The eccrine porocarcinoma (EPC) is a cancer of the sweat glands in the skin, with a diverse clinical presentation that includes plaques, ulcerated nodules, and polypoid lesions. Symptoms may involve itching, pain, and inflammation, in addition to signs of malignancy such as ulcers, bleeding, rapid growth, or pain. Histopathology shows the proliferation of atypical cells with malignant features, including pleomorphism and nuclear hyperchromasia. It can metastasize to distant organs, with common sites being bone, bladder, retroperitoneum, among others. Survival depends on the tumor size, with worse prognosis for tumors larger than 4 cm. The primary treatment is surgical removal, especially in early stages, with lymph node dissection in cases of involvement. Mohs micrographic surgery is used for localized cases. Radiation therapy and chemotherapy may be options in cases of metastasis or recurrence. Long-term follow-up is crucial, and ongoing research is needed to improve treatment strategies.
