Introduction: Testicular cancer is most common in men aged 15-35 years, their classification in seminoma and nonseminoma determines the treatment of nodal metastases and about 70% of non-seminomas present with dissemination predictable metastatic activity. In the treatment of germ cell tumors nonseminoma lymphadenectomy is an option in the case of teratoma complete surgical resection is the treatment of choice. Objective: To report the initial experience with laparoscopic retroperitoneal lymphadenectomy in the treatment of non-seminoma testicular cancer in the Hospital General del Estado de Sonora. Material and methods: We present a male patient 30 years old with diagnosis of stage IIA testicular cancer, and histopathologic results of mixed tumor (seminoma 80% and mature teratoma 20%) treated with laparoscopic retroperitoneal lymphadenectomy. Results: The harvest was 5 lymph nodes negative for malignancy. A surgical time of 230 minutes without complications, bleeding of 70 cc; hospital stay of two days and preserved antegrade ejaculation. Conclusion: Laparoscopic retroperitoneal lymphadenectomy is a feasible therapeutic option for nonseminoma testicular cancer; offers lower perioperative morbidity, fewer days of hospitalization and convalescence compared to open procedures, without compromising the oncologic outcomes.
