Background. Flow cytometry (FCM) is a technique that allows multiparametric analysis of cell populations, and is essential in biomedical research and as a diagnostic tool. This fast technique has a high sensitivity, evaluating characteristics in the population of interest such as the size, granularity, and complexity of the cell cytoplasm and proteins that allow the phenotypic and functional classification of a large number of cells. For these reasons, this technique has gained importance in the diagnosis and monitoring of diseases and hematological abnormalities, such as leukemia, myelodysplastic syndromes, and myeloproliferative syndromes, among others. Objective. This review focuses on advances in the implementation of CMF in Minimal Residual Disease (MRD) present in Acute Lymphoblastic Leukemia (ALL), which is a minimal leukemic population that is detected in a patient after supplying an oncological treatment, where its efficacy, the risk of relapse and the complete remission process are evaluated. Methodology. A non-systematic literature review is carried out in databases, from the last 15 years, where the implications of the use of flow cytometry in EMR are evaluated, and relevant aspects are extracted from this review when using the CMF for the diagnosis and monitoring of patients with leukemia. Results. CMF is a very versatile and important technique for the diagnosis and monitoring of EMR due to its high sensitivity for the detection of low numbers of cells resistant to therapy. Additionally, the importance of standardizing protocols such as EUROFLOW for proper processing and clinical analysis of patient samples is shown. [ABSTRACT FROM AUTHOR]

Neonatal congenital leukemia manifests within the first 30 days of life and is exceedingly rare in this age group, but more prevalent among patients with Down syndrome. Its etiology is primarily genetic but is also linked to alcohol consumption, marijuana, and smoking. Differential diagnosis should be made with transient myeloproliferative disorder, a benign condition that resolves spontaneously. We present a clinical case of a full-term newborn, 16 days old, initially displaying symptoms suggestive of necrotizing enterocolitis. However, upon admission to the neonatal unit, the patient exhibited a greatly distended abdomen with severe hepatosplenomegaly, severe leukocytosis, anemia, and thrombocytopenia, accompanied by 100% blasts and laboratory findings consistent with tumor lysis syndrome. Management was initiated with hyperhydration, allopurinol, and rasburicase, but there was no improvement. Therefore, cytarabine was added. Flow cytometry indicated a megaloblastic pattern, indicative of myeloid leukemia. The patient required hospitalization for a month and a half, during which expected complications might arise. However, it was considered that the newborn experienced transient myeloid disorder, given the satisfactory clinical evolution, with reduced symptoms and severe leukocytosis. [ABSTRACT FROM AUTHOR]

Background/aim: Polycythemia Vera (PV) is a myeloproliferative disorder characterized by overproduction of morphologically normal red blood cells (RBCs), granulocytes, and platelets, a phenotype that is caused by a mutation (V617F) in Janus kinase 2 (JAK2). However, JAK2 V617F is also found in approximately 50% of patients with essential thrombocytosis and primary myelofibrosis, rendering its presence nonspecific as a diagnostic test. An increased red cell mass is a major criterion for the diagnosis of PV according to World Health Organization (WHO) 2016 criteria. High hemoglobin (Hgb) or Hematocrit (Hct) are universally used as indicators of an increased red cell mass for the diagnosis of PV. However, conditions such as iron deficiency (ID) with decreased mean cell volume may mask the diagnosis due to nonelevated Hct level. The aim of this study was to investigate the clinical characteristics of the patients with unclassifiable non-CML classical myeloproliferative disease with microcytosis (MPD/M) and nonelevated Hgb and Hct levels at diagnosis and to determine if some of these cases could be real PV cases masked due to ID-related microcytosis. Materials and methods: There were 23 MPD/M cases among 208 non-CML classical MPD cases (11%). Among 22 patients who had adequate test results related to the cause of microcytosis, ID and beta-thalassemia trait (TT) were the apparent causes of microcytosis in 15 and 1 cases, respectively. Results: Clinicopathological correlations revealed consistently positive JAK2 V617F mutation status (20/20, 100%), frequently elevated RBC count (17/23, 73.9%), and PV-compatible bone marrow findings (10/12, 83.3%). These findings are compatible with PV instead of essential thrombocytopenia or primary myelofibrosis. In spite of frequent cytoreductive treatment, 3 patients developed increased Hgb/Htc levels during median 58.2 (279-63) months' follow-up. Background: Myeloproliferative neoplasms can shorten the life of patients and affect severely their quality of life as a result of the severe constitutional symptoms mediated by cytokines resulting from massive splenomegaly characteristic of these diseases. The objective measurement of symptoms of myelofibrosis is essential for the presence of general constitutional symptoms and for assessing the outcome of treatment with JAK-2 inhibitors. Objective: To assess the impact on quality of life in patients with myeloproliferative neoplasms treated with the inhibitor of JAK-1 and JAK-2 ruxolitinib. Material and method: There was applied the questionnaire Myelofibrosis Symptom Assessment Form to 16 patients included in the Compassionate Use of Ruxolitinib Program: eight men and eight women were included with a median age of 63 years; four patients had primary myelofibrosis, three had postpolycythemia vera myelofibrosis and nine essential post-thrombocythemia myelofibrosis. The patients started with a dose of 20 mg ruxolitinib orally every 12 hours. Results: The questionnaire was administered to all patients at each visit. By comparing the differences of each parameter measured between the start and at 3, 6 and 12 months were statistically significant for all variables (p < 0.001, 95%). The most significant changes were observed when comparing data at baseline and follow-up to the maximum time; however, were obvious from the first quarter of observation on fatigue and symptoms of splenomegaly in most general symptoms, which disappeared completely after three months of treatment. The variables with lower proportional change recorded were general activity, mood, walking tolerance and everyday work. Conclusions: The treatment of myeloproliferative neoplasms with ruxolitinib represents a major advance in the management of myeloproliferative diseases and achieved excellent improvement in overall constitutional symptom control, splenomegaly and the general perception of the quality of life of these patients. Skin cancer is the most common malignant neoplasm worldwide and basal cell carcinoma is the most frequent type with a benign behavior, on the contrary, giant basal cell carcinoma is a rare subtype that is characterized by an aggressive biological behavior with involvement to deep tissue and its metastatic potential is reported to be high. This paper reports the case of a patient with myelodysplastic and myeloproliferative syndrome with an unresponsive giant basal cell carcinoma that was excised by extensive surgery with a multidisciplinary team. In a second operation, reconstructive surgery was performed using a vascularized composite graft with functional preservation of the affected area. [ABSTRACT FROM AUTHOR]

INTRODUCTION: Budd-Chiari syndrome is an obstruction of hepatic venous drainage. The main causes are myeloproliferative and anti-phospholipid syndromes. The predominant clinical results are from chronic liver failure as well as its complications. Diagnosis is by imaging or liver biopsy. Treatment will depend on the cause and have a survival of 75% at ten years. CASE REPORT: A 26 year old woman with history of polycythemia vera, Budd-Chiari syndrome and spontaneous abortion secondary to placental thrombosis, consulted in the Emergency Service in Hospital San Juan de Dios of San Fernando because of red vomits associated with melena. On admission were raised the following diagnoses: Upper gastrointestinal tract hemorrhage, severe anemia, anasarca, Budd Chiari Syndrome, Polycythemia Vera and Chronic Liver failure; Child-Pugh C. The patient was hospitalized on intermediate care unit with treatment based on physiological saline, blood transfusions, omeprazole, vitamin K and furosemide. Upper gastrointestinal endoscopy revealed signs of portal hypertension and esophageal medium varices with red sign. After eight days hospitalized, gastrointestinal bleeding stops, the patient is discharged and control is decided in seven more days. DISCUSSION: Gastrointestinal bleeding is a complication that achieves 10% to 15% in patients with Budd-Chiari syndrome, and can occur in the ones who are receiving anticoagulant therapy, as well in those with portal hypertension secondary to chronic liver failure. Introduction: The JAK-2 V617F mutation has great importance in the diagnosis, classification and treatment of chronic myeloproliferative disorders. Clinical case: In a 64 year old woman thrombocytosis was detected in the preoperative analyses for blepharoplastia who developed cerebral-vascular disease after the surgery. The most recent studies showed the presence of V617F and G20210A mutations of JAK-2 and prothrombin genes, respectively; as well as short times of TP and TPT times, and different cellular anomalies. Conclusion: The present study one of the first reported in Mexico, which describes the JAK-2 V617F mutation in patients with chronic myelopro-lipherative disorders, and the first describing this mutation in a patient with essential thrombocythemia who also possess another mutation of thrombotic risk. A four-year-old, from Chilapa Guerrero with one year and six months evolution of increased volume of the abdomen, jaundice, superficial collateral veins, haematochezia; 12 cm hepatomegaly and 22 cm splenomegaly and a papulosquamous lesion on the knee. In addition, showed pancytopenia and in a bone marrow sample, amastigotes were found. ELISA test was positive for leishmaniasis. She was treated with amphotericin B and meglumine antimoniate, with a satisfactory clinical evolution.
