Se describe una nueva especie del género Bathysciola Jeannel, 1910 –Bathysciola fadriquei n. sp.– perteneciente al grupo schiodtei; ésta ha sido hallada en medio subterráneo profundo, en dos cuevas situadas en Sierra de Bernera, macizo del Bisaurín, Pirineo de Huesca, Aragón, España. Los caracteres diferenciales se encuentran básicamente en las estructuras genitales: la nueva especie se caracteriza por presentar en el complejo faneroide del saco interno del edeago unas faneras apicales muy grandes, triangulares, fuertemente esclerotizadas, con un lóbulo exterior asociado. Se discute la posición taxonómica. Se completa el estudio con ilustraciones de las estructuras que permiten su diferenciación, clave de identificación y datos de distribución.

Un nuevo Laemostenus Bonelli, 1810 (Coleoptera, Carabidae) del Parc Natural dels Ports (Cataluña, nordeste de la península ibérica) Se describe Laemostenus (Antisphodrus) portsensis sp. n., localizada en cinco cavidades del Parc Natural dels Ports. Se compara con sus vecinos geográficos, L. (A.) levantinus (Bolívar, 1919) y L. (A.) lassallei Mateu, 1989, de los que se distingue por la forma de la cabeza y el pronoto, y especialmente por la morfología de la genitalia masculina. Se completa el estudio aportando datos sobre el hábitat y la ecología de la nueva especie.

Introduction: Variations in the use of healthcare services can be defined as systematic variations of adjusted rates for certain aggregation levels of the population. The study analyses how renal replacement therapy (RRT) is used, identifying RRT variability in Catalonia from 2002 to 2012., Material and Methods: Ecological study by health area using data from the Catalan renal registry. We present incident rates, standardised incidence ratios and prevalence, while variability was calculated through direct and indirect standardisation methods., Results: From 2002 until 31/12/2012, 10,784 patients initiated RRT in Catalonia: 9,238 on haemodialysis (HD) (50 treatments per 100,000 people 2010/2012), 1,076 on peritoneal dialysis (PD) (8.2 treatments per 100,000 people 2010/2012) and 470 received an early kidney transplant (KT) (4.4 treatments per 100,000 people 2010/2012). Over the 10 years, the HD cumulative incidence rate fell (7%), while the PD and KT incidence rates increased (63% and 177%, respectively); both are higher in young patients (<45 years). 4,750 patients received a kidney transplant in this period, 49% of which were aged between 45 and 65 years. There were no significant differences in variability in HD (RV 5-95 =1.3; Empirical Bayes [EB]∼ 0), or in the prevalence of KT (RV 5-95 =1.4; EB ∼ 0). Nevertheless, we found significant geographical variability in PD; notably in the districts of the province of Lérida, where the number of cases observed was greater than expected (RV 5-95 =4.01; EB=0.08)., Conclusion: Although there was a notable rise in PD and early KT incidence rates, PD is still underused when compared to international recommendations. No territorial variability was found for HD or KT, but the use of PD was found to be higher in Lérida than in other areas. To reduce PD territorial variability and increase the uptake of this technique in the other regions, we propose 3initiatives: The development of RRT support tools for shared decision-making, the encouragement of specific PD professional training and the promotion of PD through complementary reimbursement systems., (Copyright © 2016 Sociedad Española de Nefrología. Patients on renal replacement therapy (RRT) infected with the human immunodeficiency virus (HIV) are a special group with growing interest. In order to study the epidemiological data of HIV+ patients on RRT in Spain, we collected individual information from 2004-2011 (period of use of highly active antiretroviral therapy [HAART] in the Autonomous Communities of Andalusia, Aragon, Asturias, Catalonia, Valencia, Castilla la Mancha, Castilla León, Galicia, Madrid, La Rioja and the Basque Country, comprising 85% of the Spanish population. A total of 271 incident and 209 prevalent patients were analysed. They were compared with the remaining patients on RRT during the same period. The annual incidence was 0.8 patients per one million inhabitants, with a significant increase during the follow-up period. The proportion of prevalent HIV+ patients was 5.1 per 1,000 patients on RRT (95% confidence interval [CI] 4.4-5.8. Although glomerular diseases constituted the majority of cases (42%), diabetic nephropathy was the cause in 14% of patients. The nation-wide totals for these percentages were 13 and 25%, respectively. Compared to the total of patients in treatment, the risk of death was significantly higher in the HIV+ group: hazard ratio (HR) adjusted for age, sex and diabetes was 2.26 (95% CI 1.74 - 2.91). Hepatitis C coinfection increased the risk of death in the HIV+ group (HR 1.77; 95% CI 1.10 - 2.85). The probability of kidney transplantation in HIV+ was only 17% after 7 years, comparing with total RTT patients (HR 0.15; 95% CI: 0.10-0.24). Despite the use of HAART, the incidence of HIV+ patients on dialysis has increased; their mortality still exceeds non-HIV patients, and they have a very low rate of transplantation. Introduction: The standard surgical procedure for intraductal papillary mucinous neoplasms of the main duct (IPMN-M) or side branch ducts (IPMN-Br) is pancreaticoduodenectomy. IPMN-BR is a more indolent disease with a lower incidence of malignancy., Objective: To evaluate the usefulness of organ-preserving pancreatic resections (OPPR) including duodenum-preserving pancreatic head resection (DPHR) and pancreatic head resection with segmental duodenectomy (PHRSD) in patients with IPMN-BR., Patients and Method: Surgical outcomes were evaluated in eight IPMN-Br patients: DPHR was performed in 4 patients and PHRSD was performed in 4 patients. In addition, 13 IPMN patients with Whipple resections were included in the analysis., Results: The incidence of postoperative complications was 38% after Whipple resection, 100% after DPHR and 25% after PHRSD. The mean length of hospital stay was 27 days after DPHR, 22 days after Whipple resection and 16 days after PHRSD. Invasive IPMN was found in 38% of the patients in the Whipple group, and noninvasive IPMN was found in 100% of patients who underwent organ-preserving surgery., Conclusions: Pancreaticoduodenectomy remains the treatment of choice in patients with invasive IPMN. PHRSD appears to be a useful procedure for IPMN-Br located in the head of the pancreas.

Congenital tracheal stenosis (CTS) is a rare malformation that usually require corrective surgery. Resection and tracheal anastomosis is the treatment of choice when the stenosis is short. However, such an approach is not usually viable when the CTS is long, given that anastomotic pressure would be too great. New surgical techniques have allowed this serious airway malformation to be corrected satisfactorily. We report the case of a three-year-old boy with a long CTS (51% of the length of the trachea) treated with a new reconstructive technique called slide tracheoplasty. Outcome was excellent.
