Los tiros libres indirectos son situaciones que se producen durante los partidos de fútbol, siendo acciones potencialmente trascendentales en el resultado final. Los objetivos fueron determinar la efectividad de estas acciones, identificar las variables asociadas con el éxito, y proponer un modelo de tiro libre indirecto exitoso. Tras registrar 447 tiros libres indirectos durante los 64 partidos del Mundial de Sudáfrica 2010, y de realizar análisis estadístico, los resultados indican una baja eficacia en el remate, remate entre los tres palos y el gol. El 64,3% de los goles con procedencia en un tiro libre indirectos han contribuido a sumar puntos en el resultado final. El modo de envío, número de atacantes que intervienen y la organización ofensiva son variables que se han revelado como moduladoras de la eficacia. The objectives were to determine the effectiveness of these actions, identify variables associated with success, and propose a successful indirect free kick model. After registering 447 indirect free kicks during the 64 games of the World Cup in South Africa 2010, and performing statistical analysis, results indicate a low efficiency in the shot, shot between the three posts and goal. 64.3 % of goals that came from indirect free kick contributed to score points in the final result. The mode of sending, number of attackers involved and the offensive organization are variables that have been revealed as modulators of effectiveness. These statements could be a starting point to equip coaches with new tactical tools

Las tablas tradicionales y rígidas en socorrismo acuático han sido rechazadas en muchos servicios de socorrismo por los problemas que generaban. La experiencia con tablas hinchables (AIRSUPRA) ha demostrado muchas ventajas y las objeciones sobre este material están desapareciendo y ya comienzan a utilizarse en servicios de socorrismo, que antes eran reticentes. El objetivo de este estudio fue analizar el efecto del uso de la tabla AIRSUPRA en el tiempo de aproximación en una distancia de 100 metros, con una muestra de 16 socorristas profesionales (13 hombres y 3 mujeres), certificados y actualizados en técnicas de rescate, pero sin experiencia con tablas AIRSUPRA. Los resultados demuestran que cuando se realiza la aproximación con la tabla AIRSUPRA (TR1) se obtiene un tiempo de 54.13 + 8.58, mientras que cuando se realiza la aproximación a nado el tiempo es 93.19 + 25.52, con una diferencia de 39.06 segundos, estadísticamente significativa (p, The use of traditional and rigid boards in aquatic lifesaving cause more problems than advantages, for this reason was frequently rejected in many lifesaving services. El ahogamiento es una de las principales causas de muerte en el mundo y en España. Los socorristas ejercen una labor de prevención y vigilancia pero su labor es estacional y temporal. En muchos lugares, la primera respuesta a la emergencia, cuando los socorristas no están de servicio, depende de la policía, a la que se le requiere habilidades de rescate y reanimación cardiopulmonar (RCP). El objetivo de este estudio es determinar la capacidad de rescate y efecto de la fatiga sobre la calidad de la RCP de un grupo de diez policías costeros cuya área de influencia es el litoral de Vigo. El diseño fue cuasi-experimental con dos factores (pretest basal/postest rescate). Los policías pudieron realizar el rescate acuático rápido y seguro 417 ± 54,5 seg, a nivel de lactacidema se registró 12,27 ± 2,36 mmol. La fatiga inducida por el rescate afectó negativamente a la calidad de la compresiones en la RCP (p = 0,002), Drowning is a leading cause of death worldwide and in Spain. Lifeguards exert vigilance and prevention efforts but their work is seasonal and temporary. In many places the first emergency response when lifeguards are not on duty, depends on the police, which are required rescue skills and cardiopulmonary resuscitation (CPR). The objective of this study is to determine the ability of lifesaving and effect of fatigue on the quality of CPR of a group of ten coastal police whose area of influence is the coast of Vigo. The design was quasiexperimental with two factors (basal pretest / posttest rescue). Cops with basic training could perform fast and safe water rescue 417 ± 54.5 seconds, lactate level was recorded 12.27 ± 2.36 mmol. Este estudio nace con la necesidad de establecer criterios básicos y descripción de recomendaciones para mejorar el nivel de atención de los socorristas acuáticos profesionales en los espacios acuáticos naturales, concretamente en el trabajo realizado en las playas, con el objetivo de aumentar la seguridad de estas zonas de baño. El estudio se ha desarrollado en la Comunidad Autónoma de Cataluña, concretamente en tres playas de varios municipios de la provincia de Barcelona. Se puede afirmar que tras esta investigación la mayor parte de los socorristas acuáticos que han participado en el estudio valoraron positivamente las medidas y criterios propuestos, asumiendo que con los mismos se puede desempeñar mejor su trabajo, This study arises from the need to establish basic criteria and the description of recommendations to improve the level of responsiveness of lifeguards in natural aquatic areas, specifically in the work done on the beaches, in order to increase the safety of these bathing areas. The study was developed Cataluña, in three beaches of different municipalities in the province of Barcelona. We can aver after this research that most of the lifeguards who participated in the study value positively the proposed measures and criteria, through the assumption that these features help improve their work

Objetivo: El objetivo es analizar hasta qué edad podría influir una actitud radical en la esperanza de vida de los pacientes con cánceres de próstata clínicamente localizados Gleason 8-10. Material y métodos: Se hace un estudio retrospectivo de 80 pacientes con cáncer de próstata localizado T1-2N0-XM0 Gleason 8-10. Los pacientes se estratificaron en 2 grupos: grupo A 34 pacientes que por diversos motivos, edad superior a 75 años, riesgo quirúrgico, elección del paciente u otros motivos, habían sido tratados con bloqueo androgénico y grupo B, 46 pacientes sometidos a prostatectomía radical. Las variables analizadas son: mortalidad por cáncer y supervivencia cáncer específica. Resultados: Características de los pacientes: mediana de edad grupo A 75 años(66-84) y grupo B 64 años(56-75)(p=0,033); mediana de tiempo de seguimiento grupo A 54 meses(20-180) y grupo B 37 meses(12-140)(p=0,016); mediana de Gleason grupo A 8(8-10) y grupo B 8(8-10)(p=0,144); porcentaje de T1 grupo A 24% y grupo B 41%(p=0,096); mediana de PSA grupo A 10(4-91) y grupo B 12(4-71)(p=0,269). La mortalidad cáncer específica del grupo A es de 24(71%) y en el grupo B 3(7%) (p=0.000). A los 5 años han fallecido 20(59%) pacientes como consecuencia del cáncer en el grupo A y 1(2%) de los pacientes en el grupo B. Las curvas de Kaplan-Meier y test de Log-Rank muestran diferencias significativas en la supervivencia cáncer específicas entre los 2 grupos. La regresión de Cox muestra que las posibilidades de morir por cáncer de próstata son superiores en el grupo de pacientes que no recibieron tratamiento con intención curativo, el riesgo relativo (HR 95% IC) en el grupo A es de 6.826(2.032-22.931). Conclusiones: Algo más de la mitad de los pacientes con cáncer de próstata localizado Gleason 8-10 tratados de forma conservadora, mueren por cáncer dentro de los 5 años siguientes al diagnóstico. Los pacientes con una expectativa de vida superior a 5 años pueden beneficiarse de una actitud curativa. Conclusions: More than half of the patients with localized prostate cancer Gleason 8-10 treated in a conservative way die from cancer within the next 5 years since the diagnosis. The patients with a life expectancy greater than 5 years can benefit with a curative treatment.

El objetivo de este estudio consiste en recabar información sobre los conocimientos y actitudes de una población universitaria acerca de la investigación genética, sus opiniones acerca de cómo y para qué debería ser usada la información personal de este tipo y su disposición a someterse a pruebas genéticas predictivas. La metodología consistió en la administración colectiva, anónima y voluntaria de un cuestionario adaptado de otros similares a una muestra de 232 estudiantes de primeros cursos de Enfermería y Psicología de la Universidad de Murcia. Los resultados muestran un amplio apoyo a la investigación en genética, sobre todo en lo que respecta a las nuevas posibilidades para prevenir, diagnosticar y tratar enfermedades. No obstante hay ciertas áreas que son percibidas como límites éticos, por lo que se percibe necesario un control riguroso en el uso y el almacenaje de la información genética. ABSTRACT The objective of this study consists on the gathering of information about knowledge and attitudes regarding genetic research from a university student sample, its opinions about the use of personal information of this type and its disposition to undergo predictive genetic testing. Methodology consisted on a collective, anonymous and voluntary administration of an adapted questionnaire to a sample of 232 students from first courses of Infirmary and Psychology at Universidad de Murcia. Results show a wide support of genetic research, mainly regarding the new possibilities for preventing, diagnosing and treating disease.Nevertheless there are certain areas which are perceived as ethical limits. A rigorous control in the use and the storage of the genetic information is perceives as necessary.

Presentamos dos casos clínicos de invaginación intestinal secundaria a tumores GAN yeyunales que debutan con un episodio de obstrucción intestinal que requirió intervención quirúrgica. Se analizan los métodos diagnósticos, el diagnóstico diferencial y la actitud terapéutica. We report two clinical cases of intestinal invagination due to jejunal GAN tumors that were operated on because of an intestinal obstruction. Diagnostic methods, diferencial diagnosis and therapeutic approach were analized.

- Abstract
Introduction: CT-P13 is a biosimilar drug of infliximab (IFX), effective in patients with inflammatory bowel disease (IBD). The monitoring of levels of IFX and anti-IFX antibodies is now considered part of the integral management., Objective: To compare the clinical response according to a strictly clinical (CLN) or proactive (PRO) approach based on the monitoring of levels in week 14, in clinical practice., Methods: We conducted a prospective study in IBD patients starting CT-P13. In the PRO group, levels of IFX and post-induction antibodies were systematically measured (week 14) and those with infraterapeutic levels (<3μg/ml) were intensified, irrespective of the clinical response., Results: We included 77 patients (23 ulcerative colitis and 54 Crohn's disease). Both PRO (n=41) and CLN (n=36) groups showed initial and long-term efficacy without significant differences. At week 14, 61% clinical remission (CR) (58.5% PRO, 63.9% CLN) and 80.5% at least partial response (PR) (80.5% PRO, 80.6% CLN). In week 54, 68.8% CR (61% PRO, 77.8% CLN) and 76.6% at least PR (73.2% PRO, 80.6% CLN). Of the patients in CR in week 14 (24 PRO, 23 CLN), 13 of the PRO group were intensified due to infra-therapeutic levels. In this subgroup no significant differences were observed in secondary loss of response (PRO 0%, CLN 8.7%)., Conclusion: Proactive management does not improve response or remission rates in the first year. The intensification of clinical remission patients with post-induction infratherapeutic levels does not seem to significantly prevent secondary loss of response in the first year., (Copyright © 2019 Elsevier España, S.L.U. - Abstract
Objective: The aim of this study was to evaluate the knowledge of first aid (FA) of primary and pre-school teachers and the parents of children in these education stages., Methods: A questionnaire already used in previous publications was employed and consisted of 4 sections: 1) general information; 2) assessment of FA knowledge; 3) questions about FA, and 4) attitudes about FA. The questionnaire was sent by e-mail in Google Forms format to different education centres of the Autonomous Community of Galicia. The centres were asked to send them to the school staff and parents of the pupils so that they could be completed., Results: A total of 470 questionnaires were analysed (177 teachers; 242 parents and 51 teachers with children). More than half (268, 57%) of the participants stated to have knowledge of FA. In the case of the teachers, a relationship was found between having this information and teaching these contained in the classroom (P=.008). Only 4 participants managed to arrange the basic life support sequence, and none of them correctly answered the questions on cardiopulmonary resuscitation. More than 95% of the sample considered it necessary to include FA in the school curriculum and in the study plans of degrees destined for teacher training., Conclusions: The majority of teachers and parents had training in FA, although none of them responded correctly to the questions related to basic life support. The inclusion of these contents in school curricula would be recommendable, but it requires training the teaching staff beforehand. Background: Alexithymia is the lack of mental representations of emotions leading to limited ability to understand and regulate these and can contribute to the development or maintenance of a psychosomatic illness. The aim of the study was to demonstrate that alexithymia is a feature that occurs more frequently in patients with psoriasis and that the coexistence of alexitimia-psoriasis is associated with high levels of trait anxiety., Methods: We applied the Toronto Alexithymia Scale -20 (TAS-20), Inventory of state-trait anxiety (STAI) to 16 outpatients with psoriasis of Dermatology Service of Hospital de Especialidades (Centro Médico Nacional Siglo XXI) and the results were compared with 25 control subjects., Results: 25 % of patients with psoriasis presented alexitimia, while in the control group was 8 % (p = 0.002). Correlation between the scores of the TSA-20 and STAI-trait (r = 0.6957, p < 0.0001) was observed., Conclusions: The alexitimia occurs more frequently in individuals with psoriasis than in the general population, and levels of trait anxiety in individuals with psoriasis are similar regardless of the presence of alexithymia.

- Abstract
Background: Gastrobronchial fistula is a rare complication in gastroesophageal surgical procedures. It is difficult to diagnose and handling is complex. Therefore, there have been developments for non-surgical alternatives to obliterate minor fistula mortality. Endoscopic treatment is an option for patients with small fistulas or in serious condition., Clinical Case: A 38 year old woman with evidence of gastrobronchial fistula postoperated of gastric sleeve, diagnosed during the postoperative period due to clinical variegated was initially handled as infectious respiratory symptoms; once the fistulous path was established, the intention was to close the path using endoclips. However, adding hemopneumothorax, drainage tube placement and thoracotomy were required. It was decided to chose a prosthetic esophageal endoscopic management of polytetraflouroethylene and fibrin as last therapy, because the patient had systemic inflammatory response syndrome, with favorable response to endoscopic management., Discussion: Bariatric surgery has shown satisfactory results, however, the complexity of the procedure favors severe complications such as the present case. Gastrobronchial fistulas represent a diagnostic and therapeutic challenge, this is considering from a conservative management to endoscopic procedures, as in our patient., Conclusion: Although gastrobronchial fistulas are a rare complication, the use of endoscopy in resolution should be a first class weapon in its management, since it offers a lower morbidity in a patient with habitual respiratory symptoms that are difficult to control, with satisfactory results in the medium and long term., (Copyright © 2015. Objective: To describe two cases of a rare type of renal tumor , mucinous tubular and spindle cell carcinoma (MTSC), with different pathologic features., Methods: We present: 1) the case of a 36 year-old woman 24-week pregnant, in whom during an examination for a renal colic we discovered a 5.5 cm tumor in the lower pole of the left kidney. 2) A 71-year-old woman that consulted to her doctor due to loss of weight (5 kg) and anorexia. A 15 x 12 x 9.5 cm tumor was found in her left kidney., Results: Radical nephrectomy was performed in both cases. Microscopic examination showed a myxoid matrix containing a proliferation of tubules and spindle cells, with low-grade atypia. Cells were immunoreactive for CK7, racemase, EMA and vimentin and negative for CD10. Case 1 had some foci of papillary morphology, and was pT1. Case 2 had some nests of clear cells and invaded the sinus fat focally. It was staged as pT3a. In April 2012, the patients are alive without evidence of recurrence or metastasis after 13.5 years (case 1) and 8 months (case 2) of follow up., Conclusions: MTSC is a rare type of renal carcinoma, which can appear with different clinical, gross and microscopic features. This tumor seems to share some morphological and immunohistochemical similarities with renal papillary carcinoma, and the differential diagnosis is difficult. The vast majority of cases reported had favourable evolution, like our case 1, although a remote possibility of metastasis exists, in cases with sarcomatoid differentiation, but even without it.

- Abstract
Age-related macular degeneration is the leading cause of legal blindness in people over 50 in developed countries. It is a multifactorial disease resulting from the interaction of genetic and environmental factors, and the age is the only worldwide admitted risk factor. The socioeconomic impact of the disease reaches enormous proportions, if we take into account the high cost of the available antiangiogenic therapy, the strict schedule of medical visits that it requires, and the impairment that it gives rise to. The response to treatment and the visual outcomes improve with early management of the retinal lesions, thus the early diagnosis of the disease in its initial phases, based on self-control with an Amsler grid and with regular ophthalmologic assessments, is essential., (Copyright © 2011 SEGG. Objective: To analyze histological factors not routinely assessed as potential prognostic factors in renal cell carcinoma, such as tumor necrosis, microscopic vascular invasion, and sinus fat invasion., Materials and Methods: A retrospective, analytical study was conducted of surgical specimens from 139 patients with localized renal cell carcinoma who underwent nephrectomy from 1993 to 2005. Tumor necrosis, microscopic vascular invasion, and sinus fat invasion were analyzed and compared to the classical factors: TNM classification, Fuhrman grade, and tumor size. For statistical analysis, variables analyzed were categorized as pT1, 2 vs pT3, 4; Fuhrman grade 1, 2 vs 3, 4; tumor size < 7 cm vs >or= 7cm; tumor necrosis vs no tumor necrosis; microvascular invasion of sinus fat vs no invasion. Cancer-specific survival probability and disease-free survival were calculated. A descriptive and analytical statistical analysis was performed using logistic regression for univariate and multivariate analyses. Dependent variables were used to analyze cancer-specific survival rates. Disease-free survival was estimated using a Cox regression model and Kaplan-Meier curves., Results: In the univariate analysis, all variables analyzed had a significant influence on death for renal cell carcinoma. In the multivariate analysis, the variable having the greatest influence was Fuhrman grade (p = 0,032). The variables significantly influencing disease-free survival, estimated by the Cox method, were the pT stage (p = 0.038) and Fuhrman grade (p = 0.048)., Conclusions: In patients with clinically localized renal cell carcinoma undergoing nephrectomy, pT stage and Fuhrman grade are the most important prognostic factors for survival and disease-free survival. Tumor necrosis, microscopic vascular invasion, and sinus fat invasion are prognostic factors for death from renal carcinoma which are associated to TNM classification, Fuhrman grade, and tumor size.

Introduction: Wünderlich's Syndrome is a spontaneous retroperitoneal haemorrhage characterised by sudden lumbar pain, tangible mass in the lumbar region and signs of hypovolaemic shock., Case Report: A 65 year-old female presented the classic triad indicating Wünderlich's syndrome. Imaging techniques with three-dimensional reconstruction enabled us to determine the diagnosis and aetiology, and aided in determining the treatment., Discussion: We emphasise the importance of imaging methods in diagnosing and choosing treatment for Wünderlich s Syndrome.

Objective: The aim is to analyse until which age a radical procedure could have an influence on life expectancy of patients with localized prostate cancer Gleason 8-10., Methods: We analyse retrospectively 80 patients with localized prostate cancer T1-2N0-XM0 Gleason 8-10. The patients were stratified in 2 groups: group A 34 patients who received hormonal treatment and group B, 46 patients submitted to radical prostatectomy. The analysed variables are: cancer mortality and cancer specific survival., Results: Patients characteristics: median age group A 75 years (66-84) and group B 64 years (56-75) (p = 0.033): median followup group A 54 months (20-180) and group B 37 months (12-140) (p = 0.016); median Gleason group A 8 (8-10) and group B 8 (8-10) (p = 0.144); percentage T1 group A 24% and group B 41% (p = 0.096); median PSA group A 10 (4-91) and group B 12 (4-71) (p = 0.269). The cancer specific mortality from group A is 24 (71%) and in group B 3 (7%) (p = 0.000). In the first 5 years, 20 (59%) patients died from prostate cancer in the group A and 1 (2%) in the group B. The Kaplan-Meier curves and Log-Rank test show significant differences in the survival cancer specific between the 2 groups. The Cox regression shows that the possibilities of dying from prostate cancer are higher in the group of patients who did not receive treatment with curative intention. The relative risk (HR 95% IC) in group A is 6.826 (2.032-22.931)., Conclusions: More than half of the patients with localized prostate cancer Gleason 8-10 treated in a conservative way die from cancer within the next 5 years since the diagnosis. The patients with a life expectancy greater than 5 years can benefit with a curative treatment.

- Abstract
During the splendor of Athens, Herodotus of Helicarnasus develops his life and work in the V century B.C. that led him to be considered the father of history. The available knowledge of ancient medicine and surgery is based on the descriptions of authors such as Homer, Herodotus, Hippocrates, and Plinius. Herodotus, a contemporary of Hippocrates, in his Histories, defines the limit between the epic narratives of Homer and the beginning of scientific history based on observation, reflection, and analysis. He develops an erudite, objective, and clear narrative of facts that allow him to discover the causes in order to determine their consequences in society. He describes the magic-religious model of the health-disease process, etiology, diagnostic methods, and treatment for diseases in ancient towns. He makes a description of Egyptian medicine, particularly about medical specialization, embalmment, and common diseases. He emphasizes surgical procedures: circumcision, castration, amputations of ears, nose and hands, ocular evisceration and mastectomies (punishments) and describes traumatic wounds, their prognosis and treatment. He also mentions the names and actions of the most famous physicians of the time. Herodotus lived the cultural decadence of Ionia, his native land, at the time of economic and intellectual hegemony of Athens. His work is universal and has the purposes of projection to the future and to seek causes.

- Abstract
Case Report: We present the case of a patient operated on for extracapsular cataract extraction of both eyes. Three weeks after the first operation (right eye), the patient presented a local inflammatory reaction at the incision site which was characterized by the existence of whitish exudates of different sizes surrounding the nylon suture. At one week of the second operation, the patient presented the same alteration (left eye), although it was observed sooner and had a greater intensity. The allergy skin tests for nylon and other substances were negative, however the local inflammatory reaction disappeared when the nylon suture was removed., Discussion: This type of reaction, which we have observed in several other patients, has been previously described by others authors, although no clearly established cause has been demonstrated. Both toxicity and allergy to nylon have been proposed as the underlying mechanisms. We consider that a type IV hypersensitivity reaction to nylon may be the underlying mechanism of these reactions although toxicity cannot be ruled out.

Objective: A case of a nonfunctioning ectopic adrenal tissue tumor in the epipidymis is described., Methods/results: A case of a nonfunctioning ectopic adrenal tissue tumor in the epipidymis is presented. A left testicular mass had been incidentally detected in this patient., Conclusions: Tumors of this type localized in the juxtafuniculogonadal region are usually benign. However, resection and histological analysis are always indicated in order to detect metastasis or confirm the histiological type is normal.

- Abstract
Three cases of abscesses rated as primary of the psoas, after dismissing any likely cause that can be involved in the genesis of secondary abscesses of the psoas, are presented. A review of the literature is made, emphasizing the etiological change in the psoas' abscesses as well as the possible causes involved in the etiopathology of this picture which appears to be caused by distant dissemination from a septic nucleus. This theory is also supported by over 80% cases where Staphylococcus aureus is the causative organism. Reference is made to which clinical data is most relevant and to the value of the traditional diagnostic procedures: simplex x-ray, u.i.v., ultrasound techniques, radionuclides and CAT, the latter being the ideal diagnostic procedure in these processes. Finally, different therapy approaches are presented. Percutaneous draining (supported by ultrasound scanning and CAT) appears to have greater acceptance by most authors, relegating the surgical drainage for selected cases.
