activity may involve. This study was carried out in 2007 using an ad hoc survey designed and sent to doctors at 73 prisons in Spain. 116 valid questionnaires were received. It was considered that the data received could be applied to the whole population with a level of trust of 95% and an 8% margin of error. Half the doctors referred to prescribing antiretroviral treatments, though only 10% admitted prescribing rescue combinations. It should be noted that only of half the doctors consider themselves to be appropriately prepared to deal with patients infected by the HIV virus, though half of them admit to having received specific postgraduate training on this subject. Generally speaking, doctors consider that the quality of the health care received by the HIV-infected population is quite good.

Introducción. El tratamiento antirretroviral de la infección por virus de inmunodeficiencia humana ha mejorado la supervivencia y la calidad de vida de los pacientes desde el advenimiento de la terapia combinada en 1996. Objetivos. Conocer el manejo farmacológico de la infección por VIH/sida. Materiales y métodos. Estudio descriptivo observacional con selección de pacientes con infección por VIH/sida, de ambos sexos, de todas las edades, en tratamiento, al menos, durante tres meses (julio a septiembre de 2006) localizados en diferentes ciudades colombianas y afiliados al Sistema General de Seguridad Social en Salud. Se diseñó una base de datos con registros sobre el consumo de medicamentos antirretrovirales, recolectados por la empresa que dispensa dichos fármacos a los pacientes de las entidades promotoras de salud. Resultados. Se estudiaron 997 pacientes con edad promedio de 37,7±13,2 años, 82,6% de hombres. Todos los pacientes recibían terapia con tres o más antirretrovirales; el orden de prescripción de medicamentos fue: inhibidores de nucleósidos de transcriptasa inversa (96,4%), inhibidores de transcriptasa inversa no nucleósidos (54,9%), inhibidores de proteasa (39,8%) y otros (0,4%), todos a dosis adecuadas. Las combinaciones más empleadas fueron: lamivudina-zidovudina-efavirenz (35%), lamivudina-zidovudina con lopinavir/ritonavir (8,4%), abacavir con lamivudina-zidovudina (5,5%), lamivudina-zidovudina con nevirapina (5,2 %) y otras 65 asociaciones diferentes (45,9%). Conclusiones. Todos los antirretrovirales se están utilizando a las dosis establecidas internacionalmente y predominan patrones de prescripción racionales para iniciar la terapia. Sin embargo, el encontrar 69 asociaciones diferentes de antirretrovirales lleva a considerar que no hay adecuados criterios para la implementación de los esquemas después del inicio del tratamiento, guiados según los datos científicos y quienes los prescriben adoptan múltiples opciones por fuera de lo recomendado a nivel mundial. Introduction. Antiretroviral therapy for treatment of human immunodeficiency virus type 1 (HIV-1) infection has improved steadily since the advent of combination therapy in 1996. Objective. The pharmacological therapies of the infection by HIV/AIDS were documented in order to determine if effective treatment regimes were prescribed. Materials and methods. Pharmacological therapies of the human immunodeficiency virus infection were compared in 997 patients affiliated with the health security system, of both sexes and all ages. All had been in treatment for more than three months (July-September 2006) and were distributed in 15 Colombian cities. The data were retrieved from medication consumption files which were maintained by the institutions that had distributed medications to the selected patients. Results. The average age of patients was 37.7±13.2 and 82.6% were men. All the patients received between three to five antiretrovirals. The medications most commonly prescribed were nucleoside reverse transcriptase inhibitors (96.4%), nonnucleoside reverse transcriptase inhibitors (54.9%), protease inhibitors (39.8%) and others (0.4%). All of the doses were at recommended levels.The most common combinations were lamivudine-zidovudine-efavirenz (35%), lamivudine-zidovudine+lopinavir/ritonavir (8.4%), abacavir+lamivudine-zidovudine (5.5%), lamivudine-zidovudine+nevirapine (5.2 %) and others (45.9%), consisting of 65 different combinations. Conclusions. All agents were used at internationally recommended doses and rational prescription patterns prevailed in the initial therapy. However, the use of 69 different associations suggested that after the beginning of treatment, clinicians do not have adequate criteria to prescribe in accordance with international guidelines of antiretroviral therapy; they adopt a variety of options that may be outside of established recommendations.

Introducción. La profilaxis para tuberculosis ha sido aceptada mundialmente para prevenir las formas activas de la enfermedad, especialmente, en pacientes positivos para VIH; pero en los países de alta prevalencia es aún controvertida su efectividad y sus indicaciones. Objetivo. Establecer en pacientes positivos para VIH el nivel de efectividad de dos esquemas de profilaxis contra la tuberculosis: con isoniacida durante nueve meses o pirazinamida/rifampicina durante 60 días, suministrados en forma autoadministrada, independientemente de la respuesta a la prueba de tuberculina. Materiales y métodos. Estudio observacional de cohorte. Se conformaron dos grupos, uno con 131 pacientes, quienes voluntariamente aceptaron recibir uno de los dos esquemas profilácticos, si el de pirazinamida/rifampicina no estaba contraindicado. El grupo control estuvo conformado por 200 pacientes seleccionados retrospectivamente, a partir de los registros de un programa de control de pacientes con VIH/sida. El seguimiento para ambos grupos se realizó durante dos años, mediante revisión de la historia clínica. Resultados. Los grupos no presentaron diferencias estadísticas significativas cuando se compararon sus características clínicas, ni demográficas. Una mayor proporción de pacientes del grupo control tuvieron recuento de CD4100.000. En el grupo con profilaxis, 8% manifestó efectos adversos, una persona presentó tuberculosis (0,8%) y en el grupo control 10 (5%) (RR=0,15, IC95% 0,02-1,18, p=0,07), la protección de la profilaxis fue del 80%, independiente de CD4, carga viral y terapia antirretroviral recibida. Conclusión. La profilaxis para tuberculosis mostró ser efectiva en pacientes positivos para VIH, independientemente del estado inmune, virológico y el tratamiento antirretroviral recibido. Introduction. Prophylaxis against tuberculosis has been recognized as important for preventing clinical forms of tuberculosis, mainly in HIV positive patients. However, in countries with high tuberculosis prevalence, prophylaxis application and effectiveness remains controversial. Objective. Effectiveness was established for two prophylaxis regimens -isoniazid treatment for nine months and pirazinamid/rifampin for 60 days. Materials and methods. Two cohort groups of patients diagnosed with HIV/AIDS were compared. One consisted of 131 volunteer patients, who received one of the two prophylactic regimens -pirazinamid/rifampin or isoniazid. The tuberculosis treatment drugs were self-administered and independent of tuberculin response tests. The second group consisted of 200 patients selected from the records of a HIV/AIDS control program. Follow up for both groups was conducted over a two-year period through clinical records. Results. The 2 groups were similar with respect to clinical and demographic variables. A higher proportion of patients in the control group had CD4 counts 100,000 copies. In the prophylactic group, 8% of patients reported adverse effects due to the drug, and one person had tuberculosis in that group (0.8%). Ten persons in the control group contracted tuberculosis (5%) RR=0.15, 95%CI 0.02-1.18, p=0.07. The prophylaxis protective level was calculated to be 80%, after taking into account CD4, viral load, and effective antiretroviral therapy. Conclusion. The prophylaxis against tuberculosis was effective in HIV positive patients, independently of the immune status, viral load, and highly effective antiretroviral therapy.
