Background: ILD is a common manifestation in pSS and is associated with an increased risk of death. APCA are strongly expressed by hyperplastic alveolar epithelial cells in the fibrotic lung and are associated with an accelerated decline in lung function in IPF. In the present study, we aimed to evaluate the clinical utility of APCA in ILD patients with pSS., Methods: Clinical, laboratory, PFTs and imaging data from pSS patients were reviewed, and the ESSDAI was utilized to evaluate disease activity. HRCT semiquantitative scoring was conducted. We compared the clinical characteristics of pSS patients with and without ILD and carried out logistic regression analysis of risk factors for ILD in pSS., Results: A total of 74 patients with pSS and 40 HCs were included in the study. ILD was more commonly observed in the APCA-positive group than in the APCA-negative group. The quantitative levels of APCA were positively correlated with the imaging score. Multivariate analysis found that the long disease duration, elevated APCA and elevated KL-6 level were independent risk factors for ILD in pSS patients. The area under ROC curve for APCA was 0.6618, and the threshold concentration was 153.82ng/ml (sensitivity 45.24%, specificity 87.50%)., Conclusion: APCA level is an independent risk factor and might be a potential biomarker for ILD in patients with pSS., (Copyright © 2023. Se expone el caso clínico de una paciente de 50 años de edad, que presentó síntomas sistémicos muy variables: queratoconjuntivitis seca, caída de la cola de la ceja, ambivalencia afectiva, hipotermia e intolerancia al frío, así como sequedad de las mucosas oculares, la boca (aun durante las comidas) y la piel, además de polimialgias y poliartralgias, por lo cual ingresó en el Hospital Municipal "Ezequiel Miranda", perteneciente al municipio de Julio Antonio Mella (Santiago de Cuba). Los resultados de los exámenes complementarios condujeron al diagnóstico del síndrome de Sjögren y se aplicó tratamiento con esteroides e inmunosupresores. La paciente mejoró y egresó de la institución hospitalaria, con la indicación de seguimiento ambulatorio especializado.

Introduction: Serum biomarkers are important predictive factors for development of parotid non-Hodgkin's lymphoma (NHL) complication in primary Sjogren's syndrome (pSS) patients. The aim was to evaluate the diagnostic accuracy of serum CXCL13 chemokine in pSS patients with parotid NHL complication., Material and Methods: Serum CXCL13 chemokine was assessed in 33 patients with pSS [7 with parotid NHL complication (pSS+NHL subgroup) and 26 without NHL (pSS-NHL subgroup)] and 30 healthy subjects., Results: The serum CXCL13 levels in pSS+NHL subgroup [175.2 (107.9-220.4) pg/ml] were significantly higher comparing to the healthy subjects group (p=0.018) and the pSS-NHL subgroup (p=0.048). A cut-off value of 123.45pg/ml (Se=71.4%, Sp=80.8%, AUROC=0.747) was established for parotid lymphoma diagnosis., Conclusion: The serum CXCL13 biomarker could be considered a valuable tool for the diagnosis of parotid NHL complication in pSS patients., (Copyright © 2023 Elsevier España, S.L.U. A síndrome de Sjögren (SS) primária é uma doença autoimune cujo espectro de manisfestação clínica estende-se desde um acometimento órgão-específico (exocrinopatia auto-imune) até um processo sistêmico. O envolvimento da pele é bastante comum, e a freqüência de doença vascular inflamatória é estimada entre 20% e 30%. Duas formas clínicas específicas de vasculite cutânea, a púrpura palpável e urticária crônica são predominantes, mas eritema multiforme, eritema perstans, eritema nodoso, mácula eritematosa e nódulo subcutâneo também já foram descritos. Os autores descrevem o caso de uma paciente de 46 anos, que desenvolveu quadro de síndrome de Sjögren primária, com manifestações oculares, orais, articulares e alterações laboratoriais (FAN, anti-Ro, fator reumatóide positivos, e hipergamaglobulinemia). Após dez anos do diagnóstico, apresentou úlceras em membro inferior, cuja biópsia confirmou tratar-se de lesão tipo vasculítica, com excelente resposta ao tratamento com ciclofosmida endovenosa. Foram descritos na literatura apenas dois relatos de caso de pacientes com úlceras em membros inferiores, como acometimento cutâneo da SS. Os autores ressaltam a importância do diferencial de úlceras em membros inferiores como acometimento cutâneo da SS primária.Primary Sjögren's Syndrome (pSS) is an autoimmune disease with a large spectrum of clinical manifestations extending from an organ-specific involvement to a systemic process. The skin is affected quite commonly and the estimated frequency of inflammatory vascular lesions is from 20% to 30%. Two specific, clinically recognizable forms of cutaneous vasculitis predominate, palpable purpura and chronic urticaria, but erythema multiforme, erythema perstans, erythema nodosum, erithematous macules and subcutaneous nodules have also been described. The authors report the case of a 46-year-old female patient, diagnosed as primary SS, who presented ocular and oral symptoms, poliarthritis and laboratory alterations (with a positive ANA, anti-SSA, rheumatoid factor, and hypergammaglobulinemia). Ten years after the diagnosis, she presented leg ulcers. The biopsy confirmed the presence of vasculitic process, and the ulcers improved rapidly after the treatment with endovenous cyclophosphamide. There are only two reports of chronic ulceration of the legs as cutaneous manifestation of SS. The authors stress the importance of considering ulcers in the differential diagnosis of cutaneous involvement of primary SS.

OBJETIVO: caracterizar as manifestações clínicas, exócrinas e não exócrinas, de 74 pacientes com síndrome de Sjögren primária (SSp). MÉTODOS: estudo retrospectivo dos pacientes com SSp de acordo com os novos critérios americano-europeus de 2002, seguidos no serviço de reumatologia e doenças ósseas metabólicas do Hospital de Santa Maria (HSM) nos últimos 25 anos (média de seguimento de 7,7 anos). RESULTADOS: todos os pacientes apresentaram doença exócrina dos epitélios superficiais, com destaque para a xerostomia e a queratoconjuntivite seca. Doença exócrina dos órgãos internos foi encontrada em 25 pacientes (33,8%), enquanto apenas um paciente (1,4%) desenvolveu doença linfocitária B monoclonal (pseudolinfoma pulmonar). Do total, 55 pacientes (74,3%) também apresentaram manifestações não-exócrinas, com predomínio das induzidas por mediadores da inflamação (68,9%). As manifestações vasculares inflamatórias estiveram presentes em 54% dos pacientes e as vasculares não-inflamatórias em 27%. A tiroidite auto-imune surgiu em 5 pacientes (6,8%). CONCLUSÕES: os componentes oral e ocular dominam a doença, sendo mais limitadas as manifestações orgânicas graves. Este modelo classificativo permite comparar alterações fisiopatológicas entre os pacientes e abre uma via de estudo para encontrar marcadores de atividade/cronicidade com estas relacionados.OBJECTIVE: to characterize the exocrine and non-exocrine clinical disease manifestations of 74 patients with primary Sjögren's Syndrome (pSS). METHODS: retrospective study of pSS patients according to the new 2002 American-European criteria, followed in the Hospital Santa Maria, Rheumatology Outpatient Clinics, in the last 25 years (mean follow-up of 7.7 years). RESULTS: all patients presented surface exocrine disease, especially xerostomia and keratoconjunctivitis sicca. Internal organ exocrine disease was found in 25 patients (33.8%), whilst only one patient (1.4%) developed monoclonal B lymphocyte disease (pulmonary pseudolymphoma). Fifty-five patients (74.3%) also displayed non-exocrine manifestations, in particular inflammatory mediator-induced disease (68.9%). Inflammatory vascular disease was present in 54% of patients and noninflammatory vascular disease in 27%. Autoimmune thyroiditis was found in 5 patients (6.8%). CONCLUSIONS: the disease is dominated by oral and ocular involvement, while severe organic manifestations are less frequent. This classification model allows a comparison of patients with physiopathological abnormalities and offers the possibility of finding markers of activity/cronicity related to them.

Objective: To evaluate the efficacy and safety of belimumab in patients with Primary Sjögren's syndrome (pSS)., Methods: The search included manuscripts assessing the efficacy or safety of belimumab in patients with pSS (American-European Consensus Criteria 2002) published between 2004 and 2017 in MEDLINE, EMBASE or Cochrane databases. Two reviewers independently selected the articles, extracted data and evaluated the quality of the evidence following Scottish Intercollegiate Guidelines Network (SIGN) recommendation grades., Results: Out of 135 citations, only 3 articles were included. All of them publishing results from the same study at different time points including 28 patients. At week 28 improvement was reported for visual analogue scale (VAS) dryness score and glandular manifestations in 37% and 77% of patients, respectively, which persisted at week 52 (W52). Belimumab was well tolerated and safely administered., Conclusion: Published evidence to determine the efficacy of belimumab in pSS is limited. Objective: The aim of this study is to describe the findings of a protocolised odontological evaluation of patients with primary Sjögren's syndrome (pSS) treated in rheumatology units in the Community of Madrid., Methods: Multicentric descriptive study in which pSS patients classified according to the American-European consensus of 2002 criteria were included. We collected the demographic, clinical and serological data of each patient. A complete oral examination was performed and salivary flow and the CAOD caries index were collected. The patients completed the visual analogue scale for xerostomia, the Oral Health Impact Profile-14 questionnaire and an oral health questionnaire., Results: Sixty-one patients were recruited. Ninety-eight percent were women and the mean age of the patients was 57 years. Pathological oral signs (unstimulated salivary flow/salivary glands sialography/scintigraphy) were present in 52.5% of the patients, anti-Ro/anti-La were positive in 90.2%. Ninety-two percent of the patients reported xerostomia and 61% of the patients suffered from hyposialia. Thirty-five patients presented oral mucosa lesions. CAOD index was 16.97±7.93 and visual analogue scale for xerostomia was 46.69±14.43. The results of the OHIP-14 questionnaire were 23.13±14.16. Patients with pathological oral signs obtained a significantly higher Oral Health Impact Profile-14 score (P=.03). We also found that patients with peripheral nervous system involvement obtained a significantly higher Oral Health Impact Profile-14 score (P=.001) CONCLUSIONS: The presence of xerostomia in this cohort of pSS patients was high and hyposialia was present in 61% of the patients. Oral lesions appeared in more than half of the subjects. Objectives: The aim of this study is to identify the clinical characteristics of primary Sjögren's syndrome (PSS) patients with pulmonary involvement and the associated factors for pulmonary involvement in PSS., Methods: We retrospectively reviewed clinical features, laboratory examinations, imaging tests, pathological results and therapeutic strategy of 367 PSS patients. Comparisons were made between two subgroups: PSS with pulmonary involvement and those without. Correlation between the pathology of minor salivary gland biopsy (MSGB) and diverse features with pulmonary involvement were detected by Pearson correlation analysis and associated factors were selected by multivariate logistic regression analysis., Results: The lung involved PSS patients had significantly higher level of inflammatory associated indexes (p<0.05). There is no significant correlation between pathology of MSGB and lung involvements. Age, elevated neutrophils level and hypoproteinemia are significantly associated with lung disease with in PSS cohort (p<0.05). As for therapeutic strategy, moderate dose prednisone (15-40mg/d) and cyclophosphamide (CTX) are mainly different between two subgroups., Conclusions: PSS patients with pulmonary involvements show enhanced inflammation. Age, elevated neutrophils level and hypoproteinemia are independent associated with pulmonary involvements in PSS patients. Primary Sjögren's syndrome is a systemic and chronic autoimmune disease. Renal involvement may occur in up to 30% of patients. The incidence of tubulopathies ranges from 2.6 to 33%. They are manifested by defects in the urine concentration and hydroelectrolyte alterations, mainly distal tubular acidosis and exceptionally proximal tubular acidosis. These disorders can be associated with nephrocalcinosis and renal lithiasis. We report the case of a patient with primary Sjögren who presented proximal renal tubular acidosis associated with recurrent renal colic due to renal lithiasis and nephrocalcinosis. We highlight the importance of diagnosing renal tubular acidosis in patients with Sjögren's syndrome that present alterations in urinary sediment and electrolyte disorders to avoid nephrocalcinosis and nephrolithiasis. Acidosis correction treatment aims to prevent the progression of the disorder and preserve renal function.
