Objectives: To understand the meaning of the experiences of breast cancer patients undergoing diagnostic and treatment processes. Material and methods: A descriptive qualitative study based on the hermeneutic-interpretative scientific paradigm and grounded in a phenomenological epistemological framework. The study included women aged 18 years or older diagnosed with breast cancer and treated at a university clinic in Bogotá, who were given a semi-structured interview. Procedure: Semi-structured interviews were conducted based on pre-established categories. Aspects evaluated: Knowledge about current breast cancer screening and detection patterns, current utilization patterns of healthcare services, and perceived healthcare needs. Results: Regarding Current knowledge patterns on breast cancer screening and detection, patients demonstrated limited awareness. Concerning Current utilization patterns of healthcare services, there were noted dissatisfactions with the administrative process. Regarding Perceived healthcare needs, a feeling of abandonment was identified, stemming from a lack of continuity in the care process. Additionally, two emergent categories were identified: trust in the healthcare system and predisposition to feelings of guilt due to a lack of knowledge about the disease. Conclusions: A comprehensive care process is needed that addresses the real medical needs of patients, beyond administrative concerns, and is focused on the quality of the doctor-patient relationship across the entire multidisciplinary team. 7. Resultados de la esofaguectomía mínimamente invasiva por cáncer de esófago a medio y largo plazo.

- Abstract
Introduction: Oesophagectomy is currently the curative treatment for oesophageal cancer. The aim of this study is to know the results of minimally invasive oesophagectomy in the short and medium term in patients operated on for squamous cell carcinoma and adenocarcinoma of the oesophagus in our hospital. Material and Methods: 19 patients were collected from January 2020 to June 2021 and all of them were followed up for 20 months. Different variables related to the patient, tumour, surgery and postoperative period were collected. Data were stored and processed using R-Comander statistical software assuming an α-error of 0.05. Results: The median total hospital stay was 29 days. Six patients required readmission to resuscitation: two for septic shock secondary to grade III anastomotic leak, one for upper gastrointestinal haemorrhage and three for respiratory failure. At 90 days, 5.3% of the total number of patients were readmitted. There were no deaths in the three months following surgery. All patients had a survival of more than six months. Overall survival at six, 12 and 18 months was 100, 84 and 63%. Disease-free survival at three months was 84%, at six months 63% and at one year 58%. Discussion: The results obtained in our study coincide with those reported in the literature. Bivariate failure data are common in reliability and survival studies, where estimation of dependency is often an important step in data analysis. In the literature, it known that the correlation coefficients measure the linear relationship between two variables, but strong non-linear relationship can also exist between them. Kendall's T concordance coefficient has become a useful tool for bivariate data analysis, which is used in nonparametric tests of independence and as a complementary measures of association. In the analysis of reliability data, there is a phenomenon that occurs when the value of the lifetime is partially known, which is known as censoring. In this paper, two estimation methods of Kendall's t are compared via simulation, one of them assuming normality in marginal distributions and adjusting them individually and the other based on copulas (Gaussian and Clayton), where the bivariate data are interval censored. The comparison is made using the mean squared error and the median absolute deviation. The results show that the method based on the copula approximation generally produces more precise estimates than the method of individual adjustment of the marginals. [ABSTRACT FROM AUTHOR]

Introduction: Sleep is essential for human health, and it is influenced by internal circadian rhythms that can be adjusted by external cues such as light. The nematode Caenorhabditis elegans is an invertebrate model that shows similarities to mammalian-sleep patterns. However, excessive light exposure can negatively affect stress induced sleep (SIS). Objective: To evaluate the effect of blue light (LED) and medium wave UV (UVB) on the survival of C. elegans and compare with the circadian cycle of 12 h white light exposure. Methods: An experiment was carried out using L1 larvae of C. elegans N2 cultured on NGM agar with a food source of E. coli OP50, and subjected to darkness, white, LED and UVB Lights in periods of 12 h and 24 h. Results: At the end of the experiment, an increase in mortality due to phototoxicity was evident, demonstrated by a lower survival rate in the larvae exposed for 24 h, when compared to the larvae that remained in permanent darkness, a natural condition of the parasite. The eccrine porocarcinoma (EPC) is a cancer of the sweat glands in the skin, with a diverse clinical presentation that includes plaques, ulcerated nodules, and polypoid lesions. Symptoms may involve itching, pain, and inflammation, in addition to signs of malignancy such as ulcers, bleeding, rapid growth, or pain. Histopathology shows the proliferation of atypical cells with malignant features, including pleomorphism and nuclear hyperchromasia. It can metastasize to distant organs, with common sites being bone, bladder, retroperitoneum, among others. Survival depends on the tumor size, with worse prognosis for tumors larger than 4 cm. The primary treatment is surgical removal, especially in early stages, with lymph node dissection in cases of involvement. Mohs micrographic surgery is used for localized cases. Radiation therapy and chemotherapy may be options in cases of metastasis or recurrence. Long-term follow-up is crucial, and ongoing research is needed to improve treatment strategies. Background: Children with congenital heart disease present a higher frequency of cardiorespiratory arrest (CRA) than the general pediatric population. The epidemiology of CRA is not exactly known in our setting, nor are the mortality risk or the neurological evolution factors. Objective: To describe the epidemiology and outcomes associated with pediatric cardiopulmonary resuscitation in a cardiovascular recovery unit. The primary endpoint was the survival to discharge and the secondary endpoints were the return to spontaneous circulation, the survival at 24 hours and the remote neurological condition. Methods: Descriptive, prospective, longitudinal cohort study in children under 18 years of age who required cardiopulmonary resuscitation between 2016 and 2019. Demographic variables, characteristics of cardiopulmonary arrest, resuscitation and outcome were analyzed. An uni- and multivariate analysis was performed comparing survivors and deceased. Results: Out of 1,842 hospitalized patients, 4.1% presented CRA. Fifty patients with complete records were analyzed. Seventy-eight percent (39) returned to spontaneous circulation with a high survival rate of 46%. Resuscitation > 6 min and the use of vasoactive drugs were predictors of mortality; 16/23 patients were followed up, 10 of them with normal development for age at 6 months, six had pervasive developmental disorder. Conclusions: 4.1% of patients presented CRA, with a rate of 3.4 CRA per 1,000 patient-days. Survival at hospital discharge (n = 50) was 46%. Resuscitation > 6 min and the use of vasoactive drugs were independent predictors of mortality. At six months, 63% had normal neurological development for age. [ABSTRACT FROM AUTHOR]

Introduction: Heart surgical intervention is complex, but with satisfactory results for the patient, because it increases his survival and life quality. The low cardiac output syndrome in the postoperative period is one of the entities that darkens the prognosis of this operation and elevates the morbidity and mortality indexes in these patients. Objective: To estimate the survival of patients with low cardiac output syndrome in the postoperative period according to clinical and echocardiographyc variables. Methods: A cohort investigation of survival in 56 operated patients was carried out, who were diagnosed with postoperative low cardiac output syndrome and were assisted in the Cardiology and Cardiovascular Surgery Center of Santiago de Cuba, from January, 2019 to November, 2021. Results: There was prevalence of the 65 years and over age group (60.7%), the diabetes mellitus (44.6%) as personal pathological history and patients with ejection fraction of 45% or more who presented higher quantity of deaths (29.8%). The function of the affected right ventricle was found in 52.6% of deaths. The time of extracorporeal circulation of 90 minutes or more prevailed in 67.9% of those affected, of whom 42.1% died. Conclusions: It was observed that diabetes mellitus, the time of extracorporeal circulation and the long perioperative bleeding, as well as the biventricular systolic function was related to the prognosis of low cardiac output syndrome in the postoperative period. - Abstract
Objective: We aimed to describe the main demographic, clinical, laboratory and therapeutic characteristics and to identify whether they are associated with mortality in tracheostomized patients. Material and methods. Retrospective cohort study in adult patients diagnosed with COVID-19, admitted to ICU (Intensive Care Unit) and requiring tracheostomy. Demographic, clinical, laboratory and treatment data were obtained from the medical records of patients admitted to Hospital III Daniel Alcides Carrión in Tacna. The Cox proportional hazards model was used for survival analysis and hazard ratios (HR) with their 95% confidence intervals (95%CI) were calculated. Results. We evaluated 73 patients, 72.6% were men, the most common comorbidities were obesity (68.5%), type 2 diabetes mellitus (35.6%), and arterial hypertension (34.2%). Thirty-seven percent of the participants died during their stay at the ICU. The median time from intubation to tracheostomy and the duration of tracheostomy was 17 (RIC: 15- 21) and 21 (RIC: 3-39) days, respectively. Multivariate analysis showed that the factors associated with mortality were procalcitonin > 0.50 ng/dL at the time of tracheostomy (HRa: 2.40 95%CI: 1.03-5.59) and a PaO2/FiO2 ratio less than or equal to 150 mmHg (HRa: 4.44 95%CI: 1.56-12.60). Conclusions. The factors associated with mortality at the time of tracheostomy were procalcitonin > 0.50 ng/dL and a PaO2/FiO2 ratio less than or equal to 150 mmHg. [ABSTRACT FROM AUTHOR]

Montane soils disturbed by forest loss, fire and livestock, are highly susceptible to erosion, leading to rapid degradation that reduces their fertility. Soil degradation hampers native forest restoration efforts, resulting in low natural recruitment rates and slow growth in field plantations. In order to contribute to more effective forest restoration strategies, we conducted a study to test the hypothesis that early addition of fertilizers in degraded montane soils improves the survival and growth of saplings during the initial months after planting. Our model species for this study was Polylepis australis, a tree commonly used in restoration projects in the highest mountains of central Argentina. In an area undergoing restoration with patches of degraded soils, we implemented seven treatments: three different doses of NPK (25, 34 and 67 g), three doses of urea (10, 20 and 30 g), and a control without fertilizer. After 14 months, the overall survival rate was 80%, and there was no significant correlation with the fertilization treatment. Regardless of the doses, saplings fertilized with NPK exhibited an average growth five times greater than unfertilized saplings, and more than twice that of urea-fertilized saplings. There were no significant differences in growth between unfertilized saplings and those fertilized with urea. These results suggest a deficiency of nitrogen (N), phosphorus (P), and/ or potassium (K) in the degraded soils for P. australis saplings, which apparently could be amended with any of the tested NPK doses, but no with the addition of urea alone (nitrogen only). We conclude that early nutrients addition in degraded montane soils can be an effective strategy to increase tree cover and reduce soil erosion in a shorter timeframe. [ABSTRACT FROM AUTHOR]

Newborn foals are very susceptible to potentially fatal diseases. Therefore, it is of great importance to identify variables that allow to predict the survival of these patients, and promptly start the appropriate treatment for each case. A retrospective study was carried out at the Equine Foal Care Perinatology Center, located in the municipality of Cajicá, Cundinamarca. The medical records of 106 foals admitted to hospitalization between September of 2019 and December of 2020 were reviewed, the necessary data was collect, and analyzed, in order to evaluate the predictive value of the clinical examination against the survival of hospitalized neonatal foals, and to identify which of these variables function as early markers of survival. Different statistical tests were used, such as the Kolmogorov Smirnov test, the Mann-Whitney test, the binary logistic regression, the chi-square tests, the ROC curves and the odds ratio (OR). The variables most significantly related to survival were heart rate (p = 0.019), rectal temperature (p = 0.002), lukewarm ears and limbs (p = 0.013) and concordant full strong pulse (p = 0.002). Risk factors associated with foal nonsurvival were temperature < 38 °C, heart rate < 100 bpm, cold ears and extremities, and weak pulse. [ABSTRACT FROM AUTHOR]

Newborn foals are susceptible to life-threatening diseases. Therefore, it is important to identify markers that make it possible to predict the survival of these patients, and to initiate the appropriate treatment for each case. This study was conducte at the Foal Care Equine Perinatology Center located in Cajicá, Cundinamarca. The medical records of 106 foals admitted to hospitalization between September 2019 and December 2020 were reviewed and analyzed to assess the predictive value of blood gas and electrolyte parameters against the survival of hospitalized neonatal foals. Different statistical tests were used, such as the Kolmogorov Smirnov test, Mann-Whitney U test, binary logistic regression, chi-square tests, odds ratio, and receiver operator characteristic (ROC) curve analysis. It was observed that the most common acid-base disorder upon admission to hospitalization was metabolic acidosis (37/106), and the most frequent electrolyte imbalance was hypocalcemia (89/106). The variable most significantly related to survival was pH > 7.35 (p = 0.007; OR = 0.288; CI = 95%, 0.117-0.706). The risk factors associated with non-survival of the foals were pH < 7.35, lactate > 4.5, and glucose < 119.5 mg/dl. [ABSTRACT FROM AUTHOR]

In ectothermic species, temperature is one of the most important environmental factors affecting growth, development and survival. In species with complex life cycles, such as amphibians, differential survival can lead to patterns of complex variability affecting size and structure of the population. The aim of this study was to evaluate in experimental conditions, the effect of temperature on survival, growth and development rate in tadpoles of two species of anuran Trachycephalus typhonius and Scinax nasicus. Twenty larvae of each species were reared in each of five treatments: 26, 28, 30, 32 y 34 °C. The results show that the larvae of S. nasicus are more sensitive to the effect of temperature, showing a lower survival compared to the larvae of T. typhonius. In turn, survival decreases significantly with increasing temperature, individuals exposed to 34 °C had the lowest survival rate. Temperature did not significantly affect the growth and development rates of T. typhonius tadpole, although there is a tendency to increase these parameters in the intermediate temperatures, finding the lowest values in the extremes. In contrast, S. nasicus significantly increased growth and development rates when raised at high temperatures. The results showed that high temperatures can affect the growth, development and survival of anuran larvae. Introduction: In Panama and the world, for several years, reefs have been threatened by actions such as coastal development, sedimentation, and contamination by terrestrial entities, which are the most frequent, giving rise to the implementation of restoration methodologies. Objective: To compare the efficiency of two coral garden structures in the growth of the species Acropora cervicornis in Huertas Bay, Portobelo National Park, Colon Panama. Methods: Two fixed-type structures were placed, using PVC together with hydraulic concrete bases, and two clothesline-type structures, using PVC and nylon thread. The coral fragments that were placed in the structures came from Bahía Huertas and Buenaventura Island. The growth of the corals was determined using the linear growth and tissue extension measurement method. Results: A total of 80 coral fragments were planted, representing an estimated extension of 9.71 m of coral in Huertas Bay. After nine months of study, a total of 15.3 m of totally new coral (1 525.6 cm) was estimated, where only 4.6 % belonged to the fragments of the fixe-type structure, while 95.4 % belonged to the clothesline-type structure. Conclusions: The clothesline-type structure was the most efficient and viable within the study, with better survival and growth rate compared to the fixed type structure, where it did present a mortality of 45 %. Introduction: Cervical cancer is the second most common malignant neoplasm affecting women in the Republic of Panama. Although it can be detected in early stages, patients in advanced stages associated with renal failure still arrive for care at the Institute. The objective of our work consists of reviewing the clinical evolution of patients with renal failure secondary to cervical cancer at the National Oncology Institute (ION) of Panama in a period from 2015 to 2020 in order to be able to make recommendations in the management of these. Material and Methods This is a retrospective and descriptive study. For the purposes of the study, creatinine levels greater than 2 mg/dL were considered. Quality of life was assessed by applying specific questionnaires that are not part of the Institute's patient care. In order to identify the cases, a cross-checking of data of patients with a diagnosis of cervical cancer and creatinine greater than 2 mg/dL was performed. Results. The total number of cases that could be reviewed was 65. 94% of the cases corresponded to the histological type of squamous cell carcinoma. The most frequent stage was IIIB (44%). The ECOG index 0/1 (72%). Arterial hypertension was present in 35% of cases and diabetes in 5%. There were patients in all creatinine ranges used. Of the 65 cases received some type of oncologic treatment. 36 of the 55 were treated with radiotherapy or radiochemotherapy. 10 patients could not receive any treatment due to poor general condition. The average overall survival was 7 months. The group aged 20-29 years had a better initial survival, but at 10 months this difference was lost. Conclusions The median overall survival of patients with renal failure secondary to cervical cancer in our study was 10 months and did not vary significantly according to the type of renal shunt used. - Abstract
Background: Breast cancer is the most frequent malignant tumor in women. Objective: To identify clinico-pathological and molecular markers as predictors of survival in patients with locally advanced breast cancer (LABC). Material and methods: Retrospective and observational study. The clinical factors of clinico-pathological and molecular predictors in relation with overall survival (OS) were assessed by the survival function, baseline hazard with smoothing and Cox regression. Results: 126 patients were assessed. OS at five years was significantly superior in patients with clinical stage IIIA (87%; p < 0.001), grade 2 tumor (81%; p < 0.001), pathological node stage (ypN0: 90%; p < .001), low-risk Nottingham prognostic index (86%; p < 0.001) and luminal A subtype (88%; p = 0.022). Baseline hazard with smoothing exhibited an increase in the mortality rate at 50 months for the luminal B/ HER2+ subtype compared with other subtypes. The multivariate analysis ascertained that the stage ypN2-3 (hazard ratio [HR] = 7.3; 95% confidence interval [95% CI]: 2.2 to 23.9) and the HER2+ nonluminal (HR = 7.8; 95% CI: 2 to 29.6) and triple negative (HR = 5.4; 95% CI: 1.7 to 17.2) subtypes were associated with a poor OS. Conclusions: The comprehensive evaluation of the molecular marker and clinico-pathological factors provides more accurate predictive and prognostic information. Background and objectives: 720,000 new cases of rectal cancer are reported each year. In the locally advanced stage (LARC), multimodal therapy followed by surgery with total mesorectal excision (TME) is the standard of care with potential benefits in acute toxicity, response assessment and resectability. The objective is to describe the clinicopathological characteristics and oncological results in patients treated with neoadjuvant chemoradiation therapy followed by radical surgery. Methods: A descriptive, non-experimental, cross-sectional and retrospective study in which the clinical records of 141 patients diagnosed with LARC defined as cT3T4N0 or cTXN+M0 scheduled for treatment with neoadjuvant chemoradiation therapy at the National Oncology Institute during the period from January 2014 to December 2019 were analyzed. Results: A total of 119 patients completed neoadjuvant chemoradiotherapy followed by radical surgery, resulting in a resectability of 90.1%. The overall rate of postoperative complications was 17.4%. The pCR and downstaging rate was 19.6% and 70.6%, respectively. Preoperative N category was the only variable with a statistically significant impact regarding pCR vs no pCR (p = 0.04). The median follow-up was 39 months, obtaining a 5-year overall survival of 69% and a 5-year disease-free survival of 74.4%. Conclusion: At the National Oncology Institute of Panama, the treatment of LARC with a long course of chemoradiation therapy followed by surgery with curative intent demonstrated high adherence to treatment with a rate of complete pathological response and local control in accordance with international reports. Objective. To evaluate the effect on biological, nutritional, and hematoimmune indicators of juvenile Cherax quadricarinatus were cultivated and fed with a probiotic mixture. Materials and methods. A completely randomized design (DCA) with six treatments: 0 (control), 1×102 µL, 2×102 µL, 3×102 µL, 4×102 µL and 5×102 µL of a probiotic mixture (Bacterol Shrimp Forte), with three repetitions each, 18 experimental tanks of diameter 1.7 m and area of 2.26 m2 were used, with a density of 20 juveniles (0.95 ± 0.10g and 7.78 ± 0.77mm) per tank for 60 days. Biological (weight, length, weight gain, weight increase, specific growth rate, length gain, length increase and survival), nutritional (feed conversion, feed efficiency and protein efficiency rate) and hematoimmune (total of hemocytes, differential hemocytes, phagocytic rate, superoxide dismutase and hypoxic stress) parameters were measured. Results. For biological indicators, the best results (p<0.05) were obtained when using 4×102 µL of the probiotic (final weight: 9.11 g; final length: 68.95 mm; specific growth rate: 3.74). Regarding the nutritional parameters, the best results were found with for 3×102 µL (feed conversion: 1.09, feed efficiency: 0.91, and protein efficiency: 2.61); although there were no differences between 3×102 and 4×102 µL. For the hematoimmune response, there were differences (p<0.05) for all the indicators under study, with a better performance for 4×102 µL of the probiotic mixture. Conclusions. The probiotic mixture induces the hematoimmune, biological, and nutritional response with the best response for concentrations of 3×102 µL, 4×102 µL. [ABSTRACT FROM AUTHOR]

Objective. To evaluate selected biological and histological factors associated with the survival of female dogs with mammary neoplasms. Materials and methods. An epidemiological study was performed based on the medical records of 72 female dogs diagnosed with mammary neoplasms. Associations between the characteristics of the female dogs, their reproductive status, the presence of parturition, the post-surgical survival time (m), and the final outcomes of the individual dogs were evaluated. Life tables and a Kaplan-Meier estimator were used for the statistical analysis of survival. Results. The female dogs evaluated were nulliparous (65.28%), with a mean age of 9.53±3.07 years and a weight varying between 1.5 and 49.1 kg (median 10.5 kg). The mortality incidence rate was 0.06 cases/patient/month (50 deaths/827.1 months of follow-up). Of the female dogs with mammary neoplasia, 37.50% survived, 15.28% were euthanized, and 30.56% died. The risk of death was 60.5 cases/1,000 female dogs/month, and the probability of post-surgical survival in months was 30.36%. Overall post-surgical survival was associated with the age, size, and reproductive status of the female dogs, along with size, subtype, malignancy, and TNM classification of the tumor (p<0.05). Conclusions. To ensure the survival of canine females with mammary neoplasia, early diagnosis by thorough physical examination and cytology, surgical removal when necessary, and timely consultation are imperative.
