Preeclampsia is a hypertensive complication of pregnancy, which is associated with high levels of maternal and fetal morbidity. About 5 to 8% of pregnancies are affected by this gestational disorder. Renal, cardiac, pulmonary, hepatic, neurologic dysfunction; hematologic disorders; fetal growth restriction; stillbirth; and maternal death are the serious complications that can occur in preeclampsia. Hyperuricemia is a biomarker that shows the progression of gestational hypertension and the risk of fetal and maternal complications as it induces the release of proinflammatory cytokines, such as TNF-a, IL-1ß and IL6. According to different investigations, authors indicate that high uric acid levels were notable in the second trimester in women who developed preeclampsia. Investigators estimate concentrations reported to be 6.08 ± 0.49 mg/dL versus 5.20 ± 0.19 mg/dL, and emphasize that at least 25.9% of hyperuricemic preeclamptic women (>6 mg/dL) were referred to the special care unit. The present study aims to collect current information with the purpose of clarifying the role of uric acid as a predictive biomarker in preeclampsia, in order to determine its diagnostic value in this gestational complication. Introduction: Cardiovascular diseases in pregnant women are challenging, with high maternal and perinatal morbidity and mortality, so a cardio-obstetric team is recommended for their care. Even so, little data evaluates the impact of these teams. Therefore, the present study aims to compare the obstetric, maternal, and neonatal outcomes of semi-structured follow-up (SSF) in a Cardio-obstetric clinic concerning regular or unstructured follow-up (USF) in pregnant women with heart disease. Methods: A prospective registry of pregnant women with heart disease was carried out. Patients with SSF by a cardio-obs-tetric team were compared with those with single evaluation or USF. The risk of events was calculated according to the modified World Health Organization (mWHO) classification and the CARPREG-II scale, and cardiac, obstetric, and neonatal outcomes were evaluated. Results: One hundred sixty-eight patients were evaluated, 37 with SSF and 131 with single eva-luation (USF). The primary diagnoses were congenital heart disease, arrhythmias, and valve disease. The average CAR-PREG-II in USF patients was 2.48 (SD 2.3); in SSF patients, it was 3.37 (SD 2.45; p = 0.041). The average of the mWHO in patients with USF was 2.1 (SD 1.6), and with SSF, it was 2.65 (SD 0.95; p = 0.0052). There were no significant differences in primary cardiac outcomes (13.8% in USF vs. 5.4% in SSF; p = 0.134), secondary cardiac (5.3% in USF vs. 2.7% in SSF; p = 0.410), obstetric (10% in USF vs. 16.2% in SSF; p = 0.253) and neonatal (35.9% in USF and 40.5% in SSF; p = 0.486) even though patients with SSF had a higher risk than patients with USF according to the mWHO and CARPREG-II scales. Conclusions: In pregnant women with heart disease, an SSF compared with a USF by a cardio-obstetric team did not show statistically significant differences in cardiovascular, obstetric, and neonatal outcomes. However, patients with SSF had a significantly higher risk of adverse outcomes due to the mWHO and CARPREG-II scales. This result suggests that the SSF achieves at least equal outcomes despite the higher risk of adverse events that patients in this group had. [ABSTRACT FROM AUTHOR]

Objectives: To describe the frequency of maternal complications in pregnant women with major or minor placenta previa (PP), and to assess a potential association between PP type and the presence of severe maternal bleeding and other associated outcomes. Material and methods: Retrospective descriptive cohort. The study included pregnant women with 20 weeks of gestation or more and a confirmed diagnosis of placenta previa who were seen in a high complexity hospital in Cali (Colombia), between January 2011 and December 2020. Women with a diagnosis of placenta previa and concomitant placenta accreta were excluded. The collected variables were maternal age, body mass index, smoking, obesity, parity, presence of bleeding, postpartum hemorrhage, management of postpartum hemorrhage, transfusion, and maternal ICU admission. A descriptive analysis was performed. The protocol was approved by the ethics committee of Fundaciónn Valle de Lili. Results: A total of 146 patients met the inclusion criteria. The population consisted of women with a mean age of 32 years, with no history of prior surgery, with a prenatal diagnosis of placente previa at week 22; 70% were major placenta previa cases. The most frequent complications were postpartum hemorrhage (37.9 % vs. 16.3 % for patients with major and minor placenta previa, respectively), transfusion requirement (23.3 and 9.3 %, respectively), and maternal ICU admission (40.8 % vs. 18.6 %, respectively). There were no cases of maternal death. Conclusions: There is a high frequency of complications in women with placenta previa, and it is probably higher in cases of major placenta previa. Further studies are needed to compare the frequency of maternal complications according to the type of placenta previa. [ABSTRACT FROM AUTHOR]

- Abstract
Maternal and perinatal health is one of today's global health priorities. Cardiovascular disease and stroke are the leading causes of maternal mortality. Placental abruption remains a critical concern for maternal morbidity because it has been associated with long-term vascular disease. However, there is neither much literature available in Spanish nor recent evidence elucidating some questions on this topic. Thus, this review aims to synthesize recent evidence on the long-term risk of cardiovascular and cerebrovascular disease in women with a personal history of placental abruption. It was found that, through complex pathophysiological mechanisms involving the structure and functionality of the placental vascular network with subsequent extension of vascular injury and production of proinflammatory and procoagulant factors which remain after delivery, major cardiovascular events are precipitated in the medium and long term. High-quality evidence has shown that the risk of maternal complications in women with placental abruption accounts for 2.14, rising even higher for those with severe placental abruption. In the medium and long term, the mortality risk caused by coronary heart diseases is 2.64 and by cerebrovascular disorders is 1.70, with equal risk for both ischemic and hemorrhagic strokes. It can therefore be concluded that cardiovascular and cerebrovascular risk is imminent in women with a history of placental abruption due to a number of vascular pathophysiological mechanisms. However, this risk is considerably increased when associated with traditional and non-traditional modifiable factors. [ABSTRACT FROM AUTHOR]

Introduction. The World Health Organization (WHO) estimates that more than 40% of pregnant women worldwide have anemia, and that half of them suffer from iron deficiency. The prevalence of this in Latin America is 40%, and in Colombia, 44.7%. Physiologically, a problem called "dilutional anemia" occurs during pregnancy. There are conditions in pregnant women that predispose them to suffering from pathological anemia. The latter is mainly caused by iron deficiency, hence the importance of diagnosing this entity on time and starting treatment. Iron administration is the basis of treatment of anemia caused by iron deficiency. It can be administered orally, which is the preferred option in the majority of patients. However, when this is not possible, parenteral iron must be used. However, parenteral iron is rarely used as the first line of treatment of gestational anemia. The objective of this article is to carry out a review that allows for the identification of therapy with parenteral iron as an efficient alternative for the treatment for gestational anemia, considering the pharmacological characteristics, administration, and use among the different molecules available in Colombia. Methodology. We carried out a search in databases and electronic libraries. The criteria considered were texts published between 1996 and 2020 in Spanish and English. 95 articles were obtained, of which 49 were selected. The keywords for their search were physiology, parenteral iron, gestational anemia, iron deficit, pregnancy complications, iron compounds, pharmacokinetics, diagnosis, and treatment. Division of Covered Topics. Physiology; diagnostic aids; pharmacological characteristics of parenteral iron; advantages, indications, and contraindications of parenteral iron; secondary effects and application method. Conclusions. Parenteral iron is a safe and efficient treatment to handle anemia during pregnancy. The indications and pharmacology of the molecules must be considered to choose the most appropriate option. In addition, it replaces iron reserves and hemoglobin levels more quickly. Objectives: To describe the frequency and factors associated with non-adherence to prenatal follow-up in pregnant women 35 years of age or older in the department of Cauca, Colombia, between 2016 and 2018. Material and methods: Cross-sectional, descriptive observational study of records of pregnant women between 35 and 41 years of age affiliated to the Cauca Indigenous Association Health Benefit Plan Management Organization-I (EAPB AIC-I) and registered in the prenatal care program, between 2016 and 2018. Duplicate records and records with incomplete information were excluded. Sociodemographic and clinical variables, as well as program outcomes, were analyzed using descriptive statistics. Odds ratios and their confidence intervals were calculated. Results: In 1016 patients between 35 and 41 years of age, a frequency of 61.3 % of non-adherence to at least six prenatal visits was found. History of abortion (adjusted OR: 0.46; 95 % CI 0.33-0.64) and pregnancy of five or more (adjusted OR: 3,22; IC 95 % 1,50-6,91) were the associated factors. Conclusions: Non-adherence to prenatal care by pregnant women of 35 years or more affiliated to the Cauca EAPB AIC-I is high. Further qualitative studies are needed to examine the cultural and social factors present in these communities that may affect adherence to prenatal monitoring, together with prospective studies to confirm the exploratory analysis of factors associated with non-adherence. Insurance organizations in the subsidized regime should be called upon to conduct promotion activities in these communities in order to improve adherence. [ABSTRACT FROM AUTHOR]

- Abstract
The COVID-19 pandemic starts in Peru in March 2020. It appears that progression to severe forms of the disease in pregnant women is infrequent. The most frequent complications are premature rupture of membranes, premature delivery and fetal distress. Vertical transmission has not been demonstrated. National studies are mostly only from the first stage of the pandemic. Objective: determine the epidemiological, obstetric and perinatal characteristics in pregnant women with SARSCoV-2. Material and methods: A descriptive study was conducted at the Hospital Nacional Dos de Mayo. A total of 579 SARS-CoV-2 positive pregnant women and 533 newborns were admitted. Medical records were reviewed. Descriptive statistics were performed. The study is registered in PRISA with code EI00002469. Results: SARS-CoV-2 was detected by serological antibodies in 91.0% and in 8.0% by antigenic test. Of the cases, 94.5% (547/579) remained asymptomatic. Nineteen cases (3.3%) were complicated by COVID-19 pneumonia. There were no obstetric complications in 65.5%; the most common were premature rupture of membranes (9,7%), preeclampsia (8,2%) and urinary tract infection (5,0%). Neonatal characteristics such as gestational age, birth weight and Apgar were mostly within normal limits. Conclusion: that pregnant women with SARSCoV-2 are mostly asymptomatic, have a low frequency of complications and the newborns do not present major complications. [ABSTRACT FROM AUTHOR]

14. Factores epidemiológicos asociados a recurrencia de infecciones de vías urinarias bajas en mujeres gestantes.

Background: The urinary tract infections in pregnant women are the second most frequent medical pathology in pregnancy, presenting an incidence of 5-10%. In Mexico, according to the Mexican Clinical Practice Guide, the incidence is 2%, and establishes a recurrence of 23% of them. Objective: To determine the associated epidemiologic factors with recurring infections in lower urinary tract in pregnant women during the period of May 2019 through May 2020. Material and methods: Analytical observational crosssectional and retrospective research study, where prenatal control sheets of clinical records were reviewed from the institutional program to identify frequency of recurrence and epidemiologic factors associated with lower urinary tract infections in 90 pregnant women; the variables were analyzed through descriptive statistics and analytical statistics. Results: Of a total of 90 electronic records of pregnant women, a recurrence of lower urinary tract infections was found in 41 patients (45.6%). Conclusions: The prevalence of recurring lower urinary tract infections during pregnancy in our population is alarming in whom no association was found with the presence of the epidemiologic factors studied in the present investigation; however, a tendency of recurrence urinary tract infections was identified in women that are not working, superior level of education, 2 pregnancies or less, patients under 20 years of age and in those who were treated with nitrofurantoin for a period of time less than 7 days. Introduction: The periodontal disease as risk factor of preterm births and low birth weight, as well as the existence of efficient multifactorial mathematical models to predict the intensity of this risk, open new perspectives of prevention for both entities. Objective: To determine the current state and metaanalysis of the risk relationship between the periodontal disease, preterm births and low birth weight. Methods: A literature review and a metaanalysis were carried out with search in databases MEDLINE, PUBMED, SciELO, LILACS, BBO and Cochrane of studies from 1996 to 2020, in humans, in English and/or Spanish languages. Results: Two thousand 323 articles were found; 1 429 were excluded because they were duplicated and in other languages, 742 had nothing to do with the thematic of risk action, 105 belonged to other types of studies, 32 did not meet CASPe and Newcastle - Otawa evaluation criteria and just 15 conformed the sample of metaanalysis. More than 50 % of these articles show risk association between the periodontal disease and the adverse results of pregnancy, but risk prediction is not carried out in none of them. There is a great heterogeneity in the studies regarding methodological limitations and form of measuring the periodontal disease, result of the studied pregnancy and evaluation of the gestational age. Objective: To evaluate if there is association between delay II in maternal care and complications of Pregnancy-induced hypertension (PIH) at the Alberto Sabogal Hospital in the context of the Covid19 pandemic. Material and methods: Case-control study carried out in a reference hospital in the Callao region, Peru. The study included women aged 19 or over, treated for emergency caesarean section attended in the obstetric areas COVID-19 and non-COVID-19 in the months of January to April 2021. Pregnant women with pre-existing diseases, as well as those with other obstetric complication were excluded. Patients with complications of pregnancy-induced hypertension (PIH) were included as cases, and those without these complications as controls. Age, origin, occupation, type of delay and type of complication of PIH were measured. A questionnaire was used according to the Thadeus and Mayne model and documentary review. The association between delay II and complications of PIH was evaluated through the odds ratio (OR). Results: It was found that the cases had a higher risk of having been exposed to type II delays than the controls (OR: 7.72; 95% CI: 3.44-17.35). There was an association between delay II and hypertensive complications. It was identified that delay II was associated with the origin of the patient. Conclusion: In the period of the Covid-19 pandemic, women who had complications from PIH were more exposed to type II delays than controls at the Alberto Sabogal hospital in Callao, Peru. Prospective studies are required to confirm this association and the effects of congestion of hospital services by patients with covid-19 on other maternal-perinatal outcomes in the region. Background: Studies carried out inside and outside of Cuba show that there is a high impact on the health of the product of conception of mothers with extremely serious maternal morbidity. An analysis of this problem could be reverted to adequate medical care and the achievement of excellence perinatal results. Objective: to characterize patients with extremely severe maternal morbidity and its perinatal repercussion. Methods: descriptive study, of a series of cases, carried out at the Dr. Gustavo Aldereguía Lima Hospital, in Cienfuegos. The series consisted of 145 patients with extremely severe maternal morbidity, and their products of conception, treated during the period 2016-2018. Type of delivery, causes of admission, maternal and neonatal morbidity, perinatal mortality, among other variables were analyzed. Results: patients without comorbidity, non-nulliparous and aged between 20 and 34 years predominated. The most frequent causes of admission were hypertensive disease of pregnancy (37.9%) and major obstetric hemorrhage (30.3%); as well as in neonatal therapies, they were the respiratory distress syndrome (42.5%) and neonatal asphyxia (400%). In addition, the ratio of one patient with extremely severe maternal morbidity for every 87.4 births was observed; one maternal death for every 72.5 patients; and one newborn with neonatal morbidity every 3.6 patients. Conclusion: extremely severe maternal morbidity is a condition with a high impact on perinatal morbidity and mortality. [ABSTRACT FROM AUTHOR]

Background: Preeclampsia is the main cause of morbidity, mortality, maternal and perinatal disability, making its diagnosis and prevention important. Objective: to validate the predictive value of the angiogenic test (sFlt/PIGF ratio) in hypertensive disorders of pregnancy. Methods: a descriptive cross-sectional study was carried out at the Gynecology and Obstetrics Service, Dr. Gustavo Aldereguía Lima Hospital, Cienfuegos, from January 2017 to December 2018. 125 pregnant women with suspected pre-eclampsia, from the Total number of patients admitted with high blood pressure were studied. They were divided into two groups according to the diagnostic values of the Roche angiogenic test (sFlt/PIGF ratio), and perinatal results and maternal complications were compared. The Chi square statistic (p <0.05) was used and the risk ratio (95% CI) was calculated. Results: the angiogenic test was positive more frequently in those pregnant women who developed eclampsia preeclampsia, with 90.7% vs. 9.3% in those who did not develop eclampsia preeclampsia (p = 0.000); as well as those less than 34 weeks of gestational age who developed pre-eclampsia (72%), with high sensitivity and specificity. The use of the test in the follow-up of the patients was associated with a tendency to prolong the pregnancy. Its positivity was more related to maternal, fetal and neonatal complications. Conclusion: a positive value of the test implies an increased risk of developing preeclampsia eclampsia and maternal complications, as well as adverse perinatal results. - Abstract
Background: In women of reproductive age, pregestational overweight and/or obesity could result in negative outcomes for both, mother and child. Objective: To investigate the association between pregestational body mass index (PBMI) and maternal adverse outcomes (MAO) during childbirth, in women assisted during 2014, in a public hospital in Mérida, Yucatán, Mexico. Material and methods: 427 pregnant women assisted during childbirth were consecutively included, identifying among them those who did not have any MAO (n = 137), whose demographic and clinical data, including PBMI, were compared with those who did have at least one MAO (n = 290). Odds ratios (OR) and 95% confidence intervals (95% CI) were obtained using a binary logistic regression model (LRM). Results: 33% had normal PBMI, and 37% had obesity. Urgent cesarean section was the most frequent MAO (58.8%). According to the LRM, schooling (secondary school level, OR: 0.56; 95% CI: 0.34-0.94), primigravidae (OR: 6.88; 95% CI: 3.37-12.58), and secondigravidae women (OR: 5.57; 95% CI: 3.04-10.24), the number of prenatal visits (= 5, OR: 3.49; 95% CI: 1.54-7.91), and PBMI (overweight, OR: 0.41: 95% CI: 0.23-0.72) were identified as clinically and statistically significant. Conclusions: Women with overweight were at low risk for MAO, while schooling (secondary school level) reduced risk by 44%, being a primigravidae or a secondigravidae increased risk more than 6 and 5 times, respectively. 34. Complicaciones obstétricas del dengue y la chikungunya en la paciente embarazada: estudio de casos y controles.

Background: Dengue and chikungunya infections are transmitted by the Aedes aegypti vector; they can affect the pregnant woman and her gestation product. Objective: To identify if dengue and chikungunya infections during pregnancy are factors associated with the presence of obstetric complications, in Colima, Mexico. Methods: We included in this study 45 patients with obstetric complications and 90 without complications, selected by a two-stage sampling. We collected the sociodemographic, anthropometric, gynecological, and obstetric complications' data. Descriptive statistics, nonparametric significance tests and crude and adjusted risk estimates (odds ratio [OR] and 95% confidence intervals [95% CI]) were calculated. Results: Out of 135 patients included, 15 had a diagnosis of dengue and 20 of chikungunya. There were obstetric complications in 60% of infected patients with chikungunya, 46.7% with dengue and 26% of non-infected (p = 0.007). Chikungunya infection (adjusted OR = 1.6) and antecedents of abortion and cesarean section (adjusted OR = 2.2) were independent risk factors associated with the presence of complications. Dengue had an adjusted OR = 1.3 without a statistically significant difference. Conclusions: Chikungunya infection and antecedents of abortion and cesarean section are risk factors associated with obstetric complications. 35. Resultados maternos y perinatales del tratamiento expectante de la preeclampsia severa.

Introduction: In severe preeclampsia (SP), pregnancy interruption is the first recommendation. However, some patients receive expectant treatment. Objective: To determine maternal and perinatal results of expectant treatment of SP in the intensive care unit (ICU) of a high-specialty hospital. Methods: Observational, descriptive and retrospective study; the files of 40 pregnant patients with SP managed in the ICU with expectant treatment were reviewed. The prolongation of the pregnancy, the maternal and perinatal complications, the stay in ICU and in the hospital were recorded. Descriptive statistics were used for statistical analysis. Results: Mean maternal age was 30.2 ± 5.04 years and gestational age 30.02 ± 3.18 weeks. Gestation was prolonged 7.5 ± 0.95 days. Maternal complications occurred in 60% (24 cases): thrombocytopenia 48.9%, HELLP syndrome 17.8%, pulmonary edema 4.45%, acute kidney injury 4.45%, deterioration of chronic kidney disease 4.45%, oligohydramnios 4.45%, uterine hemorrhage 4.45%, platelet transfusion 4.45%, eclampsia 2.2%, intravascular coagulation 2.2%, surgical reintervention 2.2%, without mortality. ICU stay was 3.42 ± 1.85 days and hospital stay 8.8 ± 4.82 days. 41 premature newborns were treated (100%) and complications were reported in 74.20% (23 cases): respiratory insufficiency 53.65%, neonatal intensive care 39.02%, growth restriction 21.95%, unreliable fetal status 9.75% and mortality 24.39%. Intensive care stay was 8.5 ± 5.47 days and hospital stay 26.8 ± 23.2 days. Conclusions: Prolongation of pregnancy was similar to what has been reported in previous studies. Due to the high percentage of maternal and perinatal complications, expectant treatment is not recommended. [ABSTRACT FROM AUTHOR]

Foundation: Obstetric hysterectomy is a therapeutic alternative on severe processes (hemorrhagic and/or septic) which if not solved would directly influenced in maternal morbi/mortality. Knowing about the topic allows to proceed rationally and coherently on risky situations. Objective: to charaterize the patients, series of cases who needed urgent obstetric hysterectomies in Cienfuegos 2013-2015. Methods: a descriptive study was developed, a series of cases of all the patients who were performed an obstetric hysterectomy (N=37) in the Hospital Dr. Gustavo Aldereguía Lima. Cienfuegos, from 2013 to 2015. The variables analyzed were patient age, gestational age, parity, events related to pregnancy, delivery, kind of birth and type of obstetric hysterectomy. Results: the group of patients between 31-35 years old was the most numerous, representing 40,5 % of the total. The 73% of the hysterectomized women had previous deliveries and 10 (27 %) patients were nulliparous. Uterine atony was de most frequent cause (45,9 %), mainly in the 31-35 year group (41,2 %), followed by uterine rupture and placenta accreta. The total hysterectomy without ophorectomy was the most used type(43,3 %). Conclusion: in general pregnancies arrived to the term and the atony, as in many of similar studies was the predominating indication. It was corroborated that birth by cesarean section increases the relative risk to perform a hysterectomy. [ABSTRACT FROM AUTHOR]

Objective: to identify nutritional and cultural risk factors associated with anemia in pregnant women in Somotillo. Method: study of nested cases and controls, with pregnant women between August and November of 2016 in health centers. Factors were identified by the calculation of odds ratio through the statistical program SPSS version 22. Results: the majority of pregnant women are between 14 and 24 years of age, with a marital status of free union, university academic level, occupation of housewives, with catholic religion and urban origin, with a preconceptional antecedent of the BMI greater than 23. Conclusion: it was concluded that of the nutritional variables that behaved as a risk factor was not to take supplements of folic acid and iron, and the variable of cultural factors that behaved as a risk factor was the inappropriate consumption of tacos, pozol, Corn chicha as usual food. - Abstract
Background: The aim of this work is to describe the causes of the obstetric emergency, which motivates the activation of the Immediate Response Team at a second level hospital of the Instituto Mexicano del Seguro Social, which has not an Intensive Care Unit for Adult Patients. Methods: Retrospective observational study of the causes of obstetric emergency that occurred in the domains of Admission, Hospitalization, Labor, Delivery Room, Operating Room and Recovery of the Hospital General de Zona 53 of the IMSS, during the years 2014 and 2015. Results: In 2014, were attended 3915 women due to obstetric cause; of them, 75 cases were of obstetric emergency and 3276 live births. In 2015, were 4390 women with 3842 live births and 80 cases of obstetric emergency. The main domains where performed the Immediate Response Team were: Labor, Admission and Recovery. In 2014, preeclampsia-eclampsia-Hellp ranked first as a cause of obstetric emergency: 42 cases, also in 2015: 36 cases; followed by obstetric hemorrhage with 28 and 34 cases, respectively. In 2014, there was a maternal death from severe preeclampsia and extensive liver rupture. Conclusions: Consolidation of Immediate Response Team at a hospital, according to the healthcare environment, it is essential, above all, within the meaning given by staff members. - Abstract
Objective: To describe the epidemiological characteristics of teenage pregnancy in the Regional Hospital of Coronel Oviedo, Paraguay. Material and Methods: Descriptive, observational cross-sectional study with consecutive non-probability sampling of cases, review of medical records of pregnant teenagers who attended the Regional Hospital of Coronel Oviedo, Paraguay between january and december 2011. Results: 167 medical records of pregnant adolescents were included, which accounted for 40.7% of all births attended in 2011. The age range was between 13 and 19 years, with an average age of 17.47 ± 1.42. 64.1% were of urban origin. 86.8% did not use contraception and 50.9% had a free union. The observed complications were: premature rupture of membrane at 10.2%, urinary tract infection at 8.4% and fetal death in 0.6% of cases. Conclusion: The prevalence of teenage pregnancy in this study was 40.7%; most women did not use any contraceptive method. The most common complication in this type of pregnancy was premature rupture of membrane. [ABSTRACT FROM AUTHOR]

- Abstract
Background: Maternal morbidity is a health problem for developing countries, the causes of death among obstetric patients varies according to geographic region and other factors, which include health services. The aim of this study was to identify the causes of maternal mortality and morbidity, as well as factors associated with mortality in patients attending on 2011-2012. Methods: Clinical data of the patients on admission, severity of illness, presence of comorbidity, complications that occurred during treatment and causes of death in pregnant or postpartum patients were analyzed. Results: 137 patients were analyzed, 87 (63.5 %) patients were hospitalized, 36 (41.3 %) cases with severe maternal complication (SMC); most patients with SMC were in the postpartum postcesarean. Eleven patients died, five maternal deaths and 6 late deaths. Conclusions: SMC and death were more frequent in the postpartum period. The indirect causes of death were more frequent. - Abstract
Background: stillbirth, also called late fetal death if it occurs between 28 weeks' gestation and birth, is one of the most feared complications of pregnancy. The late fetal mortality rate is an indicator of scientific progress and social development of a country. Objective: to describe the factors associated with late fetal death. Methods: a study of a series of 221 cases of late fetal deaths was conducted in Cienfuegos province from January 2006 to December 2013. The variables studied included: time and place of death, cause of death, maternal age, maternal weight, number of pregnancies, maternal diseases, pregnancy complications, gestational age and placental and umbilical cord abnormalities. Results: late fetal death rate was 6.38. Fetal deaths from unknown cause predominated, followed by those associated with hypertensive disorders of pregnancy and intrauterine growth retardation. The largest number of fetal deaths occurred antepartum, before term, outside the hospital, in obese pregnant women, in women with hypertensive disorders and diabetes, and in malformed fetuses. Placental insufficiency and obliterating angiopathy were the most common placental abnormalities. Conclusion: despite the advances in obstetrics, the late fetal death rate in Cienfuegos province remains high, similar to what have been reflected in other Cuban studies. [ABSTRACT FROM AUTHOR]

- Abstract
Objective: To determine if there is a difference in safety using expectant or active management for patients with less than 34 weeks of gestation and severe preeclampsia, in terms of maternal and foetal outcomes. Materials and methods: Historic cohort of pregnant women between 24 and 34 weeks of gestation diagnosed with severe preeclampsia, managed in accordance with an expectant or an active protocol after a course of steroids, based on the judgment of the team in charge at a referral teaching hospital in the southwest of Colombia. Consecutive sampling. The primary endpoints measured were maternal mortality and complications, and a composite measurement of poor maternal outcomes. Secondary endpoints measured were neonatal mortality, length of stay in the neonatal intensive care unit (NICU) and hours of mechanical ventilation, as well as a composite measurement of neonatal complications. Baseline and clinical characteristics of the two populations were measured, as well as neonatal outcomes, using the exact Fisher test or the chi square test for categorical variables, and non-parametric tests for continuous variables. Results: The groups were comparable in terms of their baseline characteristics. Pregnancy prolongation with expectant management was in average 8.8 days ± 8.74 days SD. There were no statistical differences between the two protocols regarding maternal or perinatal morbidity and mortality during the postpartum period, neonatal length of stay, or mechanical ventilation hours (p > 0.05). Conclusion: No advantages were found in terms of safety maternal and foetal outcomes when comparing expectant and active management of patients with early severe preeclampsia. [ABSTRACT FROM AUTHOR]
