The third cause of morbidity and mortality in many countries is cerebrovascular accident (CVA), the incidence is higher in hemodialysis patients. The presence of hemorrhagic stroke is associated with worse results; early diagnosis and treatment are essential due to its rapid expansion, which causes neurological detriment, up to a fatal outcome. The case of a 52-year-old woman is presented, admitted for emergency service on 05/23/23 with a history of three-weekly renal replacement therapy, arterial hypertension, diabetes mellitus; clinical picture with sudden headache of moderate intensity, accompanied by drowsiness, numbness at the level of the cervicodorsal spine, paraparesis in the lower extremities and hypertension; skull tomography with pre-brain stem hemorrhagic stroke; he was admitted to the Intensive Care Unit (ICU) for monitoring and management, however despite the treatment he evolved torpidly and died on 06/11/2023. Objective: To describe the epidemiologic profile of patients with cerebrovascular accidents (CVA) attended at a rehabilitation center in Lima-Peru. Methods: A cross-sectional study was carried-out at Instituto Nacional de Rehabilitación "Dra. Adriana Rebaza Flores" AMISTAD PERÚ-JAPÓN from 2013 to 2019. Information on sociodemographic, clinical, chronic underlying conditions and use of illicit drugs and medications were gathered. Results: 71.5% of our patients had ischemic CVAs and 87.6% had moderate to severe sequalae. Higher frequency of sequalae due to ischemic CVAs was found among people older than 56 years of age (80.7%), while sequalae of hemorrhagic CVAs was found predominantly above 46 years of age (61.8%). Hemiplegia was found in 81.7%; 58% had verbal communication problems; 2.1% had audition problems; 67.3% had blood hypertension and 16.1% had history of tobacco consumption. Most of the patients with any degree of sequalae were either married or cohabitant. The higher the severity of sequalae, higher percentage of old illiterate females. Conclusions: Most of the patients with CVAs had moderate-severe sequalae including hemiplegia, verbal communication problems and blood hypertension. Objective: To determine the incidence of stroke, overall and by sub-types, in Peru between 2017 and 2018. Materials and Methods: Analysis of hospital morbidity data obtained from SUSALUD (open data). Using the ICD-10 codes, the following were studied: subarachnoid hemorrhage (I60), atraumatic intracerebral hemorrhage (I61), cerebral infarction (I63), and unspecified stroke (I64). The crude and age-standardized incidence was calculated according to the population of the World Health Organization and using the national projected population number of people according to year, age and sex as the denominator. Results: In 2017, a total of 10,570 stroke cases were recorded, whereas, in 2018, there were 12,835 cases. Ischemic events were more frequent in both years. Regardless of stroke subtype and year, men were more affected than women. In the 35+ year-old population, an increase in the crude incidence of total stroke was observed between 2017 and 2018, from 80.9 to 96.7 per 100,000 person-years. The age-standardized incidence showed the same trend, but in a greater magnitude: from 93.9 to 109.8 per 100,000 person-years. Ischemic stroke was the one that increased the most, with an age-standardized rate in people aged 35+ years of 35.2 in 2017 and 46.3 per 100,000 person-years in 2018. Conclusions: The incidence of stroke is high in Peru. Ischemic cases are the most frequent and they disproportionately affect men. Our results suggest the need for a surveillance system to robustly quantify the incidence of these cases and understand their determinants. [ABSTRACT FROM AUTHOR]

Introduction: The stroke is the neurological damage that happens when one of the brain blood vessels is obstructed by a clot (embolism or clot) or it breaks (hemorrhages), which doesn't allow the brain blood flow. Objective: To characterize the patients with stroke assisted in the Emergencies Service of Dr. Joaquín Castillo Duany Teaching Clinical Surgical Hospital in Santiago de Cuba. Methods: A descriptive and cross-sectional study of 122 patients with stroke assisted in the mentioned Service from October, 2016 to the same month in 2017 was carried out. Demographic variables of interest related to the disease were taken into account and the percentage as summary measure was used. Results: The female sex (58.2%) and 61-70 age group prevailed in the series. It was demonstrated that 26.2% of those affected stayed more than 6 hours in the Emergencies Service and it was found an association of this disease with hypertension, diabetes mellitus, ischemic heart disease, nicotine addiction, alcoholism and hyperlipemia; equally, the inadequate use of the pharmacological treatment and the mechanic artificial ventilation caused a high lethality rate. Conclusions: In spite of the studies carried out in this same institution, it is important to continue deepening in the appropriate control and follow up of these accidents, as well as to develop strategies that allow training and preparation of the medical and paramedic staff of that hospital. BACKGROUND: A positive association between hyperglycemia and impaired prognosis has been described in patients with stroke. OBJECTIVE: To evaluate whether hyperglycemia has any association with increased mortality rates or prognosis in patients with stroke. MATERIAL AND METHOD: A retrospective cohort multicenter study was performed. All patients with stroke diagnosis were included. Serum glycaemia levels were recorded upon admission to the emergency unit. The diagnosis of stroke was confirmed by neuroimaging. In-hospital mortality and its association with hyperglycemia at admission were evaluated. RESULTS: A total of 308 patients were included, 52% patients had hyperglycemia at admission, of those patients 36% were diagnosed as hemorrhagic stroke and 64% as ischemic. When comparing patients with hyperglycemia vs normal serum glucose controls, significant differences were found in the prevalence of hypertension, diabetes mellitus and hypercholesterolemia between cohorts. On an unadjusted analysis an increased mortality rate was found in patients with hyperglycemia (11 vs 19%, RR 1.09); however, after adjusting for potential confounders, this association did not reach statistical significance (OR 2.05, IC95% 0.96-4.38, p = 0.06). CONCLUSION: Our study analysis revealed that hyperglycemia during admission does not seems to be directly associated with in-hospital mortality in patients with acute cerebrovascular accident. The article titled "Persistent Fever after Hospital Intervention: Nosocomial Endocarditis in Mitral Bioprosthesis" describes the case of an 80-year-old woman with arterial hypertension, persistent atrial fibrillation, and a mitral bioprosthesis, who was diagnosed with late prosthetic endocarditis. The patient presented with refractory fever, dyspnea, asthenia, and a systolic mitral murmur. Blood cultures were negative, but the echocardiogram revealed a mass on the mitral prosthesis. After the diagnosis, the patient suffered an embolism in both lower extremities and a hemorrhagic stroke without sequelae. During surgery, a coraliform vegetation was found on the bioprosthesis and a new prosthesis was implanted. Cultures were positive for Candida parapsilosis and a full cycle of antifungals was administered without complications. After six months, the patient is asymptomatic. Fungal endocarditis in bioprostheses is uncommon and may be related to the use of catheters and intracardiac devices. [Extracted from the article]
