Introduction: rotator cuff tears are one of the most common shoulder injuries. The treatment of choice is arthroscopic repair with the use of anchors. The modified Mason-Allen technique, which combines the benefits of suture bridge and mattress sutures, has shown satisfactory results. This study aims to report and analyze the clinical results using these suture techniques in rotator cuff tears. Results: preoperative mean active flexion 126o, 169o at three months, 175o at 12 months (p < 0.0001), active abduction 98o, 159o at three months and 167o at 12 months (p < 0.0001), internal rotation preoperative value 4.4 ± 3, at three months 7.1 ± 1.7 (p < 0.05) and 12 months 7.6 ± 1.1 (p < 0.001). Constant score 45.76 ± 16.35 (p < 0.0001) at three months 87.27 ± 10.94 and 12 months 91.30 ± 6.00. SSV 41.30 ± 20.89, three months 81.43 ± 18.31 and 12 months 94.37 ± 6.90 (p = 0.0001). Initial mean VAS 6.6 ± 1.6 and 12 months 0.63 ± 1.02 (p < 0.0001). Conclusion: in rotator cuff tears, using a single row with the modified Mason-Allen technique is a recommended and replicable option with satisfactory results and statistically significant clinical improvement at three and 12 months post-surgery. To promote muscle repair and regeneration, different strategies have been developed, however, there is still no single accepted treatment. Research relates the therapeutic effects of honey in the wound healing process of various etiologies. The aim of this study was to evaluate the effects of native honey formulation (Ulmoplus®) in the repair of muscle injuries by surgical incision. In five rabbits (Oryctolagus cuniculus) were made an incision 20 mm long and 3 mm wide in the anterior region of each leg, crossing the epidermis, dermis and anterior tibial muscle, until reaching the tibia periosteum. The left leg of each animal was classified as a suture group (S) and the right leg as a suture group plus Ulmoplus® (S + U). Subsequently, Ulmoplus® was applied to the entire S + U group throughout the entire length of the wound. Then, in both groups, the incision was sutured with four intradermal points (vicryl 2/0). An animal was considered as a control to obtain histological images of the anterior tibial muscle without injury. At 10 days post incision, muscle biopsies were taken and subsequently processed for observation under light microscopy. The study was carried out considering the recommendations of the Guide for the Care and Use of Laboratory Animals. At 10 days post incision, the injuries, in both groups of animals, were closed and without signs of infection. The anterior tibial muscle of the S group specimens was in the repair phase, with concomitant processes of phagocytosis of the necrotized tissue, myofiber regeneration and formation of scar connective tissue. The anterior tibial muscle of the animals of the S + U group was in the remodeling phase, since the muscular architecture was evidenced restored. Ulmoplus®, as an adjunct in the process of closing surgical wounds of skeletal muscle using conventional suture, is a good alternative, since it accelerates the muscle repair process and reduces scar connective tissue. BACKGROUND: The management of the nasal tip is of great importance in rhinoplasty. The septocolumellar suture can help the surgeon achieve a greater projection and rotations when managing the nasal tip. OBJECTIVE: To determine the changes in trans-surgical projection and rotation of the nasal tip with the use of the septocolumellar suture. MATERIAL AND METHOD: A quasi-experimental before and after study. We included patients undergoing primary rhinoplasty from April to August 2017. After managing the nasal tip, pre- and post-septocolumellar suture photographs were taken and analyzed. The nasal projection was evaluated using the Goode method, the nasolabial angle was measured to evaluate nasal tip rotation. RESULTS: There were included 30 patients, 26 (86.7%) were females. The average age was 23 ± 8 years old. In 18 patients (60%) a delivery approach was used, and cartilaginous grafts were applied; in 12 (40%) a retrograde approach was chosen and an interdomal suture was applied. Before the suture, the mean Goode's index was 0.67 ± 0.5, increasing to 0.75 ± 0.5 postsuture (p < 0.001). The nasolabial angle increased from 97 ± 10° presuture to 107 ± 9° postsuture (p < 0.001). CONCLUSIONS: The application of the septocolumellar suture after the adequate management of the nasal tip significantly increases both projection and rotation. We put in question the concept of truth because the introduction of Unconcious. Only in the manifest content, and based on the principles of the Aristotelian logic, the law of contradiction, the law of identity, the law of excluded middle, and the sufficient reason, truth can be considered in the sense of adecuacy "intelectus et rei". But if we introduce Unconcious, the being is put in question, what we say is not what we say, and truth is more in relationship witht imposibility, lack and nothing. The psychoanalyst must know how to move between "Escila" and "Caridbis" of both positions, to not fall out of the world, neither to hold on to the imaginary reality of the common sense. Introduction: Gastroduodenal perforation continues to be a relatively frequent surgical emergency, despite advances in the medical treatment of ulcer disease. Its laparoscopic approach has been increasing in the last years, although it has not been generalized. Objective: Was to analyze the postoperative results in patients with perforated ulcer treated with laparoscopic suture, and to compare them with a similar group with laparotomy suture. Our hypothesis was that laparoscopic suture is a safe option and with less morbidity than the laparotomy approach. Material and Methods: Comparative retrospective analysis of two patient cohorts: one treated with laparoscopic suture during 2014 and 2015, a period in which this approach was fully implanted in the emergency room in our hospital, and another comparable group treated by suture for open surgery during the period 2001-2003. Complications were analyzed according to Clavien-Dindo classification, conversion rate, mean stay and mortality. Results: The groups were comparable in age, sex, comorbidities and anesthetic risk. There was a trend towards superiority in favor of the laparoscopic approach in certain variables analyzed, with a conversion rate of 3%. The presence of early postoperative complications was greater in the laparotomy suture group: post-surgical septic shock (15.2% vs 6%) and wound infection (15.2% vs 3%), as well as medical complications, although not significantly. The group with laparoscopic suture had a longer surgical time, lower mean stay and lower mortality. Conclusion: The laparoscopic suture of the gastroduodenal ulcer in our center has had a very low conversion rate and a somewhat lower morbidity to the laparotomy suture, with a lower rate of reinterventions and a mean stay, despite a longer surgical time. Objective: To assess the clinical results and integrity of arthroscopic repair of rotator cuff tears using the suture bridge technique. Material and methods: Fifty-six shoulders with total thickness rotator cuff tears were assessed. Mean follow-up was 19 months. Repair integrity was analyzed postoperatively with imaging and recovery times. Results: The University of California at Los Angeles scale was used during the follow-up; the preoperative mean improved from 13.2 points to 29.7 (p\001). The rotator cuff healed completely in 66.7 of the 56 shoulders. The incidence of fatty regeneration of rotator cuff muscles was more common in individuals > 60 years of age (p = 0.002). The latter had a higher chance of recurrent tear (p\001). Discussion: There was significant pain relief in the visual analogue scale and improvement in ranges of motion. The advantages of this technique include that it allows immobilizing with a sling without the need for maximum abduction protection; it is a safer repair due to the configuration of the double band intercrossed over the tendon that provides greater fixation and stability, with greater coverage of the tear defect. Among the activities of a hospital pharmacy service is that of managing healthcare products, either overall or partially, with regard to particular processes (selection, purchasing, etc.). This creates a need for information and knowledge of the features of the different materials in order to be able both to select and purchase and to advise healthcare staff. Among these items are the different suture materials. The purpose of this discussion is to compile some basic, up-to-date notions concerning suture materials: definitions, conventional and other sutures, types of thread and their main features, and different types of materials and their uses. The advantages of cyanoacrylate based adhesives for use in cutaneous wound closure has been demonstrated, but few studies reporting the use of ethyl cyanoacrylate nationwide. The objective was to compare morphological aspects of healing in rabbit skin between ethyl-cyanoacrylate and suture. We used 10 adult male rabbits divided into 2 groups of 5 animals, the Center for Experimental Surgery, Universidad de La Frontera, Chile. The experimental group was subject to two incisions, one at each side of the dorsum of the thorax, 4 cm long, spanning epidermis and dermis. The incision was closed with ethyl right-cyanoacrylate, intradermal left with point of polyglycolic acid. Biopsies were taken at days 7, 14 and 21 days postoperatively. At day 7, the incision treated with ethyl cyanoacrylate presented a mild inflammatory reaction with moderate infiltration of PMNs. Suture biopsies showed a more advanced stage of regeneration, without presenting epidermal development. At day 14, the incisions treated with ethyl cyanoacrylate were in the final phase of the inflammatory stage, while suture biopsies were already in proliferative stage. At day 21, closing both types were remodeling stage, with thicker epidermis and basal lamina formed. The dermis showed irregular dense connective tissue, poor hair follicles and sebaceous glands. Ethyl-cyanoacrylate as Skin Adhesive is a good alternative use, showing adequate biocompatibility. Featuring a moderate inflammatory reaction, limited and of short duration dermal level. The scar achieves better cosmetic results regarding the incisions closed with sutures. Consideration should be limited to use low-tension sites not exposed to movements. [ABSTRACT FROM AUTHOR]

Patellar osteochondral lesions are difficult to diagnose, there must be a high index of suspicion in order to treat them timely. We present this case to point the keys of an early diagnosis and to expose Pritsch technique, which consists in suturing the osteochondral fragment to the anterior surface of the patella. It is necessary to slide the suture into the articulation in the arthroscopic techniques, this produce friction and abrasion of the suture, this is the principal cause of failure in the union of anchor-suture. We used a Fastak 2.4 anchor, Sawbones, No 2 Ethibond, No 2 Fiberwire and No 2 Herculine. Each suture was introduce to the anchor eyelet and was cycled in four times with 40N. The angles of traction were 0° and 45° at the same direction of the anchor eyelet and 45° with different direction of the anchor eyelet. Five sutures were used in every test. We performed the Kolmogorov-Smirnof and «t» Student tests. In all the tests there were a significant differences. The strength of the suture is affected by the abrasion in the anchor eyelet. Background: The suture is the traditional method of closing surgical wounds, and in recent years other methods of tissue adhesion as cyanoacrylates (CAL) have been proposed with limited use in inflamed skin. Objective: To determine the efficacy and safety of CAL versus simple monofilament suture in surgical wound closure of chronic skin inflammation. Patients and method: We conducted a randomized, open, controlled, single center, which included 60 patients (30 patients per group). Results: Wound dehiscence occurred in 10% of CAL and no patient with the use of simple suture. Dehiscence occurred in patients with inflamed skin.
