Redes sociais online ,Cabo Verde ,Cyberbullying ,Vulnerabilidade digital ,Juventude - Abstract

There are few published studies regarding the use of negative pressure therapy in wounds located in the head and neck regions. This probably occurs because of the anatomical complexity of these areas, which may difficult the use of a sealed dressing. We present the clinical case of a patient with a vast ulcer on the frontal region, extending to the eyebrow and with bone exposure. We decided to stimulate the growth of granulation tissue over bone surface through negative pressure therapy and proceed to an autologous skin graft, with excellent results. The use of adjuvants to the negative pressure dressing allowed us to be safe and efficient in an area of difficult approach. The anticipated knowledge of possible complications made it possible to use prophylactic measures.

[EN]: We have studied the spatial heterogeneity of 15 surface soil variables (0-10 cm) in six mixed-forest plots distributed along a pedo-climatic gradient, north-south, in the “Parque Natural Los Alcornocales” (PNLA). In three of them, the forest is composed of Quercus suber and Olea europaea (AA type), and the other three of Q. suber and Q. canariensis (AQ type). Also we estimated the spatial relationship of the soil variables with basal areas -tree-density- of the species present in the canopy. The heterogeneity of the studied soil variables could be explained by the types of forest that were measured, except for phosphorus, according to the results of the analysis of variance. Furthermore, this analysis found that the soils of the AQ plots were sandy-loam, strongly acidic, with a high C:N ratio and poor in Ca, P and K, while the AA plots had soils that were clay-loam-sandy, moderately acidic, with a C:N ratio mediumhigh, poor in P but rich in Ca and Mg. This difference in the soil of the two types of forest studied was confirmed by the results of multivariate analysis, which also stood out the important role of litterfall in the availability of phosphorus to the soils studied, which are deficient in this nutrient. On the other hand, we analyzed the spatial distribution of the variables and the covariance of these with the tree density by SADIE (spatial analysis by distance indices) analysis. This spatial analysis method has the advantage of studying the continuous variability of tree-soil relationship and exploring the entire surface of the forest stand. Aggregation was detected for most of the variables studied, although we didn’t find a level of aggregation significantly different between the two forest types. The results of the analysis of spatial covariance are very heterogeneous for different forest types, therefore, we can’t confirm the initial hypothesis and generalize that the concentration of N, K and P in surface soil (0-10 cm) is enriched underneath the trees (“footprint”). Even so, this analysis has stood out the role of lithology in the concentration of Ca and Mg and confirmed the acidophilus character of the Q. suber species. Spatial variability patterns of trees are complex and respond to various factors: lithology, water conditions, light and soil nutrients, competition processes between plants, effects of disturbances (pests, fires) and management.

Diabetes Mellitus ,Stents Farmacologicos ,Implante ,Enfermedad Coronaria - Abstract
The incidence of diabetes mellitus has continuously risen in the recent years. Patients with this chronic condition have a higher risk to develop coronary artery disease. Unfortunately, due to the frequent diffuse involvement of multiple vessels, the long term outcomes after percutaneous intervention, either with catheter-balloon or bare metal stent, has always been poor and frequently inferior to cardiac surgery in this subset of patients. Recently, the introduction of drug-eluting stents (DES) has brought enthusiasm to treat percutaneously patients with more complex disease. In this manuscript we report a successful percutaneous approach of an insulin dependent diabetic patient treated with multiple DES (Taxus™, Boston Scientific).

The degree of heterosis in the genus Capsicum spp. is considered high; however, most of the studies refer to the species Capsicum annuum L. In spite of the potential use of F1 hybrids in pungent peppers of the species Capsicum chinense, few studies are available which assess the magnitude of heterosis in this species . This study was carried out to assess heterosis and its components in F1 hybrids from a diallel cross between hot pepper lines (Capsicum chinense) and to obtain data on the allelic interaction between the parents involved in the crosses. Trials were made in Rio Branco-Acre, Brazil, from March through October 1997. A randomized complete block design with fifteen treatments and three replications was used. The treatments were five C. chinense accessions (from the Vegetable Germplasm Bank of the Universidade Federal de Viçosa - BGH/UFV) and 10 F1 hybrids derived from single crosses between them (reciprocals excluded). Diallel analyses were performed for total yield, fruit length/diameter ratio, fruit dry matter per plant, Xanthomonas campestris pv. vesicatoria incidence, capsaicin yield per plant and number of seeds per fruit. Non-additive genetic effects were larger than additive effects for all the traits assessed. Epistasis was detected for fruit dry matter per plant, capsaicin yield per plant and number of seeds per fruit. In these cases, epistasis seemed to be largely responsible for heterosis expression. Dominant gene action, ranging from incomplete dominance to probable overdominance, was responsible for heterosis in those traits where no epistatic genetic action was detected.

- Abstract
Introduction and Objectives: Transurethral resection of bladder tumor (TURBT) is crucial in the treatment of bladder tumors and when incorrectly performed can cause staging mistakes. To avoid these errors, a second resection is recommended in selected cases. The aim of this study is to evaluate the surgeon's ability to predict histologically complete primary resection of newly diagnosed bladder tumors avoiding the need for a second TURBT., Methods: This is a prospective, observational study involving 47 consecutive patients with newly diagnosed bladder tumors who had previously undergone primary TURBT, and met EAU criteria for second-look TURBT. Second-look TURBT specimens were analyzed for routine histological assessment and compared with the surgeon's impression of the tumor at initial resection., Results: Of 91 patients submitted to primary TURBT, 47 met the criteria for second-look TURBT. Second-look specimens revealed residual disease in 20.9% of patients and 3 (6.4%) of patients showed upstaging disease. The sensitivity of a senior to detect disease on second-look TURBT in relation to muscle invasion was 75%, and the specificity was 85%., Conclusions: Second-look TURBT is crucial in the treatment of bladder cancer and cannot be replaced by a surgeon's opinion, so international recommendations should be followed. Supervision of less experienced surgeons is a cornerstone., (Copyright © 2023 AEU. Published by Elsevier España, S.L.U. All rights reserved.)

- Abstract
Objective: Considering that all the evidence indicates that chronic rhinosinusitis without nasal polyps (CRSsNP) and chronic rhinosinusitis with nasal polyps (CRSwNP) are distinct entities, the aim of this study was to compare the concentrations obtained in plasma and in sinonasal mucosa with oral and nasal topical ciprofloxacin, in patients with and without nasal polyps, without evaluating the effectiveness of the use of an antibiotic., Methods: Prospective clinical study with single-blind randomization. The population consisted of patients with chronic rhinosinusitis with eligible for endonasal surgery, over 18 years old. It took place between January 2010 and December 2014. A single preoperative dose of ciprofloxacin (oral or nasal topic- spray, gel or drops) was given and samples of plasma and nasal mucosa (inferior turbinate, middle turbinate, ethmoid and maxillary sinus) were collected prior to surgery. The plasma and mucosal ciprofloxacin concentrations were assayed with high performance liquid chromatography (HPLC) with fluorescence detection (FD)., Results: The oral ciprofloxacin achieved better mucosal concentrations but had a significant plasmatic expression in all patients. None of the topical formulations achieved measurable ciprofloxacin plasmatic levels. Among the topical formulations, the gel had the best mucosal results, despite the existence of polyposis., (Copyright © 2017 Elsevier España, S.L.U. and Sociedad Española de Otorrinolaringología y Cirugía de Cabeza y Cuello. Introduction: To report a case of premacular hemorrhage treated with Neodymium (Nd):YAG laser puncture., Case Report: A 54 year-old Caucasian female was seen in the emergency department with complaints of sudden loss of vision in her right eye (RE), detected on the previous day. The best-corrected visual acuity (BCVA) in the RE was reduced to light perception, and fundoscopy evidenced a large and dense premacular hemorrhage. An Nd:YAG laser puncture was performed that day to drain the hemorrhage into the vitreous cavity. The intravitreal hemorrhage cleared in about 1 month, and the BCVA of the RE was 20/25 after 3 months., (Copyright © 2014 Sociedad Española de Oftalmología. Published by Elsevier España, S.L.U. - Abstract
Objective: our aims is to understand endoscopic findings from a preoperative systematic study of patients with hepatic cirrhosis who were candidates for transplantation and their impact on a protocol for primary and secondary prophylaxis of variceal haemorrhage., Patients and Methods: this study involves a retrospective evaluation of upper digestive tract lesions detected before inclusion and a prospective evaluation of new episodes of variceal haemorrhage, associated mortality rates, and factors that are likely to be involved in the development of this condition. Primary prophylaxis with beta-blockers was considered indicated in cases of varices of grande II or greater or with signs associated with increased risk. Secondary prophylaxis was essentially always associated with medical and endoscopic treatment., Results: of 134 patients, there were 9 deaths, with a median time on the waiting list of 3 months. Of all patients, 33.6% presented with high risk oesophageal varices, 11.2 % with gastric varices, 42.6% with portal hypertensive gastropathy, and 26.9% with peptic lesions. Primary prophylaxis was indicated in 33 of 90 patients, and was initiated in almost half of the cases as a results of the study. Optimum fulfiment of the pre-established objectives was 75.3%. The incidence of new haemorrhagic events due to varices was 10.4% and accounted for almost half of the deaths during the monitoring period. The only statistically significant predictive factors were the presence of gastrict varices and previous history., Conclusion: upper endoscopy should play a role in the preoperative examination of liver transplant candidates due to the significant impact it has on subsequent management.

Objective: To analyze the immediate results and the clinical evolution of a group of fertile age women with rheumatic mitral stenosis, in whom percutaneous balloon mitral valvuloplasty was performed before or during pregnancy., Patients and Method: Eighty-one women with mitral stenosis, submitted to balloon mitral vavuloplasty, were studied. They were divided into three groups, according to their desire of no further pregnancies (group A; n = 19), pregnancy during the follow-up (group B; n = 23) or valvuloplasty was performed during pregnancy (group C; n = 39). Patients from group B and C were controlled during pregnancy, childbirth and puerperium, and the newborns of women in group C were followed from birth to the age of 5 years., Results: Mortality in the three groups was null and the incidence of miscarriage was 2 (8.6%) in group B and 3 (9.1%) in group C. Normal delivery was predominant in group B and delivery by caesarean was predominant in group B. Success was immediate in all the cases. The procedure was repeated in 3 women due to restenosis. The media valvar area rase from 0.93 to 2.05 cm2 in group A, from 1.28 to 2.04 cm2 in group B and from 0.84 to 2.14 cm2 in group C (intergroup p = NS). The functional class improved in the three groups of patients., Conclusion: Percutaneous balloon mitral valvuloplasty is an effective, efficient method for the treatment of rheumatic mitral stenosis during pregnancy, after organogenesis, or at any time in a woman's life, as long as it is indicated according to clinical and echocardiographic evaluation criteria.

- Abstract
The authors report their experience with the percutaneous closure of the patent ductus arteriosus with the Rashkind technique. They performed 42 procedures with a success of 97.6% and one embolization episode. The mean age was 8.7 +/- 6.7 years, the mean weight was 25.0 +/- 15.8 Kg and the minimum diameter of the ductus was 4.0 +/- 1.3 mm. The immediate overall prevalence of residual shunt was 48.8% falling to 21% in a mean follow-up of 17 +/- 13 months in 29 cases. Two patients underwent late reocclusion: one with a new umbrella and the other with a coil. Another patient had an immediate severe residual shunt and it was reoccluded in the same procedure with a coil. The anatomical type and minimum internal diameter of the ductus, age and weight, size of the umbrella and the rotation of the umbrella were not predictors of residual shunt. There were no deaths, hemolysis, endarteritis or fracture of the umbrella's arms. One patient remained with a slight gradient in the left pulmonary artery. The authors conclude that the Rashkind technique is safe and efficient and has good long term results. It should be considered as the first line therapeutic modality in the treatment of selected patients with persistent ductus arteriosus.
