Angiogenesis, the growth of new capillaries from pre-existing vessels, is closely related to processes essential for the organism, which are entirely physiological, for example embryogenesis, reproductive cycle, and wound healing, and is also associated with pathological conditions such as tumor progression, metastasis, diabetic retinopathy, hemangioma, arthritis, psoriasis and atherosclerosis, among other chronic diseases. The development of specific anti-angiogenic agents arises as an attractive therapeutic approach to treat cancer and other angiogenesis-dependent diseases. Many of these agents are being evaluated in clinical trials and have shown promising antitumor activity. This review attempts to provide a comprehensive overview of key knowledge accumulated on angiogenesis and its role in cancer, including the components of signal transduction pathways that have been explored in this process. Additionally, this review focuses on the current approaches for the discovery of new compounds that inhibit angiogenesis, emphasizing on the clinical developmental status of antiangiogenic drugs. Introduction. There is a growing interest in developing new biologic markers to accurately evaluate prostate cancer; microvascular density, or angtogenesis, is one of the newest and must be evaluated as a prognostic factor in patients with locally advanced disease. Method. Patients operated of radical prostatectomy at the Hospital Central Militar from January 1999 to December 2003, and with postoperative diagnosis of a locally advanced disease, were included in this study. They were classified into two groups: with and without disease progression; tumor microvascular density was determined in each one. Results. 117 patients were operated in that period; 53 patients had a locally advanced disease; 24 patients were excluded, 29 included. From the included ones, 24 had disease progression and 5 didn't progress. Each group was divided according to tumor differentiation grade. Differences between each group were analyzed; variables were: prostatic specific antigen, microvascular density and progression time in months. Discussion. With previous studies we could suggest that angiogenesis is important in prostate cancer development and progression. This study couldn't demonstrate angiogenesis as a prognostic factor for locally advanced prostate cancer, maybe because of a reduced number of patients or a short time of observation. Conventional treatment of cancer using chemotherapeutic drugs and external beam radiation has low therapeutic index. There is damage to all proliferating cells with no discrimination between healthy and diseased tissue. Novel cancer therapies that selectively eliminate cancer cells with minimum toxicity to normal tissue are generally termed "tumor targeting." Various targeting molecules are used, like antibodies or other proteins that recognize tumor-associated targets. Other approaches are targeting of molecules that interfere with signal-transduction pathways, down regulate proto-oncogenes, or interfere with tumor angiogenesis. This is a general review of tumor targeting, its principles and history. General problems and requirements associated with tumor targeting, such as conjugate design, present status of immunotargeting, and molecular targets are discussed.
