Introduction: Suicide deaths in young people have been increasing in recent decades and are considered a major public health problem worldwide, being a partially preventable event. The prevalence of suicidal ideation is high among university students, especially in health careers. The objective of this study was to measure the prevalence of high suicidal risk in this specific group and to identify associated factors, aiming to provide empirical evidence for the construction of effective suicide prevention strategies., Methods: A cross-sectional study was conducted based on an online survey directed to healthcare students near the end of the first year of the COVID-19 pandemic to learn about the frequency of suicidal risk and its associated factors. The sample consisted of 477 students (70.8% female, mean age 21.7 ± 2.5 years) from eight healthcare majors. The data were collected in January 2021., Results: 22.6% of the young people reported a high suicide risk on the Okasha scale, and 3.4% made a suicide attempt in the previous year. Factors associated with high suicidal risk were having a non-heterosexual orientation, an irregular academic trajectory, experiences of physical and/or psychological violence, higher levels of depressive and anxious symptomatology, as well as lower levels of social support from friends and family., Conclusions: Suicide risk and attempt levels are high in this group of students, and there is a group of factors that could guide more effective actions, such as support for higher-risk groups and screening to identify and provide support to young people at high suicidal risk and with mental health conditions., Competing Interests: None to disclosure., (This work is licensed under a Creative Commons Attribution 4.0 International License.)

Introduction and Objectives: Selective serotonin reuptake inhibitors (SSRIs) are medications used in child and adolescent psychiatry mainly for the treatment of depression, anxiety and obsessive compulsive disorder. In general, these medications are safe and well tolerated. However, they can cause adverse effects such as activation syndrome, which if not identified can negatively affect adherence and response to treatment. Activation syndrome has received little attention and can be difficult to recognise due to the lack of a clear definition and objective diagnostic measures, and also because it can be confused with a worsening of the psychiatric disorder or mania triggered by the antidepressants. For all the above, it is important that professionals who prescribe antidepressants in the paediatric population are able to identify and manage activation syndrome when it occurs. Our aim was to carry out a narrative review of activation syndrome in children and adolescents treated with SSRIs in terms of definition, prevalence, pathophysiology, associated factors, relationship with suicide risk, management strategies and recommendations for reducing the risk of suicidal behaviours when using antidepressants in this population., Methods: We performed a non-systematic narrative review of activation syndrome in children and adolescents which involved finding information in PubMed, Ovid, EBSCO, ProQuest and Embase. Review articles, prospective and retrospective investigations, systematic reviews, meta-analyses and other articles related to activation syndrome in children and adolescents were selected. The search was limited to studies published in English and Spanish that involved children and adolescents and no limits were applied to the publication date or study design., Results: A total of 62 articles were included, 61 of them in English. The results were grouped into the following topics: definition; prevalence; pathophysiology; associated factors; relationship with suicide risk; management strategies; and recommendations for reducing the risk of suicidal behaviours when using antidepressants in this population. Activation syndrome refers to a set of symptoms consisting of impulsiveness, restlessness, increased activity, insomnia, irritability, disinhibition and agitation. This syndrome is poorly characterised in terms of its definition, prevalence, risk factors and pathophysiology, a situation that limits its recognition and evaluation. There are many factors that predispose the development of the syndrome such as age, differences in brain development in the paediatric population, the characteristics of the patient or the antidepressant, disorders of neurological development, and the doses and plasma levels of the medications. It has been thought that activation syndrome may be related to suicidal tendencies. However, the evidence in support of this link is inconsistent and further studies are therefore necessary., Conclusions: Activation syndrome with SSRI is a particularly important adverse effect in children and adolescents and, when it occurs, can cause lack of adherence to or discontinuation of treatment. Objective: The primary goal was to analyze the level and trend of the burden of suicide mortality in Mexico between 1990 and 2017 at a national and state scale., Methods: A secondary analysis based on the 2017 global burden of disease study. Mortality rates and years of life lost due to premature death (YLL) were reported here. A joinpoint regression analysis based on a log-linear model was used to analyze the trend of YLL., Results: The burden of disease due to suicides increased in Mexico, mainly in young males and females; though the raise that happened in the elderly is an additional challenge for the health system. The burden of disease varied substantially between states. The male mortality rates were higher than those of females during the whole period under study, nonetheless with a trend to reduce the difference between the sexes. A bimodal pattern of the burden of disease due to suicide in Mexico was also observed, with the higher rates located in those aged between 15 and 19 years, and an important increase in people older than 85 years of age., Conclusion: These results show a worrisome picture, not only from a social, economic and health point of view but also from the needs of public policies. This situation represents a wake-up call about the need to implement timely identification actions, a comprehensive multisectoral prevention strategy and the detailed study of suicide associated risk factors.

Introduction: Adolescence is one of the stages in life most affected by suicide. In Peru, 22% of suicides occur in people 10 to 19 years old. However, mental health overall and factors associated with suicidal behaviors have not been well studied., Objective: To determine the prevalence of suicidal behaviors (ideation and ideation plus suicidal planning) and associated factors in Peruvian adolescent students., Methods: A cross-sectional study analyzing data from the Global School-based Student Health Survey for Peru in 2010 was conducted to measure the prevalence of suicidal behaviors (ideation and ideation plus suicidal planning) and associated factors in Peruvian adolescent students., Results: Of the 2521 students evaluated, 19.9% (95% CI: 17.8 to 22.2) presented suicidal ideation and 12.7% (95% CI: 11.1 to 14.5) presented suicidal planning in the last 12 months. Females had a higher prevalence of both ideation (27.5%, 95% CI: 24.9 to 30.4) and ideation plus suicidal planning (18.5%, 95% CI: 16.4 to 20.7). Multivariate analysis found that being female, having little parental support, having felt loneliness, having suffered from physical aggression, having been bullied, and alcohol consumption, were associated with ideation and ideation plus suicidal planning in adolescent students., Conclusion: Ideation and ideation plus suicidal planning is a problem in the Peruvian adolescent population and is associated with several factors. Strategies are needed to identify and register suicide in adolescents in Peru and to develop prevention programs.

Introduction: A priority for the WHO by 2020 is to have reduced the rates of suicide; they indicate difficulties in giving precise estimations due to a wide variety of factors, which include aspects related to the statistical measurements themselves of suicidal behaviour. The proportion of deaths from suicide is 8.5% among young people between 15-29 years of age., Objective: To review the methodology used to express the frequency of suicidal behaviour in young people and to describe the methodological characteristics of the studies reviewed., Method: A systematic review of longitudinal studies registered on PROSPERO. The extracted information included the following: year of publication, journal, population size, sample, country, design, age, percentage of men, follow-up time and losses, suicidal behaviour, risk factors, ethical aspects, fundamentally, evaluating the measures of frequency used., Results: Eighty-two articles were selected from 37,793 documents. None of the studies define the measure of frequency used for suicidal behaviour, there are currently up to 9 different ways of measuring it. The populations are students or the general population (66%), birth cohorts (16%) and specific groups. Follow-up was from 24 weeks to 30 years. Only 24.1% of the studies took ethical aspects into consideration., Conclusions: Researchers must make an effort to reach an agreement on the measures of frequency used in suicidal behaviour studies, as the methodological and terminological variability currently used impedes making any comparisons between different studies or understanding the real dimension of the problem., (Copyright © 2017 SEP y SEPB. Objective: To evaluate the coverage, characteristics and the risk of suicide in the indigenous people of Tabatinga in the Brazilian Amazon., Methods: An active surveillance strategy for suicide cases was used: records of the Ministry of Health, the Municipal Health Secretariat, the Special Indigenous Health District of the Upper River Solimões (Distrito Sanitario Especial Indígena Alto Río Solimões), the Military Hospital of Tabatinga, the National Indian Foundation (Fundación Nacional del Indio) and the civil registry offices were examined from 2007 to 2011 for individuals over 9 years of age. Adjusted rates were estimated using the direct method and according to age. A descriptive analysis was performed and the hypothesis tests were considered significant if p-values were <0.05., Results: The coverage of indigenous suicide was 82.8%, since 11 (17.2%) were classified as hidden suicides. For men between 15 and 29 years of age, and for women aged from 12 to 20 years, the probability of suicide was around 70.0%. In 17.2% of the sample there was a record of alcohol consumption before death and relationship between victims. The corrected adjusted mortality rate was 111.7/100,000 (95% CI, 84.6-148.6)., Conclusions: The risk of suicide in the indigenous people of the Tabatinga is very high. Coping strategies should consider the complex relationship between suicide and alcohol consumption, gender differences and the existence of vulnerable groups, such as young people, especially those with close relatives who have committed suicide., (Copyright © 2018 Asociación Colombiana de Psiquiatría. Background: Analyze the long-term sex specific trends in suicide rates for Ecuadorian children, adolescents and young people, as well as to describe the evolution of the male/female ratio between 1990 and 2017., Method: Population and mortality data were obtained from the National Institute of Statistics and Censuses of Ecuador. Suicide rates were calculated for each year, age group (10-14, 15-19, 20-24 years) and sex. We used the Joinpoint Regression Program 4.6.0.2 to analyze temporal trends and report annual percent change (APC) by each age group and sex., Results: An average of 313 suicides per year happened in Ecuador between 1990 and 2017, with increases of 480% in young boys and 322 % in girls aged between 10-14, 111% in male adolescents and 57% in young adults, while there was a decrease in adolescent girls (19%) and young adults (37%). The suicide rate increased annually among boys (APC = 5.10%) and girls (2.57%), as well as for male adolescents (3.07%) and young adults (10.42%), while for female adolescents (0.81%, not significant) and young women (1.16%) the trend decreased after reaching a peak around 2006. While up until the beginning of the XXI century young females aged between 10 and 19 years had higher suicide rates, during the last decade men had higher rates across all age groups (between 1.5:1 and 4.3:1 in 2017)., Conclusions: Suicide in the population under 25 years in Ecuador is a major public health issue and increased between 1990 and 2017, particularly among children, and males between 15-24 years. The intersectorial strategy proposed in 2018 in order to reduce suicide is pending further evaluation and interpretation.

The scope of this article is to analyze the risk of death by suicide in Colombia based on the covariables of sex, age group, year, region and/or department from 2000 to 2013. The databases of deaths by suicide and population projections of DANE were used. A longitudinal and analytical study was conducted. From 2000 to 2013, annual trends of deaths by suicide by means of modeling of covariables were established and risk estimates were collected. The Poisson regression model (PRM) was used. IRR was used according to the MRP with a level of significance of (P <0.05). Colombians between the years 2000-2013, male, 15 to 34 years old, from the Central and Eastern regions and from the Departments of Vaupés, Huila and Quindío were those with the highest risk of death by suicide. The suicide rate decreased slightly as the years went by. The highest risk of suicide occurred among men of productive age, older adults, and in underdeveloped regions. Colombia requires a mental health policy that implements strategies for primary care, health promotion and disease prevention, aimed at promoting the quality of life, detection of suicidal ideation, treatment and rehabilitation of these people from a comprehensive rights and care approach.

Introduction: Suicide is a major public health problem worldwide, and an approach is necessary due to its high potential for prevention. This paper examines the main characteristics of people admitted to hospitals in the Community of Madrid (Spain) with suicidal behaviour, and the factors associated with their hospital mortality., Material and Methods: A study was conducted on patients with E950-E959 codes of suicide and self-inflicted injuries of the International Classification of Diseases, Ninth Revision, Clinical Modification, contained in any diagnostic field of the minimum basic data set at hospital discharge between 2003 and 2013. Sociodemographic, clinical and health care variables were assessed by uni- and multivariate logistic regression analysis in the evaluation of factors associated with hospital mortality., Results: Hospital suicidal behaviour predominates in women (58.7%) and in middle-age. Hospital mortality is 2.2% (1.6% in women and 3.2% in men), increasing with age. Mental disorders are detected 3-4 times more in secondary diagnoses. The main primary diagnosis (>74%) is poisoning with substances, with lower mortality (∼1%) than injury by hanging and jumping from high places (≥12%), which have the highest numbers. Other factors associated with increased mortality include different medical comorbidities and severity of the injury, while length of stay and mental disorders are protective factors. Type of hospital, poisoning, and Charlson index are associated differently with mortality in men and women., Conclusions: Hospitalised suicidal acts show a low mortality, mainly related to comorbidities and the severity of injuries., (Copyright © 2016 SEP y SEPB. Introduction: Monitoring and preventing suicidal behaviour requires, among other data, knowing suicide deaths precisely. They often appear under-reported or misclassified in the official mortality statistics. The aim of this study is to analyse the under-reporting found in the suicide mortality statistics of Tarragona (a province of Catalonia, Spain)., Method and Materials: The analysis takes into account all suicide deaths that occurred in the Tarragona Area of the Catalan Institute of Legal Medicine and Forensic Sciences (TA-CILMFS) between 2004 and 2012. The sources of information were the death data files of the Catalan Mortality Register, as well as the Autopsies Files of the TA-CILMFS. Suicide rates and socio-demographic profiles were statistically compared between the suicide initially reported and the final one., Results: The mean percentage of non-reported cases in the period was 16.2%, with a minimum percentage of 2.2% in 2005 and a maximum of 26.8% in 2009. The crude mortality rate by suicide rose from 6.6 to 7.9 per 100,000 inhabitants once forensic data were incorporated. Small differences were detected between the socio-demographic profile of the suicide initially reported and the final one. Supplementary information was obtained on the suicide method, which revealed a significant increase in poisoning and suicides involving trains., Conclusions: An exhaustive review of suicide deaths data from forensic sources has led to an improvement in the under-reported statistical information. Introduction: Suicide is an important public health problem, it represents one of the major causes of unnatural death, and there are many factors that affect the risk of suicidal behaviour. The present study analyzes the temporal and spatial variations of mortality by suicide in Spain and its relationship with gross domestic product (GDP) per capita., Material and Methods: A retrospective study was performed, in which deaths by suicide, sex and age group in 50 Spanish provinces between 2000 and 2012 were analyzed. The annual trend of suicide mortality was assessed using Kendall's tau-b correlation coefficient. Seasonality and monthly and weekly behaviour were evaluated by performing the ANOVA test and the Bonferroni adjustment. Finally, the relationship between GDP per capita and suicide was studied., Results: Between 2000 and 2012, 42,905adult people died by suicide in Spain. The annual average incidence rate was 95 suicides per million population. The regions located in the south and in the northwest of the country registered the highest per capita mortality rates. There is a decreasing trend in mortality by suicide over the period studied (CC=-.744; P=.0004) in adults over the age of 64, and a seasonal behaviour was identified with summer maximum and autumn minimum values (f=.504; P<.0001). The regions with the highest GDP per capita showed the lowest mortality by suicide (r=-.645; P<.0001) and the relationship is stronger among older age groups., Conclusions: Mortality by suicide does not follow a homogenous geographical distribution in Spain. Mortality in men was higher than in women. Over the period of study, there has been a decrease in mortality by suicide in Spain in adults over the age of 64. The seasonal cycle of suicides and the inverse relationship with GDP per capita found in this study, provide information which may be used as a tool for developing prevention and intervention strategies., (Copyright © 2017 SEP y SEPB. Introduction: Studies have pointed to rurality as an important factor influencing suicide. Research so far suggests that several sociodemograpic and psychiatric factors might influence urban-rural differences in suicide. Also, their contribution appears to depend on sex and age. Unfortunately, studies including a comprehensive set of explanatory variables altogether are still scare and most studies have failed to present their analyses split by sex and age groups. Also, urban-rural differences in suicide in Spain have been rarely investigated. The present study aimed at explaining rural-urban differences in suicidality in the province of Castellon (Spain). A comprehensive set of sociodemographic and psychiatric factors was investigated and analyses were split by sex and age., Material and Method: The sample comprised all suicides recorded in the province of Castellon from January 2009 to December 2015 (n=343). Sociodemographic data included sex, age, and suicide method. Psychiatric data included the history of mental health service utilization, psychiatric diagnosis, suicide attempts, and psychiatric hospitalization., Results: Consistent with past research, suicide rates were highest in rural areas, especially in men and older people. We also found that urban-rural differences in sociodemographic and psychiatric variables were sensitive to sex and age. Our results indicated that specialized mental health service use and accessibility to suicide means might help understand urban-rural differences in suicide, especially in men. When exploring urban-rural differences as a function of age, general practitioner visits for psychiatric reasons were more frequent in the older age group in rural areas., Conclusions: Study implications for suicide prevention strategies in Spain are discussed., (Copyright © 2017 SEP y SEPB. Background: Suicidal behaviour (fatal and non-fatal) has become a serious public health problem in many countries. The aim of the study was to describe the differential characteristics of emergency calls due to suicidal behaviour made to the Emergency Coordinating Centre (CCUE) in the province of Málaga, in comparison with calls due to physical or psychiatric problems., Methods: Retrospective observational study of the calls recorded in the database of the Public Company for Emergency Health during one year. Multivariate logistic regression analyses were carried out including age, gender and the following variables related with the demand: hours of the day, type of day (working days or bank holidays), months of the year and trimesters, number of resources mobilized and types of resolution., Results: The analyses were carried out on 163,331 calls, of which 1,380 calls were due to suicidal behaviour (0.8%), 9,951 for psychiatric reasons (6.1%) and 152,000 for physical reasons (93%). The emergency calls for suicidal behaviour were mainly made by females, between 31-60 years, in the evening and at night, and required transfer to hospital and more than one mobilized resource. Calls due to completed suicide were more frequently made by older men. Calls due to suicidal tendencies predominated over those due to attempted or threatened suicide during the first trimester of the year, while the opposite was the case during the third trimester., Conclusions: The results indicated differential characteristics of suicide calls that are potentially relevant for prevention in spite of the limitations of the present study.

Using official data, this study analyzed violent deaths (homicide, suicide, events of undetermined intent and deaths due to legal intervention) in Brazil and Mexico in the three-year periods 2002-2004 and 2012-14, the impact of these causes of death on life expectancy in both countries and the role of the different age groups in years of life expectancy lost (YLEL). Abridged life tables were constructed for both countries for both periods. Temporary life expectancy and YLEL between zero and 80 years by selected causes and age groups were calculated for each triennium. The leading cause of YLEL among men was homicide in both periods in Brazil (1.5 years) and in the second period in Mexico (one year). Violent deaths (VD) accounted for around 16% of YLEL in Brazil and 13% in Mexico in 2012-2014. Among women, YLEL due to homicides and suicides showed the greatest relative increase in both countries, although VD accounted for barely 3% of total YLEL. The highest percentage of YLEL due to VDwas found among the 15 to 29 year age groups in both countries and for both sexes. The increase in rates of VD in Mexico, above all among young people, has curbed further increases in life expectancy in recent years, especially among men. Likewise, the high rates of VD in Brazil in both periods have hindered the growth of life expectancy.

Background: The aim of this study was to know the suicide mortality rates in Navarra and Spain and to compare their time trends during the period 2000-2015., Methods: Suicide data were obtained from the Instituto Nacional de Estadística [INE], Spain’s National Statistics Institute. During all the period, the coding of cause of death in Navarra was carried out using data from forensic autopsies, while in some Spanish regions this process started after 2010. A Joinpoint time series regression model was used to assess the time trend and estimate the annual percent change [APC] of the rates by sex and 4 age groups., Results: Suicide mortality in Spain showed a significant downward trend in the overall rate between 2000-2011 [APC of -1.93% in males and -2.19% in females] followed by a significant increase in 2012-2015 [APC: 3.65% in men and 7.60% in women]. No inflection points were observed in men aged 25-44 [APC:1.26%], 45-64 [APC: 1.63%], and women over 65 years [APC:-2.18%]. In Navarra, global rates in men showed a non significant decrease [APC:-1.56%] and a significant decrease in the group of men between 25-44 years [APC:-3.19%]. Among women we observed a non significant increase [APC: 0.74%]., Conclusions: The rise in overall suicide rates observed in Spain since 2011, that was not observed in Navarre, could be influenced by the improvements in coding methods in several Spanish regions. However, this could not explain the decrease observed during the all period in men aged 25-44, that was even larger in Navarra.

Introduction: The aim of the present work is to determine the association between unemployment and suicide, and to investigate whether this association is affected by changes in the economic cycle or other variables such as age and sex., Methods: A time-trend analysis was conducted to study changes in the number of suicides between 1999 and 2013 in Spain. Pearson's correlation coefficients and regression models were used to find the association between unemployment and suicide., Results: A significant positive association was found between unemployment and suicide in the pre-crisis period in men. In that period (1999-2007), each 1% annual increase in unemployment was associated with a 6.90% increase in the annual variation of suicide in the total population, and with a 9.04% increase in the annual variation of suicide in working age men., Conclusions: The correlation between unemployment and suicide is significant in periods of economic stability, but has weakened during the recent financial crisis. We report the results of a retrospective, descriptive, qualitative study of suicide among university students in Bogotá, Colombia. The objective of this study was to document the magnitude, principal characteristics, and impact of this phenomenon in the selected population. A semi-structured survey was employed to collect information from 66 individuals linked to the universities. A total of 45 cases of suicide were documented in the study period (2004 - 2014). Of these, 69% occurred in males and 31% in females. The age range was 17 - 27 years, with 62% of the cases in the 19 - 22 year-old group. The most common mechanisms employed were suffocation and poisoning, followed by intentional falls, use of a firearm, and drug overdose. The selected location was the place of residence in 52% of cases and the university campus in 16% of cases. The distribution of students by area of knowledge showed a predominance of social and human science (44%) followed by engineering (22%). A history of difficulties in family and affective relationships was common among victims, as was a history of exposure to intolerance of differences in sexual orientation. The individuals surveyed expressed a wide range of interpretations of the significance of suicide, both positive (courage, self-affirmation, autonomy) and negative (defeat, despair, and an inability to adapt).

The aim of this study was to estimate and analyze the evolution of mortality rates due to suicide in Peru between 2004 and 2013. National death records from the Peruvian Ministry of Health were analyzed, calculating the regional mortality rates due to suicide standardized by age. Similarly, rates grouped in 5-year periods were geospatially projected. There were 3,162 cases of suicide (67.2% men); the age range with the highest incidence was 20 to 29 years (28.7%) and 49.2% were due to poisoning. Suicide rates increased from 0.46 (95% confidence interval [CI] = 0.38-0.55) to 1.13 (95% CI = 1.01-1.25) per 100,000 people from 2004 to 2013, respectively. The highest rates of suicide were identified in Pasco, Junín, Tacna, Moquegua, and Huánuco. The suicide issue in Peru requires a comprehensive approach that entails not just identifying the areas with the highest risk, but also studying its associated factors that may explain the regional variability observed.

Introduction: Suicides are one of the main public health issues globally. Objective: To analyze the trends and impact of suicide mortality in Colombia and México between 2000 and 2013, nationally, by sex and age groups. Materials and methods: Mortality vital statistics from the Colombian Departamento Administrativo Nacional de Estadística and the Mexican Instituto Nacional de Estadística y Geografía were used. We conducted a descriptive and cross sectional study for which we calculated standardized mortality rates and years of life lost in people between 0 and 100 years of age. Results: In Colombia, the suicide mortality rate decreased between 2000 and 2013 for both sexes (28% for men and 38% for women); an opposite trend was observed in México (with an increase of 34% for males and 67% for females). In 2013, the years of life lost in Colombia were 0,32 among men and 0,15 among women, with a decreasing trend since 2000, whereas in México a level of 0,42 was observed in men and 0,2 in women, with an increasing trend since 2000. The age groups where suicides had a bigger impact were those of men 15 to 49 years of age in both countries, while suicides were more uniformly distributed among women between 15 and 84 years of age., Conclusion:  Suicide mortality increased gradually in México, whereas in Colombia an opposite trend was observed. Suicides can be prevented, so it is fundamental to implement public health policies focused on timely identification, comprehensive prevention strategies and the study of associated risk factors.

Bariatric surgery is considered the most effective treatment for severe obesity and its benefits include improvement in medical comorbidities. However, a higher rate of suicides after this type of surgery has been reported. We performed a literature review on the subject, and concluded that the mentioned increase in suicide rates, compared to the general population, is probably caused by conditions that the patient had before surgery, especially psychiatric disorders such as depression or eating disorders. These are risk factors for suicide, and are more common in the population with indication for bariatric surgery. Therefore, it is necessary to thoroughly evaluate these patients before surgery searching for suicide risk factors, deriving them to a mental health professional if necessary and follow their mental health after surgery. Considering that the literature on the topic is inconsistent, further research is needed.

Introduction: Suicide is a serious public health problem worldwide, affecting all population groups, regardless of age, gender, or area of residence. The aim of this investigation was to characterise the recorded suicides in Colombia, between the years 2000 and 2010, according to the variables of the person, time and place., Methods: Descriptive quantitative study with information from secondary sources, from the death certificates of deceased people by suicide registered with the National Bureau of Statistics. The behaviour of the deaths and mortality, were determined using the denominator population projections of Colombia and the Amazon, Andean, Atlantic Coast, Eastern Plains and Pacific regions. Descriptive measurements and mortality rates were calculated using these., Results: A total of 24,882 suicides were recorded in the eleven years studied, with a mean of 6.2 people per day, which increased to 8.0 during holidays The mean age of death was 34.5 years (men 36.4, women 27.7), with male deaths (78.1%) and urban areas (66%) predominating. The greatest risk of dying from this cause was recorded in the Eastern Plains, in young adults and seniors, and residents in rural areas., Conclusions: Suicide rates in Colombia show a downward trend, with a mean of 5.3/100,000 inhabitants, and with an increased risk in men (3.7 times the risk in women, in young adults (9/100,000). This article analyses and compares the epidemiology of mental disorders and relevant public policies in Chile and Finland. In Chile, a specific mental health law is still lacking. While both countries highlight the role of primary care, Finland places more emphasis on participation and recovery of service users. Comprehensive mental health policies from Finland, such as a successful suicide prevention program, are presented. Both countries have similar prevalence of mental disorders, high alcohol consumption and high suicide rates. In Chile, the percentage of total disease burden due to psychiatric disorders is 13% and in Finland 14%. However, the resources to address these issues are very different. Finland spends 4.5% of its health budget on mental health, while in Chile the percentage is 2.2%. This results in differences in human resources and service provision. Finland has five times more psychiatric outpatient visits, four times more psychiatrists, triple antidepressant use and twice more clinical guidelines for different psychiatric conditions. In conclusion, both countries have similar challenges but differing realities. This may help to identify gaps and potential solutions for public health challenges in Chile. Finland’s experience demonstrates the importance of political will and long-term vision in the construction of mental health policies.

Unlabelled: Lesbian, gay, bisexual and trans (LGBT) populations have a high prevalence of suicide behaviors., Aim: To review the literature on suicide among LGBT populations., Material and Methods: A systematic review of the scientific literature published between 2004 and 2014 on suicidality among LGBT people was conducted. Forty-five articles were selected for a thematic analysis., Results: The research on suicide among LGBT people has increased during the last decade. The vast majority of studies have been conducted in the U.S. and other English speaking countries. At the same time, in a great part, these studies have been published in journals specializing in mental health and public health of sexual minorities. In general, they provide a significant foundation on protective and risk factors for suicide as well as their epidemiology., Conclusions: The focus of research on suicide among LGBT populations is mainly exploratory. More attention on these at-risk groups is required from the suicide research community in order to provide useful evidence for prevention and development of focalized and effective interventions.

Introduction: Suicide is an important Public Health problem. One of the most relevant known risk factors for suicide is suffering from a mental health disorder, identified in up to 90-95% of completed suicides, with this risk being increased if comorbidity is present. Findings from international research on the most common psychiatric disorders are dichotomous, divided into mood disorders and psychotic disorders. In Spain, data of this kind are scarce., Methods: This study describes the psychiatric and forensic characteristics of completed suicide cases (n=79) ocurred in a psychiatric hospital healthcare area (in Spain), between 2007 and 2010. The forensic data were obtained from the Institute of Legal Medicine of Catalonia and the clinical data by reviewing the clinical records., Results: Most of the subjects in this sample were males (78.5%, 95% CI; 68.4%-87.3%). Almost half of the sample (45.4%, 95% CI; 33.8%-57.1%, 35/77) had records in the Mental Health Services Network (including substance misuse services). Two of the 79 were under 18, so we were not able to access the records. More than half (54.3%, 95% CI; 37.1%-71.4%) of those with psychiatric history suffered from a mood disorder; 37.1% (95% CI; 22.9%-51.4% from a depressive disorder; 14.3% (95% CI; 2.9%-25.7%) from a bipolar disorder, and 17.1% (95% CI; 5.7%-31.4%) suffered from a psychotic disorder. With regard to substance misuse, 42.9% (95% CI; 25.7%-60.0%) presented substance misuse, and 48.6% did not., Conclusions: Psychiatric and forensic characteristics of completed suicide in this Spanish sample confirm previous findings from international studies: there is a high rate of psychiatric disorders in those who complete suicide, and there is a specific pattern as regards the method used to complete it., (Copyright © 2013 SEP y SEPB. The objective of this study was to analyze the burden of disease due to suicide in Mexico using years of life lost (YLL) between 2000 and 2012 by sex, age group (for those under 85 years of age) and jurisdiction. Vital statistics on mortality and population estimates were used to calculate standardized mortality rates and years of life lost due to suicide. Between 2000 and 2012 a sustained increase in the suicide mortality rate was observed in Mexico. The age group with the highest rate was 85 years of age or older for men, and 15-19 years of age for women. The highest impact in life expectancy due to suicide occurred at 20 to 24 years of age in men and 15 to 19 years of age in women. The states with the highest mortality due to suicide were located in the Yucatan Peninsula (Yucatan, Quintana Roo and Campeche). Mortality due to suicide in Mexico has increased continually. As suicides are preventable, the implementation of health public policies through timely identification, integral prevention strategies and the detailed study of associated risk factors is imperative.

Aims: Many studies have demonstrated that the risk of suicide in prison is higher than in the general population. This study has two aims. First, to explore the risk of suicide in men sentenced in Andalusian prisons. And second, to study the sociodemographic, criminal and, especially, psychopathological factors associated with this risk., Methods: An assessment was made of 472 sentenced inmates in two Andalusian prisons, and included a sociodemographic interview, the IPDE personality disorders questionnaire, the SCID-I diagnostic interview (DSMIV), and the Plutchick suicide risk questionnaire. The interviewers were experienced clinical psychologists with training in prison environments. Adjusted ORs were calculated using a logistic regression., Results: A risk of committing suicide was detected in 33.5% of the sample. The diagnoses (lifetime prevalence) of affective disorder (adjusted OR 3329), substance dependence disorders (adjusted OR 2733), personality disorders (adjusted OR 3115) and anxiety disorder (adjusted OR 1650), as well as a family psychiatric history (adjusted OR 1650), were the predictors that remained as risk factors after the regression analysis. No socio-demographic risk factor was significant in the regression analysis., Conclusions: The psychopathological variables are essential and the most powerful factors to explain suicide risk in prisons. A correct and systematic diagnosis, and an appropriate treatment by mental health professionals during the imprisonment are essential to prevent the risk of suicide., (Copyright © 2013 SEP y SEPB. In spite of the high prevalence of mental health disorders in Chile, there is a significant financing deficit in this area when compared to the world's average. The financing for mental health has not increased in accordance with the objectives proposed in the 2000 Chilean National Mental Health and Psychiatry Plan, and only three of the six mental health priorities proposed by this plan have secure financial coverage. The National Health Strategy for the Fulfilment of Health Objectives for the decade 2011-2020 acknowledges that mental disorders worsen the quality of life, increase the risk of physical illness, and have a substantial economic cost for the country. Thus, this article focuses on the importance of investing in mental health, the cost of not doing so, and the need for local mental health research. The article discusses how the United States is trying to eliminate the financial discrimination suffered by patients with mental health disorders, and concludes with public policy recommendations for Chile.

Objective: Determine the trend in mortality from external causes in pregnant and postpartum women and its relationship to socioeconomic factors., Methods: Descriptive study, based on the official registries of deaths reported by the National Statistics Agency, 1998-2010. The trend was analyzed using Poisson regressions. Bivariate correlations and multiple linear regression models were constructed to explore the relationship between mortality and socioeconomic factors: human development index, Gini index, gross domestic product, unsatisfied basic needs, unemployment rate, poverty, extreme poverty, quality of life index, illiteracy rate, and percentage of affiliation to the Social Security System., Results: A total of 2 223 female deaths from external causes were recorded, of which 1 429 occurred during pregnancy and 794 in the postpartum period. The gross mortality rate dropped from 30.7 per 100 000 live births plus fetal deaths in 1998 to 16.7 in 2010. A downward curve with no significant inflection points was shown in the risk of dying from this cause. The multiple linear regression model showed a correlation between mortality and extreme poverty and the illiteracy rate, suggesting that these indicators could explain 89.4% of the change in mortality from external causes in pregnant and postpartum women each year in Colombia., Conclusions: Mortality from external causes in pregnant and postpartum women showed a significant downward trend that may be explained by important socioeconomic changes in the country, including a decrease in extreme poverty and in the illiteracy rate.

Background: The suicide mortality rate in Andalusia is higher than the Spanish average, and it shows a different evolution over time. This study analyzes recent changes of suicide mortality in Andalusia, its geographical distribution and its relation with antidepressant use, altitude and socioeconomic inequality., Methods: An ecological study of the Basic Health Zones in Andalusia has been conducted. Age-standarized suicide rates in 2012 and during 2007-2011 were calculated. The correlation among variables and their association with the geographical differences was assessed with Spearman's coefficient and generalized linear models were also estimated., Results: Andalusia registered 7,58 suicides per 100.000 inhabitants per year from 2007 to 2011, with a range of 0,96 to 27,71 in the different areas. Positive correlation has been observed between altitude and suicide rates in 2012 (r=0,29; p<0,001) and during the period 2007-2011 (r=0,47; p<0,001). Antidepressant use in 2012 was not correlated with suicide rates in that year (r=0,03; p=0,67), but a positive correlation was observed with the precedent five years suicide rates (r=0,18; p=0,008). The Deprivation Index had a positive correlation with suicide rates in 2012 (r=0,29; p<0,001) and in 2007-2011 (r=0,33; p<0,001)., Conclusions: There is a clear geographical pattern in the distribution of suicide mortality in Andalusia and it remains stable over time. Central areas and in those with the highest altitude concentrate the highest suicide rates, which are associated with a larger use of antidepressants and also with higher levels of material deprivation.

Introduction: Suicidal behavior shows high numbers in prison population; hence, the following research was developed, which aimed to identify the sociodemographic, family, personal and imprisonment factors that are associated with suicidal ideation in inmates of a prison of Antioquia, Colombia., Material and Methods: A cross descriptive study was carried out. It was applied the Suicide Orientation Inventory Test ISO-30, as well as a survey, in order to investigate sociodemographic, family and imprisonment aspects of a group of 154 inmates. Univariate and bivariate analysis were applied to the collected data by using Excel and OpenEpi programs., Results: 14.9% had high suicidal ideation. By analyzing the distribution of high suicidal ideation in the variables studied, it was found that the highest rates occurred in those who had previous attempts, 61.1%; a history of domestic violence, 50.0%; no schooling, 33.3%; those with a family history of suicide, 28.6%; and those without a definite sentence 22.9%. A significant association was found between the ideation and having committed previous suicide attempts (P<.01; rp=7.4), belonging to households with domestic violence (P=.03; rp=4.0), being single (P=.04; rp=2.2) and being under 30 years old (P=.04; rp=2.1)., Conclusions: The high suicidal rates are much greater than those found in the general population; it is therefore recommended to the professionals of the institution to address the risk factors found here, in order to develop prevention and intervention programs., (Copyright © 2014 Asociación Colombiana de Psiquiatría. Objective: To determine the number of suicides and the main social and demographic characteristics, time frames and methods involved during the period 2010-2013 in Navarra by gender., Methods: A study of the Electronic Clinical Records provided by Navarra's Healthcare Service in which suicides in Navarra are detailed., Results: One hundred and eighty individuals committed suicide. The number of suicides remained stable: 41 in 2010, 51 in 2011 and 44 in 2012 and 2013. Seventy-five point four percent were males (n=136) and 24.6 % (n=44) were females. In the 13 to 26 age group, 12 (92.3%) out of 13 suicides were committed by males. In terms of employment status, 49.3% (n=70) were pensioners. The highest rate of suicides was reached in summer (n=71) and spring (n=39). Monday was the day with the highest rate of suicides (n=37) and the time period between 8:00 and 12:00 hours was when the highest number of suicides (n=80) took place. The most usual ways of committing suicide were hanging (n=80), falling from a height (n=41) and pharmacological overdose (n=23). Males used violent methods more frequently. It is necessary to highlight the fact that in some of the sociodemographic characteristics there was up to 60% of cases without enough information., Conclusion: The results obtained show some specific features of the phenomenon of suicide in Navarra that should be considered for its prevention. Furthermore, the implementation of effective protocols of data collection is recommended to develop prevention strategies.

Introduction: Income inequality is directly related to the mental health of the population. However, the relationship between income inequality and suicide rates in Colombia has still not been explored., Objective: To estimate the relationship between inequality and suicide rates in Colombia from 1994 to 2013., Methods: An ecological study was conducted, in which the correlation was estimated (Spearman) between inequality (Gini coefficient) and suicide rate between 1994 and 2013, according to official information available from the National Administrative Department of Statistics (DANE), and the National Institute of Forensic Science and Forensic Medicine., Results: A Gini coefficient between 0.53 and 0.60 (median, 0.65 [interquartile range, 0.54-0.57]) was observed, and suicide rates were between 3.84 and 5.26 (median, 4.20 [4.08-4.86]). The correlation between inequality and suicide rates was positive and statistically significant (r=.70; p<.001)., Conclusions: There is a positive association between economic inequality and suicide rate in Colombia. Introduction: Approximately 3,500 people commit suicide every year in Spain. The main aim of this study is to explore if a spatial and temporal clustering of suicide exists in the region of Antequera (Málaga, España)., Methods: Sample and procedure: All suicides from January 1, 2004 to December 31, 2008 were identified using data from the Forensic Pathology Department of the Institute of Legal Medicine, Málaga (España). Geolocalisation. Google Earth was used to calculate the coordinates for each suicide decedent's address. Statistical analysis. A spatiotemporal permutation scan statistic and the Ripley's K function were used to explore spatiotemporal clustering. Pearson's chi-squared was used to determine whether there were differences between suicides inside and outside the spatiotemporal clusters., Results: A total of 120 individuals committed suicide within the region of Antequera, of which 96 (80%) were included in our analyses. Statistically significant evidence for 7 spatiotemporal suicide clusters emerged within critical limits for the 0-2.5 km distance and for the first and second semanas (P<.05 in both cases) after suicide. There was not a single subject diagnosed with a current psychotic disorder, among suicides within clusters, whereas outside clusters, 20% had this diagnosis (X2=4.13; df=1; P<.05)., Conclusions: There are spatiotemporal suicide clusters in the area surrounding Antequera. Background and Objectives: To be able to prevent suicides, The exact magnitude of suicides must be known in order to prevent them. There is evidence that data provided on the number of suicides by the Spanish Statistical Office (Instituto Nacional de Estadística [INE]) differs from that identified by forensic pathologists. This paper aims to determine whether the INE data are to the same as those recorded by the agencies responsible for conducting autopsies in Spain (IML), and calculate the number of missing cases., Method: IML were requested to provide the number of suicides in a five year period (2006-2010) and this was compared with those offered by INE in its site. A new rate of suicides per 100,000 was obtained and used to calculate the number of suicides in Spain., Results: For every year of the studied period, the number of suicides reported by the INE was lower than that obtained directly from the IML. Using the highest annual suicide rate registered by province and the total, a count was obtained of identified suicides that for some reason had not been recorded in the INE or the IML data. Based on these figures, the mean rate of suicides per 100,000 was 0.97 (SD 0.10) higher than official INE data, which indicates a mean loss of 443.86 (SD 46.72) cases reported each year. Possible factors that may influence the missing recorded cases are discussed., Conclusions: According to the results, there is a discrepancy between INE and IML about the number of suicides in Spain. Objectives: To analyze the trend in suicide mortality in Andalusia from 1975 to 2012 and its relationship with unemployment and the use of antidepressants., Methods: Poisson's segmented regression models were used to estimate changes over time. The association between suicide and the factors examined was measured using Spearman's correlation coefficient., Results: Suicide mortality patterns in men and women are rising. The largest increase was found in people aged from 15 to 44 years, with an annual percentage rate change of 1.21 (95%CI: 0.7-1.7) for men and 0.93 (95%CI: 0.4-1.4) for women., Conclusions: Mortality by suicide has increased in Andalusia since 1975 in all age and gender groups except for women aged 65 years or above. During the last few decades, an upward trend has been observed in young people and a stable or falling trend in the remaining population. The main mission of Spanish Institutes of Legal Medicine (ILMs) is to serve the justice system. We review the potential broader role of the work done by ILMs, with an emphasis on forensic pathology. The relevance of forensic information to increase the quality of mortality statistics is highlighted, taking into account the persistence of the low validity of the external causes of death in the Mortality Register that was already detected more than a decade ago. The new statistical form and reporting system for the deaths under ILMs jurisdiction, as introduced by the Spanish Instituto Nacional de Estadística in 2009, are also described. The IMLs role in the investigation of the following mortality causes and of their determinants is reviewed in detail: traffic accidents, suicide, drugs of abuse, child deaths and sudden deaths. We conclude that an important public role of IMLs is emerging beyond their valuable service to the justice system, mainly through the gathering of data critical to assess and prevent several medical and public health and safety issues of great social impact and through their participation in epidemiologic research and surveillance., (Copyright © 2014 Elsevier España, S.L. Background: Suicide is an important public health problem accounting for an important proportion of deaths in Chile., Aim: To describe trends in suicide mortality in Chile between 1998 and 2011, by year, region, sex and age., Material and Methods: A population survey study was conducted using suicide mortality data of Chile from 1998 to 2011, provided by the statistics department of the Ministry of Health, Coroners' offices and the Pan American Health Organization (PAHO/WHO). Age-adjusted suicide mortality rates were calculated per 100,000 habitants. Variables analyzed included year, age, sex, etiology and geographical distribution. An analysis of the average annual variation (AAV) was performed and the logarithm of the age-adjusted rates by year and region was fitted by applying linear regression models. Relative risks (RR) by sex, geographical distribution and age were also calculated., Results: The mean suicide rate in Chile, was 12.11 per 100.000 in the period 2000-2011. The rates were higher in men, with a higher AAV in women and a relative risk for men was 5.14 higher than in women. The rates were higher in the southern regions of the country. Atacama had the highest AAV. By age, the rates are consistently higher in subjects aged 40 to 59 years until 2006, when this pattern started to change. Between 2006 and 2009, subjects aged 25-39 years had the highest rates. Subjects aged 0 to 14 years, exhibit the highest AAV., Conclusions: The distribution of suicide rates by sex in Chile is similar to other countries, but it is different by age. The age range with the highest suicide rates changed over time.

Objective: To assess the satisfaction of persons with suicidal behaviour and their relatives using patient information material included in the Clinical Practice Guidelines on Prevention and Treatment of Suicidal Behaviour., Method: The sample was made up of 57 patients with suicidal ideation or behaviour, and 52 relatives. The participants were recruited through a suicide prevention programme (Programa de intervención intensiva en conducta suicida [PII] - Suicidal Behaviour Intensive Intervention Programme) and a family association (Federación de Asociaciones de Familiares y Personas con enfermedad mental de Galicia [FEAFES] - Galician Federation of Associations of Relatives and Persons with mental diseases). An ad-hoc questionnaire was designed to ascertain the degree of perceived satisfaction and usefulness of using the information included in the guidelines., Results: The descriptive data of the sample is presented, along with an exploratory factorial analysis of the questionnaire that yielded two dimensions, i.e., format and usefulness. Patients scored significantly lower than the relatives in two dimensions; nevertheless, no significant differences were found between the two groups in the level of general satisfaction. The socio-demographic variables did not influence the results. Similarly, no differences were observed between patients with and without history of suicidal behaviour. Participants stressed that Primary Care was the setting best suited for dissemination of this type of information., Conclusions: In general, both patients and relatives displayed a high level of satisfaction with the patient information material assessed. Introduction: Psychosocial behaviors are the object of indifference, which has lead to a generalized increase in attitudes favoring violence. Among these, we can find young people as victims or killers., Objective: To describe the behavior of mortality due to homicide and suicide in young population between 15 and 24 years old in the departments of Colombia from 1998 to 2008., Materials and Methods: This was a descriptive study form a secondary source. We used 150,433 death certificates with homicide or suicide as cause of death in 15 to 24 years old population in Colombia during the period 1998-2008. We calculated an index for favorable conditions for violence according to economical, social and educational variables., Results: Mortality rate was 95.6 deaths/100,000 people in this age range for homicide, and 10.1 for suicide. Men showed greater risk than women, with it being 12.3 higher for homicide and 1.2 for suicide. This evidences the way in which violence takes productive and reproductive lives, thus affecting the structure by sex and age and decreasing the opportunity window, but having a higher risk in some departments more than in others., Conclusion: According to the index for conditions for violence, the departments of Vaupés, Guaviare, Arauca and Guainía are the ones with more favorable conditions for an increased risk of dying by homicide and suicide in people from 15 to 24 years old, regardless of other departments having higher deaths rates.

Aim: To analyze the mortality trend by accidental falls in Mexico, from 1979 to 2010., Material and Methods: We used official mortality data in México. Variables included were mortality cause, place of lesion, age, sex, year of mortality registration, size of the town of occurrence, state, occupation, workplace falls., Results: During that period, 124,509 deaths by accidental and intentional falls were registered; of these 77.03% were men. From 1989 (3.20 per 100,000) to 2010 (1.99 per 100,000), the mortality trend was falling (β = -0.053/ 100,000, p ≤ 0.001). In the majority of states, we found a decreased between 1993-1997 and 2006 to 2010, except in Aguascalientes, Puebla, Baja California Sur, Durango and Nuevo León., Conclusions: This is the first study in México that showed the descending trend of mortality in accidental falls in the last two decades.
