Se presenta el caso de un paciente que acudió al cuerpo de guardia del CIMEQ en marzo del 2007. Presentaba un cuadro clínico de abdomen agudo quirúrgico y fue intervenido con el diagnóstico de apendicitis aguda complicada. En el transoperatorio se constató la presencia de un gran divertículo de Meckel, que inicialmente se inflamó (diverticulitis aguda) y posteriormente evolucionó hasta la necrosis, por lo que afectó la irrigación de un segmento intestinal en el íleon terminal, el ciego y el tercio inferior del colon ascendente. El apéndice no tenía ninguna alteración, por lo que se realizó una hemicolectomía derecha. El paciente evolucionó favorablemente y fue dado de alta a los 10 días, con una evolución favorable.This is the case presentation of a patient treated in Emergency Department in March 2007 presenting with a clinical picture of surgical acute abdomen and operated on due to complicated acute appendicitis. At transoperative period presence of a Meckel's diverticulum was verified that initially becomes inflamed (acute diverticulitis) and later process evolved until necrosis affecting the blood stream of an intestinal segment in terminal ileum, cecum ant the lower third of ascending colon. Appendix was not altered and a hemicholectomy was performed. Patient had a favorable course and was discharged at 10 days.

Goals: The current trend in the treatment of non-complicated diverticulitis is the outpatient management with antibiotic or even anti-inflammatory regimens in selected cases. We present a comparison of the results in our hospital with different protocols applied in 2016 and 2017., Material and Methods: All patients selected for this study were diagnosed with diverticulitis grade Ia of Hinchey's classification according to radiological findings on abdominal CT. We have analyzed two retrospective cohorts: 100 patients treated in 2016 according to the old protocol and 104 patients treated in 2017 with a new protocol. In 2016, the candidates for ambulatory treatment remained under observation for 24 hours before being discharged. The treatment consisted of 14 days of ciprofloxacin and metronidazole. In 2017, only patients with more acute symptoms were observed 24 hours and amoxicillin-clavulanic acid was prescribed for only 5 days., Results: The persistence of the disease in 2016 was 6% and in 2017 was only 5.77% (p = 0.944). Recurrence during the first year was 13% in the first group, while in the second it was 5.7%, although this difference was not statistically significant. Likewise, a significant reduction in health costs was achieved., Conclusions: Outpatient treatment of acute uncomplicated diverticulitis with oral treatment seems to be a safe and effective therapeutic strategy in selected patients with low comorbidity., (Copyright © 2020 Elsevier España, S.L.U. Introduction: Acute diverticulitis (AD) is increasingly seen in Emergency services. The application of a reliable classification is vital for its safe and effective management. Objective: To determine whether the combined use of the modified Neff radiological classification (mNeff) and clinical criteria (systemic inflammatory response syndrome [SIRS] and comorbidity) can ensure safe management of AD. Material and methods: Prospective descriptive study in a population of patients diagnosed with AD by computerized tomography (CT). The protocol applied consisted in the application of the mNeff classification and clinical criteria of SIRS and comorbidity to guide the choice of outpatient treatment, admission, drainage or surgery. Results: The study was carried out from February 2010 to February 2016. A total of 590 episodes of AD were considered: 271 women and 319 men, with a median age of 60 years (range: 25-92 years). mNeff grades were as follows: grade 0 (408 patients 70.6%); 376/408 (92%) were considered for home treatment; of these 376 patients, 254 (67.5%) were discharged and controlled by the Home Hospitalization Unit; 33 returned to the Emergency Room for consultation and 22 were re-admitted; the success rate was 91%. Grade Ia (52, 8.9%): 31/52 (59.6%) were considered for outpatient treatment; of these 31 patients, 11 (35.5%) were discharged; eight patients returned to the Emergency Room for consultation and five were re-admitted. Grade Ib (49, 8.5%): five surgery and two drainage. Grade II (30, 5.2%): ten surgery and four drainage. Grade III (5, 0.9%): one surgery and one drainage. Grade IV (34, 5.9%): ten patients showed good evolution with conservative treatment. Of the 34 grade IV patients, 24 (70.6%) underwent surgery, and three (8.8%) received percutaneous drainage. Conclusions: The mNeff classification is a safe, easy-to-apply classification based on CT findings. Together with clinical data and comorbidity data, it allows better management of AD.

INTRODUCTION THE AIM: was to analyse the stoma reversal rate after surgery for complicated acute diverticulitis (CAD), and more specifically the end-stoma-reversal, as well as the delay, feasibility, complications and risk factors for stoma maintenance., Methods: A multicentre retrospective study of patients who had undergone urgent surgery for CAD with stoma formation in ten hospitals during a period of 6 years. The frequency of reversal over time and the factors affecting the decision for reversal were analysed., Results: Out of 385 patients operated for CAD, 312 underwent stoma creation: 292 end colostomies and 20 diverting stomas. During follow-up, stoma reversal surgery was performed in 161 patients (51.6%) after a median of 9 months. The main causes for not performing stoma reversal were comorbidities and the death of the patient. Advanced age was an adverse factor in the multivariate analysis, and the actuarial rate of reversal was higher in men and in patients with no previous Hartmann's operation. Stoma reversal surgery was completed in all but one patient, and a loop ileostomy was associated in four. Morbidity and mortality rates were 35.7% and 1.9%, respectively. A total of 8.4% of patients underwent re-operation, and 6% experienced an anastomotic leak. Twelve patients remained with a stoma after the attempted reconstruction surgery., Conclusions: Surgery for CAD is frequently associated with an end stoma, which will ultimately not be reversed in almost 50% of patients. El éxito en el tratamiento de las infecciones necrotizantes de los tejidos blandos (INTB) depende de la precocidad diagnóstica y de la agresividad terapéutica, basada en el desbridamiento quirúrgico, la antibioticoterapia de amplio espectro y el soporte intensivo. Se presenta un caso de INTB secundaria a diverticulitis aguda perforada (Hinchey 4) con el fin de ilustrar las consideraciones particulares en el manejo de la INTB de localización en la pared abdominal, apoyadas en la evidencia disponible en la literatura científica. Se identifican como puntos clave la precocidad y la agresividad del desbridamiento quirúrgico, la antibioticoterapia dirigida por el cultivo y su suspensión guiada por la negativización microbiológica; y el empleo de técnicas combinadas de autoplastia y prótesis apoyadas en la terapia de presión negativa en la restauración de la pared abdominal, con lo que se ha obtenido un buen resultado., Successful treatment of necrotizing soft tissue infections (NSTI) depends on early diagnosis and therapeutical aggressiveness, based on surgical debridement, broad spectrum antibiotics and intensive support. A case of perforated diverticulitis (Hinchey 4)-secondary-NSTI is presented in order to illustrate the particular considerations in the management of abdominal wall located NSTI, supported on the evidence available in scientific literature. Several key points are identified, such as prompt and aggressive surgical debridement; culture-guided antibiotherapy and its suspension based on microbiological negativization; and the utilization of combined techniques of autoplastic repair and replacement, also supported on negative pressure therapy in the abdominal wall restoration, which have shown a good outcome., (Copyright: © 2018 Permanyer.)

The authors present three cases report of appendicular diverticulum with associated diverticulitis, one of them asymptomatic. The clinical distinction between acute apendicitis and apendical diverticulitis is very difficult, the later usually with earlier suppuration. The are no consensus regarding preventive surgery for apendical diverticulitis. It is possible that incidence of apendicular diverticulum depends on careful anatomopathological exams.
