OBJECTIVE: To compare three methods: Biochemical tests, high-performance liquid chromatography (HPLC) and polymerase chain reaction-restriction fragments length polymorphism (PCR-RFLP), for the identification of mycobacteria, and to perform a cost-benefit analysis to define an optimum identification algorithm. MATERIAL AND METHODS: One-hundred-and-seven mycobacteria isolates were identified by the three methods at Instituto de Diagnóstico y Referencia Epidemiológicos, between February of 1999 and January of 2000 and the results were compared with those of a reference laboratory using the Q-Cochran statistical test. RESULTS: PCR-RFLP was the most rapid and specific procedure but also the most expensive; biochemical tests excelled for identification of Mycobacterium tuberculosis, but were lengthy and expensive for other mycobacteria; HPLC ranked in the middle for price, speed and specificity. CONCLUSIONS: Considering the expected proportion of M. tuberculosis, the following algorithm was proposed: Initially, biochemical tests should be performed; if the results indicate a non-tuberculous mycobacteria, the isolate should be analyzed with HPLC; if results are unclear, the isolate should be analyzed using PCR-RFLP. Isolates showing a previously undescribed PCR-RFLP pattern should be characterized by DNA sequencing.OBJETIVO: Comparar tres métodos: pruebas bioquímicas, cromatografía líquida de alta resolución (HPLC, por sus siglas en inglés) y reacción en cadena de la polimerasa-polimorfismo del tamaño de fragmentos de restricción (PCR-RFLP) para identificar micobacterias a nivel especie, analizando costo-beneficio y proponiendo un algoritmo de identificación. MATERIAL Y MÉTODOS: Entre febrero de 1999 y enero de 2000, en los laboratorios del Instituto de Diagnóstico y Referencia Epidemiológicos se tipificaron 107 aislados de micobacterias y los resultados se compararon con los obtenidos en un laboratorio de referencia utilizando la prueba estadística Q de Cochran. RESULTADOS: Se encontró que el PCR-RFLP fue el método más específico y rápido pero también el más caro. Las pruebas bioquímicas fueron confiables para la identificación de Mycobacterium tuberculosis, pero lentas e inespecíficas para otras micobacterias. El HPLC estuvo en un nivel medio tomando en cuenta costo, tiempo y especificidad. CONCLUSIONES: Considerando la proporción esperada de M. tuberculosis, se propone el siguiente algoritmo: si las pruebas bioquímicas indican una micobacteria no tuberculosa, el aislado será analizado por HPLC; si la identificación no es clara, el aislado será analizado usando PCR-RFLP. Si el aislado no pertenece a un patrón descrito, se identificará por secuenciación de ADN.

Com o objetivo de enfatizar a importância da escolha adequada de um parâmetro foi analisado o diagnóstico de deficiência de ferro de 855 crianças, com 60 meses e menos de idade, utilizando o padrão proposto pela OMS (11,0 g/dl) e outros. A inclusão de maior número de "falsos-positivos", resultante de parâmetros de maior sensibilidade do que o da OMS, é compensada pela garantia de tratamento à maior parte das crianças ferro-deficientes. Ao mesmo tempo que tal ampliação de programa é importante em sociedades de países em desenvolvimento, onde a prevalência de anemia é elevada, esta extensão é factível de ser executada dado o baixo custo do suplemento de ferro.A comparison of diagnoses of iron-deficiency, using the index proposed by WHO (11.0 g/dl) and other parameters of great sensitivity, in a population of children of 60 months or less, was made. The inclusion of a great proportion of "positives" through the use of these standards is compensated for by the guarantee of adequate treatment for the majority of iron-deficient children. This is an important attitude in developing societes with high prevalence of iron-deficiency anemia and is also practical because of the low cost of iron supplement.

Foram estudados, comparativamente, em 500 pacientes, os métodos de exames de fezes de Lutz, Faust modificado e Kato-Katz para o diagnóstico parasitológico de fezes. O método de Kato-Katz proporcionou maiores índices de positividade do que as outras duas técnicas no diagnóstico de ancilostomideos, T. trichiurus e S. mansoni. Nenhuma diferença foi observada quanto ao diagnóstico de A. lumbricoides. Para o diagnóstico das protozooses não houve diferença significativa entre os métodos de Faust modificado e de Lutz. Baseado nos dados obtidos, eficiência, simplicidade e rapidez de execução recomenda-se a associação dos métodos de Kato-Katz e Faust modificado na rotina de diagnóstico parasitológico de fezes.A comparative study was of the Lutz, Faust (modified), and Kato-Katz stool examination methods for parasitological diagnosis using 500 patients. The Kato-Katz method provided higher rates of positivity than the other two techniques in the diagnosis of Ancylostomidae, T. trichiurus, and S. mansoni. No differences were observed in the diagnosis of A. lumbicoides. Nor was a statistically significant difference observed in the diagnosis of protozoosis when using the Lutz and modified Faust methods. The data obtained led to a recommendation for an association of Kato-Katz and modified Faust methods for routine parasitological diagnosis due to Us simplicity, rapidity, and efficiency.
