Objective: to characterize pregnant women with severe preeclampsia and their quality of life in a highly complex obstetric institution. Methodology: descriptive observational study. Pregnant women with severe preeclampsia hospitalized between January and October 2020 were included. The EuroQOL-5D-3L questionnaire was used at admission and discharge to measure quality of life. Results: 75 % pregnant were 33 years old or younger, 25 % of them with term pregnancies, predominantly from rural areas and low socioeconomic level. The most frequent severity criteria were moderate-severe headache and blood pressure >160/110 mmHg. Throughout the stay there was a tendency to improve pain-discomfort and anxietydepression, while mobility, personal care and performance of daily activities presented a tendency to deteriorate. Conclusions: severe preeclampsia not only clinically affects the maternal-fetal relationship, but also its psychosocial dimensions. Efforts should be directed towards safe and humanized care with high standards of pain control and early detection of anxietydepression, which aim at a better quality of life for pregnant women with preeclampsia. Cardiovascular disease is the leading cause of death in the world; In turn, preeclampsia is the leading cause of maternal death worldwide. Hypertensive disorders of pregnancy, framed mainly by preeclampsia, translate into consequences that can be immediate, such as an increase in maternal and perinatal morbidity and mortality; however, its implications go further and it is estimated that its presentation contributes to the subsequent development of cardiovascular disease even years after pregnancy. In this order of ideas, the presentation of preeclampsia should be considered as an alert for women’s health, implementing prevention tools that impact modifiable risk factors and recommending the acquisition of healthy lifestyles with the aim of avoiding the presentation future cardiovascular disease and mortality from this cause. Preeclampsia isa pathology with high morbidity and mortality worldwide. In this disease, the placenta is an organ of shock where inflammation and the immune response generate the damage that results in the characteristic clinical scenario. The classic triad in preeclampsia is made up of hypertension, edema. and protei n uria, so it is thought that the endcthel ium must be affected by inflammatory-immunological activity. The immune system acts in the development of pregnancy and does so at different timesand regulating physio logically. Both, cellular and humoral components of the innate and acquired response have been studied in patients with preeclampsia and it has been determined that their participation is decisive in the pathophysiology of this disease. The involvement of the immune system in the pathophysiology of preeclampsia reaches a high level of complexity since it interacts with other systems (coagulation, renal, cardiovascular and endocrinological among others) thus favoring the disease. For this reason, treatment must be comprehensive, with a holistic vision of the condition and requires a multidisciplinary team that acts harmoniously to achieve the greatest therapeutic success with the least frequency of sequelae for the mother-fetus or mother-newborn dyads. Preeclampsia is a hypertensive complication of pregnancy, which is associated with high levels of maternal and fetal morbidity. About 5 to 8% of pregnancies are affected by this gestational disorder. Renal, cardiac, pulmonary, hepatic, neurologic dysfunction; hematologic disorders; fetal growth restriction; stillbirth; and maternal death are the serious complications that can occur in preeclampsia. Hyperuricemia is a biomarker that shows the progression of gestational hypertension and the risk of fetal and maternal complications as it induces the release of proinflammatory cytokines, such as TNF-a, IL-1ß and IL6. According to different investigations, authors indicate that high uric acid levels were notable in the second trimester in women who developed preeclampsia. Investigators estimate concentrations reported to be 6.08 ± 0.49 mg/dL versus 5.20 ± 0.19 mg/dL, and emphasize that at least 25.9% of hyperuricemic preeclamptic women (>6 mg/dL) were referred to the special care unit. The present study aims to collect current information with the purpose of clarifying the role of uric acid as a predictive biomarker in preeclampsia, in order to determine its diagnostic value in this gestational complication. Objective. This study aims to investigate the risk factors associated with hypertension in pregnant women through a comprehensive literature review. Methodology. A retrospective bibliographic investigation was conducted by searching for relevant information in both Spanish and English languages using databases such as PubMed, Scielo, Medline, NIH, and Elsevier. Results. A total of 33 articles were included in this research, covering various aspects including definitions, etiology, classification, risk factors, and laboratory examinations related to hypertension during pregnancy. Conclusions. Hypertension during pregnancy is a pathological condition that not only affects the mother but also poses risks to the fetus. The mortality rate among pregnant women with hypertension is estimated at 35%. Failure to timely detect and diagnose hypertension can result in complications such as hemorrhages, early placental detachment, spontaneous abortions, or preterm births, which may lead to maternal and fetal mortality. Objective: To define normal blood pressure at first trimester of pregnancy by performance for pre-eclampsia prediction. Method: A cohort study in singleton pregnancies, according to the competing risks model for pre-eclampsia. Blood pressure was measured according to a standardized method at 11 to 14 weeks' gestation. Results: Among 797 pregnancies included, there were 40 (5.0%, IC95 3.6; 6.4) deliveries with pre-eclampsia. The systolic, diastolic, and mean arterial blood pressures were respectively of 101(9), 68(8) and 79(8) mmHg in pregnant women who did not develop pre-eclampsia, against 109(10), 75(8) and 86(8) in those who did (p < 0.001, t-test). The areas under the curves were 0.707 (0.637; 0.777), 0.728 (0.661; 0.795), and 0.738 (0.673; 0.803). At a 3% of false positive rate, the cut-off points were 119, 83 and 94 mmHg with predictive values of negative of 95.6%. Introduction: nutrient and trace element deficiency are risk factors for the development of preeclampsia; these nutrients induce changes in the hematologic state, which can be used to prevent complications during pregnancy. Objective: this research will analyze the nutritional and hematological status during the 3 trimesters of gestation in pregnant women with preeclampsia to evaluate its association with gestational alterations. Method: a cross-sectional, descriptive, and analytic study was conducted on 78 pregnant women who attended a prenatal control clinic, 11 of whom were diagnosed with preeclampsia. The hematological parameters were determined through hematometry. To get the nutritional intake profile of the pregnant women, a frequency of food consumption questionnaire was used. Results: the normotensive group's arterial pressure (PA) was significantly lower than that of pregnant women with preeclampsia. The hematological profile changed during pregnancy, and platelet value was much higher in preeclamptic women. The nutritional status of pregnant women in the third trimester consumed more macronutrients and micronutrients than in the first and second trimesters, and this pattern was observed in pregnant women with preeclampsia. When compared to pregnant women with preeclampsia, normotensive women received more minerals and vitamins. Conclusion: hematological values are modified during each trimester of gestation, and pregnant women with preeclampsia had high platelet values compared to normotensive women. Our data show a greater intake of minerals and vitamins in normotensive pregnant women as compared to those who developed preeclampsia; therefore, it is recommended that pregnant women be provided with nutritional guidance in order to prevent pregnant complications. The aim of our study was to investigate the effect of inflammation in the placenta on the pro-apoptotic development after severe preeclampsia. Placenta tissue samples of 15 HELLP syndrome and 15 healthy 35-38th week-pregnant women were involved in the study. Tissue samples were taken only from the maternal side of the placenta and fixed in 10 % formaldehyde, then blocked in paraffin wax and 4-6 mm-thick sections were cut and stained with Harris Hematoxylene-Eosin. Antigen retrieval was performed for sections, incubated with FAS antibody and anti-IL-6 antibody. After the application of streptavidin peroxidase followed by AEC chromogen solution, sections were counterstained with Harris hematoxylin. Significant thickening of the fibrinoid layer, degeneration and apoptotic change in decidua cells, marked increase in the hyalinized area, degenerative changes in the syncytial regions of the chorionic villus and an increase in syncytial nodes and bridges and IL-expression were observed as positive. FAS expression was positive in the pycnotic nuclei of decidual cells in the maternal region and in the syncytial regions. It was observed that the proapoptotic process increased as a result of severe preeclampsia. It was concluded that the control of cytokine activity and reduction of pro-apoptotic signal during the inflammation process will slow down the development of HELLP syndrome. [ABSTRACT FROM AUTHOR]

Background: The ophthalmic artery is the first branch of the internal carotid artery. It comes out just distal to the cavernous sinus. The ophthalmic artery is an easily accessible vessel for Doppler evaluation that provides less accessible information about intracranial circulation. Preeclampsia is a hypertensive disorder in pregnancy related to 2% to 8% of pregnancy-related complications worldwide. Methodology: a narrative analysis was carried out through different databases from January 2014 to January 2022; the exploration and selection of articles was carried out in journals indexed in the English language. Among the keywords we use: Doppler, Artery, Ophthalmic, Prediction, Preeclampsia. Results: the internal carotid artery is divided into branches, the first being the ophthalmic artery. At the ophthalmic level, in women with preeclampsia, there has been a decrease in flow impedance and an increase in the speed in the waveform of the flow rate from the ophthalmic artery. It has been identified that between weeks 11 to 14, and between 20 to 28, patients who develop preeclampsia, present an increase in the second PSV, compared to patients in pregnancy without involvement. Another of the parameters evaluated in the Doppler of the ophthalmic artery is the maximum mesodiastolic velocity. Conclusions: this review provides updated and detailed information on the prediction of preeclampsia through the elective Doppler method of ophthalmic artery. [ABSTRACT FROM AUTHOR]

A sixteen-year-old female patient with a 32-week pregnancy which was interrupted due to maternal well-being due to the development of severe preeclampsia and obstetric hemorrhage, the patient suffers from acute pulmonary edema and heart failure, disseminated intravascular coagulation during the immediate puerperium as a complication of the disease hypertensive, aggravated by the presence of endomyometritis which motivates the performance of a total abdominal hysterectomy in order to evacuate the source of infection. After being assessed by the critically ill maternal care commission and taking the necessary measures before the intervention and knowing the high cardiovascular surgical risk that it represents, it is decided by anesthesiology to carry out anesthetic management with a total intravenous technique such as ataranalgesia (midazolam, ketamine, fentanyl), standard monitoring, maintain the ventilatory strategy established by intensive medicine, apply fluid restrictive therapy, the patient remains clinically stable during the perioperative period, the intervention is performed, she is delivered to the post-anesthetic care unit without anesthetic or surgical complications, After her evolution in the cardiology room, she is discharged home [ABSTRACT FROM AUTHOR]

The article describes the case of a pregnant patient with preeclampsia and segmental contractility disorders. The patient presented symptoms such as chest pain and changes in the electrocardiogram. The presence of severe preeclampsia and acute myopericarditis was confirmed through diagnostic tests. Despite the complications, both the patient and the baby had a satisfactory outcome. The increased risk of cardiovascular events in patients with preeclampsia during pregnancy is highlighted. The approach to obstetric patients with cardiovascular events should be coordinated between cardio-obstetric clinics. [Extracted from the article]

- Abstract
Introduction: Metabolic syndrome and insulin resistance are predictors of cardiovascular risk in pregnancy. Objective: To determine the presence of metabolic syndrome in pre-eclampsia and its relationship with cardiovascular risk. Method: Descriptive, prospective, observational, cross-sectional study was carried out with 180 pregnant women, from 2019 to 2020. Body mass index was calculated, blood pressure was measured and the diagnosis of metabolic syndrome was established, according to the existing criteria. Triglyceride, HDL cholesterol and blood glucose concentrations were quantified. Data were processed with SPSS v. 20.0 statistical software. Results: Seventy-three of the 180 pregnant women developed pre-eclampsia, in whom serum HDL cholesterol levels lower than 1.29 mmol/L (84.93%) predominated; as well as triglyceride levels greater than 1.7 mmol/L (67.12%), and abdominal circumference greater than 88 cm (71.23%). Metabolic syndrome was diagnosed in 63 pregnant women (35.0%), with predominance in the group with preeclampsia (49.31%). In 96.3% of the 107 pregnant women who had not pre-eclampsia, the cardiovascular risk was low, while 50.7% of the 73 pregnant women with pre-eclampsia had a moderate, high and very high cardiovascular risk. Conclusions: Half of women with pre-eclampsia had moderate, high, and very high cardiovascular risk. A high prevalence of metabolic syndrome was found in women who had pre-eclampsia, with a statistically significant association between these variables. It means that pregnant women with metabolic syndrome criteria present cardiovascular risk and a high risk of pre-eclampsia. [ABSTRACT FROM AUTHOR]
