- Abstract
Early, adequate antibiotic treatment of urinary tract infection (UTI) in young children alleviates acute symptoms and may also limit long-term sequelae. Antibiotics should ideally be prescribed only to those. As, there is no rapid point of care for a UTI using an antibiotic with the narrowest effective spectrum. Empirical treatment is universal in primary care, given that urine culture results take several days. As yet, there are no rapid point-of-care tests that give a sufficiently robust indication of an etiology and the susceptibility of infecting organisms. One of the complications during an acute coronary syndrome event is the presence of arrhythmias. Among them, those of the supraventricular type, especially atrial fibrillation, carry a poor prognosis both in the short and long term, being the cause of situations such as cerebrovascular event, ventricular arrhythmias, and increased mortality. The arrhythmia tends to appear in a certain population group with particular risk factors during the index event in approximately 10% of cases. Appropriate treatment at the time of its onset, thanks to the use of drugs that modulate heart rate, rhythm, and anticoagulant management in the most vulnerable groups, will lead to a less bleak outcome for these patients. Payment for ecosystem services (PES) is an economic tool that seeks to compensate landowners for the conservation of their forested lands and reduce the negative impacts of land use change in forest areas that are important for the provision of ecosystem services (ES). However, it is essential to verify that the implementation of this mechanism is focused on where it is most needed. In this study, an analysis of the evolution of PES in the Pixquiac sub-basin in Veracruz was carried out, and the degree of coincidence between the supported areas and hydrological priority areas was evaluated. Information regarding the fiscal years of PES in the region from 2003 - 2016 was statistically analyzed, and the priority areas and areas of PES were determined using geographic information systems. Results showed that payments amounted to USD 1,076,782 in an area of 5,450 ha during the period analyzed. However, only 881.58 ha (24.5 % of the total hydrological priority areas) were considered in this payment scheme. Decision-makers must have up-to-date information that allows them to focus available resources at a more precise spatial scale. In addition, efforts must be made by the actors involved to ensure greater participation by forest owners and ecosystem service users. [ABSTRACT FROM AUTHOR]

7. Aciduria glutárica tipo I: reporte de un caso con diagnóstico tardío.

- Abstract
BACKGROUND: Glutaric aciduria type I (AG-I) is an inborn error of metabolism caused by variants in GCDH, this affects the function of the enzyme glutaryl-CoA dehydrogenase and produces accumulation of neurotoxic metabolites, patients with this disfunction are characterized with striatal lesions and movement disorders (dystonia, choreoathetosis). CASE REPORT: The patient is a 3-year-old Mexican male with a history of an encephalopathic crisis at 10 months old, after this episode he suffered development regression, hypotonia and movement disorder, he has also other pathological antecedents: laryngomalacia, recurrent hordeolum, chronic productive cough, diarrhea-constipation, generalized hyperhidrosis and multiple hospitalizations for acute respiratory infections. MRI reported expansion in the frontotemporal subarachnoid space and the Sylvian fissures, also bilateral atrophy, T2/FLAIR hyperintensity and restricted diffusion in the basal ganglia; organic acids analysis showed increased glutaric acid levels; GCDH sequencing reported two pathogenic variants: p.Arg402Trp and p.Thr429Met. CONCLUSIONS: This is a report of a late diagnosed AG-I case with two pathogenic variants in GCDH, previously unreported in the Mexican population. Early AG-I treatment radically changes the prognosis; however, it depends on screening methods for diagnosis that are not widely available in some populations. [ABSTRACT FROM AUTHOR]

The article analyzes the perception of science and scientific culture in Cuban society, highlighting the lack of scientific knowledge and scientific culture. Surveys were conducted with students of Agronomy, Medicine, and teachers from middle schools and physics, revealing a low level of knowledge and understanding of scientific concepts, especially in physics. The results also show that less than 50% of teachers recognize the scientific method as the main research method in the sciences, and that there is little knowledge of the history of physics and the term "epistemology". With the objective of evaluating the phenotypic diversity of Cuban rice cultivars obtained by INCA, supported by morphoagronomic characters and multivariate analysis techniques, this work was developed in Scientific and Technological Base Unit belonging to the National Agricultural Sciences Institute of Cuba. 30 qualitative and 23 quantitative characters were evaluated and data processed through a multiple correlation analysis and multivariate analysis of automatic classification (clusters). The results showed that 11 characters qualitative were homogeneous and 10 were distinctive of some cultivars, within them, the color of the ligule, the leaf blade, node, internode, subnodal ring, stigma, as well as, the position of the apex of the first leaf below the flag leaf, lemma, palea and leaf pubescence, and panicle exertion. There is a positive and significant correlation between yield and grains per panicle; also, between mesocotile and coleoptile length with industrial yield and tillering capacity, respectively. Variation ranges are appreciated in all the quantitative traits evaluated and although the cultivars obtained are genetically related, they differ phenotypically. [ABSTRACT FROM AUTHOR]

Intelectual destacado e impulsor infatigable de varios de los proyectos culturales más importantes en México y América Latina durante el siglo XX, Jaime García Terrés es un personaje fundamental en el ámbito universitario. Su labor como dedicado editor, funcionario, gestor y promotor de la cultura en esta institución hacen de él una figura insustituible de la historia de la Universidad Nacional Autónoma de México. Sin duda, el músculo anímico que mantuvo vivas y en equilibrio todas esas facetas de su vida profesional fue la poesía. Y el del poeta fue un camino que diariamente vislumbraba con pasión en medio de las arduas faenas burocráticas y la trivialidad dialógica de la vida cotidiana: se trataba -en sus palabras- de'un ritual a ciegas'. En el marco del centenario de su nacimiento, este libro invita a (re) valorar la obra de Jaime García Terrés a partir de una aproximación a su poética, a su visión sobre el oficio y a algunas reflexiones sobre la recepción de su trabajo, todo como una antesala a sendas lecturas de tres poemas singulares: uno que imagina los inicios de la tradición lírica novohispana, otro que intenta fijar una experiencia psicodélica personalísima y, finalmente, uno más en que el autor de este volumen -en uso de una licencia menos poética que teatral- asume la voz fantasma del poeta para presentarnos una'Carta viviente'que expone, desde la ultratumba literaria, una última voluntad.

This review focuses on the technique known by the name of cavitation for wastewater treatment. There are several ways to make a fluid cavitate; the focus of this document is hydrodynamic cavitation and acoustic cavitation. Cavitation occurs when a fluid abruptly changes phase due to a variation in its pressure: the cavities have a considerable amount of energy that favors reactions inside and at the interface of each bubble or cavity that forms within the liquid, as well as in the fluid. This treatment technique is favorable since does not imply the use or dosage of chemical inputs to reduce the concentration of contaminants in the water. The sources consulted indicate that to achieve greater effectiveness of the cavitation process, it is necessary to intensify the process by the simultaneous implementation of other treatment techniques, including advanced oxidation processes, electrochemical processes, and water clarification. The combinations of cavitation with other wastewater treatment techniques have been studied for different recalcitrant pollutants, achieving very favorable results. Se busca analizar este concepto desde distintas variantes, poniendo el foco en nuestro territorio y en las universidades. Catorces textos inéditos se intercalan con seis entrevistas a jóvenes innovadores latinoamericanos. Una pieza en construcción de nuestra historia que busca explorar las voces jóvenes, con sentido ciudadano, en esta conversación que también les pertenece.

Objective: To assess the relation between the minimal Apparent Diffusion Coefficient (ADC) and Gleason scale in prostate cancer suspicious patients. Materials and methods: Retrospective study which included 13 patients who underwent Prostate Multiparametric MRI (MP-MRI) before biopsy. Suspicious lesions were located in ADC maps. Quantitative measurements were performed using a size adjusted ROI. Minimal ADC diagnostic accuracy was assessed with Pearson coefficient. Results: Clinically significant prostate cancer (Gleason Score ≥ 7) was diagnosed in 7 cases. Non-significant prostate cancer was diagnosed in 6 cases. Minimal ADC values obtained in clinically significant cancer were 0.68x10-3 mm²/s. Median values in non significant prostate cancer were > 0.81x 10-3 mm²/s. Conclusions: Clinically significant prostate cancer showed lower ADC values compared with non significant cancer, therefore there is a negative correlation with Gleason Score values. This can contribute for an improvement in the diagnostic and terapeutic prostate cancer strategies. [ABSTRACT FROM AUTHOR]

¿Cuál es el rol del derecho y de la administración en la reconfiguración de las ciudades a escala mundial, en especial desde una noción como la de ciudad inteligente? A partir de este interrogante, este libro propone reflexionar sobre los nuevos conceptos y dispositivos tanto jurídicos como tecnológicos que repercuten en el derecho administrativo y en la administración pública. En particular, los ligados a las nuevas tecnologías, como la big data y la inteligencia artificial, y cómo ellos afectan la participación política del ciudadano en el gobierno abierto, la movilidad, la integración social de grupos excluidos por género o discapacidad, las ciudades ecosostenibles y las dinámicas de la administración local y la propia definición de ciudad. Las preguntas acá planteadas representan los primeros riesgos jurídicos identificados que puede conllevar la implementación de “ciudad inteligente”. Y allí es donde el derecho administrativo debe comenzar a mutar en sus conceptos para prevenir jurídica y técnicamente la construcción de grupos sociales desiguales desde la falsa idea de la neutralidad tecnológica y la eficiencia económica o energética.

Mexican peach production [Prunus persica (L.) Batsch] faces an economic crisis, as a result of the gradual decrease the area planted, production, prices, and national consumption per capita. Therefore, the producers have a low income. The objective of the study was to assess whether or not organic peach production would contribute to the improvement of the income of producers in the Estado de Mexico. The economic viability of turning the conventional production system into the organic production system was evaluated using the real options theory, while the abandonment option was assessed through the binomial option pricing model. In contrast to traditional valuation methods, the real options theory considers flexibility in decisions when the profitability of investments faces volatility and uncertainty. The quantified value of the crop by-products and residues was added to the farmer’s final income to find out if the amount would help to improve his income, during and after the conversion process. Given the positive value of the options, investing in organic production is feasible, even when the project abandonment option was considered. Introduction: Globally, mangroves are declining; characterizing their structure and regeneration at different disturbance levels can help understand their responses to stressful situations. Objective: The study's primary goal was to analyze the mangrove community structure and to estimate the populations' trajectories of Avicenia germinans at different mangrove sites that present three levels of disturbance. Methods: Eight transects of approximately 500 m in length and laid perpendicular to the Tamapamchoco lagoon, Veracruz, intersected three levels of disturbance currently present on the mangrove forest. On each transect, 10 x 10 m, 5 x 5 m, and 1 x 1 m quadrants served to account for all mangrove trees by species and stage of maturity in 2017 and 2018. Results: The most abundant species was: A. germinans followed by Rhizophora mangle and Laguncularia racemosa. Based on their abundance, we found significant differences for the three levels of disturbance (ANOSIM R = 0.6, P < 0.001) presented in a non-metric ordination (nMDS). A life table for A. germinans allowed a population analysis that showed an intrinsic rate of population increase (r) that was negative at the disturbed sites (-0.0027 and -0.0774) and positive (0.0289) at the apparently undisturbed site. Seedling to juvenile stage survival ranged from 50 % at the undisturbed site to 5 and 4 % at the disturbed sites. Conclusions: Measures to increase the survival of seedlings are necessary to reverse the decreasing population trajectories at the disturbed sites; otherwise, the mangrove will have reduced viability in the medium term. El Manual de electrocardiografía básica es una herramienta práctica que incluye los aspectos más importantes a tener en cuenta en la interpretación del electrocardiograma. En él, los estudiantes de medicina, médicos generales y especialistas de diferentes áreas pueden adquirir una visión práctica de la electrografía. Se buscó la manera más sencilla y adecuada de entender las principales manifestaciones eléctricas que pueden detectarse en el corazón, de modo que se tenga una aproximación diagnóstica precisa de las principales patologías cardiovasculares. Se recopiló información pertinente y veraz de diferentes bibliografías acreditadas, para luego consolidarlas en una obra, como guía teórica de entendimiento básico asociado a un texto práctico y adaptado mediante términos sencillos y fáciles de comprender. Este texto es el resultado de la investigación, el rigor científico y la experticia que da la atención diaria de los pacientes, en busca de una experiencia más amena y de rápida comprensión para quienes se encuentran en el proceso de formación médica o carreras afines, trabajan profesionalmente en el área de la salud o requieren una profundización en el tema.

Objective: To express the clinical foundations for the therapeutic choice in a case that presents the permanent canines of the maxilla that have erupted ectopically from the vestibular, and its therapeutic possibilities. Method: The case of a 17-year-old male patient with ectopic canines is presented. The bracket system used is passive self-ligating. The treatment lasted 41 months. The extractions were made and the appliances were placed using 0.014" NiTi archwires. Results: The active treatment lasted 41 months, the canines were involved in the archwire with 5/16" night-wear elastics of 2.5 oz. A stable occlusion is obtained, and an upper and lower Hawley plate retainer is used. Conclusion: The decision-making of the treatment plan for a particular case should be based on the needs and limitations of the patient. The decision to extract a tooth should be supported with scientific information, hoping for a long-term stability of the results. Each tooth has a specific function in the craniofacial system, since its anatomy is designed according to the place it occupies in the arches. [ABSTRACT FROM AUTHOR]

The structure and the trend of the fiscal system reflect the orientation of the development model of Mexico in past decades and they determine its future model and its ability to meet the main social challenges (development, equity, wellbeing, environmental sustainability, etc.). The objective of this paper is to study the relative dimension, the tax structure and the trends in tax collection, as well as the nature and the objectives of the incentives and the tax benefits that have been enforced in Mexico in recent years. This exercise allows an evaluation of the 2014 tax reform to be made with the perspective of the past six years and the formulation of future challenges. A descriptive and qualitative approach was used based on data obtained from the Inter-American Center of Tax Administration (ciat), the Organisation for Economic Co-operation and Development (oecd), and the Secretaría de Hacienda y Crédito Público (shcp). The structure of tax collection is analyzed, with a focus on the main federal taxes, as well as the volume and the composition of tax expenditure, by breaking down the existing incentives and the tax benefits on each of the taxes. The main results indicate a passive evolution in the collection power that grew from 12.5% of gross domestic product (gdp) in 1990 to 15.6% in 2019, principally based on consumer taxes, wage income tax and a strong dependence on the fossil fuels sector. This shows that some of the stated objectives of the 2014 reform--significantly increasing collection capacity and reducing the jungle of incentives and tax benefits--have hardly been met. In conclusion, some of the challenges that a tax reform should face in terms of inclusion, equity and sustainability are formulated. [ABSTRACT FROM AUTHOR]

This article is the product of research supported by a data analysis methodology. The main objective is to study the theoretical position of the Mexican Supreme Court of Justice and the Judicial Power of the Federation before and after issuing the new Amparo Law regarding the applicability of the principle of non-retroactivity in the case of jurisprudence (the system of precedents in Mexico). The main findings are that even when the traditional doctrine of the Supreme Court of Justice was surpassed by the Legislative Power, by establishing the Law of Amparo the guarantee for the governed within the scope of judicial precedents, the proper development of the judicial doctrine in the jurisprudence of the Judicial Power has been reluctant to achieve a consistent position on the subject. Transfusion-Related Acute Lung Injury (TRALI) is a relatively unusual, life threatening clinical syndrome, characterized by acute respiratory failure, hypotension, and non-cardiogenic pulmonary edema during or within 6 hours after a blood product transfusion. Although its true incidence is unknown, it has been attributed one case per 5.000 transfusions of any blood product and has been one of the most frequent causes of transfusion-related death. We present a case of TRALI in the perioperative period of cardiac surgery with extreme clinical manifestations, whose therapeutic approach was extremely difficult for the medical surgical team, due to its complex clinical setting: cardiac surgery with cardiopulmonary bypass due to diagnosis of infective endocarditis, previous lung injury and history of other post-transfusion reaction.
