The present article reports our experience with endopyelotomy, a technique we have performed at our hospital since 1985. Sixty-two patients comprise this series. Of these, 57 were evaluated more than 6 months after treatment (65 cases of ureteropelvic junction syndrome (UPJS); 26 were primary UPJS (success rate 81%) and 39 secondary UPJS (success rate 84.5%). The complication rate was low. Patients in whom endoscopic treatment had failed were submitted to open surgery with no particular difficulty. A comprehensive review of the literature highlighting the indications, techniques and results achieved with this procedure is performed.

A total of 140 pancreatic transplants have been performed in 132 insulin-dependent diabetic patients at Lyon (France). Most cases received a double pancreatic and renal transplant. The duct obstruction technique was utilized in 113 segmental transplants. A total pancreatic transplant was performed in 27 patients, with duct obstruction in 27, GI tract derivation in 14, urinary tract derivation in 11 and intraductal obstruction in 2 patients. Utilizing quadruple immunosuppression, the one year survival rate was 90% and the transplant survival was 60%.

Six patients carrying bulky renal cysts located in the kidney lower pole were treated by percutaneous resection of the cystic wall. No complications were observed during the peri and postoperational period. In all cases disappearance of symptoms was observed, and the total success rate (absence of residual cavity) recorded was 4/6, two cases presenting persistence of a small residual cavity which originated one case of relapse. The paper presents the technique used, and the indications and possible therapeutic actions for the treatment of simple kidney cysts are discussed.

Sub uretric injection of teflon was used in 62 adults patients (92 ureteral units, 30 bilateral cases), 49 females, 13 males for treatment of congenital VUR, in 76 cases the refux was permanent. Degree (D) I = 18 Ureters (U), D II = 28 U., D III = 22 U., D IV = 7 U and D V = 1 U. Follow-up consisted in ultrasound examination cystogram and IVP performed at one month (92 U.) three months (81 U), one year (64 U) and two years (12 U). The procedure was performed under general anaesthesia. The quantity of polytef varied from 1 to 2 cc. Correction of VUR was observed in 70/92 U (76.1%), 11 persistent VUR were treated by reinjection of politef and in six VUR disappeared (83% total correction). Recurrence was observed in 4 patients, at 3 months, one year and two years (2 cases). 3 patients had reinjection (2 success, 1 failure). Through long term results are still to be evaluated. The results at one and two years seems to confirm the inoucuity and the effectiveness of endoscopic procedure for treatment of congenital VUR.
